Mid-term results of orbital atherectomy in heavily calcified lesions above and below the knee.

Background: Severe arterial calcification poses a major challenge to endovascular therapy (EVT) in peripheral artery disease (PAD). Orbital atherectomy (OAS) is used to modify plaque and enhance luminal gain, potentially improving EVT outcomes, but long-term evidence remains limited. Patients and methods: This retrospective single-center study assessed the mid-term outcomes of OAS in heavily calcified infrainguinal disease. We included symptomatic PAD patients (Rutherford 2-6) with ≥70% stenosis undergoing OAS for above- and below-the-knee (BTK) arterial lesions. Results: From 2018 to 2023, 78

Factors Associated With Vessel Injury Caused by Atherectomy in the Treatment of Severely Calcified Femoropopliteal Lesion.

BACKGROUND: The factors predicting vessel injury during the use of the Jetstream atherectomy system (JS) for femoropopliteal (FP) lesions have not been fully investigated. AIM: This study aimed to identify factors associated with vessel injury when using the JS in severely calcified FP lesions. METHODS: This was a single-center, retrospective observational study. A total of 57 patients with 68 FP lesions underwent endovascular therapy (EVT) using the JS between April 2024 and May 2025. Vessel injury was defined as evidence of medial or adventitial disruption on intravascular ultrasound evalu

Comparison of intravascular lithotripsy versus rotational atherectomy for the treatment of severe coronary artery calcification.

BACKGROUND: Calcified lesions are one of the most challenging cases for PCI, where optimal angiographic results and satisfying outcomes are hard to achieve. METHODS: We evaluated the baseline clinical, procedures characteristics and outcomes of patients with severe coronary artery calcification (CAC) who underwent coronary intravascular lithotripsy (IVL) and rotational atherectomy (RA). RESULTS: Respectively 152 and 238 patients who underwent IVL and RA are enrolled from January 2023 to November 2023. Regarding demographic characteristics, the gender proportion, medical history of PCI and sm

Optimized strategy of rotational atherectomy of underexpanded coronary stents in patients with acute coronary syndrome.

BACKGROUND: Stent under-expansion is a main cause of acute coronary syndrome (ACS), which can lead to serious clinical outcomes. The rotational atherectomy of underexpanded coronary stents (academically called stent ablation, SA) by intravascular ultrasound (IVUS) may provide more visual reference in the intervention. We aim to analyze the procedural and long-term outcomes of the optimized strategy of SA in patients with ACS and to provide real-world data on this technique. METHODS: A total of 11 patients with ACS who underwent SA between April 2017 and January 2019 were analyzed. Clinical fo

Adverse Events and Modes of Failure Related to Rotational Atherectomy System: The Utility of the MAUDE Database.

BACKGROUND/PURPOSE: Coronary artery calcification is a marker of advanced atherosclerosis and a predictor of adverse clinical outcomes. Rotational atherectomy (RA) can effectively modify calcified lesions, optimizing procedural outcomes. We interrogated the most commonly reported adverse events involving rotational atherectomy systems (Rotablator and Rotapro) by analyzing post-marketing surveillance data from the Food and Drug Administration Manufacturer and User Facility Device Experience (MAUDE) database. METHODS/MATERIALS: We queried MAUDE from September 1, 2016, through December 31, 2019.

Feasibility and safety of orbital atherectomy for the treatment of in-stent restenosis secondary to stent under-expansion.

BACKGROUND: Debulking and ablative techniques are sometimes used for the treatment of in-stent restenosis (ISR) secondary to resistant stent under-expansion (SU). The safety and effectiveness of orbital atherectomy (OA) in this cohort of patients has not been reported. METHODS: We retrospectively evaluated consecutive patients treated with OA for ISR secondary to balloon undilatable SU at two academic tertiary care centers between October 2016 and June 2019. Angiographic or intravascular imaging identified SU. Technical success was defined as residual 0% stenosis with TIMI III flow. RESULTS:

Orbital atherectomy for the treatment of severely calcified coronary lesions: evidence, technique, and best practices.

The presence of severe coronary artery calcification is associated with higher rates of angiographic complications during percutaneous coronary intervention (PCI), as well as higher major adverse cardiac events compared with non-calcified lesions. Incorporating orbital atherectomy (OAS) for effective preparation of severely calcified lesions can help maximize the benefits of PCI by attaining maximal luminal gain (or stent expansion) and improve long-term outcomes (by reducing need for revascularization). Areas covered: In this manuscript, the prevalence, risk factors, and impact of coronary ar

Plaque Excision in Management of Lower Extremity Peripheral Arterial Disease with the SilverHawk Atherectomy Catheter.

