Benefit and risk management for steroid treatment in upper airway diseases.

Because of their efficacy, intranasal corticosteroids (INCS) remain the cornerstone in the treatment of upper airway diseases. Although INCS are extremely efficacious, local and systemic adverse effects have been associated with their use. However, none of these effects is of sufficient frequency or magnitude to warrant a reduction in their use or any action more than careful drug and dose selection and monitoring when indicated. Recent publications focus on newer aspects of efficacy in rhinitis and on the adverse effects of growth suppression, cataract formation, and histologic effects on the

Inhibition of GM-CSF secretion by topical corticosteroids and nedocromil sodium. A comparison study using nasal polyp epithelial cells.

Nasal epithelial cells maintain eosinophil survival by secreting granulocyte/macrophage colony-stimulating factor (GM-CSF). Corticosteroids antagonize eosinophil viability induced by GM-CSF. We investigated the effect of topical corticosteroids and nedocromil sodium on the release of GM-CSF from nasal polyp epithelial cells. Epithelial cells were obtained from 19 patients undergoing nasal polypectomy and cultured. After reaching confluence, cultured cells were stimulated with 10% foetal calf serum in the absence and presence of four topical corticosteroids and nedocromil sodium for 48 h. GM-CS

Prophylactic drugs in the management of childhood asthma.

Recurrences of explosive, severe asthma in children can often be prevented by immunotherapy and constant surveillance of environmental and dietary control. In addition, judicious and preserving use of pharmaceuticals may be employed for prophylaxis in childhood asthma. These include cromolyn sodium by inhalation, beclomethasone dipropionate aerosols, round-the-clock methylxanthines and beta 2 adrenergic bronchodilator agents. Their application is discussed.

Corticosteroid treatment in nasal polyposis with a three-year follow-up period.

OBJECTIVES/HYPOTHESIS: The management of nasal polyposis is undoubtedly a controversial subject. The part played by surgery seems to be steadily growing, if the number of published reports dedicated to this approach is any yardstick. Although the medical treatment remains the undisputed therapeutic mainstay, trials dedicated to the long-term assessment of its overall efficacy are scarce. STUDY DESIGN: Retrospective medical record review. METHODS: The aim of the study is focused on the evaluation of a dual modality, topical and systemic, over a follow-up period of 3 years. In all, 100 patient

Intranasal steroid sprays in the treatment of rhinitis: is one better than another?

The treatment of allergic rhinitis has been revolutionized by the introduction of topical nasal steroids, which are one of the commonest prescriptions from otolaryngology departments. With so many different sprays available on the market, the literature was reviewed for the efficacy, side-effect profile and relative cost of each product and the following conclusions made: (1) A meta-analysis of randomized controlled trials comparing the efficacy of intranasal corticosteroids and oral antihistamines in the treatment of allergic rhinitis showed a clear benefit in favour of intranasal steroids in

Clinical Inquiry. Do inhaled steroids reduce bone mineral density and increase fracture risk?

No, except perhaps at high doses. Inhaled corticosteroids at low to medium doses (<1500 mcg beclomethasone hydrofluoroalkane per day) for asthma and chronic obstructive pulmonary disease don't increase the risk of significant bone loss or fracture at 2 to 3 years follow-up. Higher doses, however, may raise the risk of nontraumatic fracture over 1 to 4 years of follow-up.

Prevalence and clinical features of cough variant asthma in a general internal medicine outpatient clinic in Japan.

OBJECTIVE: The aims of the present study were to examine the prevalence and clinical features of cough variant asthma (CVA) among patients with chronic and persistent cough at an outpatient clinic in Japan, and the efficacy of treatment with an inhaled corticosteroid. METHODOLOGY: This prospective study was conducted at a general internal medicine outpatient clinic in Japan over a 12-month period. CVA was diagnosed as chronic cough without wheezing or any apparent cause, that had persisted for more than 8 weeks, with a normal CXR and spirometry but with bronchial hyperresponsiveness to methac

Stridor in childhood asthma.

Three asthmatic children are reported with shortness of breath due to stridor. The stridor was preceded by a deterioration in asthma control necessitating the introduction or increased dosage of a pressurized beclomethasone inhaler. The stridor mimicked asthmatic symptoms and was only differentiated by auscultation directly over the neck and by response to nebulized adrenaline. One child had a laryngeal foreign body which probably gained entry via a pressurized inhaler.

From alpha to beta: a two-step strategy for stereoinversion of C16 glucocorticoids.

C16-Methylcorticoids are important anti-inflammatory agents whose activity depends on the C16 methyl configuration. However, access to C16β analogues remains challenging. Herein, we report the first concise two-step stereoinversion of C16α-methylcorticoids to C16β diastereomers through C17 hydroxyl elimination followed by Mn-catalyzed Mukaiyama hydration. Betamethasone, beclomethasone, clobetasol, clobetamethasone, and difluorolaxone were obtained in useful yields with high diastereoselectivities (19 : 1 to >99 : 1 dr).

[Pharmacomodulation of hypersensitivity rhinitis].

INTRODUCTION: Hypersensitive rhinitis is characterized by accumulation of mast cells and eosinophils (believed to be primary effector cells) in the nasal epithelium. Treatment of hypersensitive rhinitis is directed towards reducing either tissue accumulation of these activated cells or the end-organ effects of the released mediators. The aim of this study was to examine effects of local corticosteroid treatment on the number of eosinophils in nasal secretion of patients with isolated hypersensitive rhinitis. MATERIAL AND METHODS: A total of 55 subjects were included in a prospective controlle

Inhaled corticosteroids compared with oral prednisone in patients starting long-term corticosteroid therapy for asthma. A controlled trial by the British Thoracic and Tuberculosis Association.

Inhaled beclomethasone dipropionate and inhaled betamethasone valerate have been compared with oral prednisone in the treatment of 75 patients with asthma who were starting long-term corticosteroids for the first time. Both of the inhaled corticosteroids controlled asthma as well as did oral prednisone in those who had responded to therapy in the initial period of the trial. A daily dose of 400 mug of inhaled drug was approximately equivalent to 7-5 mg daily of prednisone. Prednisone suppressed the adrenal response to tetracosactrin, whereas the mean responses in the groups receiving inhaled c
