A simple method for the continuous noninvasive estimate of cardiac output using the Maxima breathing system. A pilot study.

The Maxima is a new breathing system which makes it possible to monitor cardiac output (Q) noninvasively and to follow trends continuously. When used as a rebreathing system in controlled ventilation, the Maxima selectively eliminates alveolar gas and no fresh gas (VF) is eliminated. Without the need for a mixing chamber, eliminated CO2 (FeCO2) may be measured directly. With steady state conditions, and assuming a respiratory quotient of 1, carbon dioxide production (VCO2) = VF x FeCO2. The indirect Fick principle applied to CO2 exchange (Q = VCO2/ venous-to-arterial CO2 content difference (Ca

Tetany During Intravenous Conscious Sedation in Dentistry Resulting From Hyperventilation-Induced Hypocapnia.

Hyperventilation can be a manifestation of anxiety that involves abnormally fast breathing (tachypnea) and an elevated minute ventilation that exceeds metabolic demand. This report describes a case of hyperventilation-induced hypocapnia resulting in tetany in a 16-year-old girl undergoing orthodontic extractions under intravenous conscious sedation. Pulse oximetry is the gold standard respiratory-related index in conscious sedation. Although the parameter has great utility in determining oxygen desaturation, it provides no additional information on respiratory function, including, for example,

[Anesthesia accidents: accidental esophageal intubation].

Accidental oesophageal intubation is a common mistake in inexperienced anaesthetists, but unrecognized oesophageal intubation is fortunately a rare event because, in anaesthetic malpractice claims, it frequently resulted in death or brain damage. The connection of these complications with the lack of experience of the anaesthetist and/or the difficult intubation is not so evident in the literature. The precocious detection of tube dislocation depends on the systematic verification of endotracheal tube position after insertion. The paper describes the clinical and instrumental tests used for de

[Successful use of propofol for anesthesia for plasty of eyelids in epidermolysis bullosa].

A 21-year-old male with recessive dystrophic epidermolysis, required anesthesia for plastic surgery of eyelids. We took special care for the attachment of monitoring equipment and placing an intravenous cannula. Anesthesia was induced by intravenous propofol 1 mg.kg-1 and maintained with local injection of 1.0% lidocaine and continuous infusion of propofol 3-4 mg.kg-1.hr-1. Tracheal intubation was not performed and oxygen was given through a nasal cannula and spontaneous breathing was maintained throughout the anesthetic management. Respiratory condition was monitored by measuring Spo2 and end

The use of bromothymol blue and sodium thiopentone to confirm tracheal intubation.

The possibility of using chemical changes to confirm correct tracheal tube placement was investigated with a view to their use in developing countries where more sophisticated methods are unavailable. The effect of bubbling expired gases through a 10% solution of bromothymol blue and a 0.25% solution of thiopentone led to chemical changes producing, in the case of bromothymol blue, a colour change and in that of thiopentone, precipitation, probably due to a change in pH caused by carbon dioxide. We also discovered that the time to precipitation of the thiopentone could be greatly reduced if it

[Respiratory arrest during sleep. Facts and hypothyses].

The writers analyze 29 nyctohemeral recordings in 20 patients with sleep apneas. In addition to the standard polygraphy, use is made of pneumotachography and capnography. The results (central apnea with open or secondarily closed glottis; obstructive apnea with closed glottis or with expiratory valve phenomenon) raise the hypothesis of a central physiopathological mechanism common to the various types of apneas through a more or less dissociated inhibition of the activity of the respiratory, laryngeal, intercostal, and diaphragmatic muscles:--in central apnea: inhibition of the thoracoabdomina

Anesthesia information system-assisted detection and effective management of sugammadex-induced anaphylaxis: a case report.

BACKGROUND: Sugammadex, a neuromuscular blockade reversal agent, has a reported incidence of severe anaphylaxis of approximately 0.005% in Japan, typically occurring within 5 min of administration. However, detailed case reports describing the full clinical course and associated changes in monitoring parameters—such as vital signs, capnographic waveforms, and airway pressure—remain limited. CASE PRESENTATION: We report the case of an 80-year-old man who underwent laparoscopic partial hepatectomy under combined general and epidural anesthesia. Postoperatively, 200 mg of sugammadex was administe

Clinical efficacy of COMBO endoscopy of oropharyngeal airways in elderly patients undergoing painless gastroenteroscopy under anaesthesia: a randomized controlled trial protocol.

BACKGROUND: Respiratory depression and airway obstruction are the most common respiratory adverse reactions in elderly patients undergoing gastroenteroscopy under sedation. Additionally, sharing the airway with endoscopists can complicate airway management. Here, we plan to apply COMBO (Capnography Monitoring Bite Block Oxygenation) endoscopy of the oropharyngeal airway, a novel airway management device with integrated capnography monitoring and dual oxygenation support, to investigate the incidence of adverse events such as hypoxaemia and airway obstruction in elderly patients during anaesthe

Assessment of Bohr and Enghoff Dead Space Equations in Mechanically Ventilated Children.

