Are we ready for a gender-specific approach in interventional cardiology?

Gender differences in the pathophysiology of atherosclerosis, cardiovascular risk factors, and diagnosis of coronary artery disease and valvular heart disease are well known. Such differences have also been outlined in the management and outcomes after acute coronary syndromes and valvular repair. Regarding the atherosclerotic process, pathological experimental studies suggest that plaque composition and burden may differ by gender. Female gender is associated with worse outcomes in the case of ischemic heart disease and, compared with men, women are less likely to undergo interventional cardi

Should an emergency physician be a "surgeon" in a rural area? A case of blunt cardiac rupture successfully treated by an emergency physician.

OBJECTIVE: Blunt cardiac rupture is a life-threatening injury that requires surgical repair by cardiovascular or trauma surgeons. We report a case of blunt cardiac rupture in a rural area in which emergency physicians performed emergency department thoracotomy and surgical repair to save the patient's life. PATIENT AND METHODS: This case involved an 18-year-old female who was injured in a traffic accident and underwent emergency thoracotomy and surgical repair. RESULTS: The patient's left thorax was deformed, and sonographic assessment revealed pericardial effusion. She experienced cardiopul

Noteworthy literature in cardiac surgery for 2014.

Year 2014 has provided insight into the optimal care of surgical patients with cardiovascular disease. In mitral disease, the management of degenerative mitral regurgitation and ischemic mitral insufficiency are moving in the opposite directions. In aortic disease, the description of arch procedures is becoming more definable for comparisons across approaches, while the treatment of acute type B dissections is becoming more aggressive, yet deliberate.

Classification of potential risk factors for trigeminocardiac reflex in craniomaxillofacial surgery.

PURPOSE: Trigeminocardiac reflex (TCR) in craniomaxillofacial surgery can lead to severely life-threatening situations. At least mild forms are probably much more common than the existing surgical literature suggests. Therefore, the aim of this presentation of cases and literature review was to evaluate the predisposing factors leading to a classification of risk factors for potential TCR and to give information concerning preventive measures and management procedures. PATIENTS AND METHODS: All surgery reports from the Department of Cranio-Maxillofacial and Oral Surgery in the University Hosp

Cardiovascular phenotypes of children and adolescents with Turner syndrome from a single-center cohort study.

BACKGROUND: Approximately half of the patients with Turner syndrome (TS) have congenital or acquired cardiovascular diseases. The objectives of this study were to improve the early diagnosis of TS and to predict the risk of severe cardiovascular diseases in patients with TS by analyzing the main features of cardiovascular diseases in these patients. METHODS: This study included 107 patients with TS who underwent echocardiography and were admitted to the Shanghai Children's Medical Center between November 2019 and November 2024. In this study, the height, weight, age, karyotype, cardiac imagin

Establishing a Standardized Renal Artery Ligation Procedure for Hypertension in Syrian Golden Hamsters.

Hypertension is considered a major global health threat, which can lead to severe complications, including vascular remodeling, heart failure, myocardial infarction, and end-stage renal disease. Syrian golden hamsters, a model with naturally expressing cholesteryl ester transfer protein (CETP), are susceptible to diet-induced hyperlipidemia. To establish a reliable and reproducible hamster model for studying the relationship between renal artery stenosis (RAS), secondary hypertension, and cardiovascular disease, the two-kidney, one-clip (2K1C) Goldblatt model was employed. This study details

Management dilemma of a child with long-segment tracheal stenosis, double outlet right ventricle, and left pulmonary artery sling: For single or staged repair?

This is a case of a 3-year-old child who presented with long-segment tracheal stenosis (LSTS) and complex congenital cardiovascular disease for surgical intervention. The ideal operation for this child will include slide tracheoplasty and multiple cardiac lesion repairs. However, the low birth weight and extremely young age present a dilemma as to whether to perform total repair simultaneously or in stages. Age and weight by which the first surgical procedure was done, cardiovascular abnormality details, preoperative respiratory support, preoperative tracheobronchomalacia, cardiopulmonary bypa

Periprocedural Complications in Adults with Mediastinal Masses.

OBJECTIVES: Respiratory and cardiovascular catastrophes are feared complications in patients with mediastinal masses undergoing anesthesia. Only a few of these events in the adult population have been noted in rare case reports, however. We aimed to investigate the complications related to surgical resection or biopsy involving mediastinal masses in clinical settings. METHODS: We performed a retrospective review of patients undergoing procedures related to their mediastinal mass and report on the perioperative outcomes. RESULTS: In a 10-year span, we identified 122 patients who underwent pro

Should valve replacement and related procedures be performed in elderly patients?

