Craniofacial malformations and their syndromes. An overview for the speech and hearing practitioner.

Congenital malformations of the craniofacial region represent an important class of human developmental disorders. Abnormalities of speech and hearing frequently occur in this vast array of conditions. Knowledge of the diagnostic criteria, genetics, and natural history of these conditions is important for audiologists and speech-language pathologists because of their close involvement with the children and families in their care-providing role. Awareness of the important distinction between individuals with isolated defects vs. individuals who have their facial defect as part of a syndrome is

Validity of the Multiple Auditory Processing Assessment-2: A Test of Auditory Processing Disorder.

Purpose A normative study using the Multiple Auditory Processing Assessment-2 (MAPA-2; Schow et al., 2018) was recently completed. With access to these data, the authors extend that work and support a definite construct for auditory processing disorder (APD). The goal here is to examine MAPA-2 reliability and validity (construct, content, and concurrent). Evidence for the APD construct is further buttressed by measures of sensitivity and specificity. Results of MAPA-2 testing on children diagnosed with learning disability (LD), attention-deficit/hyperactivity disorder (ADHD), and specific lang

An amplitude-bandwidth expansion method for hearing-aid adjustment.

The basic concept for a technique that facilitates the adjustment of a hearing aid by a person of normal hearing is proposed. The technique involves processing using an amplitude-bandwidth expansion method to expand the hearing-aid output to fit to the user's hearing requirements and loudness recruitment. The expansion is a reversal of the cochlear compression model. As the expansion method introduces distortion of the waveform and a reduction of the expansion rate, the support technique presented here includes solutions to these problems. The difference between the original speech signal and

A Possible Incomplete Form of Treacher Collins Syndrome: A Case Report.

Treacher Collins syndrome (TCS) is a rare genetic disorder that affects craniofacial development due to malformation of the first and second branchial arches. The TCOF1 gene is mainly responsible for this condition. Here, we present a case of a 13-year-old adolescent girl with complaints of maligned teeth with conductive deafness. On clinical examination, she had retrognathia, a broad nose, maligned teeth, a high arch palate, and midfacial hypoplasia. On the basis of the clinical findings, a diagnosis of a mild-variant TCS was made as eyes were not involved and supportive treatment was given t

Clinical consensus document for fitting non-surgical transcutaneous bone conduction hearing devices to children.

This clinical consensus document addresses the assessment, selection, and fitting considerations for non-surgical bone conduction hearing devices (BCHD) for children under the age of 5 years identified as having unilateral or bilateral, permanent conductive or mixed hearing losses. Children with profound unilateral sensorineural hearing losses are not addressed. The document was developed based on evidence review and consensus by The Paediatric Bone Conduction Working Group, which is composed of audiologists from North America who have experience working with BCHDs in children. The document ai

Acoustic-phonetic approach toward understanding neural processes and speech perception.

This review paper describes an "acoustic-phonetic" experimental approach aimed at understanding normal and abnormal speech perception processes from both a behavioral and an electrophysiologic perspective. First, we consider the relevant acoustic characteristics of speech and identify a set of acoustic-phonetic classes that represent the parameters most important for making an acoustic signal sound like speech. Second, we review what is known about the neurophysiologic representation of acoustic-phonetic speech parameters in animal and human subjects. Third, we describe how an acoustic-phoneti

Electrocochleography: methods and clinical applications.

The use of electrocochleography (ECoG) as a tool in the diagnosis, assessment, and monitoring of certain neuro-otologic disorders has attracted renewed attention in the United States. The use of ECoG among otologists and audiologists has increased in recent years, primarily due to the availability of relatively easily applied, noninvasive extratympanic electrodes. Information derived from ECoG recordings has been shown to be helpful in the objective identification and monitoring of Meniere's disease and endolymphatic hydrops. In addition, ECoG may be used to enhance our ability to observe the

A critical evaluation of the glycerol test in Ménière's disease.

The glycerol test is currently widely used to evaluate Menière's disease, particularly in regard to its suitability for decompressive surgical therapy. Our previous findings have shown that patients with Menière's syndrome respond extremely well to placebo treatment. We have investigated the effect of psychological factors in the outcome of the glycerol test. It was administered twice to each of a group of patients with a typical Menière's syndrome. On one occasion the patient was told the hearing would improve, and on the other that the hearing would worsen. The same dose of glycerol was prov

Future directions for screening and treatment in congenital hearing loss.