BACKGROUND: Recent findings suggest that using alveolar PCO2 (PACO2 ) estimated by volumetric capnography in the Bohr equation instead of PaCO2 (Enghoff modification) could be appropriate for the calculation of physiological dead space to tidal volume ratio (VD/VT Bohr and VD/VT Enghoff, respectively). We aimed to describe the relationship between these 2 measurements in mechanically ventilated children and their significance in cases of ARDS. METHODS: From June 2013 to December 2013, mechanically ventilated children with various respiratory conditions were included in this study. Demographic

PRODIGY score predicts respiratory depression in the post-anesthesia care unit: A post-hoc analysis.

Surgical patients who experience respiratory depressive episodes (RDEs) during their post-anesthesia care unit (PACU) admission are at a higher risk of developing subsequent respiratory complications in general care wards. A risk assessment tool for PACU RDEs has not been previously assessed. The PRediction of Opioid-induced respiratory Depression In patients monitored by capnoGraphY (PRODIGY) score is an assessment tool that uses baseline patient variables to categorize patients into low, intermediate, or high risk groups for RDEs in general care wards. This study assessed whether PRODIGY gro

Hypoxic burden and sleep hypoventilation in obese patients.

INTRODUCTION: Novel biomarkers of hypoxic load have emerged, as sleep apnea-specific hypoxic burden which provides more precise assessment of intermittent hypoxemia severity. Our main objective was to assess the potential benefit of hypoxic burden to identify obesity-related sleep hypoventilation. We hypothesized that hypoxic burden may help diagnose obesity-related sleep hypoventilation better than usual sleep respiratory measures (i.e., apnea-hypopnea index (AHI), mean SpO2, time with SpO2 < 90 %). METHODS: This retrospective study was conducted from June 2022 to October 2023 at the Univers

Duration of prone position sessions: a prospective cohort study.

BACKGROUND: Prone position (PP) is highly recommended in moderate-to-severe ARDS. However, the optimal duration of PP sessions remains unclear. We searched to evaluate the time required to obtain the maximum physiological effect, and to search for parameters related to patient survival in PP. METHODS AND RESULTS: It was a prospective, monocentric, physiological study. We included in the study all prone-positioned patients in our ICU between June 2016 and January 2018. Pulmonary mechanics, data from volumetric capnography and arterial blood gas were recorded before prone positioning, 2 h after

[Airway management in preclinical emergency anesthesia with respect to specialty and education].

BACKGROUND AND OBJECTIVE: Difficult airway management is a key skill in preclinical emergency medicine. A lower rate of subjective difficult airways and an increased success rate of endotracheal intubation have been reported for highly trained emergency physicians. The aim of this study was therefore to analyze the effect for different specialists and the individual state of training in the German emergency medical system. MATERIAL AND METHODS: In a retrospective register analysis of 6024 preclinical anesthesia procedures, the frequencies of airway devices, neuromuscular blocking agents, capn

Comparing the Effects of Two Different Levels of Hyperoxygenation on Gas Exchange During Open Endotracheal Suctioning: A Randomized Crossover Study.

BACKGROUND: Endotracheal suctioning is required for mechanically ventilated patients to maintain a patent airway. Studies show that open endotracheal suctioning affects respiratory mechanics and gas exchange. The aim of this study was to compare the effectiveness of hyperoxygenation with FIO2 + 0.20 above baseline and hyperoxygenation with FIO2 1.0 in preventing hypoxemia, and to determine the impact of open endotracheal suctioning on the pulmonary ventilation of critical subjects receiving mechanical ventilatory support. Detection of hypoventilation during deep sedation in patients undergoing ambulatory gynaecological hysteroscopy: a comparison between transcutaneous and nasal end-tidal carbon dioxide measurements.

BACKGROUND: Transcutaneous measurement of carbon dioxide (Tc(co(2))) provides a non-invasive estimation of arterial carbon dioxide (Pa(co(2))). Nasal capnography (Pe'(co(2))) is used to assess ventilation during monitored anaesthesia care (MAC) with sedation since it can readily detect apnoea. We compared the agreement between Tc(co(2)) and Pe'(co(2)) with Pa(co(2)) and the ability to detect hypercarbia in patients under deep sedation. METHODS: Forty healthy female subjects receiving deep sedation for hysteroscopy were studied. A Tc(co(2)) (TOSCA 500, Radiometer, Inc., Westlake, OH, USA) elec

[Out-of-hospital cardiopulmonary resuscitation : when and how should it be stopped ?].

Managing out-of-hospital cardiac arrest involves significant challenges, including the decision to discontinue resuscitation efforts, which requires a comprehensive assessment that takes into account the clinical history, prognostic factors, ethical considerations and the patient's wishes, as conveyed by their healthcare representative. The presence of clear signs of death or valid advance directives results in therapeutic abstention. The withdrawal of resuscitation efforts is based on the absence of spontaneous circulation, of a shockable rhythm and of any reversible causes. The role of capno

Inability to detect unintentional endobronchial intubation by conventional signs and its detection by the radiologist during fluoroscopy for cerebral aneurysm coiling.

It is routine practice to auscultate the chest for equal bilateral breath sounds after tracheal intubation to verify that the tracheal tube tip is properly positioned. Signs of bronchial intubation also include desaturation, an increase or decrease in end-tidal carbon dioxide, and an increase in peak airway pressures. We report a case in which, during the course of endovascular coiling of a cerebral aneurysm during general anesthesia, desaturation occurred despite seemingly equal air entry on chest auscultation. The clinical picture was confused further by the presence of normal airway pressur
