[Apraxia of the larynx].

Disturbances of vocal function in patients with apraxia of speech are well known in the literature and have often been described. Although apraxia of phonation is presumed, only 1 case is mentioned in the literature. We present a 51-year-old man (with missing signs of dysarthria and aphasia after spontaneous right temproparietal hemorrhagia) who had aphonia and missing respiration during speaking, whereas articulation was nearly undisturbed. After differential diagnostic exclusion of other central disorders of phonation we classify this patient as having apraxia of phonation or laryngeal aprax

Anti-neutrophil cytoplasmic antibody-associated paraneoplastic vasculitis.

A 68 year old man presented with a systemic necrotizing vasculitis and elevated levels of anti-neutrophil cytoplasmic antibody (ANCA) which responded to treatment with steroids and cyclophosphamide, with a decrease in the titre of ANCA until its disappearance. Four months later he presented with weakness, loss of weight, aphonia and dysphagia. A computerized tomography scan showed a solid mass in the anterior mediastinum, and histological studies revealed an undifferentiated adenocarcinoma. Vasculitis improved although the malignancy progressed and ANCA was persistently negative. Our case demo

Anterior Neck Trauma.

In brief: Sports-related injuries to the anterior neck can cause serious airway compromise, requiring life-saving fieldside skills on the part of physicians and trainers covering the event. After reviewing the case of a recreational lacrosse player whose laryngeal injury resulted in severe respiratory distress, the authors discuss field management of anterior neck trauma, including the options for reestablishing an injured athlete's airway and the clinical signs-such as aphonia, hemoptysis, tracheal deviation, or palpable crepitus-that necessitate immediate transport to the hospital. The autho

Psychiatric presentation of Jakob-Creutzfeldt disease.

A case of presenile dementia, which on a post-mortem examination proved to be due to Jakob-Creutzfeldt Disease, is described. The most dramatic early clinical feature was that of hysterical aphonia, temporarily removable by hypnotic suggestion. Anxiety symptoms and irritability also occurred early on, followed by a transient hallucinatory psychosis. The hysterical features persisted throughout. In view of the lack of dementing features or neurological signs, a psychogenic diagnosis was repeatedly made by several neurologists. This case illustrates the fact that primary hysterical neurotic feat

Prevalence of phonatory symptoms in patients with type 2 diabetes mellitus.

To look at the prevalence of phonatory symptoms in patients with type 2 diabetes mellitus. The correlation between these symptoms with duration of the disease, glycemic control, and neuropathy will be described. A total of 105 consecutive patients diagnosed with type 2 diabetes mellitus by their primary endocrinologist were evaluated. A control group consisting of 33 healthy subjects was recruited for this study. Demographic data included: age, gender, allergy, smoking, duration of the disease, glycemic control, and presence or absence of neuropathy. Subjects were also asked about the presence

Intraoperative neurological event during cesarean section under spinal anesthesia with fentanyl and bupivacaine: Case report and review of literature.

Neurological events similar to transient ischemic attack in a peripartum woman are uncommon. Cerebral complications of preeclampsia, thrombo-embolic phenomena, or high spinal can mimic such situations. Spinal anesthesia with local anesthetic and opioid is an established anesthetic technique for cesarean section. Although intrathecal opioids are safe for both the mother and fetus; some unusual complications such as dysphagia alone or associated with facial numbness, aphasia, have been reported. We report a case of transient aphonia and tingling sensation over the face without any dysphagia afte

Speech development in aphonic children.

It has been well established that pre-lingually hearing-impaired children experience difficulties in speech development. Little appears in the literature, however, about speech development in aphonic children. The authors have had the opportunity to manage several children who, for various reasons, were unable to phonate from a pre-lingual age to approximately the age of 5 or 6 years. The children in this report developed various compensatory means of communication, including unusual laryngeal, non-laryngeal and nonverbal communication. Representative cases are reviewed and observations are ma

Something's "Fishy" With That Sore Throat..

A 9-year old male presented to the emergency department for the evaluation of a sore throat. Initial history of present illness stated in the triage note was that "the child complained of a sore throat," suggesting possible pharyngitis. There was no evidence of stridor, sialorrhea, hypoxia, or aphonia. Further investigation of the history of present illness identified the concern for a possible esophageal foreign body. Imaging studies of the neck identified a fishbone in the esophagus. This case presentation discusses an atypical presentation of an esophageal foreign body and the subtle findin

[CHAOS in neonatal emergency care? : Tracheal agenesis in the obstetric theatre].

