Salmonella bacteremia in renal transplant recipients.

Three cases of salmonella bacteremia in renal transplant recipients are reported. Two patients had prolonged salmonella infections with recurring bacteremia. The longest interval between bacteremic relapses was 15 years. 27 cases from the literature of salmonella bacteremia in renal transplant recipients are reviewed. The courses of salmonellosis in renal transplant recipients are more serious than in other non-compromised patients, being complicated by bacteremia in 70% of the cases. Renal transplant recipients have prolonged carrier states and frequent relapses or recurrences of salmonellosi

Occult bacteremia with nontypeable Haemophilus influenzae.

A 2-year-old boy had occult bacteremia with nontypeable Haemophilus influenzae 6 weeks after receiving H. influenzae type b polysaccharide vaccine. Evaluation of his host defense was normal. As determined by outer membrane protein electrophoresis and Southern hybridization analysis, this strain was not related to type b strains. Its virulence in rats was similar to that of another nontypeable strain and less than that of a type b strain.

Uvulitis and Hemophilus influenzae b bacteremia.

Uvulitis and Hemophilus influenzae b (HIb) bacteremia occurred in two infants. Uvulitis may be associated with HIb bacteremia, with or without epiglottitis. An acutely inflamed uvula in a febrile infant should alert the physician to the possibility of HIb bacteremia and potentially serious sequelae. Hospitalization and parenteral antibiotic therapy are recommended.

Bacteremia in Charleston County, South Carolina.

To describe the epidemiology of bacteremia in a large, well defined population, the authors reviewed medical records for residents of Charleston County, South Carolina, who had bacteria isolated from blood in the period 1974 to 1976. The incidence was 80 cases per 100,000 population per year. The most common organisms were Escherichia coli, Staphylococcus aureus, Klebsiella, and Streptococcus pneumoniae. The incidence was highest for neonates, infants, and those 70 years of age and older with annualized attack rates of 1,864,250, and 446 cases per 100,000 population, respectively. The incidenc

Group B streptococcal bacteremia in men.

Twenty-three episodes of bacteremia due to group B Streptococcus (GBS) in men were reviewed for both clinical and microbiologic features. The commonest sources of bacteremia were infections of the respiratory tract (seven cases), skin (five cases), urinary tract (four cases), and intravenous access sites (four cases). Sixteen (70%) of the cases of bacteremia were hospital acquired, and 10 (43%) were polymicrobial (seven with Staphylococcus aureus). GBS bacteremia was generally low-grade and brief as assessed by the percentage and persistence of positive blood cultures. Among patients bacteremi

Does bacteremia occur during flexible sigmoidoscopy?

Up to 10% of patients may have bacteremia after rigid sigmoidoscopy. The aim of our study was to determine the frequency of bacteremia accompanying flexible sigmoidoscopy. Blood samples for aerobic and anaerobic cultures were obtained before, during, and after flexible sigmoidoscopy in 100 patients who were examined a mean distance of 49.5 cm, range 15-60 cm, after a bowel preparation of two Fleet enemas. In one patient, a transient bacteremia with Streptococcus intermedius was documented and was attended by no associated clinical manifestations. This organism has been previously isolated from

Bacteremia and meningitis following fiberoptic bronchoscopy.

Bacteremia and meningitis due to Streptococcus pneumoniae developed in a 52-year-old man 28 hours after an otherwise uncomplicated fiberoptic bronchoscopy. The patient responded to antimicrobial therapy and supportive care and later underwent pneumonectomy for carcinoma. This report reinforces previous observations that bacteremia may occasionally be associated with the performance of fiberoptic bronchoscopy.

Antiserum treatment of gram-negative bacteremia.

In order to lower the mortality rate from gram-negative bacteremia, 136 patients were treated with human antiserum against core glycolipid, or with control nonimmune human serum, in a double-blind clinical trial. The antiserum was prepared by immunizing healthy young men with a vaccine composed of heat killed cells of the J5 mutant of E. coli 0111 B4. Since the core glycolipid in this mutant is not encumbered with "O" side chains, it can stimulate antibody against the core glycolipid possessed in common by the different species of gram-negative bacteria responsible for lethal bacteremia in pat

Polymicrobic bacteriuria: significant association with bacteremia.

