Atrial flutter--update on the mechanism and treatment.

Atrial flutter is a common and usually benign but symptomatic supraventricular tachycardia. There is a striking similarity between patients with atrial flutter suggesting a common substrate despite the presence or absence of underlying heart disease. In man, the mechanism is a single reentrant circuit originating in the right atrium whose center appears to be functional within the anatomical constraints of the right atrium. The reentrant circuit of atrial flutter contains an area of slow conduction in the inferior right atrium but the size and exact location is uncertain. Drug therapy directed

Right Atrial Thrombus and Challenges in its Management.

Right atrial thrombi is a serious complication of central venous cannulation although incidence is less as compared to left atrial thrombi.1 Right atrial thrombi develops at anastomotic foci or on injured endothelium, implanted devices or foreign bodies including tumours, pacemakers and indwelling right atrial catheters. They have been associated with triple-lumen catheters for chemotherapy, intravenous fluids or parenteral nutrition, pulmonary artery catheters, hemodialysis and implantable venous access devices.2 Right atrial thrombi can have severe consequences leading to pulmonary embolism,

Effect of patient age at surgical intervention on long-term right ventricular performance in atrial septal defect.

Controversy exists about the influence of patient age on the benefit of surgery in atrial septal defect (ASD). Tissue Doppler echocardiography (TDE) when applied to atrioventricular annuli provides variables reliably reflecting the performance of the corresponding ventricle. We sought to investigate the effect of timing of surgery on biventricular functions by comparing the conventional echocardiography variables and TDE profiles of right and left atrioventricular annuli in patients treated at various ages. Conventional echocardiography and TDE analysis of mitral and tricuspid annuli were perf

Pronounced effect of procainamide on clockwise right atrial isthmus conduction compared with counterclockwise conduction: possible mechanism of the greater incidence of common atrial flutter during antiarrhythmic therapy.

INTRODUCTION: It has been shown that the induction rates of common and reversed common atrial flutter are comparable during baseline control study, whereas the rate is significantly greater for common flutter than reversed common flutter during administration of antiarrhythmic agents. The mechanism of this discrepancy is not known. METHODS AND RESULTS: The study consisted of 15 patients (group 1) with clinically documented common atrial flutter either with (n = 10) or without (n = 5) Class I antiarrhythmic therapy, and 15 patients with paroxysmal supraventricular tachycardia (group 2). Surgery for atrial fibrillation.

OBJECTIVE: The mechanisms of atrial fibrillation arc multiple reentry circuits spinning around the atrial surface, and these baffle any attempt to direct surgical interruption. The purpose of this article is to report the surgical experience in the treatment of isolated and concomitant atrial fibrillation at the Cardiac Surgical Institute of the University of Pavia. METHODS: In cases of atrial fibrillation secondary to mitral/valve disease, surgical isolation of the left atrium at the time of mitral valve surgery can prevent atrial fibrillation from involving the right atrium, which can exert

Right ventricular function in congenital heart defects assessed by regional wall motion.

OBJECTIVES: To develop a simple method to assess right ventricular function by angiography. BACKGROUND: Conventional methods of evaluating right ventricular function are inaccurate, cumbersome, and expensive. METHODS: We analyzed biplane right ventricular angiograms taken in the posterior-anterior and lateral projections using software to measure right ventricular volumes and regional wall motion in 78 patients with normal hearts (n = 29), atrial septal defects (ASD n = 13), pulmonary valve stenosis (PVS n = 21), and postoperative atrial switch patients (n = 15). We also measured the shorten

[Sick sinus syndrome: comparative study of atrial and ventricular stimulation tests].

