Obstructive azoospermia associated with chronic sinopulmonary infection and situs inversus totalis.

We describe 2 cases of obstructive azoospermia associated with situs inversus and sinopulmonary infection due to ciliary defects. Electron microscopy of testicular sperm flagella demonstrated normal morphology with nine peripheral doublets surrounding a central pair and complete sets of inner and outer dynein arms. Electron microscopy of the nasal mucosa revealed partial defects of the dynein arms of cilia, although the "9+2" morphology was preserved. Our cases were considered unique variants of Young's syndrome but also had characteristic features of Kartagener syndrome, and thus support the

Identification of biomarkers for non-obstructive azoospermia based on microRNA and bioinformatics screening.

MicroRNAs (miRNAs) play an important role in the occurrence of non-obstructive azoospermia (NOA). Nevertheless, there is still a lack of research on the molecular mechanisms by which miRNAs regulate target genes to mediate NOA at present. In this study, we obtained NOA-related miRNA datasets from the GEO database and applied differential expression matrices combined with weighted correlation network analysis (WGCNA) and LASSO regression to identify four key miRNAs. Based on the miRDB database, the target genes of these miRNAs were predicted and intersected with the differentially expressed gen

Artificial Intelligence in Non-Obstructive Azoospermia: State of the Art in Sperm Retrieval.

Non-obstructive azoospermia (NOA) is the most severe form of male infertility, characterized by defective spermatogenesis and a complete absence of sperm in the ejaculate. Biological fatherhood for affected patients relies on identifying rare foci of active spermatogenesis within the testes through microdissection testicular sperm extraction (micro-TESE) which is the current gold standard approach. Optimizing sperm retrieval is challenging since success depends on both surgical precision and laboratory detection accuracy. Artificial intelligence (AI) has recently emerged as a promising tool to

A novel hemizygous missense variant in the BEND2 gene is associated with nonobstructive azoospermia.

Nonobstructive azoospermia (NOA), the most severe form of male infertility, frequently arises from genetic defects that disrupt spermatogenesis. In this study, a novel hemizygous missense variant (NM_001184767.2 [c.G1069A; p.V357I]) is identified in the X-linked BEN domain-containing 2 ( BEND2 ) gene of a patient with NOA characterized by spermatocyte maturation arrest. Whole-exome sequencing and Sanger validation confirmed that this rare variant is absent in fertile controls and that no pathogenic variants were detected in established NOA genes. Computational analysis predicted potential stru

Obstructive Azoospermia Revealing CFTR-Related Disorder in an Adult With Recurrent Pancreatitis.

Obstructive azoospermia represents a critical diagnostic category in male infertility, with congenital bilateral absence of the vas deferens being a well-established etiology associated with Cystic Fibrosis Transmembrane Conductance Regulator (CFTR) gene variants. We report a 38-year-old man presenting with primary infertility and a history of recurrent pancreatitis. Evaluation revealed azoospermia with low ejaculate volume, normal hormonal profile, and bilateral absence of the vas deferens on physical examination. Genetic testing identified compound heterozygosity for a pathogenic CFTR varian

Genetic insights into non-obstructive azoospermia: Implications for diagnosis and TESE outcomes.

BACKGROUND: Non-obstructive azoospermia (NOA) is considered one of the most severe forms of male infertility. Despite the limited range of testicular phenotypes, NOA exhibits considerable genetic heterogeneity. The aim of this study was to uncover the etiopathogenesis of NOA and provide insights into the outcomes of testicular sperm extraction (TESE). MATERIAL METHOD: To elucidate the potential causes of testicular pathogenesis, a cohort of 61 patients was analyzed. The genetic etiology was assessed using our developed gene panel, based on genes with prior functional studies conducted specifi

The Use of Testicular Sperm Extraction in Male Infertility Patients Without Azoospermia.

