Spontaneously reported hepatic adverse drug events in Korea: multicenter study.

Hepatic adverse drug reactions (ADRs) to certain drugs may differ within each country, reflecting different patterns of prescription, socioeconomic status, and culture. The purpose of this study was to assess the suspected cause of hepatic ADRs using the spontaneously reported pharmacovigilance data from Korea. A total of 9,360 spontaneously reported adverse drug events (ADEs) from nine Pharmacovigilance Centers were analyzed. Risk of hepatic ADEs was assessed by calculating the reporting odds ratio (ROR). Of the 9,360 cases, 567 hepatic ADEs were reported. The most frequently prescribed drug

Multi-target mechanisms underlying the antiviral and anti-inflammatory activities of tobacco-derived natural small molecules.

Tobacco (Nicotiana tabacum L.) is widely cultivated worldwide and is traditionally associated with adverse health effects due to cigarette consumption. However, from a phytochemical and pharmacological perspective, tobacco plants represent a rich yet underexplored reservoir of bioactive natural small molecules. Phytochemical investigations have identified more than 2,500 specialized metabolites in tobacco, including alkaloids, terpenoids, phenolic acids, and flavonoids, many of which exhibit antiviral and anti-inflammatory activities. Conventional antiviral drug development has largely followe

Cationic amphiphilic scandenone derivatives as membrane-active anti-MRSA agents.

Methicillin-resistant Staphylococcus aureus (MRSA) remains a major challenge in anti-infective therapy, highlighting the need for antibacterial agents with novel structures and mechanisms of action. Herein, a gram-scale preparation of the natural antibacterial isoflavone scandenone was established, enabling systematic structural optimization of this prenylated isoflavonoid scaffold. Guided by a cationic amphiphilic design strategy, a series of scandenone-derived quaternary ammonium derivatives were synthesized to enhance antibacterial activity and membrane targeting. Among them, compound 16c d

Identification and characterization of Zika virus NS5 RNA-dependent RNA polymerase inhibitors.

The current outbreak of Zika virus (ZIKV) is the impetus for novel, safe and efficacious anti-ZIKV agents. ZIKV non-structural protein 5 RNA-dependent RNA polymerase (RdRp) is essential for viral replication and is logically regarded as an attractive drug target. This study used a fluorescence-based polymerase assay to find an anti-infective drug 10-undecenoic acid zinc salt (UA) which could inhibit RdRp activity with a half maximal inhibitory concentration (IC50) of 1.13-1.25 µM. Molecular docking and site-directed mutagenesis analyses identified D535 as the key amino acid in the interaction

Benzophenone Derivatives as a New Class of Anti-Infectives Against Acanthamoeba spp.

The rare but sight-threatening ocular disease Acanthamoeba keratitis represents a therapeutic challenge due to its causative agents, Acanthamoeba spp., being able to penetrate the corneal epithelium and persist in the corneal stroma as dormant and highly resistant cysts. To combat the scarcity and limitations of current treatment options, we developed a new class of anti-infective agents against Acanthamoeba spp. based on Ro 48-8071 (3), a known inhibitor of the human oxidosqualene cyclase (hOSC). The compound design was guided by docking studies using a homology model of the cycloartenol synt

Lymphoproliferative Disease and Hepatitis B Reactivation: Challenges in the Era of Rapidly Evolving Targeted Therapy.

Reactivation of hepatitis B virus (HBV) is a known complication that occurs in patients receiving chemotherapy especially for malignant lymphoma. The increased risk in lymphoma patients parallels the potency of the immunosuppressive treatment regimens that are provided. B-cell-depleting therapy such as anti-CD20 monoclonal antibodies, especially when combined with conventional chemotherapy, significantly increases the risk of HBV reactivation, even in patients with resolved HBV infection. The first reports of HBV reactivation with anti-CD20 therapy emerged only 4 years after its US Food and Dr

Infections, inflammation, and risk of neuropsychiatric disorders: the neglected role of "co-infection".

Neuropsychiatric disorders (NPDs) have multiple etiological factors, mainly genetic background, environmental conditions and immunological factors. The host immune responses play a pivotal role in various physiological and pathophysiological process. In NPDs, inflammatory immune responses have shown to be involved in diseases severity and treatment outcome. Inflammatory cytokines and chemokines are involved in various neurobiological pathways, such as GABAergic signaling and neurotransmitter synthesis. Infectious agents are among the major amplifier of inflammatory reactions, hence, have an in

Advances in the treatment of invasive neonatal candidiasis.

INTRODUCTION: Invasive candidiasis is responsible for ∼ 10% of nosocomial sepsis in very-low-birth-weight infants and is associated with substantial morbidity and mortality. Over the last two decades, the antifungal armamentarium against Candida spp. has increased; however, efficacy and safety studies in this population are lacking. AREAS COVERED: We reviewed the medical literature and extracted information on clinical and observational studies evaluating the use of antifungal agents in neonates with invasive candidiasis. EXPERT OPINION: Efficacy and safety data for antifungals in neonates a

HIV-Leishmania co-infections in Italy: serological data as an indication of the sequence of acquisition of the two infections.

