Management of Low-Grade Appendiceal Mucinous Neoplasms (LAMN): An International Survey of Surgeons Performing CRS and HIPEC.

BACKGROUND: Low-grade appendiceal mucinous neoplasms (LAMN) are commonly managed by community surgeons at diagnosis. There is variability in the treatment of LAMN, both by community and specialist providers. We assessed current management practices for LAMN across surgeons with expertise in peritoneal surface malignancies (PSM). METHODS: An online survey was sent to 106 international surgeons specializing in PSM. The survey assessed demographics, favored pre-referral management, and definitive management practices for LAMN. RESULTS: The response rate was 40% (67% USA, 33% international). Res

Advanced Computational Modeling and Machine Learning for Risk Stratification, Treatment Optimization, and Prognostic Forecasting in Appendiceal Neoplasms.

BACKGROUND: Appendiceal neoplasms account for less than 1% of gastrointestinal cancers but are increasing in incidence worldwide. Their marked histological variations and differences create multiple challenges for prognosis and management planning, as current staging systems are limited in certain aspects for capturing the entire disease complexity. METHODS: We synthesized data from 18 large observational studies, including 67,001 patients diagnosed between 1973 and 2024. Using advanced computational modeling, we combined multiple statistical methods and machine learning techniques to improve

Misdiagnosis of appendiceal neoplasms as ovarian tumors: Impact of prior gynecologic surgery on definitive cytoreduction and HIPEC.

BACKGROUND: Female patients with pelvic/adnexal masses often undergo gynecologic operations due to presumed ovarian origin. The diagnosis of an appendiceal tumor is often only made postoperatively after suboptimal cytoreduction has been performed. We hypothesized that an index gynecological procedure increases the morbidity of definitive cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS/HIPEC) in patients with appendiceal mucinous tumors. METHODS: A single-center retrospective review was performed to identify female patients undergoing CRS/HIPEC for appendiceal tumors f

Progression to pseudomyxoma peritonei in patients with low grade appendiceal mucinous neoplasms discovered at time of appendectomy.

BACKGROUND: The discovery of a low grade appendiceal mucinous neoplasm (LAMN) during appendectomy is a rare scenario. These neoplasms can progress to pseudomyxoma peritonei (PMP), however the incidence of progression is not well known. METHODS: The records of all patients with a diagnosis of localized LAMN found during appendectomy were identified, and demographic, tumor, surveillance, and outcome variables were analyzed. RESULTS: Progression to PMP occurred in 20% of patients in an average of 12.4 months after appendectomy with median follow-up of 18 months. Tumor variables such as margin p

Survival Profile and Prognostic Factors for Appendiceal Mixed Neuroendocrine Non-neuroendocrine Neoplasms: A SEER Population-Based Study.

INTRODUCTION: Mixed neuroendocrine non-neuroendocrine neoplasm (MiNEN) is a rare form of neuroendocrine neoplasms (NENs). The purpose of this study was to investigate the characteristics and survival profile of appendiceal MiNENs, with a view of providing robust clinical features of this rare disease. METHODS: Patients were selected from the Surveillance, Epidemiology, and End Results database (2004-2016). The prognosis of MiNEN (n = 315) was compared with other histological subtypes including neuroendocrine tumor (NETs) (n = 1734), neuroendocrine carcinoma (NECs) (n = 375), goblet cell carci

Duplicated Appendix With Mucocele Mimicking an Appendiceal Neoplasm in a 45-Year-Old Woman: A Case Report.

BACKGROUND Appendiceal duplication is an extremely rare congenital anomaly, with an incidence of 0.004% to 0.009%. An association with an appendiceal mucocele is even rarer. It can complicate the diagnosis and management of right iliac fossa pathology and remains difficult to identify preoperatively. Failure to recognize it can result in recurrent symptoms, missed pathology, intraoperative challenges, and medicolegal consequences. This report describes the case of a 45-year-old woman with history of a loculated peri-appendiceal abscess managed with percutaneous drainage and intravenous antibio

Case Report: Lumbar herniation of a low-grade appendiceal mucinous neoplasm mimicking a chronic abscess.

Low-grade appendiceal mucinous neoplasms (LAMNs) are uncommon tumors that may lead to peritoneal dissemination if not completely excised, yet they often present with nonspecific symptoms. We describe a 53-year-old male with a three-year history of recurrent right lumbar "abscesses" whose contrast-enhanced CT suggested herniation of the ileocecal region into the abdominal wall. Laparoscopic exploration revealed a superior lumbar hernia containing an enlarged appendix with surrounding purulent fluid. The patient underwent laparoscopic ileocecal resection with side-to-side ileocolic anastomosis a

Low-Grade Appendiceal Mucinous Neoplasm With Discontinuous Mucin Infiltration Into the Cecal Wall: A Case Report.

Low-grade appendiceal mucinous neoplasm (LAMN) can lead to intraperitoneal dissemination and subsequent pseudomyxoma peritonei (PMP). Therefore, achieving complete resection, typically by appendectomy, is considered important for curative treatment. We report a case in which discontinuous mucin infiltration into the cecal wall was identified only after secondary resection, providing insight into the extent of surgical resection. To the best of our knowledge, such cases appear to be rare, with limited reports in the literature. A 60-year-old female was incidentally found to have an enlarged app

Incidentally Detected Low-Grade Appendiceal Mucinous Neoplasm Following Laparoscopic Appendectomy for Acute Appendicitis.