Testicular sperm extraction (TESE) in its various versions has been traditionally used in male infertility patients with azoospermia, with the intention of utilizing surgically retrieved sperm for in vitro fertilization (IVF) with or without intra-cytoplasmic sperm injection (ICSI). Possible risks of TESE include bleeding, infection, and inability to find sperm. Microdissection TESE, a more extensive type of surgical sperm retrieval procedure, may be associated with a transient post-operative decline in serum testosterone levels. More recently, the possibility of using TESE in non-azoospermic

Loss of Adgra3 causes obstructive azoospermia with high penetrance in male mice.

The adhesion receptor ADGRA3 (GPR125) is a known spermatogonial stem cell marker, but its impact on male reproduction and fertility has not been examined. Using a mouse model lacking Adgra3 (Adgra3-/- ), we show that 55% of the male mice are infertile from puberty despite having normal spermatogenesis and epididymal sperm count. Instead, male mice lacking Adgra3 exhibited decreased estrogen receptor alpha expression and transient dilation of the epididymis. Combined with an increased estradiol production, this indicates a post-pubertal hormonal imbalance and fluid retention. Dye injection reve

Loss-of-function variants in KCTD19 cause non-obstructive azoospermia in humans.

Azoospermia is a significant cause of male infertility, with non-obstructive azoospermia (NOA) being the most severe type of spermatogenic failure. NOA is mostly caused by congenital factors, but our understanding of its genetic causes is very limited. Here, we identified a frameshift variant (c.201_202insAC, p.Tyr68Thrfs∗17) and two nonsense variants (c.1897C>T, p.Gln633∗; c.2005C>T, p.Gln669∗) in KCTD19 (potassium channel tetramerization domain containing 19) from two unrelated infertile Chinese men and a consanguineous Pakistani family with three infertile brothers. ESX1 gene as a potential candidate responsible for male infertility in nonobstructive azoospermia.

Infertility is a problem that affects approximately 15% of couples, and male infertility is responsible for 40-50% of these cases. The cause of male infertility is still poorly diagnosed and treated. One of the prominent causes of male infertility is disturbed spermatogenesis, which can lead to nonobstructive azoospermia (NOA). Whole-genome sequencing (WGS) allows us to identify novel rare variants in potentially NOA-associated genes, among others, in the ESX1 gene. The aim of this study was to activate the ESX1 gene using CRISPRa technology in human germ cells (testicular seminoma cells-TCam-

Hereditary spastic paraplegia (SPG 48) with deafness and azoospermia: A case report.

Hereditary spastic paraplegias (HSP) are inherited neurodegenerative disorders characterized by progressive paraplegia and spasticity in the lower limbs. SPG48 represents a rare genotype characterized by mutations in AP5Z1, a gene playing a role in intracellular membrane trafficking. This study describes a case of a 53-year-old male patient with SPG48 presenting spastic paraplegia, infertility, hearing impairment, cognitive abnormalities and peripheral neuropathy. The Sanger sequencing revealed a homozygous deletion in the chr 7:4785904-4786677 region causing a premature stop codon in exon 10.

Novel exon mutation in SYCE1 gene is associated with non-obstructive azoospermia.

Non-obstructive azoospermia (NOA) is a common cause of male infertility, and genetic problems, such as chromosomal abnormalities and gene mutations, are important causes of NOA. Our centre received a case of NOA, in which no mature sperm was found during microdissection testicular sperm extraction. A postoperative pathological examination revealed that testicular spermatogenesis was blocked. Target region capture combined with high-throughput sequencing was used to screen for male infertility-related gene mutations. Sanger sequencing further confirmed that the SYCE1 gene, a central component o

Genetic Factors of Non-Obstructive Azoospermia: Consequences on Patients' and Offspring Health.

Non-Obstructive Azoospermia (NOA) affects about 1% of men in the general population and is characterized by clinical heterogeneity implying the involvement of several different acquired and genetic factors. NOA men are at higher risk to be carriers of known genetic anomalies such as karyotype abnormalities and Y-chromosome microdeletions in respect to oligo-normozoospermic men. In recent years, a growing number of novel monogenic causes have been identified through Whole Exome Sequencing (WES). Genetic testing is useful for diagnostic and pre-TESE prognostic purposes as well as for its potenti