Twenty-two sera from visceral leishmaniasis (VL) patients with human immunodeficiency virus (HIV) infection (50% with the acquired immune deficiency syndrome) were examined for anti-Leishmania immunoglobulin G (IgG) antibodies and compared with 35 sera from VL patients without HIV (controls). Significant titres of specific IgG were found in 81.8% of co-infections. However, while control sera showed a restricted range of anti-Leishmania IgG titres, sera from co-infection cases displayed a considerable degree of variability, both quantitative and qualitative. They were clearly divided into 2 gro

Combination of compound screening with an animal model identifies pentamidine to prevent Chlamydia trachomatis infection.

Chlamydia trachomatis (Ct) is the most common cause for bacterial sexually transmitted infections (STIs) worldwide with a tremendous impact on public health. With the aim to unravel novel targets of the chlamydia life cycle, we screen a compound library and identify 28 agents to significantly reduce Ct growth. The known anti-infective agent pentamidine-one of the top candidates of the screen-shows anti-chlamydia activity in low concentrations by changing the metabolism of host cells impairing chlamydia growth. Furthermore, it effectively decreases the Ct burden upon local or systemic applicati

Repurposing Nitroimidazoles: A New Frontier in Combatting Bacterial Virulence and Quorum Sensing via In Silico, In Vitro, and In Vivo Insights.

The global antibiotic resistance crisis demands innovative strategies targeting bacterial virulence rather than survival. Quorum sensing (QS), a key regulator of virulence and biofilm formation, offers a promising avenue to mitigate resistance by disarming pathogens without bactericidal pressure. This study investigates the repurposing of nitroimidazoles as anti-QS and anti-virulence agents at subminimum inhibitory concentrations (sub-MICs). In Silico analyses, including molecular docking and molecular dynamics (MD) simulations, were performed to investigate ligand-receptor interactions with s

Infective and inflammatory diarrhoea: mechanisms and opportunities for novel therapies.

There have been significant advances in unravelling the cellular mechanisms of diarrhoea in common gut infections and colonic inflammation, as well as in the identification of targets for potential antidiarrhoeal drugs. Infective diarrhoea reflects activation of electrogenic Cl⁻ secretion, inhibition of electroneutral NaCl absorption and in some cases, downregulation of tight junctional proteins and increased apoptosis. In colonic inflammation, diarrhoea mainly reflects impairment of colonic Na⁺ and Cl⁻ absorption by inflammatory cytokines, leading to decreased water absorption. Stimulation of

Post-traumatic infection of the lower limb caused by rare Enterobacteriaceae and Mucorales in a young healthy male.

Enterobacter amnigenus and Leclercia adecarboxylata are gram-negative aerobic bacilli of the family Enterobacteriaceae that have been isolated from water and, rarely, from various clinical specimens. Absidia is a filamentous fungus of the class Zygomycetes that is ubiquitous in nature and can cause infection, primarily in immunocompromised hosts. Here, we describe an infection of the left lower limb caused by E. amnigenus and L. adecarboxylata with subsequent isolation of Absidia spp. in a patient with multiple traumatic injuries after a major motor vehicle accident. The severity of the clinic

Peptide of Trichinella spiralis Infective Larval Extract That Harnesses Growth of Human Hepatoma Cells.

Trichinella spiralis, a tissue-dwelling helminth, causes human trichinellosis through ingestion of undercooked meat containing the parasite's infective larvae. However, benefits from T. spiralis infection have been documented: reduction of allergic diseases, inhibition of collagen-induced arthritis, delay of type 1 diabetes progression, and suppression of cancer cell proliferation. Since conventional cancer treatments have limited and unreliable efficacies with adverse side effects, novel adjunctive therapeutic agents and strategies are needed to enhance the overall treatment outcomes. This st

Repurposing Anidulafungin for Alzheimer's Disease via Fragment-Based Drug Discovery.

The misfolding and aggregation of beta-amyloid (Aβ) peptides have been implicated as key pathogenic events in the early stages of Alzheimer's disease (AD). Inhibiting Aβ aggregation represents a potential disease-modifying therapeutic approach to AD treatment. Previous studies have identified various molecules that inhibit Aβ aggregation, some of which share common chemical substructures (fragments) that may be key to their inhibitory activity. Employing fragment-based drug discovery (FBDD) methods may facilitate the identification of these fragments, which can subsequently be used to screen n

Post-infection sequelae of COVID-19 and other infectious diseases-a nationwide Danish study with 40-month follow-up.

Nationwide follow-up studies of long-term post-acute COVID-19 sequelae compared with sequelae of other infections have been lacking. Using nationwide registers, we analyzed all SARS-CoV-2 PCR test results, prescriptions for anti-infective agents, and hospitalizations with COVID-19 or other infections in Denmark from March 2020 to June 2023, including up to 40-month follow-up. We used Cox proportional hazards models with time-varying exposures to estimate the rates of first-time mental disorders (n = 5,306,132) and general medical conditions (n = 3,517,630). Here we show that positive SARS-CoV-
