Right Atrial Fibroelastoma Presenting as Typical Atrial Flutter: Rare Disease in Unusual Location.

Typical atrial flutter as initial presentation of papillary fibroelastoma involving the cavotricuspid isthmus is not described before in literature. To our knowledge only 14 cases have been reported in literature involving the right atrium. Very unusual location is at the junction between inferior vena cava (IVC) and right atria as only 1 case has been reported.

Right atrial substrate properties associated with age in patients with typical atrial flutter.

BACKGROUND: Data detailing the age-related difference in the atrial substrate for formation of typical atrial flutter (AFL) are sparse. OBJECTIVE: The purpose of this study was to characterize the difference in the right atrial substrate related to aging using noncontact mapping of the right atrium. METHODS: A total of 54 patients (23 young [<60 years; 45 +/- 12 years] and 31 old [>or=60 years; 74 +/- 6 years]) with typical AFL who underwent three-dimensional noncontact mapping of typical AFL were enrolled in the study. The atrial substrate was characterized according to (1) regional wavefro

Atrial flutter after surgical radiofrequency ablation of the left atrium for atrial fibrillation.

BACKGROUND: Left atrial radiofrequency ablation is the most common technique for the treatment of atrial fibrillation during mitral valve surgery. Reported failure rates range between 15% and 30%, with some patients remaining in atrial fibrillation and others experiencing atrial flutter. The incidence and nature of the postoperative atrial flutter is not yet well defined. METHODS: The study group consisted of 50 patients with atrial fibrillation who underwent mitral valve surgery combined with left atrial radiofrequency ablation, and were followed for a mean period of 15 +/- 7 months. The maj

Catheter ablation of atrial flutter due to amiodarone therapy for paroxysmal atrial fibrillation.

AIMS: Antiarrhythmic drug treatment for atrial fibrillation can cause atrial flutter-like arrhythmias. The aim of this study was to clarify the effect of catheter ablation of the tricuspid annulus-vena cava inferior isthmus on amiodarone-induced atrial flutter and to determine the incidence of atrial fibrillation after catheter ablation of amiodarone-induced atrial flutter in comparison to regular typical flutter. METHODS AND RESULTS: Among 92 consecutive patients with typical atrial flutter who underwent isthmus ablation 28 patients had atrial flutter without a history of previous atrial fib

Ablation for Complex Atrial Flutter After Surgical Valve Replacement.

BACKGROUND: After surgical valve replacement, the atria develop extensive scarring and low-voltage zones, leading to significant abnormal substrate. This causes abnormal conduction of atrial activation and predisposes to complex atrial tachycardia. CASE SUMMARY: We report a case of complex atrial flutter in a patient with a history of surgical mitral valve replacement and previous catheter ablation for atrial fibrillation. Using high-density mapping, the re-entrant circuit was precisely located, and the tachycardia was successfully terminated by ablation. DISCUSSION: Complex atrial flutter i

Feasibility of reversible electroporation mapping in human atrial flutter.

BACKGROUND: Reversible pulsed field ablation (PFREV) can temporarily block conduction and may emerge as a novel clinical mapping tool to accurately identify critical isthmuses before permanent lesions are created. OBJECTIVE: We aimed to assess the feasibility and performance of PFREV as a mapping tool in humans. METHODS: PFREV pulses were delivered with a 9-mm lattice-tip ablation catheter in 24 atrial flutters, targeting sites in and outside the circuit. Only nonpropagated PFREV pulses without change in activation sequence were analyzed. RESULTS: In 21 patients, 100 nonpropagated PFREV pul

Atypical Atrial Flutter: Electrophysiological Characterization and Effective Catheter Ablation.

Atrial flutter (AFL), defined as macro-re-entrant atrial tachycardia, is associated with debilitating symptoms, stroke, heart failure, and increased mortality. AFL is classified into typical, or cavotricuspid isthmus (CTI)-dependent, and atypical, or non-CTI-dependent. Atypical AFL is a heterogenous group of re-entrant atrial tachycardias that most commonly occur in patients with prior heart surgery or catheter ablation. The ECG pattern is poorly predictive of circuit anatomy but may still provide mechanistic insight. AFL is difficult to manage medically and catheter ablation is the preferred

Atypical Cases of Typical Atrial Flutter? A Case Study.

Ablation of typical atrial flutter has a high safety and efficacy profile, but hidden pitfalls may be encountered. In some cases, a longer cycle length with isoelectric lines is associated with a different or more complex arrhythmogenic substrate, which may be missed if conduction block of the cavotricuspid isthmus is performed in the absence of the clinical arrhythmia. Prior surgery may have consistently modified the atrial substrate and complex or multiple arrhythmias associated with an isthmus-dependent circuit can be encountered. In these cases, electroanatomic mapping is useful to guide t

Atrial Flutter With Exercise-Induced 1:1 Atrioventricular Conduction.