Tracheal agenesis (TA) is a very rare congenital malformation of unknown aetiology. It is often associated with polymalformative syndromes; the neonates commonly present a critical condition during post-natal treatment. Pathology revolves around the triad of aphonia, respiratory distress syndrome and impossibility of endotracheal intubation. In contrast to the most important differential diagnosis, i. e., congenital high airway obstruction syndrome (CHAOS), surgical airway management is also impossible due to the absence of tracheal structures. In most cases, prognosis is very poor. The case r

Phoniatric management of 3 clinical cases of voice disorders involving the recurrent nerve.

These three cases involve aphonia or dysphonia as a consequence of entrapment of one of the recurrent nerves, in the absence of any prior surgical history. Depending on the particular case, one to two months was needed to normalize phonatory function; One case relapsed after a year. The phoniatric management of these cases involved: New functional and acoustic pattern, recorded by the patient as a form of daily maintenance; Laryngeal and perilaryngeal manual therapy, applied according to the "osteovox" method. Manual therapy and different phonatory habits, in association with alternative medic

Pharyngeal traumatic neuromas and traumatic neuromas with mature ganglion cells (pseudoganglioneuromas).

Over a period of 4 years, 14 cases of traumatic neuroma were detected in laryngoscopic biopsy specimens. The approximate incidence was 10% of biopsy specimens from this area. The most frequent symptoms in descending order of frequency, were hoarseness, dysphagia, choking spells, intermittent aphonia, and cough. Six (43%) of the 14 lesions contained mature ganglion cells. These lesions have not been reported in the pharyngeal region, and I suspect that in the past a few similar conditions have been diagnosed as ganglioneuromas. Because the lesion appears to be the cause of the symptoms and is a

Laryngoscopy and Bronchoscopy in an Infant With a Congenital Laryngeal Web Complicated by Bilateral Pneumothoraces: A Case Report.

Neonates with laryngeal webs pose unique challenges to the anesthesiologist. We present a 2-day-old neonate with aphonia and stridor who underwent microdirect laryngoscopy. Intraoperatively, a Cohen type 4 laryngeal web was diagnosed not immediately amenable to resection. Therefore, the decision was made for endotracheal intubation and subsequent tracheostomy. After endotracheal intubation, there was acute respiratory compromise and oxygen desaturation that improved moderately after urgent tracheostomy. A chest radiograph revealed a large pneumothorax. Our experience suggests that in the prese

Ewing's sarcoma of the larynx. Effective treatment with organ preservation.

Extraskeletal Ewing's sarcoma arising in the head and neck region is an extremely rare malignant neoplasm. We describe the unusual case of a tumor originating in the larynx of a 68-year-old male with hoarseness and occasional aphonia. We report successful treatment with sequential chemo- and radiotherapy. Complete regression and larynx preservation with voice function recovery was achieved. To our knowledge, this is the first report of this type of tumor in the larynx with cartilage invasion that documents the effectiveness of radiotherapy as an alternative to surgical management. At present,

Complete traumatic laryngo-tracheal separation.

Laryngotracheal separation injuries are rare and potentially fatal. Immediate respiratory signs may include dysphonia, aphonia, hemoptysis, subcutaneous emphysema and a sucking wound. Patients with this injury usually die at the site of the trauma. The absolute life saving intervention for patients with laryngotracheal injury is airway control via routine intubation or emergency tracheostomy. We present an extremely rare case of complete laryngotracheal separation in a teenager driving a quad bike in a 'clothes line' type injury with chicken wire. This case highlights the need for prompt airwa

Arytenoid adduction to treat impaired adduction of the vocal fold due to rheumatoid arthritis.

Rheumatoid arthritis (RA) is a chronic inflammatory disease affecting the synovial membrane and causing joint damage and bone destruction. The symptoms of cricoarytenoid joint (CJ) arthritis often include hoarseness, and a sense of pharyngeal fullness in the throat. Sometimes, in cases with bilateral CJ involvement, an urgent tracheostomy might be required for acute airway obstruction. In this report, we describe a woman suffering from aphonia due to hampered adduction of the vocal fold which was caused by RA with unilateral CJ involvement. Arytenoid adduction surgery on the affected side was