Subjects with a ventriculo-atrial block and normal sinus function exhibit acceleration of the sinus rhythm during incremental right ventricular pacing; this acceleration is nor observed or is hardly visible in patients with sinus node dysfunction. Incremental right ventricular pacing up to 150/min was performed in 16 patients with ventriculo-atrial block (Group A) referred for investigation of sinus node dysfunction and the results compared with the classical tests of atrial pacing (Strauss'graph, atrio-sino-atrial conduction time, sinus node recovery time). The same tests were carried out in

Rapid Left Ventricular Recovery After Correction of a Secundum Atrial Septal Defect: Understanding the Hemodynamics.

Closure of an atrial septal defect (ASD) may lead to a change in the function of both ventricles. Although right ventricular function typically improves, the left ventricle (LV) may behave in different ways. This has been a matter of much debate, with some authors reporting a decline in LV function after ASD closure and others reporting delayed improvement or no identifiable change. We report the case of a 41-year-old female with a large left-to-right ASD shunt (Qp:Qs 2.3:1 and shunt volume 3.6 L/min) who presented with biventricular systolic dysfunction that improved within 24 hours of ASD cl

Noninvasive estimation of right atrial pressure using cardiac magnetic resonance in severe tricuspid regurgitation.

BACKGROUND: While right atrial pressure (RAP) reflects disease severity and carries prognostic value in patients with severe tricuspid regurgitation (TR), its noninvasive estimation remains limited in accuracy. The aim of this study was to evaluate the ability of cardiovascular magnetic resonance imaging (CMR) to estimate RAP by identifying relevant imaging parameters in patients with severe TR. METHODS: Between September 2021 and January 2026, 47 consecutive patients who underwent cardiac catheterization and CMR for severe TR were enrolled from a tertiary center. In addition to conventional

Declining cardiopulmonary exercise capacity is not associated with worsening systolic systemic ventricular dysfunction in adults with transposition of great arteries after atrial switch operation.

BACKGROUND/OBJECTIVE: Cardiopulmonary exercise capacity is often reduced in patients with transposition of the great arteries after atrial switch operation. Reduced exercise capacity may be caused by deterioration of systemic right ventricular function over time. This study analyzed serial changes in systemic right ventricular function and cardiopulmonary exercise capacity in young adults with transposition of the great arteries after atrial redirection surgery. METHODS: Twenty-one patients (37% female, mean age 23.2 ± 3.3, mean age at surgery 12.8 ± 14 years) with transposition of the great

Flail tricuspid leaflet after multiple biopsies following orthotopic heart transplantation: echocardiographic and hemodynamic correlation.

Flail tricuspid leaflet has been reported as a relatively uncommon complication of endomyocardial biopsy in orthotopic heart transplant recipients. However, the relationship of this complication to the number of biopsies performed and to the site of access for biopsy is not known. The objectives of this study were to assess the prevalence of flail tricuspid leaflet/torn chordae tendineae in our recent transplant population, define the relationship of this complication to endomyocardial biopsy, and to correlate echocardiographic assessments of tricuspid regurgitation severity with hemodynamic d

Global cardiac alterations detected by speckle-tracking echocardiography in Fabry disease: left ventricular, right ventricular, and left atrial dysfunction are common and linked to worse symptomatic status.

The aim of this study was to test the hypothesis that in patients with Fabry disease, 2D speckle-tracking echocardiography (2DSTE) could detect functional myocardial alterations such as left ventricular (LV), right ventricular (RV), and left atrial (LA) dysfunction, even when conventional cardiac measurements are normal. In addition, we hypothesized that these global cardiac alterations could be linked to worse symptomatic status in these patients. Fifty patients with Fabry disease and a control group of 118 healthy subjects of similar age and gender were included. The myocardial function and

[Study on right and left atrial dynamics in chronic constrictive pericarditis by esophageal echocardiography].

In 9 normal subjects (N) and 4 patients with chronic constrictive pericarditis (CP) with normal sinus rhythm, esophageal echocardiography was performed to obtain simultaneous echograms of the right atrial free wall and the interatrial septum, and both atrial dynamics were evaluated. In spite of increased mean right atrial and mean pulmonary arterial wedge pressures in CP, there were no significant differences in left, right and total atrial end-diastolic dimensions between N and CP. This indicates that there is a decrease in compliance of atrio-pericardial system in CP. Atrial shortening of bo

Longitudinal Evaluation of Atrial Function in Patients with Tetralogy of Fallot.

