BACKGROUND CONTEXT: The safety of allograft material has come under scrutiny because of recent reports of allograft-associated bacterial and viral infections in tissue recipients. Gamma irradiation, although being one of the most effective ways of terminal sterilization, has been shown to affect the biomechanical properties of allograft bone. It may also have detrimental effects on the osteoinductivity of allograft material such as demineralized bone matrix (DBM) by the denaturation of proteins because of heat generated by irradiation. Sterilization of DBM material is an important variable in

BACKGROUND: Hip decompression effectively treats early-stage osteonecrosis of the femoral head (ONFH) by slowing disease progression and potentially delaying joint replacement. Biological adjuvants like bone marrow aspirate concentrate (BMAC) and platelet-rich plasma (PRP) support bone regeneration and improve outcomes1-7. The present video article demonstrates a simple, coreless hip decompression technique with BMAC and PRP injection for early-stage ONFH.
DESCRIPTION: The procedure is performed in the same operating room setting as traditional core decompression, with the patient supine on a

Different noninvasive techniques for the assessment of the individual fracture risk in osteoporosis are introduced, and the relation between structural properties of high-resolution computed tomography (HR-CT) images of vertebral bodies, their bone mineral density (BMD) and the fracture load is analyzed. In 24 unfractured lumbar vertebrae with different degrees of demineralization from six specimens, the trabecular and cortical BMD was determined using quantitative CT. A lateral X-ray image revealed the number of fractures in the entire spine. A structural analysis of spongy and cortical bone

BACKGROUND: Demineralized dentin matrix (DDM) can serve as a novel scaffold for dental tissue regeneration. This study aimed to assess the impact of two distinct sterilizing methods on morphology of Allo-DDM as well as on the viability, metabolic activity, and alkaline phosphatase (ALP) activity of bone marrow mesenchymal stem cells (BMMSCs).
METHODS: Allo-DDM scaffolds were prepared from human-extracted premolars and sterilized using two different techniques. These scaffolds were examined using scanning electron microscopy (SEM) to assess their morphology and by Energy Dispersive X-ray Spect

UNLABELLED: Although most patients who undergo anterior cruciate ligament (ACL) reconstruction achieve long-term functional stability and symptom relief, graft rupture rates range from 2% to 10%1,2. A small subset of these patients require a 2-stage revision ACL reconstruction because of tunnel osteolysis or tunnel malposition that will interfere with the planned revision tunnel placement3. In the present article, we describe the hybrid use of arthroscopically delivered injectable allograft matrix in the femur and pre-shaped bone dowels in the tibia for the treatment of lower-extremity bone de

OBJECTIVE: To assess the efficacy of osteogenic protein-1 (OP-1) for long bone nonunion.
CLINICAL NEED: Although most fractures heal within a normal period, about 5% to 10% do not heal and are classified as delayed or nonunion fractures. Nonunion and segmental bone loss after fracture, reconstructive surgery, or lesion excision can present complex orthopedic problems, and the multiple surgical procedures often needed are associated with patient morbidity and reduced quality of life. Many factors contribute to the pathogenesis of a delayed union or nonunion fractures, including deficiencies of

BACKGROUND: Gingival recession defects (GRDs) pose functional and esthetic concerns and may be associated with unfavorable tooth positions. Surgically facilitated orthodontic treatment (SFOT) with clear aligners can be a valuable option for adults with severe malocclusion and GRDs.
METHODS: A 28-year-old male presented with severe dental crowding, Class III dental malocclusion, localized tooth crossbites, and tapered maxillary arch. He exhibited multiple gingival recessions type I up to 6 mm in depth, at least 2 mm of keratinized tissue throughout, and a thick scalloped gingival phenotype. Cl

INTRODUCTION: Primary hyperparathyroidism (PHPT) is a common endocrine disease especially in postmenopausal women and in older adults, with elevated parathyroid hormone (PTH) levels by parathyroid glands. The main symptoms of PHPT are hypercalcemia, often associated with hypercalciuria, urolithiasis and bone demineralization that results in osteopenia or osteoporosis and increases overall fracture risk. Parathyroidectomy is today the only definitive treatment for patients to prevent worsening of symptoms. Minimally invasive targeted approach for parathyroidectomy can be offered to patients wit

UNLABELLED: Complex intra-articular calcaneal fractures often resulted in secondary pain and deformity, requiring subsequent subtalar arthrodesis1-3. The literature suggests that primary subtalar arthrodesis in the acute period has good functional results2,3. The literature also demonstrates that posterior arthroscopic subtalar arthrodesis for chronic arthritis has favorable results5. Thus, we propose an approach to treating these difficult intra-articular calcaneal fractures that utilizes a posterior arthroscopic primary subtalar arthrodesis technique-aptly named Calcaneus Fracture and Poster

