Relationship between biologic therapy and cytokine levels in patients with inflammatory arthritis.

Biological agents are frontline treatments for ankylosing spondylitis (AS) and rheumatoid arthritis (RA); however, their efficacy varies owing to differences in patient autoimmune status. This study aimed to evaluate peripheral blood cytokine profiles in patients with AS and RA and assess the impact of different treatment modalities. Data from 145 patients with AS, 491 patients with RA, and 125 healthy controls were collected. Cytokine levels (tumor necrosis factor [TNF]-α, interleukin [IL]-6, IL-8, IL-17, IL-5, etc) were analyzed in healthy controls and patients with AS and RA, with a focus o

Safety of biologic therapy.

Several biologic agents have been assessed in patients with inflammatory bowel disease (IBD; Crohn's disease [CD] and ulcerative colitis [UC]). Until recently, only infliximab (humanized monoclonal anti-TNF-alpha antibody) had been approved by the Food and Drug Administration (FDA) to induce and maintain remission in patients with active mild to moderate and/or fistulizing Crohn's disease who are refractory to conventional therapy. Two recent trials, ACT 1 and ACT2, observed high efficacy of infliximab in inducing and maintaining clinical remission, mucosal healing, and corticosteroid-sparing

Iraq's biological weapons. The past as future?

Between 1985 and April 1991, Iraq developed anthrax, botulinum toxin, and aflatoxin for biological warfare; 200 bombs and 25 ballistic missiles laden with biological agents were deployed by the time Operation Desert Storm occurred. Although cause for concern, if used during the Persian Gulf War, Iraq's biological warfare arsenal probably would have been militarily ineffective for 3 reasons: (1) it was small; (2) payload dispersal mechanisms were inefficient; and (3) coalition forces dominated the theater of war (ie, they had overwhelming air superiority and had crippled Iraq's command and cont

Pain in spondyloarthritis: A neuro-immune interaction.

Pain in spondyloarthritis is usually attributed to inflammation. However, treatment with potent biologic agents that control inflammation does not always control the pain. Pain is hence likely to be multifactorial. Fatigue is another prominent feature of this condition which again tends to respond poorly to potent biologic agents. There is also a growing interest in coexisting fibromyalgia in this group of patients and how this affects response to biologic therapies. Advances in neuroimaging have helped in better understanding the dynamic nature of brain networks in the perception of pain. Ani

Biologics for ANCA-associated vasculitis.

The antineutrophil cytoplasmic antibody (ANCA)-associated vasculitides (AAV) are a group of necrotizing vasculitides with a potential fatal outcome. Conventional therapy is based on the use of glucocorticoids (GCs) and cyclophosphamide (CYC), which is associated with severe toxic effects and is unable to control the disease activity in some refractory and relapsing cases. Several authors focused their efforts on the identification of safe and more efficient drugs, primarily investigating biological agents. Rituximab (RTX) demonstrated to be an alternative to CYC as remission-induction therapy

Cost-effectiveness of moderate-to-severe psoriasis treatment.

Psoriasis is a common, chronic inflammatory disease that can cause as much disability as cancer, diabetes or other major medical illnesses. Traditional therapies for treating moderate-to-severe psoriasis include phototherapy, methotrexate, oral retinoids and ciclosporin. New biological treatments provide further therapeutic options, but add to the already considerable cost of managing psoriasis. Expert panels have published guidelines for the use of biological agents in managing moderate-to-severe psoriasis; however, few if any of these guidelines appropriately consider the cost-effectiveness

A nondenaturing solid phase pharmaceutical carrier comprised of surface-modified nanocrystalline materials.

Numerous carrier systems have been developed for the controlled delivery of biologically active molecules such as drugs and diagnostic agents. The biophysical interactions between the biologically active molecules and their carriers, however, may denature the former and lead to reduced biological activity. In this study, a model nondenaturing carrier comprised of a nanocrystalline (10 -7 m) tin oxide core and a surface-charge-reducing organic bonding layer (GF292) was synthesized. A subsequently bound protein (human transferrin) showed significant retained conformation by immunoelectron micros

[Toxicological effects of weapons of mass destruction and noxious agents in modern warfare and terrorism].

