Liver damage during organ donor procurement in donation after circulatory death compared with donation after brain death.

BACKGROUND: During the past decade the number of livers recovered and transplanted from donation after circulatory death (DCD) donors has increased significantly. As reported previously, injuries are more frequent during kidney procurement from DCD than from donation after brain death (DBD) donors. This aim of this study was to compare outcomes between DCD and DBD with respect to liver injuries. METHODS: Data on liver injuries in organs procured between 2000 and 2010 were obtained from the UK Transplant Registry. RESULTS: A total of 7146 livers were recovered from deceased donors during the

Metabonomics Analysis of Brain Stem Tissue in Rats with Primary Brain Stem Injury Caused Death.

OBJECTIVES: To explore the potential biomarkers for the diagnosis of primary brain stem injury (PBSI) by using metabonomics method to observe the changes of metabolites in rats with PBSI caused death. METHODS: PBSI, non-brain stem brain injury and decapitation rat models were established, and metabolic maps of brain stem were obtained by LC-MS metabonomics method and annotated to the HMDB database. Partial least square-discriminant analysis (PLS-DA) and random forest methods were used to screen potential biomarkers associated with PBSI diagnosis. RESULTS: Eighty-six potential metabolic marke

Predictive patterns of sensory evoked potentials in comatose brain injured patients evolving to brain death.

OBJECTIVE: To assess whether Somatosensory Evoked Potentials (SEPs), recorded within 24h after ICU admission, are reliable predictors of brain death (BD) in comatose patients with acquired brain injury of various aetiologies. METHODS: SEPs were classified as absent (A), pathological (P), and normal (N). Considering SEP recordings from both hemispheres, 6 patterns were identified: NN, NP, PP, NA, AP, and AA. The final endpoint was BD. RESULTS: Of the 203 patients included in the study, 70 (34%) evolved toward BD. The survival analysis indicated that the combination of SEP patterns in a two-gr

Integrated Necroptosis Within Programmed Cell Death in Traumatic Brain Injury.

Traumatic brain injury (TBI), a major global public health burden, is characterized by complex pathophysiology involving primary and secondary injuries, with programmed cell death (PCD) pathways significantly impacting neuronal damage and neurological dysfunction. Substantial evidence over time highlights necroptosis-a caspase-independent, inflammatory form of PCD mediated by RIPK1/RIPK3/MLKL-as a key driver of secondary injury after TBI. However, the dynamic interplay between necroptosis and other PCD pathways after TBI remains poorly understood. In this review, we systematically describe the

Fulminant neuropsychiatric lupus leading to brain death: a case report.

BACKGROUND: Neuropsychiatric systemic lupus erythematosus (NPSLE) represents a severe and heterogeneous manifestation of systemic lupus erythematosus (SLE), occasionally progressing to catastrophic outcomes. Although rare, fulminant forms with rapid neurological decline and brain death are exceptionally uncommon. CASE DESCRIPTION: We report a 21-year-old woman with an 8-year history of SLE (articular, cutaneous, renal, and hematologic involvement) and hypothyroidism, who presented with acute neuropsychiatric symptoms and rapidly deteriorated to coma and brain death in less than 72 hours. CDK3 induces neuronal death and brain atrophy in Alzheimer's disease.

Progressive neuronal loss and brain atrophy are principal determinants of cognitive decline in Alzheimer's disease (AD), yet most mouse models fail to recapitulate these features. Here we identify cyclin-dependent kinase 3 (CDK3) as a key driver of neurodegeneration in AD. CDK3 is elevated in human AD brains and correlates with disease severity. As laboratory mice carry a nonfunctional Cdk3 mutation, we generated two models with restored CDK3 activity and then crossed to AD backgrounds. Both models exhibited substantial neuronal loss, hippocampal atrophy, and cognitive deficits. Single-nucleus

Cannabidiol potentiates phenobarbital-induced cell death in the developing brain.

Epilepsy, affecting about 1 % of the global population, is more prevalent in children. The primary treatment is antiseizure medications (ASMs), with phenobarbital (PB) being the most common for pediatric cases. However, PB is effective in only two-thirds of patients and can cause side effects like cell death in developing brains. Early-life epilepsy treatment is particularly challenging, as many patients continue to experience poorly controlled seizures. Due to the limitations of current ASMs, cannabidiol (CBD) has emerged as a promising alternative, offering fewer side effects, neuroprotectiv

Intestinal microbiota dysbiosis and liver metabolomic changes during brain death.

BACKGROUND: Whether a causative link exists between brain death (BD) and intestinal microbiota dysbiosis is unclear, and the distortion in liver metabolism associated with BD requires further exploration. METHODS: A rat model of BD was constructed and sustained for 9 h (BD group, n=6). The sham group (n=6) underwent the same procedures, but the catheter was inserted into the epidural space without ballooning. Intestinal contents and portal vein plasma were collected for microbiota sequencing and microbial metabolite detection. Liver tissue was resected to investigate metabolic alterations, an

Cases Abusing Brain Death Definition in Organ Procurement in China.

