Foot Malalignment and Proximal Fifth Metatarsal Fractures.

BACKGROUND: Proximal fifth metatarsal fractures are common injuries that are classified into 3 zones according to their anatomical localization. While zone 1 and 2 fractures typically are traumatic, zone 3 fractures may be linked to foot alignment abnormalities, such as hindfoot varus and metatarsus adductus. The aim of the study was to explore the association between hindfoot alignment and different fracture zones, as well as the relationship between traumatic and atraumatic fracture origin and foot alignment. METHODS: We conducted a retrospective cohort study of patients with proximal fifth

Screw Fixation Without Bone Grafting for Delayed Unions and Nonunions of Minimally Displaced Scaphoids.

Delayed unions and nonunions of the scaphoid are most often treated by open reduction and internal fixation with bone grafting. We sought to evaluate a large consecutive series of nondisplaced or minimally displaced scaphoid nonunions and delayed unions treated by a compression screw without bone grafting by 2 fellowship trained hand surgeons. A total of 23 patients (19 males, 4 females) were identified who had fractures located at the distal third (2), the waist (18), and the proximal third (3). Of the 23 patients, 19 had a complete follow-up (mean follow-up period, 5.2 months) with evidence

Wrist pain in adult athletes.

As more and more adults become physically active, primary care physicians are seeing an increasing number of wrist injuries. Effective treatment is based on (1) accurate diagnosis, (2) maintenance of bone and joint-surface alignment until healing of bone, soft tissue, or both occurs, and (3) institution of an appropriate rehabilitation program. After symptoms have subsided, patients can gradually return to athletic participation. The long-term sequelae of wrist injuries are yet to be determined. If it is assumed that such injury causes a form of joint trauma, then analogies may be drawn to oth

TomoFix: a new LCP-concept for open wedge osteotomy of the medial proximal tibia--early results in 92 cases.

High Tibial Osteotomy (HTO) is an established treatment for unicompartmental osteoarthritis of the knee with malalignment. The classic procedure for correcting varus deformity is the lateral closed wedge osteotomy of the tibia with osteotomy of the fibula. The disadvantages of this technique are well known. Open wedge osteotomy from the medial side eliminates the risk of compartment syndrome and peroneal nerve injuries. A new fixation device (TomoFix) with an adapted surgical technique allows stable fixation of the osteotomy without the need to fill the osteotomy gap with bone grafts. In a pro

Correction of Tetratorsional Malalignment of the Lower Extremities Improves Patient-Reported Outcomes.

BACKGROUND: Axial malalignment of the bilateral femurs and tibias, previously known as "miserable" malalignment, now renamed tetratorsional malalignment (TTM), presents with hip and/or knee pain refractory to nonoperative treatment. PURPOSE: We sought to investigate whether bilateral rotational osteotomy of the femur and tibia leads to improvement in a deformity-specific patient-reported outcome measure (PROM). METHODS: A retrospective review of patients who underwent staged rotational correction of the bilateral femur and tibias was performed. Computed tomography (CT) was used to measure th

[Fractures of the thoracic and lumbar spine].

Fractures of the thoracic and lumbar spine result from high velocity trauma, assuming bone density is normal. The main location of fractures is the thoracolumbar junction. Most injuries can be treated conservatively; however, patients transferred to hospitals and spine centers represent a preselection with more severe trauma and a higher incidence of operative treatment. There is a large variety of operative techniques that can be used, which can be principally differentiated by the approach: posterior or anterior. Dorsal approaches are differentiated by the instrumentation for spondylodesis a

Observer variability in measurements of carpal bone angles on lateral wrist radiographs.

Determinations of carpal bone angles are used in the clinical evaluation of carpal malalignment. Eleven frequently referred radiological measures in lateral projection of the wrists in 23 wrists were assessed using different definitions of axes. Interobserver- and intraobserver variations were calculated. The standard deviation of the interobserver variation ranged from 2.60 degrees to 18.15 degrees, and the intraobserver variation from 1.89 degrees to 4.66 degrees depending on the angles measured. The use of three angles for the least observer variability in assessment of carpal alignment is

[Focus on the shoulder : Precise diagnostics for complex pathological alterations].

