Sepsis in burn care: incidence and outcomes.

BACKGROUND: Although sepsis is known to be the leading cause of morbidity and mortality in adult burn patients, its epidemiology and impact are poorly understood. This study aims to address these gaps by further characterizing predictors of sepsis and comparing outcomes between septic and non-septic burn patients in different age groups. METHODS: We included patients (≥ 18 years) with thermal burn injuries ≥ 5% total body surface area (TBSA) admitted to two burn centers between 1 January 2006 and 30 June 2021, and 1 January 2023 and 6 April 2025. Patients were stratified by age into adults (1

Clinical practice variation in acute severe burn injury.

The New South Wales (NSW) Statewide Burn Injury Service Database was reviewed to identify variations in clinical practice with respect to care of severely burn-injured patients in intensive care. We compared differences in practice relating to duration of endotracheal intubation and surgical grafting. In this retrospective observational study, we reviewed all intensive care unit (ICU) admissions to the two NSW adult burns centres, ICU A and ICU B, between January 2008 and December 2015. Data were analysed for association between duration of intubation and outcome. There were 855 admissions to

Microbial contamination in burn patients undergoing urgent intubation as part of their early airway management.

Inhalation injuries occur in approximately one third of all major burns and account for a significant number of deaths in burn patients each year. Previous studies have examined ventilator-associated pneumonia in patients with inhalation injury, but no study to date has evaluated the incidence of bacterial contamination of the airways on admission in patients with inhalation injuries. Because pulmonary complications have been found to cause or directly contribute to mortality in as high as 77% of patients, with combined inhalation injury and thermal injury, early detection of community-acquire

Predicting mortality from burns: the need for age-group specific models.

Traditional burn mortality models are derived using all age groups. We hypothesized that age variably impacts mortality after burn and that age-specific models for children, adults, and seniors will more accurately predict mortality than an all-ages model. We audited data from the American Burn Association (ABA) National Burn Repository (NBR) from 2000 to 2009 and used mixed effect logistic regression models to assess the influence of age, total body surface area (TBSA) burn, and inhalation injury on mortality. Mortality models were constructed for all ages and age-specific models: children (<

Characteristics and outcome of children with carbon monoxide poisoning with and without smoke exposure referred for hyperbaric oxygen therapy.

OBJECTIVES: To describe the clinical characteristics and outcome of children with carbon monoxide (CO) poisoning with and without smoke exposure referred for hyperbaric oxygen therapy (HBOT), and to determine the association between any of these characteristics and death. SETTING: Regional hyperbaric referral center. PATIENTS: The medical records of 150 children with CO poisoning (COP) who were treated with HBOT between August 92 and September 95 were reviewed. MEASUREMENTS/MAIN RESULTS: COP was defined as a history of probable exposure to CO, with either a carboxyhemoglobin level (COHb) >

Hydroxocobalamin is not associated with methemoglobinemia in patients with inhalation injury and suspected cyanide toxicity and a proposed algorithm for hydroxocobalamin administration.

BACKGROUND: Cyanide poisoning poses a significant threat to burn patients exposed to smoke in residential or workplace fires, leading to central nervous system dysfunction, hemodynamic instability, cardiovascular collapse, and death. Prompt administration of an effective antidote is critical. Hydroxocobalamin, a form of vitamin B12, is the gold standard treatment for cyanide toxicity, by binding to cyanide molecules and converting them into non-toxic cyanocobalamin that is eliminated by the kidneys. This mechanism is distinct from previous cyanide antidotes, which induce the formation of methe

The role of fibreoptic bronchoscopy in the management of respiratory burns.

We found that fibreoptic bronchoscopy was a useful, simple, safe and accurate method in the diagnosis of inhalation injury, determining the type, the site and the extent of damage sustained. The most common mucosal damage was erythema and oedema of the supraglottic and glottic tissues, though in a few cases more severe and extensive injury was seen. Death seemed to be determined largely by the extent of skin burn but since there did not appear to be a close relationship between the severity of skin burn and the presence of inhalation burn, we suggest that the two types of injuries be evaluated

Epidemiology of work-related burn injuries: A ten-year retrospective study of 429 patients at a referral burn centre in the north of Iran.

Work-related burns can have a destructive impact; however, knowledge of the epidemiology of work-related burn injuries in Iran is limited. This study aimed to describe epidemiological characteristics of work-related burn injuries in a burn centre in the north of Iran. This study was a retrospective single-centre study of the medical records of work-related burns between 2011 and 2020. Data collection was done using the hospital information system (HIS). The data were analysed by using descriptive statistical methods and SPSS 24.0 software. Of the 9220 cases treated in the burn centre, 429 (4.6

Invasive Aspergillus outbreak in inhalation injury: a case presentation and literature review.