Organ donation after brain death has been practiced in China since 2003 in the absence of brain death legislation. Similar to international standards, China's brain death diagnostic criteria include coma, absence of brainstem reflexes, and the lack of spontaneous respiration. The Chinese criteria require that the lack of spontaneous respiration must be verified with an apnea test by disconnecting the ventilator for 8 min to provoke spontaneous respiration. However, we have found publications in Chinese medical journals, in which the donors were declared to be brain dead, yet without an apnea t

Intensive care practices in brain death diagnosis and organ donation.

We conducted a multicentre study of 1844 patients from 42 Spanish intensive care units, and analysed the clinical characteristics of brain death, the use of ancillary testing, and the clinical decisions taken after the diagnosis of brain death. The main cause of brain death was intracerebral haemorrhage (769/1844, 42%), followed by traumatic brain injury (343/1844, 19%) and subarachnoid haemorrhage (257/1844, 14%). The diagnosis of brain death was made rapidly (50% in the first 24 h). Of those patients who went on to die, the Glasgow Coma Scale on admission was ≤ 8/15 in 1146/1261 (91%) of pat

Transcranial amelioration of inflammation and cell death after brain injury.

Traumatic brain injury (TBI) is increasingly appreciated to be highly prevalent and deleterious to neurological function. At present, no effective treatment options are available, and little is known about the complex cellular response to TBI during its acute phase. To gain insights into TBI pathogenesis, we developed a novel murine closed-skull brain injury model that mirrors some pathological features associated with mild TBI in humans and used long-term intravital microscopy to study the dynamics of the injury response from its inception. Here we demonstrate that acute brain injury induces

Diagnosing brain death by CT perfusion and multislice CT angiography.

INTRODUCTION: Although the diagnosis of brain death (BD) is usually based on clinical criteria, in sedated patients, ancillary techniques are needed. This study was designed to assess the accuracy of cerebral multislice computed tomographic angiography (CTA) and CT perfusion (CTP) in diagnosing BD. METHODS: Prospective observational study in 27 BD patients. RESULTS: All patients were diagnosed as BD based on clinical and electroencephalogram findings. After BD diagnosis, CTP was performed followed by 64-detector multislice CTA from the aortic arch to the vertex. Images were reconstructed fro

Transcranial Doppler ultrasonography to confirm brain death: a meta-analysis.

OBJECTIVE: Barbiturate therapy or hypothermia precludes proper diagnosis of brain death either clinically or by EEG. Specific intracranial flow patterns indicating cerebral circulatory arrest (CCA) can be visualized by transcranial Doppler ultrasonography (TCD). The aim of this study was to assess the validity of TCD in confirming brain death. DESIGN: Meta-analysis of studies assessing the validity of TCD in confirming brain death. METHODS: A systematic review of articles in English on the diagnosis brain death by TCD, published between 1980 and 2004, was performed. An oscillating or reverbe

Variability among hospital policies for determining brain death in adults.

OBJECTIVE: In the absence of federal requirements or state statutes, criteria to certify brain death are specified by medical staff and administrative policies in individual hospitals. Variability among such policies may allow inconsistency in the declaration of death by neurologic criteria. Our intent was to partially quantify diversity among hospital standards used in brain death certification. DESIGN: Survey. SETTING: Six hundred randomly selected hospitals. PATIENTS: None. INTERVENTIONS: A survey was conducted of 600 hospitals randomly selected from the American Hospital Association re

Fetal brain death syndrome--a case report and literature review.

This case describes a new feature of fetal brain death syndrome, abnormal movements mimicking fetal convulsions being subsequently found to be decerebrate hypertonicity in a brain-dead fetus. It also confirms the diagnostic criteria of fetal brain death, both clinical and ultrasonic. The development of polyhydramnios both prior to and after the presumed neurological event is suggested as an association with the diagnosis of fetal brain death. Increased awareness of this event and the heterogeneity of the presentation may prevent further unnecessary Caesarean sections, as to date only 4 of the

Evidence of brain ischemia in early neonatal sudden death syndrome.

Ten early neonatal sudden death victims (ENSD) were selected for neuropathological and immunohistochemical examinations. The gestational ages of the victims ranged from 36 to 42 weeks, and the postnatal ages from 8 hours to 10 days of life. In 6 of 10 patients, softening with rarefaction was observed in the subcortical or intermediate white matter, which was associated with minimal astrogliosis in the whole white matter. In 7 of the 10 ENSD victims, the number of GFAP-positive glia was significantly greater in both the deep and subcortical white matter than in controls. The brainstem showed mi
