Management of acute bursitis: outcome study of a structured approach.

In patients with septic bursitis the indications for admission and surgical intervention remain unclear, and practice has varied widely. The effectiveness of a conservative outpatient based approach was assessed by an outcome study in a prospective case series. Consecutive patients attending an emergency department with acute swelling of the olecranon or prepatellar bursa were managed according to a structured approach, subjective and objective outcomes being assessed after two to three days, and subsequently as required until clinical discharge. Long-term outcomes were assessed by telephone f

Olecranon and pretibial bursitis in atopic dermatitis: coincidence or association?

BACKGROUND: Bursal fluid accumulations are not usually associated with skin disease; however, several cases have been observed in a large atopic dermatitis (AD) clinic. OBJECTIVE: We sought to characterize the clinical features of bursitis in AD and to assess the evidence for a mechanistic association between these conditions. METHODS: We retrospectively evaluated the clinical presentation of bursitis in six adults with lifelong severe AD. The course of the bursitis and possible causative factors were assessed for each patient. RESULTS: Ten episodes of bursitis were documented in six patien

Use of contrast radiography in the investigation of tenosynovitis and bursitis in horses.

Contrast radiography, using a 25 per cent solution of sodium diatrizoate, has been used for the investigation of tenosynovitis and bursitis in horses. The procedure was undertaken on a series of 32 clinical cases and on specimens obtained at autopsy. Lesions affecting the extensor tendon sheaths at the carpus, the digital sheath, the tarsal sheath and the bursae at the elbow, hock and fetlock were examined by this means. The results showed the procedure offered the clinician useful information about the nature of the interior of these structures, particularly as regards the presence of adhesio

Septic and Aseptic Iliopsoas Bursitis - A Systematic Review of the Literature.

Iliopsoas bursitis (IB) is a rare entity and usually presents with ambiguous clinical presentation similar to other musculoskeletal conditions in the anatomic area. In septic IB, patients usually present with vague complaints of hip pain and fever. To better understand this vague clinical entity, a systematic review of cases of iliopsoas bursitis published in the open literature was performed to better characterize the clinical presentation, demographics and clinical outcome in patients with IB. An analysis of 50 articles and 54 patients including our index case revealed that hip pain was the m

Hamular bursitis as a cause of orofacial pain. A case report.

Pterygoid hamular bursitis is an infrequent cause of orofacial pain due to a hypertrophic pterygoid hamulus. The pain is often referred to the pharynx, temporomandibular region and facial zone, and requires a differential diagnosis with other craniofacial entities. This paper describes a patient with pterygoid hamular bursitis that presented pain of neuropathic characteristics in the left retromolar region, associated with odynophagia and temporomandibular joint disorders. Based on the clinical and radiological findings, a surgical resection of the pterygoid hamulus was decided. After the surg

Endoscopic Resection of Recalcitrant Tuberculous Bursitis of the Popliteal Fossa.

Tuberculosis of the musculoskeletal system is uncommon and constitutes 1% to 5% of all forms of tuberculosis. In total, 30% of skeletal tuberculosis involves the joints, the knee being the third most commonly affected after the spine and the hip. Knee tuberculosis commonly presents as synovitis or arthritis, and infected Baker's cyst is a rather rare presentation. It is believed to result from the propagation of tuberculosis of the knee joint into the cyst, as approximately one half of the Baker's cyst communicates with the knee joint and it appears as swelling in the medial aspect of the popl

Management of septic and aseptic prepatellar bursitis: a systematic review.

BACKGROUND: Despite contributing to significant morbidity in working-age adults, there is no consensus on the optimal treatment for prepatellar bursitis. Much of the existing literature combines prepatellar and olecranon bursitis. This systematic review aims to determine the optimal management of prepatellar bursitis. STUDY DESIGN AND METHODS: A primary search of electronic published and unpublished literature databases from inception to November 2019 was completed. Articles over 25 years old, case reports with less than four patients, paediatric studies, and non-English language papers were

Medial collateral ligament bursitis in a patient with knee osteoarthritis.

BACKGROUND: Medial collateral ligament (MCL) bursitis has been described as a distended and inflamed bursa between the superficial and deep portions of the MCL. It is a rare but important cause of medial knee pain. CASE DESCRIPTION: A 65-year-old woman with knee osteoarthritis (OA) presented with severe pain and swelling in her left knee. She did not have a traumatic injury. After a clinical examination, a slight decrease in the range of motion of the left knee, and a painful swelling and tenderness over the medial side of the knee and proximal tibia were detected. The pain was exacerbated by

[Current treatment concepts for olecranon and prepatellar bursitis in Austria].

BACKGROUND: The limited evidence available on the diagnosis and treatment of olecranon and prepatellar bursitis indicates nationally varying treatment approaches. Therefore the aim of this study was to survey the current treatment concepts of olecranon and prepatellar bursitis in Austria. MATERIAL AND METHODS: An online questionnaire comprising of demographic data, questions regarding diagnostics and differentiation between septic bursitis (SB) and non-septic bursitis (NSB) as well as two case reports for therapy appraisal were sent to members of the Austrian Society of Orthopaedics and Ortho

[Trochanteric bursitis due to tuberculosis in an immunocompetent young woman].

Soft tissue infection due to Mycobacterium tuberculosis can affect muscle, tendons, fascia, bursa and synovial tissue. Tuberculous trochanteric bursitis is a rare entity that usually affects immunocompromised patients. Manifestations usually occur insidiously, which delays diagnosis and treatment. We present the case of an immunocompetent young woman who came to our department for chronic left hip pain. The study confirms the diagnosis of tuberculous trochanteric bursitis. This case demonstrates the importance of considering a possible infectious origin of bursitis in immunocompetent patients.

A cluster of septic olecranon bursitis in association with infantry training.

Septic olecranon bursitis (OB) is caused primarily by Staphylococcus aureus (SA) usually in association with occupations involving trauma to the elbows. An outbreak of septic OB was identified in an infantry platoon. The severity of the injury to the skin overlying the elbows was scored and compared to a parallel platoon of the same unit. All soldiers were tested for SA carriage and pulse field gel electrophoresis (PFGE) was performed on available isolates. Nine cases of septic OB were identified only in platoon A. The significant risk factor for septic OB was a moderately or severely injured

Avascular necrosis of the femoral head presenting as trochanteric bursitis.

Five patients are described with avascular necrosis of the femoral head who presented with ipsilateral trochanteric bursitis, in the absence of clearcut hip joint disease. Avascular necrosis was indicated by magnetic resonance imaging. It is suggested that clinical trochanteric bursitis, especially when refractory to local corticosteroid treatment, may be the initial sign of hip disease. In the patient with risk factor(s) for avascular necrosis that diagnosis should be considered and evaluated with appropriate studies, such as magnetic resonance imaging, to prevent weight bearing at an early s

Septic and aseptic olecranon bursitis in patients on maintenance hemodialysis.

Over a period of four years, seven of 110 maintenance hemodialysis patients developed olecranon bursitis. In all patients it appeared in the arm on which AV access was constructed but one patient also had bilateral bursitis. None of the patients gave a history of trauma or gout. Despite similar clinical presentation, culture of bursal aspirate demonstrated septic bursitis in two patients. (One diabetic, the other on steroids). Sustained pressure on the olecranon bursa during maintenance hemodialysis and/or milk trauma in the anticoagulated state may be responsible for the lesion. Infection can
