The Canadian veterinary journal = La revue veterinaire canadienne2026eng
An 18-month-old dog was presented to the emergency department with a marked discrepancy in lactate measurements obtained on different analyzers (lactate gap), an elevated anion gap, and a renal azotemia, which raised concern for ethylene glycol intoxication. The dog was treated with intermittent hemodialysis until the lactate gap was normal. Key clinical message: It is hypothesized that the lactate gap can be used by veterinary clinicians as both a diagnostic tool and a therapeutic tool to determine when intermittent hemodialysis treatment can be stopped.

McKittrick-Wheelock syndrome is characterized by extracellular volume depletion and severe fluid and electrolyte imbalance caused by secretory tumors of the rectum, usually villous adenomas. Patients usually present signs of dehydration, hyponatremia, hypokalemia, and prerenal azotemia due to the chronic diarrhea, rich in ions and water. Since this is a potencially lethal entity without treatment, and a very unusual condition, we report a clinical case. Treatment consists in surgical resection of the tumor, although endoscopy polypectomy has also been reported.

Veterinary radiology & ultrasound : the official journal of the American College of Veterinary Radiology and the International Veterinary Radiology Association1998eng
A 3-month-old foal was presented for correction of bilateral angular limb deformities. Azotemia was detected as an incidental finding. Small, misshapened, hyperechoic kidneys with decreased corticomedullary demarcation were noted with ultrasonography. Additionally, the internal renal architecture was abnormal in that the intrarenal vessels and distant collecting system were not clearly seen in either kidney. Ultrasound-guided renal biopsy was suggestive of congenital renal dysplasia, which was later confirmed at necropsy. Clinical, sonographic, and pathologic features of equine renal dysplasia

The acute tumour lysis syndrome in humans as well as in dogs is caused by the acute lysis of tumour cells following chemotherapy. The release of intracellular products and their metabolites result in hyperkalemia, hyperphosphatemia, and hyperuricemia. The precipitation of uric acid and calciumphosphate in the kidneys can lead to hypocalcemia and acute renal failure which leads to fatal azotemia and arrhythmia's due to electrolyte disturbances. In this article the occurrence of the acute tumour lysis syndrome in a dog with malignant lymphoma is described. Suggestions to prevent the tumour lysis

It is reported on a septennial observation of a 43 year old woman, who was admitted to the hospital in 1968 with at first unclear symptoms and who developed an azotemia within a few days. The infusion pyelogram showed a hydronephrosis with enlargement of the upper part of the ureter; the lower part of the ureter was normal. After displacement of the ureters out of the fibrotic clasping into a duplicate of the peritoneum, the urine flow has normalized. After 7 years the patient is now healthy and able to work. The cause of the urinary stasis was histologically cleared as retroperitoneal fibrosi

A six-year-old, intact male German Shepherd, weighing 43 kg, was presented with generalized edema formation and acute kidney injury due to a suspected delayed type III hypersensitivity reaction and vasculitis 2 weeks after the administration of human serum albumin (HSA). At presentation, the patient had a moderately reduced general condition, a heart rate of 96/min, rectal temperature of 37.0 °C, generalized edema, hematoma and petechiae on all limbs and the abdomen, and scleral hemorrhage. The initial blood work showed a moderate anemia, a severe azotemia, and a moderate hypoalbuminemia. Mark

Pauci-immune crescentic glomerulonephritis (GN) is a rapidly progressive form of GN typically associated with antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) and may present with rare but severe histologic findings such as medullary angiitis. We describe a 69-year-old male patient with no prior medical history who presented with worsening renal function following a recent hospitalization for what is thought to be pneumonia and presumed prerenal azotemia. On readmission, he was found to have an elevated serum creatinine of 5.42 mg/dL, significant hematuria and proteinuria

A three-year-old, intact female mix-breed dog, weighing 30 kg, was presented due to vomitus and diarrhea. At presentation, the patient had a slightly reduced general condition and moderately enlarged mandibular and popliteal lymph nodes. The initial blood work showed severe azotemia and hypoalbuminemia. In the urinalysis, marked proteinuria with a urine protein/creatinine ratio (UPC) of 4.69 was found. Further workup showed a high leishmania antibody titer. The dog was diagnosed with leishmaniosis and glomerulonephritis. Initial treatment consisted of intravenous fluid therapy, allopurinol, mi

CASE SERIES SUMMARY: A 2-year-old female spayed Abyssinian cat was evaluated for lethargy and inappetence that first occurred approximately 4 days prior. In addition, urination had not been observed by the owner for 5 days. A Lilium species plant had been brought into the house approximately 5 days before initial evaluation, and intoxication was therefore suspected. Bloodwork revealed anemia, severe azotemia and hyperkalemia. As the cat was anuric with severe azotemia, hyperkalemia and fluid overload, intermittent hemodialysis was recommended. Attempts were made to place a hemodialysis cathete