Typical atrial flutter is a common atrial tachyarrhythmia. Symptoms are typically related to the rate of ventricular response. Although atrial flutter with 1:1 atrioventricular conduction is rare, it is important to recognize because it may precipitate rapid hemodynamic compromise. The authors present a case of exercise-induced 1:1 atrioventricular conduction in a patient with typical atrial flutter.

Common atrial flutter catheter ablation without discontinuing oral anticoagulation.

Aim: To determine if performing catheter ablation under oral anticoagulation is associated with a higher risk of thromboembolic or bleeding complications. Methods: Patients with common atrial flutter that underwent catheter ablation of the cavo-tricuspid isthmus were consecutively included in the study. All patients were taking oral anticoagulants at least 3 weeks before the ablation. Results: A total of 102 patients (mean age 67.9 ± 10.3 years; 83.3% male) were included. Among them 80.6% patients were taking acenocoumarol, 10.7% dabigatran, 7.8% rivaroxaban and 0.9% apixaban. After the proced

[Surface ECG characteristics of right and left atrial flutter].

INTRODUCTION: Atrial tachycardia in virtually all areas of both atria has become more important in the clinical management of patients with previous complex atrial fibrillation ablation. Accurate interpretation of surface electrocardiogram (ECG) characteristics is of paramount importance to localize the origin of atrial tachycardia, particularly for planning interventional treatment. This article highlights the ECG features of different types of right and left atrial tachycardia. DEFINITION: Typical right atrial flutter through the cavotricuspid isthmus conducts septally in a cranial directio

Atrial flutter: more than just one of a kind.

Since its first description about one century ago, our understanding of atrial flutter (AFL) circuits has considerably evolved. One AFL circuit can have variable electrocardiographic (ECG) manifestations depending on the presence of pre-existing atrial lesions, or impaired atrial substrate. Conversely, different (right sided or even left sided) atrial circuits including different mechanisms (macroreentrant, microreentrant, or focal) can present with a very similar surface ECG manifestation. The development of efficient high-resolution electroanatomical mapping systems has improved our knowledg

Multiple external electrical cardioversions for refractory neonatal atrial flutter.

This report describes a case of atrial flutter in a fetal/neonatal patient who did not respond to adenosine, a propafenone bolus, and three electrical external cardioversions. A fourth direct current cardioversion after propafenone premedication resolved the atrial flutter. The arrhythmia did not recur with propafenone therapy during a 12-month follow-up period.

Elevated impedance during cardioversion in neonates with atrial flutter.

Direct-current cardioversion is a common treatment modality for acute termination of atrial flutter in neonates. Studies in children have demonstrated that cardioversion is often successful with as little as 0.25-0.5 J/kg with the current biphasic devices. We hypothesize that during cardioversion of atrial flutter in neonates, however, the impedance may be high and more energy may be required for successful cardioversion. A retrospective chart review of our institutional experience from 2005 through 2008 was performed. Neonates with atrial flutter requiring cardioversion who had strips availab

A atrial flutter consisted of two different conduction pathways.

The patient is a 64-year-old male who had been implanted with a DDD-pacemaker for sick sinus syndrome in 2003. In January 2006, he experienced palpitations and visited a local hospital on the second day of occurrence. After an electrocardiography (ECG) revealed atrial flutter (AFL) during palpitation, which was resistant to administered verapamil, the patient was admitted to our hospital for curative treatment. Electrophysiological study revealed a common AFL (AFL1) initiated by atrial burst pacing. We performed isthmus radiofrequency (RF) ablation against AFL1 successfully, after which anothe

DP+1: another simple endpoint for atrial flutter ablation.

BACKGROUND: When double potentials (DP) on the line of block are difficult to see, we propose another simple method to verify complete bidirectional block (CBDB) at the end of an atrial flutter ablation. We measured the interval between the electrograms immediately on either side of the line of block on a multipole catheter spanning the isthmus. We called this interval "DP+1" because it is one pair of electrodes away from the DP on the line of block. METHODS: Fifty consecutive patients (age 62 +/- 13 years, LVEF 54 +/- 11%, mean cycle length 241 +/- 34 ms) underwent an atrial flutter ablation

Atrial flutter following placement of an umbilical venous catheter.

Atrial flutter is a rare complication of the insertion of an umbilical venous catheter. We report a neonate who had an umbilical venous catheter placed within the first 24 hours of life and developed a hemodynamically stable tachyarrhythmia. The arrhythmia was successfully managed with synchronized cardioversion, with no further complications. The presentation and management are discussed. Physicians should be watchful for such potentially life-threatening complications from the placement of indwelling lines.