BACKGROUND: Invasive pulmonary aspergillosis often occurs in patients with poor immune function, who abuse steroids or broad-spectrum antibiotics, or who use intravenous drugs. Among the Aspergillus genus of pulmonary infection, Aspergillus fumigatus is the most important pathogen, followed by Aspergillus flavus, Aspergillus niger, and Aspergillus terreus. Inhalation injury complicated by Aspergillus infection has atypical clinical manifestations. Diagnosis is difficult, and it is easy to make mistakes in treatment. Moreover, there are few cases of burn inhalation injury complicated with pulmo

[Fire at the Viennese hotel "Am Augarten"--a contribution to the toxicology of combustion gas poisoning].

The fire at the Vienna hotel "Am Augarten" (28.9 1979) caused 25 deaths. 24 victims died as a result of inhalation of fire gases; 10 of them also suffered 2nd and 3rd degree burns. In one case, death was due to internal injuries (sustained when jumping from a window). The fact that none of the bodies was charred facilitated identification. In all cases autopsy revealed the typical symptoms of inhalation of fire gases (soot deposition in the respiratory tract, presence of carboxyhaemoglobin in the blood). In public discussion the question was raised whether the inhalation of hydrogen cyanide (l

Circulating blood volume in burn resuscitation.

Circulating blood volume (CBV) was prospectively measured in patients with almost solely smoke inhalation injury (Group I: 10 patients) and in patients with severe cutaneous burn (Group B: 6 patients) consecutively until 96 hours after injury, to assess the effect of either injury on intravascular volume status by the pulse dye-densitometry method. All participants were treated by an ordinary fluid regimen based on the Parkland formula with an hourly urine output of 1.0 to 2.0 ml/kg for the resuscitation endpoint. CBV was also measured in 15 elective surgical patients for a control value (76.7

The usefulness of combined high-frequency percussive ventilation during acute respiratory failure after smoke inhalation.

Inhalation injury and bacterial pneumonia represent some of the most important causes of mortality in burn patients. We describe 11 severely burned patients with acute respiratory failure due to inhalation injury who did not respond adequately to conventional respiratory support. High-frequency percussive ventilation (HFPV) is a recent ventilatory mode, which combines the advantages of conventional ventilation with some of those of high-frequency ventilation. Seven patients developed pulmonary infection during the acute phase; one patient died of multiple organ failure on day 25. All the other

[Clinical and pathophysiologic problems associated with smoke inhalation injury].

Smoke inhalation injury is one of the primary determinants of survival following major burn injury. The primary site of injury in smoke inhalation appears to be the small airway rather than the alveoli, and thus small airway occlusion caused by edema and pseudomembrane formation are the primary mechanisms of progressive hypoxia. Ventilation-perfusion (VA/Q) alterations after smoke inhalation are characterized by increased blood flow to low VA/Q compartments, although an increase in true shunt (VA/Q = 0) was not a consistent finding. This differs considerably from most adult respiratory distres

[Smoke poisoning].

UNLABELLED: SYSTEMIC AND LOCAL EFFECT: Smoke inhalation causes systemic and local, mainly respiratory, toxicity due to the asphyxiant and irritant properties of toxic gases. OXYGEN DEPRIVATION AND INTOXICATION: The syndrome of oxygen deprivation and intoxication by asphyxiant gases is caused by combustion-induced oxygen deprivation and exposure to carbon monoxide (CO) and cyanide (CN), but also to other toxic gases. A loss of consciousness is a good sign of systemic toxicity; however, the respective role of CO, CN and other toxic gases cannot be determined. The presence of apnea, lactic acido

Microalbuminuria: a marker of endothelial dysfunction in thermal injury.

INTRODUCTION: Systemic endothelial dysfunction (SED) and capillary leak occur following severe burn. SED can be assessed as low-level albuminuria (microalbuminuria) detectable only by sensitive immunoassay. This study compared the magnitude and duration of microalbuminuria with burn surface area and associated aggravating factors. METHODS: Serial urine specimens were collected from 2 to 36 h after injury from 43 adult burn patients with a mean total body surface area (TBSA) of 32% (range 15-68%) and during 44 episodes of wound manipulation within the same period. Urinary albumin was expressed

Initial management of patients with burns and combined injuries for acute care surgeons: What you need to know.

There are nearly 700,000 adults and children with burn injuries who seek care in America each year. While most patients with major burn injuries are cared for at American Burn Association-verified burn centers, many of them present to nonburn centers initially or present with combined burns and other injuries. Despite this, burn surgery is no longer mandated by the Accreditation Council for Graduate Medical Education for general surgery residency and is rarely a meaningful component of surgical critical care fellowships. However, general surgeons are called to participate in and, occasionally,

Combined Extracorporeal Carbon Dioxide Removal (ECCO2R) With Renal Replacement Therapy and Blood Purification in a Severe Inhalation Injury Patient-Case Report.

Severe burns complicated by inhalational trauma represent a clinically challenging combination, particularly in elderly patients with multiple comorbidities. The following case report highlights the successful management of an elderly patient with second-degree burns and inhalational trauma following a residential fire. Given the severity of the clinical presentation, a combined approach incorporating extracorporeal CO2 removal (ECCO2R) to allow for early protective ventilation, continuous renal replacement therapy for precise hydroelectrolytic management, and blood purification for early immu
