Artificial Intelligence Across the Surgical Research Workflow: A Stage-Specific Framework for Benefits, Failure Modes, and Oversight.

Artificial intelligence (AI) tools, particularly large language models (LLMs), have rapidly integrated into surgical and biomedical research, prompting fundamental questions about their appropriate role. This review presents a stage-specific framework mapping each phase of the surgical research workflow (literature review, data analysis, manuscript preparation, and research synthesis) to the role AI plays, the benefits it provides, the failure modes it introduces, and the oversight it requires. Artificial intelligence-powered query systems are designed to speed literature synthesis across thou

Special considerations in the design and implementation of pediatric otoprotection trials.

PURPOSE: Cisplatin-induced hearing loss (CIHL) is a common late effect after childhood cancer treatment having profound, lifelong consequences that lower quality of life. The recent identification of intravenous sodium thiosulfate (STS) as an effective agent for preventing pediatric CIHL represents a paradigm shift that has created new opportunities for expanding STS usage and developing additional otoprotectants. The purpose of this paper is to discuss key considerations and recommendations for the design and implementation of future pediatric otoprotection trials. METHODS: An approach synth

Osteoporosis and risk of fracture: reference class problems are real.

Elselijn Kingma argues that Christopher Boorse's biostatistical theory does not show how the reference classes it uses-namely, age groups of a sex of a species-are objective and naturalistic. Boorse has replied that this objection is of no concern, because there are no examples of clinicians' choosing to use reference classes other than the ones he suggests. Boorse argues that clinicians use the reference classes they do because these reflect the natural classes of organisms to which their patients belong. Drawing on a thorough exploration of how the disease osteoporosis is defined in adults,

MICHELINdb: a web-based tool for mining of helminth-microbiota interaction datasets, and a meta-analysis of current research.

BACKGROUND: The complex network of interactions occurring between gastrointestinal (GI) and extra-intestinal (EI) parasitic helminths of humans and animals and the resident gut microbial flora is attracting increasing attention from biomedical researchers, because of the likely implications for the pathophysiology of helminth infection and disease. Nevertheless, the vast heterogeneity of study designs and microbial community profiling strategies, and of bioinformatic and biostatistical approaches for analyses of metagenomic sequence datasets hinder the identification of bacterial targets for f

Fistula in ano in infants: who recurs?

INTRODUCTION: Fistula in ano is a common malady in infancy. However, relatively little literature is devoted to it. Our aim was to describe the natural history and identify predictors of which children will ultimately recur. METHODS: A retrospective review of patients less than 3 years old undergoing anal fistulotomy was performed between May 2002 and November 2007 at a tertiary children's hospital. Demographics, preoperative, operative, and postoperative characteristics were collected in each group and evaluated by biostatistical analysis. P values <0.05 were considered significant. RESULTS

Reducing unnecessary morbidity from percutaneous thermocoagulation in the treatment of trigeminal neuralgia--part C: a starting point for a somatotopic map of the human Gasserian ganglion.

OBJECTIVES: An original method and technique has been designed to reduce the significant morbidity associated with techniques currently used for percutaneous thermocoagulation in the treatment of trigeminal neuralgia. The current report deals with the mathematical and biostatistical analysis of verbal responses gathered using such a method in an attempt, as a starting point, to establish the somatotopic organization of the human gasserian ganglion. METHOD: A correspondence analysis was used to validate verbal responses. These were ordered in three 34 x 34 matrices, according the initial seque

[Data base for patients with balance disorders].

INTRODUCTION: With the aim of integrating clinical thinking and obtaining epidemiological and biostatistical results in long series of patients who present balance disorders, we have developed a neuro otological protocol and its corresponding database. This offers answers to the questions of how to assess the patient, how to conduct a clinical interview correctly, and which complementary studies should be requested and how. RESULTS: The information from the protocol is stored and analysed in our database, which was designed in the Epi Info software application, produced by CDC (NIH, USA), in

Clinical trials with velnacrine: (PROPP) the physician reference of predicted probabilities--a statistical model for the estimation of hepatotoxicity risk with velnacrine maleate.

