Fracture of the shaft of the femur in children.

Sixty-five cases of fracture of the femur shaft caused by trauma in children under age 12 were studied. All were treated by various conservative methods. Two patients later underwent open reduction and internal fixation. Children below 5 years of age were treated by Bryant's suspension traction method. Older children were treated by balanced skin traction. In a few cases where the position was not acceptable, the fracture was reduced under general anesthesia and fixed traction was applied. Patients were discharged with a hip spica when the fracture site was nontender (average three weeks). Und

Standardized anesthetic and ventilation protocol to prevent atelectasis during robotic-assisted bronchoscopy: retrospective observational study.

BACKGROUND: Robotic-assisted bronchoscopy (RAB) relies on stable intrathoracic geometry during cone-beam CT (CBCT) acquisition to ensure accurate navigation and lesion targeting. However, anesthesia-induced atelectasis develops rapidly after induction and can distort airway anatomy, critically impacting the stable intrathoracic geometry required for accurate RAB navigation and lesion targeting. This study evaluated an standardized anesthetic and ventilation protocol designed to prevent atelectasis and maintain physiologic stability during RAB. METHODS: This single-center retrospective observat

General anesthesia vs procedural sedation for failed NeuroThrombectomy undergoing rescue stenting: intention to treat analysis.

BACKGROUND: There is little data available to guide optimal anesthesia management during rescue intracranial angioplasty and stenting (ICAS) for failed mechanical thrombectomy (MT). We sought to compare the procedural safety and functional outcomes of patients undergoing rescue ICAS for failed MT under general anesthesia (GA) vs non-general anesthesia (non-GA). METHODS: We searched the data from the Stenting and Angioplasty In Neuro Thrombectomy (SAINT) study. In our review we included patients if they had anterior circulation large vessel occlusion strokes due to intracranial internal caroti

[Intraocular pressure values in intubation anesthesia in patients with and without chronic open angle glaucoma].

BACKGROUND: A number of clinical studies have investigated the influence of hypnotics and relaxants on intraocular pressure. All of them referred to the normal eye. Our goal was to show if there is a difference in intraocular pressure profile during general anesthesia between eyes with and without primary open angle glaucoma. PATIENTS AND METHODS: We prospectively examined two groups of 20 patients with and without primary open angle glaucoma. Intraocular pressure (IOP) readings were taken using a Perkins hand held applanation tonometer during routine surgery on cataract or glaucoma on the fe

Assessments of prolonged effects of desflurane and sevoflurane on motor learning deficits in aged AppNL-G-F/NL-G-F mice.

As the proportion of elderly in society increases, so do the number of older patients undergoing surgical procedures. This is concerning as exposure to anesthesia has been identified as a risk factor for Alzheimer's disease (AD). However, the causal relationship between clinical AD development and anesthesia remains conjectural. Preclinical studies have demonstrated that anesthesia, such as halothane, isoflurane, and sevoflurane, induces AD-like pathophysiological changes and cognitive impairments in transgenic mouse models of AD. Desflurane does not have these effects and is expected to have

[Removal of the laryngeal mask airway in the post-anesthesia care unit. A means of process optimization?].

BACKGROUND: Removal of the laryngeal mask airway in the post-anesthesia care unit could potentially contribute to a faster turnover from one operation to the next. The aim of this study was, therefore, to obtain an insight into the potential time saving and the safety of planned removal of the ProSeal™-LMA (PLMA) in the post-anesthesia care unit. METHODS: In this study 120 adult patients with American Society of Anesthesiologists (ASA) classification I-II, age range 18-85 years, undergoing a surgical procedure under general anesthesia in which the PLMA was used were randomly assigned to one o

[The TUR-syndrome and its importance to the anesthetist (author's transl)].

The so called TUR-syndrome is even today not to be underestimated a complication of the transurethral prostatectomy. In a retrospective study covering a time of three years and dealing only with transurethral prostatectomies under spinal anesthesia the frequency and degree of severity of the TUR-syndrome, the correlation between the early symptoms and the incidence and between the length of resection and the degree of severity of the syndrome were investigated. Furthermore the possibilities of prophylaxis and optimal therapy were inquired and recommendations were given for both of them. Finall

Awake prone lateral transpsoas lumbar interbody fusion with simultaneous posterior decompression and fusion: illustrative case.

BACKGROUND: Awake spine surgery under spinal anesthesia may reduce perioperative risk in older or medically complex patients. This report presents the first documented case of an awake, single-position prone lateral transpsoas interbody fusion at L3-4 with simultaneous posterior decompression and fusion to treat a 77-year-old male with neurogenic claudication and mechanical back pain. A technical approach was developed to complete the procedure within the approximate 3-hour duration of spinal anesthesia. Prone positioning enabled simultaneous access to lateral and posterior corridors. Intraope

Effects of Anesthetic Techniques on the Risk of Postoperative Complications Following Lower Extremity Amputation in Diabetes Patients with Coagulation Abnormalities: A Retrospective Cohort Study Using Propensity Score Analysis.