The shoulder joint combines three main joints and two auxiliary joints, which enable a unique freedom of movement. Stability is primarily ensured by muscles and ligaments, making injuries, overuse or degenerative processes common causes of complaints. A systematic examination includes the anamnesis, inspection, palpation, range of motion assessment and functional tests. During the inspection the pattern of movement, muscle atrophy and malalignments are assessed. Bone and soft tissue structures are tested by palpation. Functional tests help to differentiate pathological alterations. A structure

Salvage of pilon fracture nonunion and infection with circular tensioned wire fixation.

Nonunion, malunion, and infection are complications encountered when treating fractures of the distal tibia extending into the joint surface. There is rank order of severity of those complications extending from a varus collapse of the medial column of the distal tibia with a reduced joint surface to an infected nonunion with destruction of the joint surface, segmental bone loss, and damaged soft tissue envelope. Malunion and malposition of nonunion of the distal tibia have a combination of angular deformity, translation, rotational malalignment, and shortening. These deformities require corre

Influence of the angle between tibial and prosthesis mechanical axes on tibial bone remodeling in total knee arthroplasty.

Osteoarthritis of the knee is one of the most common diseases that affect the quality of life in the elderly population, and Total Knee Arthroplasty is considered the only real treatment for it, and as with any other surgery, a suboptimal technique may lead to an undesirable outcome. This paper aims to investigate the effects of the angle between mechanical axes of the tibia and the implant on the bone remodeling process. A 3D model was reconstructed using CT images, which was then used in an ABAQUS model with a USDFLD subroutine to simulate bone remodeling post TKA. The USDFLD subroutine comp

Statistical shape modelling provides a responsive measure of morphological change in knee osteoarthritis over 12 months.

OBJECTIVES: Responsive biomarkers are needed to assess the progression of OA and their lack has hampered previous clinical trials. Statistical shape modelling (SSM) from radiographic images identifies those at greatest risk of fast-progression or joint replacement, but its sensitivity to change has not previously been measured. This study evaluates the responsiveness of SSM in knee OA in a 12-month observational study. METHODS: A total of 109 people were recruited who had undergone knee radiographs in the previous 12 months, and were grouped based on severity of radiographic OA (Kellgren-Lawr

The importance of axial rotation of the lower limb : a systematic review of measurement methods.

AIMS: Assessment of lower limb alignment is a cornerstone of orthopaedics. Few studies look at rotational alignment in the axial plane as measured by femoral version (FV) and tibial torsion (TT), both of which have implications for hip, knee, and ankle pathology. This review provides an overview of the axial rotation and evaluates CT-based measurement methods for FV and TT to identify the most reliable and reproducible techniques for use in clinical practice. METHODS: A systematic PRISMA-guided review assessed original CT-based methods, examining inter- and intraobserver reliability (intracla

[Angular and torsional malalignments of the paediatric knee joint].

BACKGROUND: Abnormal angular and torsional alignment of the lower extremities during growth are a common reason for consultation of a paediatrician or orthopaedist. There is often uncertainty about the pathological value of these abnormalities and the need for treatment, even among specialists. In the course of physiological growth and development processes, the axial and torsional alignment of the legs sometimes change considerably and in characteristic patterns. DIAGNOSTICS: The ability to assess whether abnormal angular or torsional alignments of the lower limb in the course of growth shou

The Windswept Foot: Dealing with Metatarsus Adductus and Toe Valgus.

The windswept foot remains a reconstructive challenge. The hallux valgus associated with the medially displaced lesser metatarsal heads is hard to correct. Either the lesser metatarsal heads need to be displaced laterally or the deformity accepted. With the deformity, all the toes tend to be aligned into valgus with the position of the flexor and extensor tendons. Several treatment alternatives exist and may require a combination of open and percutaneous surgery. The authors think that, in severe metatarsus adductus, proximal correction of the first, second, and third metatarsals is required.