Safety observations during the clinical development of Mentane (velnacrine maleate) have included the occurrence of generally asymptomatic liver enzyme elevations confined to patients with Alzheimer's disease (AD). The clinical presentation of this reversible hepatocellular injury is analogous to that reported for tetrahydroaminoacridine (THA). Direct liver injury, possibly associated with the production of a toxic metabolite, would be consistent with reports of aberrant xenobiotic metabolism in Alzheimer's disease patients. Since a patient related aberration in drug metabolism was suspected,

Metabolomic breath landscape analysis unravels lipid biomarker candidates in patients with genetic and idiopathic Parkinson's disease.

Parkinson's disease (PD) is the fastest growing neurodegenerative disorder. The current lack of efficient early diagnostic tools necessitates novel approaches to biomarker discovery. We propose an untargeted metabolomics approach using non-invasive exhaled breath analysis. Breath samples, collected from 73 PD patients, encompassing both genetic (LRRK2: n = 12, GBA1: n = 35, PRKN: n = 6) and idiopathic PD (n = 20), 4 unaffected LRRK2 pathogenic variant carriers, and 90 controls underwent extreme-resolution FT-ICR-MS analysis. Findings were compared with metabolomics data from blood plasma. Bios

ACE2 genetic polymorphisms and non-genetic factors were associated with a history of musculoskeletal injuries in high-performance athletes.

High-intensity exercise affects angiotensin-converting enzyme 2 (ACE2) levels and can modulate regeneration of the skeletal muscle; however, genetic polymorphisms can influence ACE2 expression, which could lead to musculoskeletal injury (MSKi). This case-control study aimed to evaluate the influence of ACE2 polymorphisms, and their combination with non-genetic predictors on the history of MSKi in high-performance athletes. Four hundred eighty-eight athletes genotyped for ACE2 SNPs (rs2285666, rs4240157, and rs2074192). Of the 300 MSKi cases, 51% reported only muscle injuries and 13% only tendi

Study design challenges and strategies in clinical trials for rare diseases: Lessons learned from pantothenate kinase-associated neurodegeneration.

Substantial challenges in study design and methodology exist during clinical trial development to examine treatment response in patients with a rare disease, especially those with predominant central nervous system involvement and heterogeneity in clinical manifestations and natural history. Here we discuss crucial decisions which may significantly impact success of the study, including patient selection and recruitment, identification and selection of endpoints, determination of the study duration, consideration of control groups including natural history controls, and selection of appropriat

Infections by human gastrointestinal helminths are associated with changes in faecal microbiota diversity and composition.

Investigations of the impact that patent infections by soil-transmitted gastrointestinal nematode parasites exert on the composition of the host gut commensal flora are attracting growing interest by the scientific community. However, information collected to date varies across experiments, and further studies are needed to identify consistent relationships between parasites and commensal microbial species. Here, we explore the qualitative and quantitative differences between the microbial community profiles of cohorts of human volunteers from Sri Lanka with patent infection by one or more par

Role of gene-expression profiling in chronic myeloid leukemia.

Gene-expression profiling enables disease classification and risk stratification, and provides important insights into possible pathogenetic mechanisms. The clinical management and prognosis of chronic myeloid leukemia (CML) has substantially changed after the introduction of targeted therapies, such as imatinib and the second-generation tyrosine kinase inhibitors nilotinib and dasatinib. Although exact characterization of CML pathogenetics has been performed by showing the causal pathogenetic relevance of the reciprocal translocation between chromosomes 9 and 22 for CML development, the disea

Biomarkers of human gastrointestinal tract regions.

Dysregulation of intestinal epithelial cell performance is associated with an array of pathologies whose onset mechanisms are incompletely understood. While whole-genomics approaches have been valuable for studying the molecular basis of several intestinal diseases, a thorough analysis of gene expression along the healthy gastrointestinal tract is still lacking. The aim of this study was to map gene expression in gastrointestinal regions of healthy human adults and to implement a procedure for microarray data analysis that would allow its use as a reference when screening for pathological devi

An early Transcriptomic Investigation in Adult Patients with Spinal Muscular Atrophy Under Treatment with Nusinersen.

Spinal muscular atrophy (SMA) is a rare degenerative disorder with loss of motor neurons caused by mutations in the SMN1 gene. Nusinersen, an antisense oligonucleotide, was approved for SMA treatment to compensate the deficit of the encoded protein SMN by modulating the pre-mRNA splicing of SMN2, the centromeric homologous of SMN1, thus inducing the production of a greater amount of biologically active protein. Here, we reported a 10-month transcriptomics investigation in 10 adult SMA who received nusinersen to search for early genetic markers for clinical monitoring. By comparing their profil
