Thrombolytic therapy in acute cerebral infarction complicating diagnostic cardiac catheterization.

Diagnostic and interventional percutaneous coronary catheterization is associated with stroke. Many of such strokes are asymptomatic, but some are devastating. Once the diagnosis of acute cerebral infarction is confirmed, thrombolytic therapy should be administrated within the time window of 3 hours. We report a 61-year-old woman who suffered from an acute cerebral infarction during diagnostic cardiac catheterization for unstable angina, which manifested as sudden onset of global aphasia, right hemiplegia and gaze preponderance to the left side. Computed tomography of the head performed immedi

Vascular injury following cardiac catheterization, coronary angiography, and coronary angioplasty.

All vascular injuries occurring at this hospital department over a 5-year period (1987-91) as a result of cardiac catheterization, coronary angiography, or coronary angioplasty (PTCA) and requiring transfusion, surgical consultation, or repair, are reviewed. Such complications may occur late and, to detect cases not apparent from the protocol accompanying every examination, a questionnaire was sent to all surgical clinics in the region asking for details of vascular surgical intervention after angiography. The present review of 4879 examinations disclosed 18 patients with 19 vascular injuries

Retrograde left atrial catheterization with a new steerable cardiac catheter.

A new type of guiding catheter that may be used for retrograde transmitral catheterization of the left atrium is described. The catheter has a 9F diameter and incorporates a steering arm by means of which the catheter tip may be made to curve through an arc of 0 to 180 degrees, entirely through external manipulation. In this way the tip of the guiding catheter may be positioned below the mitral valve orifice and used for the introduction of a smaller catheter into the left atrial cavity. We tested the system in 20 patients, eight of whom had mitral stenosis. Left atrial catheterization was suc

Iatrogenic central retinal artery embolization: a complication of cardiac catheterization.

A 57-year-old man with a myocardial infarction was subjected to cardiac catheterization as a follow-up study for his heart problem. The catheter, made for one time use only, was reautoclaved and used sometime before catheterizing this man. A small fragment of this catheter dislodged while guiding the parent catheter into coronary vessels with a metallic guide-wire. The fragment traveled getting lodged in the central retinal artery of the right eye leaving the eye with only light perception vision. The catheter fragment could be seen in the central retinal artery at the optic nerve head.

Routine surgical management of brachial artery occlusion after cardiac catheterization.

From 1965 through 1980, 1,108 patients (1.5%) underwent thrombectomy and local arterial reconstruction because of brachial artery occlusion after a total of 73,750 cardiac catheterization procedures performed at the Cleveland Clinic. In a study group of 100 consecutive patients, 91 had simple arteriotomy revision and nine had segmental arterial resection with either axial reanastomosis or a saphenous vein interposition graft. Thirteen patients sustained early recurrent thrombosis after limited arteriotomy revisions, and 11 of these had successful reoperations. Ninety-eight patients were discha

Biomarker-based prediction of radial artery occlusion after cardiac catheterization.

Thrombotic occlusion of the radial artery is the most frequent complication of transradial angiography. This study sought to investigate biomarker-based predictors of radial artery occlusion (RAO) after coronary angiography (CAG) and/or percutaneous coronary intervention (PCI). Consecutive patients who underwent cardiac catheterization through radial artery route for diagnostic or therapeutic purposes were included in the study retrospectively. The clinical, laboratory, and angiographic data were obtained from hospital records. All patients were invited for a follow-up visit one week after dis

[Coronary angiography: From cardiac catheterization to advanced interventional cardiovascular imaging].

Coronary angiography has evolved from the first cardiac catheterization performed by Werner Forssmann into a highly precise imaging modality of the coronary arteries. Modern techniques such as intravascular ultrasound, optical coherence tomography, and physiological measurements including FFR and iFR enable combined structural and functional assessment of coronary stenoses. At the same time, non-invasive coronary computed tomography, particularly with photon-counting technology, is gaining importance for diagnosis and procedural planning. Cardiovascular imaging is therefore becoming central to

Inferior Subtle ST Elevation. Would You Activate Cardiac Catheterization Laboratory?

BACKGROUND: According to guidelines, patients with inferior myocardial infarction only qualify for emergent reperfusion if they have at least 1 mm of ST elevation in two contiguous inferior leads. Although this has remained the standard for years, acute coronary occlusion may occur in patients with nondiagnostic ST elevation. Accordingly, a paradigm change is instigated, shifting the focus to physiopathology (occlusion myocardial infarction) rather than ST criteria. CASE REPORT: A middle-aged man presented to our emergency department with chest pain and subtle nondiagnostic electrocardiograph

Acetylcholine Use in Modern Cardiac Catheterization Laboratories: A Systematic Review.