Weapons of mass destruction (WMD) best portray the twisted use of technological achievements against the human species. Despite arm control efforts, WMD threat continues to exist and even proliferate. This in turn calls for improvement in defensive measures against this threat. The modern soldier is exposed to a number of chemical, biological, and radiological agents in military and peace operations, while civilians are mainly exposed to terrorist attacks. Regardless of origin or mode of action, WMDs and other noxious agents aim for the same - to make an organism dysfunctional. Because their e

Molecular Mechanisms of New Bone Formation in Axial Spondyloarthritis.

Axial spondyloarthritis (axSpA) is a disease characterised by new bone formation. Biologic agents targeting TNFα or IL-17 are used widely and are very effective in controlling symptoms and improving quality of life in these patients. However, the effect of biologics on radiographic progression is still not entirely known. The most crucial question to be addressed is whether new bone formation in the context of axSpA is linked to the inflammatory process. If new bone formation and inflammation are interconnected, then long-term suppression of inflammation with biologic agents may eventually lea

Complications of biological therapy for inflammatory bowel diseases.

PURPOSE OF REVIEW: The purpose of this review is to analyze the complications associated with treatment of inflammatory bowel disease with biologic agents. RECENT FINDINGS: There have been various biologic agents evaluated in patients with inflammatory bowel disease; that is, Crohn's disease and ulcerative colitis. Thus far only infliximab has been approved by the US Food and Drug Administration as induction and maintenance treatment in patients with active Crohn's disease (moderate-to-severe and/or fistulizing) who are refractory to conventional therapy. Potential applications of 1,8-cineole on biofouling control in microalgae cultivation.

Biological contamination poses a critical challenge to microalgae cultivation, yet effective and microalga-safe control agents remain limited. In this study, 1,8-cineole, a major component of eucalyptus oil, was evaluated as an insecticide to investigate its effects on biological contaminants Euplotes vannus, Brachionus plicatilis, and Tigriopus japonicus during the cultivation of microalage Synechococcus sp., Chlorella sp., and Isochrysis sp., respectively. Results indicated that 1,8-cineole significantly reduced the abundance of the three biological contaminants while causing a negligible ef

Comparative cytogenotoxicity and physiological stress endpoints of a Trichoderma asperellum-based biopesticide versus mancozeb and abamectin in plant bioassays.

The global transition toward sustainable agricultural systems has intensified the search for fungal-based biopesticides, such as Trichoderma asperellum, to replace synthetic pesticides. Although considered safer, these products are not biologically inert and may elicit cellular responses in non-target organisms, making comparative ecotoxicological evaluation with conventional pesticides, such as mancozeb and abamectin, essential. This study aimed to evaluate and compare the cytogenotoxic effects, nucleolar alterations, and phytotoxicity of a T. asperellum-based biopesticide in relation to comm

Effect of Paecilomyces lilacinus, Trichoderma harzianum and Trichoderma virens fungal extracts on the hatchability of Ancylostoma eggs.

BACKGROUND: Ancylostoma species have demanded attention due to their zoonotic potential. The use of anthelmintics is the usual method to prevent environmental contamination by Ancylostoma eggs and larvae. Nematophagous fungi have been widely used in their biological control due to the fungus ability to capture and digest free nematode forms. AIMS: The aim of this study was to evaluate the effect of four different fungal extracts of Paecilomyces lilacinus (n=2), Trichoderma harzianum (n=1) and Trichoderma virens (n=1) isolates on the hatchability of Ancylostoma eggs. METHODS: Fungal extracts

Anaplastic thyroid carcinoma: A comprehensive review of current and future therapeutic options.

Anaplastic thyroid carcinoma (ATC) is the rarest, but deadliest histologic type among thyroid malignancies, with a dismal median survival of 3-9 mo. Even though ATC accounts for less than 2% of all thyroid tumors, it is responsible for 14%-39% of thyroid carcinoma-related deaths. ATC clinically presents as a rapidly growing mass in the neck, associated with dyspnoea, dysphagia and vocal cord paralysis. It is usually locally advanced and often metastatic at initial presentation. For operable diseases, the combination of radical surgery with adjuvant radiotherapy or chemotherapy, using agents su

Diagnosis, management, and containment of smallpox infections.

The US public health system and primary health care providers must be prepared to address varied biologic agents, including pathogens that are rarely seen in the United States. The increased frequency of international travel, combined with the current national concerns regarding bioterrorism, create an imperative for health practitioners to be more knowledgeable about specific biologic agents. Smallpox poses a serious bioterrorist threat to the US population at this time. Thus, if an outbreak occurred, prompt recognition and the implementation of control measures would be important.