BACKGROUND: Demineralized bone matrix (DBM) is used for the treatment of osseous defects. Conditioned medium from native bone chips can activate transforming growth factor (TGF)-β signaling in mesenchymal cells. The aim of this study is to determine whether processing of native bone into DBM affects the activity of the conditioned medium.
METHODS: Porcine cortical bone blocks were subjected to defatting, different concentrations of hydrochloric acid, and various temperatures. DBM was lyophilized, ground, and placed into culture medium. Human gingiva and periodontal fibroblasts were exposed to

BACKGROUND: Previous research on spinal alignment and postoperative outcomes after cervical and upper thoracic fixation has suggested that clinical and patient-reported outcomes are improved when certain anatomical parameters are maintained. These parameters include the cervical sagittal vertical axis (cSVA), C2 and T1 slopes, and cervical lordosis (CL). For patients with primary and metastatic tumors involving the subaxial cervical and/or upper thoracic spine, there is minimal guidance on how to apply these parameters. Surgeons must make critical decisions when designing the optimal construct

BACKGROUND: Definitive treatment strategies for bisphosphonate-related osteonecrosis of the jaw (BRONJ) have not been developed. Cell-based therapy is an attractive treatment method for intractable diseases in the medical and dental fields; however, approval has been challenging in dentistry. Recently, we developed quality- and quantity (QQ)-controlled peripheral blood mononuclear cells (PBMNCs) that have anti-inflammatory and pro-angiogenesis effects. The aim of this study was to investigate the effects of QQ-controlled PBMNC transplantation on BRONJ-like lesions in mice.

BACKGROUND: The management of bisphosphonate related necrosis of the jaw has become clinical routine. While approximately two thirds of the lesions are in the mandible, one third is located in the maxilla. In 40-50 % of maxillary necrosis the maxillary sinus is involved, leading to maxillary sinusitis and oro-antral communications.
METHODS: This retrospective single center study includes all patients with diagnosis of BP-ONJ of the maxilla and concomitant maxillary sinusitis. The information collected includes age, gender, primary disease, bisphosphonate intake, involving type of bisphosphona

Bone is one of the key components of the musculoskeletal system. Bone and joint disease are the fourth most widespread disease, in addition to cardiovascular disease, cancer, and diabetes, which seriously affect people's quality of life. Bone organoids seem to be a great model by which to promote the research method, which further could improve the treatment of bone and joint disease in the future. Here, we introduce the various bone and joint diseases and their biology, and the conditions of organoid culture, comparing the in vitro models among 2D, 3D, and organoids. We summarize the differin

INTRODUCTION: Aneurysmal bone cyst (ABC) is a benign intraosseous lesion, usually seen before the age of 20 years and is a lesion filled with blood cavities causing a blowout distension of the bone. It constitutes to about 1% of benign bone tumors. Although benign, a large lesion is liable to develop pathological fracture, so needs prompt addressal. Surgical resection or curettage in large lesions can lead to bone defects, deformities, and even functional abnormalities, especially in children. This article describes a large aggressive ABC of proximal metaphyseodiaphyseal region of proximal hum

Tooth extraction in patients treated with bisphosphonates (BPs) for osteoporosis or cancer exposes the patient to the risk of osteonecrosis of the jaw. An autologous membrane using platelet-rich fibrin (PRF) is an innovative technique to promote wound healing, which allows obtaining a hermetic closure of the post-extractive surgical site without the need of mucoperiosteal flaps or periosteal releasing incisions. Here, we report the case of a 70-year-old woman, in therapy with alendronate for 12 years, requiring the upper right premolar extraction because of a crown fracture. After the tooth ex

BackgroundVertebral compression fractures (VCFs) constitute a major health care problem, not only because of their high incidence but also because of their direct and indirect negative impacts on both patients' health-related quality of life and costs to the health care system. Two minimally invasive surgical approaches were developed for the management of symptomatic VCFs: balloon kyphoplasty and vertebroplasty. The purpose of this study was to evaluate the effectiveness and safety of balloon kyphoplasty in the treatment of symptomatic VCFs.MethodsBetween July 2011 and June 2012, one hundred

Bone metastases are the most severe and prevalent consequences of prostate cancer (PC), affecting more than 80% of patients with advanced PC. PCBMs generate pain, pathological fractures, and paralysis. As modern therapies increase survival, more patients are suffering from these catastrophic consequences. Radiographically, PCBMs are predominantly osteosclerotic, but the mechanisms of abnormal bone formation and how this pathological increase in bone density is related to fractures are unclear. In this study, we conducted a comprehensive analysis on a cohort of 76 cadaveric PCBM specimens and 1

Metastases to the upper cervical spine are uncommon, and subsequent instability is rare. We report the details of a patient with metastatic breast cancer to the axis manifesting as a pathologic fracture and C1/C2 instability. This was treated by preoperative reduction to appropriate alignment with use of crown halo traction, followed by posterior occipitocervical fusion and postoperative radiation therapy.