Appendiceal mucinous neoplasms are rare entities that account for a small proportion of all gastrointestinal malignancies and are most often discovered incidentally on histopathological examination of appendectomy specimens. We present the case of a 36-year-old woman who presented to the emergency department with acute right lower quadrant abdominal pain and was found to have acute appendicitis on computed tomography imaging. She underwent an uncomplicated laparoscopic appendectomy, and final pathology revealed a low-grade appendiceal mucinous neoplasm (LAMN) confined to the appendix, staged a

Mechanical Small Bowel Obstruction Secondary to Low-Grade Appendiceal Mucinous Neoplasm: A Case Report.

Low-grade appendiceal mucinous neoplasm (LAMN) is an uncommon epithelial tumor that is frequently diagnosed incidentally and only rarely produces mechanical small bowel obstruction, with previous reports limited to isolated cases. We describe a 53-year-old man with a four-day history of periumbilical pain, obstipation, nausea, and bilious vomiting. Examination showed abdominal distension, decreased bowel sounds, and rebound tenderness. Contrast-enhanced computed tomography (CT) demonstrated a 7 cm heterogeneous right hemiabdominal mass with pericecal fat stranding that favored, but did not def

Diagnostic uncertainty and management of low-grade appendiceal mucinous neoplasm-a case report and review of the literature.

Low-grade appendiceal mucinous neoplasm (LAMN) is a rare entity identified in ~1% of patients undergoing appendectomy. The presentation often varies, making diagnosis challenging. Timely identification and treatment are critical to prevent rupture, which may lead to pseudomyxoma peritonei. We describe the case of a 41-year-old male who presented for evaluation of acute right lower quadrant abdominal pain. The clinical impression was consistent with appendicitis with a clinical suspicion for underlying malignancy. The patient was brought to the operating room for an exploratory laparotomy and r

[A Case of Small Bowel Obstruction Caused by an Intraoperatively Recognized Low-Grade Appendiceal Mucinous Neoplasm].

An 89-year-old woman with an allergy to contrast dye and with a history of cholecystectomy visited emergency department complaining of vomiting and abdominal pain. The diagnosis of adhesive bowel obstruction near terminal ileum was made with plain computed tomography. Although intestinal decompression was attempted with nasogastric tube, followed by intestinal tube, no improvement was achieved, so adhesiolysis was planned. Intraoperatively, an extrinsic mass was detected near the terminal ileum, and ileocecal resection was performed. the pathological diagnosis was low grade appendiceal mucinou

A Case of Low-Grade Appendiceal Mucinous Neoplasm That Led to Surgery After 12 Years of No Treatment.

Low-grade appendiceal mucinous neoplasm (LAMN) is a relatively rare, non-invasive appendiceal tumor. We experienced a case of LAMN that led to surgery after 12 years of no treatment. Until now, LAMN has been reported to progress more slowly than other tumors, but there have been no reports of long-term follow-up of appendiceal tumors without treatment. Although the tumor had grown over the course of 12 years, there was no mixing or migration of other histological types, and it did not lead to pseudomyxoma peritonei. As this course is considered to be relatively rare, we report it along with a

A giant low-grade appendiceal mucinous neoplasm (LAMN) presenting as ileocecal intussusception: a case report.

We report the rare case of a giant low-grade appendiceal mucinous neoplasm (LAMN), presenting as ileocecal intussusception. An 80-year-old woman presented in the emergency department of our institution with progressively worsening diffuse abdominal pain during the last 24 h. A CT scan revealed a giant abdominal mass (98.7 × 127.3 × 107.6 mm) with air-fluid level and imaging characteristics of ileocecal intussusception. An emergency exploratory laparotomy was performed, and a well-circumscribed cystic mass deriving from the appendix was found. A right hemicolectomy was performed, and the histop

True Incidence of Appendiceal Neoplasms in Patients Presenting with Complicated Appendicitis: A Call for a Revised Management Algorithm.

BACKGROUND: Prior studies suggest an association between complicated appendicitis and underlying appendiceal neoplasms, but the reported incidence varies widely. This study aimed to determine the incidence and predictors of appendiceal neoplasms among patients with radiographically diagnosed complicated appendicitis. METHODS: This study analyzed adult patients with radiologic diagnoses of complicated appendicitis (perforation, abscess, phlegmon, gangrene, or mucocele) at a tertiary center (2010-2024). To ensure diagnostic accuracy, an expert surgeon reviewed radiologic images with uncertain f

FDG PET/CT and Histopathologic Characteristics of Low-Grade Appendiceal Mucinous Neoplasm with Pseudomyxoma Peritonei.

A 50-year-old woman presented a rapidly proliferative cystic lesion in the left pelvis as displayed by sonography. Exploratory laparoscopy with omental biopsy suggested pseudomyxoma peritonei (PMP). An 18F-FDG PET/CT revealed scalloping of the liver surface with an associated extrahepatic lesion exhibiting moderate FDG avidity and strand-like FDG uptake within a left-sided septated ovarian cyst. The diagnosis of low-grade appendiceal mucinous neoplasms was proven after cytoreductive surgery. This case provided important imaging features on 18F-FDG PET/CT that directed the pre-operative diagnos