Current clinical practice states that a urine culture yielding three or more isolates must be considered a contaminated specimen and discarded. The validity of this practice was tested by reviewing all patients at the West Haven Veterans Administration Hospital with positive blood cultures for 1 year to determine how many were associated with polymicrobic bacteriuria (two or more urine isolates each present at greater than 10(5) colonies/ml). During 1973 there were 18 patients who had bacteremia associated with a urinary tract infection in which the same organism with the identical antibiogram

Transient bacteremia associated with barium enema.

A group of 175 patients had barium enema. Pour-plate blood cultures were obtained immediately before and after the procedure and 5, 10, 15, and 30 minutes later. Bacteremia was demonstrable in 20 (11.4%) patients. In some, blood cultures were positive for as long as 15 minutes after barium enema; all were negative at 30 minutes. Among the bacteria associated with the 20 episodes of bacteremia were Escherichia coli, Klebsiella, enterococci, Proteus morganii, Bacteroides, and Veillonella. The incidence of bacteremia among patients with ulcerative colitis, regional enteritis, rectal polyps, colon

Vibrio fluvialis Bacteremia With Bullous Cellulitis.

Vibrio fluvialis is a gram-negative bacillus commonly implicated in outbreaks of gastroenteritis. The bacterium has been identified as an emerging pathogen in recent years, with increasing reports of extraintestinal manifestations. This report discusses a case of bacteremia presumed secondary to bullous cellulitis of the left lower extremity, with blood cultures positive for Vibrio fluvialis and Streptococcus dysgalactiae. The patient presented with progressively worsening quadriplegia and falls, with one week of progressive discoloration of the left lower extremity. He denied any gastrointest

Erysipelothrix rhusiopathiae bacteremia treated with linezolid.

Erysipelothrix rhusiopathiae is a common zoonotic pathogen that rarely causes diseases in humans. It has three main disease manifestations: a localized cutaneous, a disseminated cutaneous, and a systemic form of infection, typically characterized as bacteremia with or without endocarditis. Human infections are often associated with occupational exposure to animals, animal products, or their excreta. We present a case of a 60-year-old woman found to have E. rhusiopathiae bacteremia associated with a leg laceration sustained after she fell into a sewer drain. Germane animal exposures were not id

Acute cellulitis with Shewanella algae bacteremia.

Shewanella algae are gram-negative bacteria commonly found in aquatic environments. Infections caused by this agent are rarely documented; however, they are increasingly reported, mainly in countries with warm to temperate climates. Herein, we present a case of a 46-year-old immunocompetent woman with acute cellulitis and S. algae bacteremia (the first isolation culture performed at our hospital). To better understand the epidemiology, clinical outcomes, and treatment possibilities for S. algae bacteremia, we searched literature for similar cases; however, we did not find any cases of infectio

Bacteremia in Pediatric Liver Transplant Recipients.

BACKGROUND: We aimed to determine the frequency of bacteremia, septic shock and bacterial meningitis in pediatric liver transplant recipients (pLTRs) in the outpatient setting and to identify clinical factors associated with bacteremia. METHODS: Multicenter retrospective study of pLTRs evaluated in the emergency department or outpatient clinic between 2010 and 2018 for suspected infection, defined as fever ≥38 °C or a blood culture obtained. We excluded patients with nontransplant immunodeficiency, multiorgan transplants or intestinal failure. The primary outcome was bacteremia; secondary out

Bacteremia Secondary to Epiploic Appendagitis (EA).

Epiploic appendages are fat-filled sacs that are generally located along the surface of the large intestine. In most cases, epiploic appendagitis (EA) is described as an ischemic infarction of an epiploic appendage as the result of torsion or spontaneous thrombosis of the central draining vein of the epiploic appendage. The patient described in this report presented with a sudden onset of diffuse abdominal pain, nausea, and fever. CT scan of the abdomen and pelvis with oral contrast revealed EA of the sigmoid colon. Along the course of the admission, the patient became septic with blood cultur