Hearing loss is the most common neurosensory deficit. It results from a variety of heritable and acquired causes and is linked to multiple deleterious effects on a child's development that can be ameliorated by prompt identification and individualized therapies. Diagnosing hearing loss in newborns is challenging, especially in mild or progressive cases, and its management requires a multidisciplinary team of healthcare providers comprising audiologists, pediatricians, otolaryngologists, and genetic counselors. While physiologic newborn hearing screening has resulted in earlier diagnosis of hea

[Language development disorders--studies of preschool children of the Leipzig special language school].

Children with severe speech disorders were examined to determine the causes of their abnormal speech development. Thirty-seven preschool and school-age children were followed up and results compared. All children had low intelligence quotients, with most of the children coming from low social backgrounds. However, both of these factors (intelligence and low social background) had no influence on the improvement of speech levels after therapy. Two-thirds of the children with moderate conductive hearing losses showed improvement after therapy, so that the effect of this hearing loss was believed

Audiological and medical considerations for children with cochlear implants.

Recent advancements in technology have resulted in the development of implantable devices--cochlear implants--designed to maximize the sensation of hearing in the hearing-impaired population. For children who meet the numerous preselection criteria, the cochlear implant can be effective and beneficial. The implants must be inserted surgically, which involves placing an electrode array in the cochlea and attaching the receiver/stimulator to the skull. The child can usually be discharged from the hospital by the third postoperative day. The external device is fitted and initially stimulated abou

Predicting hearing sensitivity and audiometric slope for mentally retarded persons.

The sensitivity prediction from the acoustic reflex (SPAR) test was administered to 90 subjects who were classified according to four levels of mental retardation and five levels of hearing sensitivity. SPAR results showed accurate hearing level prediction for 58% of the subjects, one-category errors for 29%, and two- or three-category errors for 13%. Audiometric slope predictions were also made and were accurate 58% of the subjects. SPAR results indicated that the greater the hearing loss, the less likely that normal hearing would be predicted. Hearing level predictions for the retarded popul

Down syndrome: a multidisciplinary perspective.

Trisomy 21, a genetic disorder resulting from a chromosomal abnormality, is one of the most common forms of mental disability in the United States. Individuals with Down syndrome frequently present with a constellation of medical problems including conductive hearing loss and, to a lesser degree, sensorineural hearing loss. As part of a health care team, audiologists must be sensitive to and aware of medical conditions prior to establishing intervention strategies. Medical conditions, by necessity, precede audiologic interventions and, therefore, a close working relationship among team members

Tablet and web-based audiometry to screen for hearing loss in adults with cystic fibrosis.

INTRODUCTION: Individuals with chronic lung disease (eg, cystic fibrosis (CF)) often receive antimicrobial therapy including aminoglycosides resulting in ototoxicity. Extended high-frequency audiometry has increased sensitivity for ototoxicity detection, but diagnostic audiometry in a sound-booth is costly, time-consuming and requires a trained audiologist. This cross-sectional study analysed tablet-based audiometry (Shoebox MD) performed by non-audiologists in an outpatient setting, alongside home web-based audiometry (3D Tune-In) to screen for hearing loss in adults with CF. METHODS: Hearin

Hearing Aid Silicone Impression Material Discovered During Exploration of the Middle Ear: 2 Case Reports and Literature Review.

It is unusual to see complications with the preparation of ear mold in order to get hearing aids for children who are in need. However, we came across 2 cases who had a foreign body retained in the middle ear after a long period of time from taking silicon mold impression for hearing aid fitting. One patient presented after 2 years, and the other patient presented after 10 years of hearing aid fitting. We are reporting 2 cases with silicon impression material left in the middle ear for a long period of time after taking an impression for hearing aid fitting and found unexpectedly during explor

Surgical treatment of long-term sensorineural hearing loss due to labyrinthine fistula.

Perilymphatic fistula, usually in the round or oval window, causes a variety of symptoms, including sensorineural hearing loss. Surgical repair of these fistulas has resulted in restoration of hearing in some cases. It has been suggested that surgery must be performed within 2 months of the trauma if improvement in hearing is to occur. This paper presents the case of a patient whose fistula and resulting hearing loss had persisted ten years before surgical repair. Restoration of normal hearing and discrimination of speech occurred in the ear which had previously presented a severe sensorineura

Self-Adjusted Amplification Parameters Produce Large Between-Subject Variability and Preserve Speech Intelligibility.

The current study used the self-fitting algorithm to allow listeners to self-adjust hearing-aid gain or compression parameters to select gain for speech understanding in a variety of quiet and noise conditions. Thirty listeners with mild to moderate sensorineural hearing loss adjusted gain parameters in quiet and in several types of noise. Outcomes from self-adjusted gain and audiologist-fit gain indicated consistent within-subject performance but a great deal of between-subject variability. Gain selection did not strongly affect intelligibility within the range of signal-to-noise ratios teste