BACKGROUND: The use of acetylcholine for the diagnosis of vasospastic angina is recommended by international guidelines. However, its intracoronary use is still off-label due to the absence of safety studies. We aimed to perform a systematic review of the literature to identify adverse events related to the intracoronary administration of acetylcholine for vasoreactivity testing to fill this gap. METHODS AND RESULTS: We conducted a systematic review of observational studies and randomized controlled trials dealing with the intracoronary administration of acetylcholine. Articles were searched

Role of Cross-Sectional Imaging in Pediatric Interventional Cardiac Catheterization.

Management of congenital heart disease (CHD) has recently increased utilization of cross-sectional imaging to plan percutaneous interventions. Cardiac computed tomography (CT) and cardiac magnetic resonance (CMR) imaging have become indispensable tools for pre-procedural planning prior to intervention in the pediatric cardiac catheterization lab. In this article, we review several common indications for referral and the impact of cross-sectional imaging on procedural planning, success, and patient surveillance.

Distal Transradial Access For Cardiac Catheterization in Patients Undergoing Hemodialysis.

OBJECTIVES: Although a distal radial artery (DRA) approach has recently been used in patients undergoing cardiac catheterization, no studies have so far investigated the safety and feasibility of DRA in patients undergoing hemodialysis (HD). We aimed to investigate the incidence of conventional radial artery (CRA) occlusion and puncture site complications after DRA puncture in patients undergoing HD. METHODS: We retrospectively analyzed the data of 117 consecutive patients with HD who underwent coronary angiography or percutaneous coronary intervention via a DRA approach at our institution fr

Double Vision: Isolated Third Cranial Nerve Palsy After Cardiac Catheterization.

Neurological complications after cardiac catheterization are rare. We report an unusual case of isolated third cranial nerve palsy in a 72-year-old male patient whose past medical history was significant for diabetes mellitus and coronary artery disease (CAD). He presented for elective cardiac catheterization for stable angina, which revealed multivessel CAD and no intervention was done. Two hours after the procedure, the patient suddenly started complaining of new-onset double vision in his left eye. Ophthalmologic exam revealed ptosis of the left eye lid, sluggish pupillary reflex and impair

Radial artery pseudoaneurysm after transradial cardiac catheterization: A case presentation.

Cardiac catheterization through the radial artery has reduced the percentage of complications seen previously in the femoral approach. Computed tomographic angiography is a noninvasive technique that can accurately diagnose pseudoaneurysms and other vascular pathologies. A 93-year-old female presented to the emergency department with pain and swelling of her right distal arm and wrist after a transradial cardiac catheterization procedure, as part of a transcatheter aortic valve replacement evaluation for severe aortic stenosis. Angiography is the standard diagnosing technique for aneurysms and

Operator-Directed Procedural Sedation in the Congenital Cardiac Catheterization Laboratory.

OBJECTIVES: The purpose of this study was to evaluate the association between the method of procedural sedation and outcomes for congenital cardiac catheterization procedures. BACKGROUND: The safety of operator-directed sedation (ODS) in the pediatric/congenital cardiac catheterization laboratory has been questioned. To our knowledge, the relative safety of ODS versus general anesthesia (GA) in these cases has not to date been critically evaluated. METHODS: A single-center retrospective cohort study was performed to compare the relative safety, cost, and times of catheterization procedures p

Contrast-Induced Nephropathy After Cardiac Catheterization: Culprits, Consequences and Predictors.

BACKGROUND: Contrast-induced nephropathy (CIN) is a common complication after radiocontrast exposure. METHODS: A retrospective medical record review of 513 hospitalized patients who underwent cardiac catheterization from June-December 2014 was done, of which 38 patients with end-stage renal disease and 57 patients without preprocedural creatinine were excluded. Serum creatinine concentration before the procedure and each day for 3 days after the procedure was recorded. CIN was defined as an increase in serum creatinine concentration by ≥25% or ≥0.5mg/dL from the preprocedural value within 72h

[Prolonged reversible cortical blindness over 5 days after cardiac catheterization].

We present a case of a 49-year-old man with angina pectoris. He developed transient cortical blindness (TCB) with focal neurological symptoms following coronary angiography. Two cranial magnet resonance studies within 3 days showed no morphological changes. Clinically the patient developed complex focal epilepsy, which was symptomatically treated. Under intensive monitoring, including hypertension control, the patient improved after 5 days of blindness with restoration of his vision on day 6. The exact mechanism of TCB is still speculative, but could be caused in this case by a toxic effect of