The cases of 135 consecutive elderly patients 70 years old or older who had valve replacement and related surgical procedures from October, 1977, through April, 1982, were reviewed. There were 75 men and 60 women. The mean left ventricular ejection fraction was 50.16 +/- 5%. The overall operative mortality was 8% (11 patients). The early operative deaths were related mainly to cardiac failure, low cardiac output, sepsis, and renal and multiorgan failure. To assess the operative risk, these 135 patients were compared with 312 younger patients (less than 70 years old) who had undergone similar p

Fluoroquinolones and surgical prophylaxis.

The objective of surgical prophylaxis is to prevent wound infections associated with surgery. The rates of wound infections vary according to the procedure: less than 3 infections per 100 for clean procedures; up to 4 per 100 for clean-contaminated procedures; and up to 9 per 100 for contaminated procedures. Surgical antimicrobial prophylaxis has been shown in many randomised clinical trials to reduce the incidence of postoperative wound infections. Such prophylaxis is actually recommended in many clean-contaminated and some clean procedures. Because of their antimicrobial, pharmacokinetic, an

Surgical intervention in children after heart transplantation.

One hundred sixty-eight children underwent heart transplantation from January 1989 to December 1992 at Loma Linda University Medical Center. Maintenance immunosuppression consisted of cyclosporine and azathioprine. Perioperatively, 19 patients required peritoneal dialysis, with a 42% hospital mortality rate. During a follow-up period ranging from 3 months to 4 years, major surgical procedures were performed on 72 patients (43%). Cardiovascular and thoracic interventions included balloon angioplasty of aortic coarctation (10 patients, 2 of whom later required surgical repair), permanent pacemak

[Experience with surgical therapy in snoring and sleep apnea].

Knowledge of the connection between habitual snoring diseases and obstructive sleep apnoea has stimulated research into sleep disturbance in patients at high risk of cardiovascular disease. Any disturbance of nasal ventilation leads to a higher resistance to breathing and may cause cardiovascular complications in the long run. Anatomical and functional conditions that lead to intermittent incomplete blockage or complete occlusion of the upper respiratory tract must be detected and surgically eliminated, if possible. Surgical procedures for treating snoring and obstructive sleep apnoea may be s

Cardiac operations for North American children with rheumatic diseases: 1985-2005.

Certain pediatric rheumatic diseases are known to affect the heart, sometimes requiring surgical intervention. The Pediatric Cardiac Care Consortium database was used to characterize cardiac surgical intervention among children with rheumatic diseases from 1985 to 2005. From this large database, the records for patients younger than 21 years who underwent cardiac surgery for any rheumatic disorder were extracted. The data collected included the type of procedure performed, the age at the time of the procedure, and the year the procedure was performed. The 261 pediatric patients identified unde

Medical management of acute type a aortic dissection in association with early open repair of acute limb ischemia may prevent aortic surgery.

BACKGROUND: Acute type A aortic dissection (AAAD) is a cardiovascular emergency with a high potential for death. Rapid surgical treatment is indicated to prevent fatal complications. Aggressive appropriate medical management starts at first suspicion and is essential to prevent exacerbation or rupture of the dissection. Despite improved surgical techniques, perioperative care and the development of specialized cardiovascular centers, mortality remains high. Organ ischemia is a catastrophic manifestation of aortic dissection, demanding acute surgical intervention in specialized cardiovascular c

Pancreaticoduodenectomy for biliary tract carcinoma with situs inversus totalis: difficulties and technical notes based on two cases.

Situs inversus totalis (SIT) denotes complete right-left inversion of the thoracic and abdominal viscera. Diagnosis and surgical procedures for abdominal pathology in patients with SIT are technically more complicated because of mirror-image transposition of the visceral organs. Moreover, SIT is commonly associated with cardiovascular and hepatobiliary malformations, which make hepatobiliary-pancreatic surgery difficult. Two cases of pancreaticoduodenectomy for biliary tract carcinoma in patients with SIT are presented. Both patients had an anomaly of the hepatic artery. Advanced diagnostic im

[Cardiovascular Surgery for Patients Under Immunosuppressive Therapy].

Immunosuppressive agents including steroids are generally given to patients with collagen disease or organ transplant recipients. Cardiovascular surgery for these patients can potentially associate with increased rate of postoperative infection or wound healing complications. Here, some key points for perioperative management in patients under immunosuppressive therapy are reviewed. Before an elective surgery, steroids need to be tapered down as much as possible, because even small amount of steroid can lead to adverse postoperative outcomes. Exploring the impact of the COVID-19 pandemic on provision of cardiology services: a scoping review.

The coronavirus disease-19 (COVID-19) pandemic has forced hospitals to prioritize COVID-19 patients, restrict resources, and cancel all non-urgent elective cardiac procedures. Clinical visits have only been facilitated for emergency purposes. Fewer patients have been admitted to the hospital for both ST-segment elevation myocardial infarctions (STEMI) and non-ST segment elevation myocardial infarctions (NSTEMI) and a profound decrease in heart failure services has been reported. A similar reduction in the patient presentation is seen for ischemic heart disease, decompensated heart failure, and