Diabetic foot amputation is associated with high morbidity and mortality rates. To prevent cardiovascular complications along with vasculopathy in the course of diabetes mellitus, a high number of patients receive anticoagulant therapy. However, anticoagulants are contraindicated in neuraxial anesthesia limiting available anesthetic modalities. Therefore, in this retrospective study, we aimed to compare between general anesthesia and peripheral nerve block (PNB) with respect to postoperative complications following lower extremity amputation (LEA) in patients with coagulation abnormalities. Midline vs. flank laparotomy- criteria for choosing the optimal surgical technique for uterine torsion correction in the mare.

Uterine torsion in mares belongs to maternal pregnancy disorders, accounting for 5-10% of complications in the last trimester of pregnancy. Two surgical techniques for repositioning uterine torsion are used: flank laparotomy in local anesthesia on a standing mare (SFL) and midline laparotomy carried out under general anesthesia (MI). The study aims to present the exact protocol used by the authors to qualify a mare with uterine torsion for surgery using one of the above-mentioned methods.A total of 19 mares were operated on, of which 13 underwent midline laparotomy under general anesthesia, an

Pharmacology of thoracic anesthesia.

PURPOSE OF REVIEW: Thoracic anesthesia is one of the most demanding subspecialties, requiring the integration of surgical conditions with patient safety. Procedures such as lung resections, video-assisted thoracic surgery, robotic surgery, and transplantation impose unique physiologic stress, particularly during one-lung ventilation, which challenges gas exchange and cardiovascular stability. This review summarizes current evidence on anesthetic strategies in thoracic surgery, including induction and maintenance techniques, opioid use, neuromuscular blockade, and hemodynamic management. Lateral ankle ligament anesthesia significantly alters single limb postural control.

Lateral ankle anesthesia has been used as a model to explore effects of ligament deafferentation related to ankle sprain on single limb postural control with conflicting results. Time-to-boundary (TTB) is a postural control measurement technique found to be sensitive in detecting subtle deficits in postural control in those with chronic ankle instability. The objective of this study was to determine the effects of lateral ankle ligament anesthesia on TTB measures of single limb postural control in healthy adults. Twenty-two healthy adults with no history of lower extremity injury within the pa

[Separation of conjoined twins: the anesthesiologist's perspective].

Managing anesthesia care for separation of conjoined twins poses a special challenge for the anesthesia team. Early preparation, thorough understanding of the anatomical and the physiological consequences of this complex anomaly, as well as careful coordination with the teams of surgeons, nurses and operating room personnel are mandatory to assure successful separation of the conjoined twins. It is recommended that the mother be transferred to and the babies be delivered at a tertiary care medical center, like the Children's Hospital. Special attention is devoted during the surgical procedure

Magnetic Resonance Imaging-Guided Focused Ultrasound Lesioning under General Anesthesia: A Case Series.

INTRODUCTION: Real-time monitoring during MR-guided focused ultrasound (MRgFUS) procedures has been considered essential to monitor tremor improvement and side effects in the alignment and/or verify phase before the actual MRgFUS treatment and following the ablative sonications. However, a subgroup of patients does not tolerate being awake during the entire procedure for a variety of reasons. CASE PRESENTATIONS: We performed MRgFUS treatments in three Parkinson's disease/Parkinsonism patients under general anesthesia. These patients had previously failed an attempt to undergo the procedure aw

The neonatal lung--physiology and ventilation.

This review article focuses on neonatal respiratory physiology, mechanical ventilation of the neonate and changes induced by anesthesia and surgery. Optimal ventilation techniques for preterm and term neonates are discussed. In summary, neonates are at high risk for respiratory complications during anesthesia, which can be explained by their characteristic respiratory physiology. Especially the delicate balance between closing volume and functional residual capacity can be easily disturbed by anesthetic and surgical interventions resulting in respiratory deterioration. Ventilatory strategies s

Suitability of sedative usage in patients undergoing cardiovascular surgery.

INTRODUCTION: The provision of optimal anesthesia care is essential to the safe conduct of cardiac surgery. Nevertheless, evidence regarding ideal choices for anesthetic induction and maintenance remain conflicting. Individualized regimens balancing the patient's cardiac pathology and the expected hemodynamic perturbations from the anesthetics are likely optimal, underscoring the rationale for this review of contemporary anesthetic strategies in cardiac surgery. AREAS COVERED: Descriptions of cardiac surgery trends and advancements that necessitate concomitant anesthesia, including the challe