Urolithiasis is a common condition in male small ruminants where predisposing factors have been identified. Occasionally, urolithiasis is diagnosed in South American camelids (SACs). However, nephrolithiasis is rarely diagnosed in ruminants. To our knowledge, this is the first report focusing on a combined appearance of nephrolithiasis and urolithiasis in an alpaca cria. A 7-month-old alpaca cria suffering from impaired urinary flow was presented for examination. On admission, the alpaca had a wet prepuce and showed a standing posture with a wide-based stance. Ultrasonographic examination of t

A 15-year-old, 113 kg intact male Sumatran tiger (Panthera tigris sumatrae) was evaluated for weight loss, polydipsia, and intermittent hematuria. The tiger was immobilized for diagnostic testing including blood work, urinalysis, and abdominal ultrasound. Laboratory testing demonstrated macro- and microhematuria, azotemia, and an increased urine protein:creatinine ratio. Abdominal ultrasound revealed bilateral ureterolithiasis as well as hydronephrosis and ureteral dilation. Ultrasonography performed 5 months later revealed worsening of the right-sided hydronephrosis and hydroureter and a decr

The purpose of this study was to review recent cases of leptospirosis seen at referral centers in New York State and to identify differences in clinical or clinicopathologic aspects of the disease among different suspected infecting serogroups. Medical records at the Cornell University Hospital for Animals and the Animal Medical Center in New York City were reviewed to identify dogs diagnosed with leptospirosis from September 1996 to August 2002. Records of 55 dogs met the inclusion criteria for the study. The suspected infecting serogroups included 21 occurrences of Grippotyphosa, 12 of Pomon

Recombinant interleukin-2 (IL-2) infusions have recently been evaluated as a new form of immunotherapy for the treatment of malignancies. This form of therapy has been complicated by the development of fluid retention, azotemia, and hypophosphatemia. To evaluate the effects of IL-2 on renal function, we prospectively studied eight patients who received IL-2 (10(5) micron/kg every eight hours intravenously [IV]) for five days as the initial phase of a treatment protocol using IL-2 plus lymphokine activated killer (LAK) cells. Dopamine and fluids were used to maintain blood pressure and all pati

Myohemoglobinuric acute renal failure (ARF) was induced in dehydrated, salt-deficient, salt-loaded, and untreated rats by intramuscular injection of glycerol, and the renal vasculature was studied after 24 hours. Kidneys were prepared for examination by rapid freezing in vivo to -160 degrees C and freeze substitution in -80 degrees C alcohol, and by perfusion fixation with 1% glutaraldehyde in Ringer's solution at 120 mm Hg. Frozen kidneys were examined by light microscopy after paraffin and epoxy resin embedding. Techniques used in examining the perfusion-fixed kidneys were: (1) vascular inje

Serum levels, urinary excretion, and clearances of several proteins of different molecular weights were studied in 18 patients with mono- and myelomonocytic leukemia. Nine patients had normal renal function (group A) and nine had impaired renal function with azotemia (group B). The majority of patients in both groups had increased concentration of immunoglobulins, particularly IgG, IgA, and IgM; IgD level was normal. Serum transferrin and alpha(2)-macroglobulin were frequently reduced while the level of ceruloplasmin was often increased, especially in patients with azotemia. The activity of ly

INTRODUCTION: Wernicke encephalopathy - most often observed in alcoholic patients - is due to severe thiamine deficiency.
CASE: We report here the case of a 30-year-old woman who presented with hyperemesis and vomiting during the first trimester of pregnancy (hyperemesis gravidarum). Hyperemesis can lead to severe, symptomatic thiamine deficiency and to severe dehydration with prerenal azotemia.
DISCUSSION: Wernicke encephalopathy is a rare complication of hyperemesis gravidarum. It should be diagnosed as early as possible to prevent long-term complications.

Cholesterol crystal embolism can be spontaneous or iatrogenic, and it can involve any organ of the gastrointestinal tract, presenting with common gastrointestinal symptoms such as bleeding, perforation, obstruction, and inflammation. It is therefore considered the "great masquerader," requiring a high level of suspicion because the condition is associated with increased morbidity and mortality. We present a 69-year-old man who presented with gastrointestinal bleeding and azotemia. He was found to have cholesterol crystal embolization in the kidney and a duodenal leiomyoma, the latter being an

We report a case of bilateral renal vein thrombosis in a neonate, and reviewed available literature. The neonate was a macrosomic male born to a mother with glycosuria in pregnancy. There was delay in commencing breasttfeeding for up to 36 hours due to lack of lactation by themother. Clinical and laboratory examination showed enlarged palpable kidneys and azotemia. Diagnosis of bilateral renal vein thrombosis was confirmed by renal ultrasonography. The child is being managed conservatively. Measures aimed at prevention of the disease because of its poor outcome were highlighted.