Atherectomy, the removal of plaque from diseased vessels, is theoretically appealing with respect to various procedures used for revascularization of lower extremity vessels. Instead of damaging the native vessel by pushing aside plaque with a balloon or stent, the plaque is removed. Many atherectomy devices have been designed in attempts to achieve this goal. The SilverHawk device is the latest percutaneous device employing this treatment strategy. We discuss patient selection, technical considerations, and strategies for both above and below the knee revascularization, as well as pitfalls in

[In-hospital major complications associated with rotational atherectomy: experience with 800 patients at a single center].

INTRODUCTION: Rotational atherectomy is usually performed in patients with angiographically determined high risk coronary lesions. The aim of this study was to evaluate the rate of major adverse cardiac events (death, Q-wave infarction or new revascularization) after rotational atherectomy, as well as to identify the clinical characteristics associated with this incidence. PATIENTS AND METHODS: The study population included 800 patients treated with rotational atherectomy from 1993 to 1999: 512 (64%), for de novo lesions, and 288 (36%) for restenosis. Balloon dilation and coronary stenting wa

Rotational atherectomy for in-stent restenosis: acute and long-term results of the first 100 cases.

OBJECTIVES: This study evaluated the clinical safety and long-term results of rotational atherectomy (RA) followed by low-pressure balloon dilatation (percutaneous transluminal coronary angioplasty [PTCA]) for the treatment of in-stent restenosis (ISR). BACKGROUND: In-stent restenosis is associated with a high incidence of recurrence after interventional treatment. Because ISR is due to neointimal hyperplasia, rotational ablation may be a more effective treatment than PTCA. METHODS: Between November 1995 and November 1996, 100 consecutive patients with first-time ISR were treated by RA. Quan

Directional Coronary Atherectomy as a Diagnostic Adjunct in Suspected IgG4-Related Coronary Artery Disease.

BACKGROUND: Immunoglobulin (Ig) G4-related disease can involve the cardiovascular system, and isolated vessel involvement is diagnostically challenging. CASE SUMMARY: A 79-year-old Japanese man with a history of percutaneous coronary intervention presented with exertional angina. Imaging demonstrated in-stent occlusion of the left anterior descending artery, severe distal left circumflex artery stenosis, and marked adventitial thickening of the coronary arteries and aorta. Serum IgG4 levels were markedly elevated. Directional coronary atherectomy (DCA) enabled retrieval of intimal tissue. Alt

Technical note: Improving orbital atherectomy efficacy for calcified nodules using a curved guide catheter.

ObjectivesEndovascular therapy (EVT) for calcified nodules in patients with peripheral artery disease (PAD) remains challenging in achieving favorable outcomes. This study aims to investigate the effectiveness of orbital atherectomy (OA) for calcified nodules using an IM catheter to precisely control the device and achieve optimal engagement with the target lesion.MethodsWe performed EVT for a calcified nodule in the right common femoral artery using an OA. Due to the large vessel size, controlling the OA to effectively engage the lesion was challenging. To overcome this, we utilized an IM cat

White thrombi on optical coherence tomography after rotational atherectomy of severely calcified coronary lesions.

PURPOSE: Rotational atherectomy (RA) has improved percutaneous treatment of severely calcified coronary lesions, but the "no-reflow" phenomenon remains a serious complication. Platelet activation by RA may contribute to no-reflow, and the use of optical coherence tomography (OCT) to test the effect of RA on white thrombus could confirm platelet activation indirectly. METHODS: We analyzed 53 consecutive patients with severely calcified lesions on coronary angiography. All patients were examined with OCT. In total, 20 patients who received RA and for whom OCT imaging was performed before and af

Coronary orbital atherectomy treatment of Hispanic and Latino patients: A real-world comparative analysis.

OBJECTIVES: To assess coronary orbital atherectomy (OA) use in Hispanic or Latino (HL) patients compared to non-HL patients. BACKGROUND: HL patients are at greater risk of cardiovascular disease mortality compared with Whites with similar coronary artery calcium (CAC) scores. The safety and efficacy of coronary atherectomy in the HL patient population is unknown due to the under-representation of minorities in clinical trial research. METHODS: A retrospective analysis of consecutive patients undergoing coronary OA treatment of severely calcified lesions at the Mount Sinai Medical Center, Mia

Predictors of clinical outcome after rotational atherectomy-facilitated percutaneous coronary intervention in hemodialysis patients.

BACKGROUND: Percutaneous coronary intervention (PCI) in hemodialysis patients with severely calcified and diffused lesions is associated with extremely high rates of major adverse cardiovascular events (MACE), even when facilitated by rotational atherectomy (ROTA). Potential risk factors for MACE with ROTA-facilitated PCI in hemodialysis patients should be identified. METHODS: We retrospectively analyzed a consecutive cohort of patients from the Sapporo Cardiovascular Clinic database, who were on maintenance hemodialysis with severe calcified lesions and treated with ROTA-facilitated PCI. Comparison of atherectomy, drug-eluting balloon, and combined treatment efficiency by near infrared spectroscopy.