Atrial function provides insight into ventricular diastolic function. Invasive assessment of left ventricular (LV) diastolic function correlates with development of sustained ventricular tachyarrhythmias in patients with repaired tetralogy of Fallot (rTOF). Non-invasive assessment of atrial function may prove key towards assessment of diastolic function. We longitudinally evaluated the progression of biatrial function in patients with rTOF, regardless of pulmonary valve replacement (PVR). Patients with rTOF who had multiple CMR were identified. CMR examinations were retrospectively reviewed. L

Left ventricular geometry and function in adults with Ebstein's anomaly of the tricuspid valve.

We postulated that the abnormal shape, size, and function of the right heart and adjoining ventricular septum in adults with Ebstein's anomaly of the tricuspid valve might in turn alter the shape and function of the left ventricle. Seven adult patients with uncomplicated Ebstein's anomaly were studied. Left ventricular geometry was determined by two-dimensional echocardiography. Left ventricular function was assessed by treadmill exercise and radionuclide angiography at rest and with exercise. Paradoxic ventricular septal motion was consistently present. Left ventricular eccentricity (ratio of

"Pulmonary valve replacement diminishes the presence of restrictive physiology and reduces atrial volumes": a prospective study in Tetralogy of Fallot patients.

Pulmonary valve replacement (PVR) reduces right ventricular (RV) volumes in the setting of long-term pulmonary regurgitation after Tetralogy of Fallot (ToF) repair; however, little is known of its effect on RV diastolic function. Right atrial volumes may reflect the burden of RV diastolic dysfunction. The objective of this paper is to evaluate the clinical, echocardiographic, biochemical and cardiac magnetic resonance (CMR) variables, focusing particularly on right atrial response and right ventricular diastolic function prior to and after elective PVR in adult patients with ToF. This prospect

Impact of Severe Pulmonary Arterial Hypertension on the Left Heart and Prognostic Implications.

BACKGROUND: Severe pulmonary arterial hypertension (sPAH) results in a dilated and dysfunctional right ventricle (RV) together with a small left ventricle (LV) with preserved systolic function. RV size and function parameters have an established association with poor prognosis in sPAH. We sought to determine the impact of RV geometry and function on LV mechanics and its relationship with mortality. METHODS: We studied 114 patients (54 ± 13 years) with sPAH, normal LV ejection fraction (LVEF), and complete two-dimensional transthoracic echocardiograms (TTE) and compared them with 70 normal con

Temporary epicardial cardiac resynchronisation versus conventional right ventricular pacing after cardiac surgery: study protocol for a randomised control trial.

BACKGROUND: Heart failure patients with stable angina, acute coronary syndromes and valvular heart disease may benefit from revascularisation and/or valve surgery. However, the mortality rate is increased- 5-30%. Biventricular pacing using temporary epicardial wires after surgery is a potential mechanism to improve cardiac function and clinical endpoints. METHOD/DESIGN: A multi-centred, prospective, randomised, single-blinded, intervention-control trial of temporary biventricular pacing versus standard pacing. Contractile properties of the right atrial myofilaments in patients with myxomatous mitral valve degeneration.

BACKGROUND: Myxomatous degeneration of the mitral valve is a common pathological finding in mitral valve surgery and the most common reason for severe mitral valve regurgitation. Considering the importance of right ventricular remodeling and global function after mitral valve surgery we tried to elucidate a possible association of myxomatous mitral valve and impairment of right atrial and ventricular function, which might have an impact on global ventricular performance after mitral valve surgery. METHODS: Right atrial tissue was harvested from 47 patients undergoing mitral valve surgery. We
