Neurodevelopmental functioning in children with FAS, pFAS, and ARND.

OBJECTIVE: The purpose of this article is to compare the neurodevelopmental profiles of 78 foster and adopted children with fetal alcohol syndrome (FAS), partial FAS (pFAS), or alcohol-related neurodevelopmental disorder (ARND). METHOD: Seventy-eight foster and adopted children underwent a comprehensive diagnostic evaluation. By using criteria more stringent than those required by current guidelines, the children were placed in 1 of 3 diagnostic categories: FAS, pFAS, or ARND. Each child was evaluated across the domains of neuropsychological functioning most frequently affected by prenatal ex

Child Maltreatment Among Children in Out-of-Home Care: Secondary Analysis of NCANDS and AFCARS Data.

This study uses data from the National Child Abuse and Neglect Data System (NCANDS) and the Adoption and Foster Care Analysis and Reporting System (AFCARS) to examine the incidence of maltreatment among children in out-of-home care (OOHC), the nature of that maltreatment (neglect, physical abuse, sexual abuse, or emotional maltreatment), the source of maltreatment reports, and the relationship between maltreatment in OOHC and child characteristics (i.e., age, gender, race, ethnicity, and disability). We found that 6.5% of children in OOHC in a given reporting year were the subject of a child m

Bidirectional associations among maternal depressive symptoms, child social skills, and child executive function development using latent change scores.

This study examined the bidirectional associations among maternal depressive symptoms, child social skills, and child executive function between 3 (T1) and 4 years (T2) of child age. Participants included 1,292 families and children (49% girls; 41% Black) from the Family Life Project, living in rural, low-income areas of the United States. Using an autoregressive and cross-lagged latent change score model, we found significant bidirectional associations between maternal depressive symptoms and child social skills, such that lower maternal depressive symptoms at T1 predicted a greater increase

Perinatal substance abuse: the impact of reporting infants to child protective services.

OBJECTIVE: The purposes of this study were to follow the judicial placement of newborns with positive toxicology screening results and to determine how long such infants remained in foster care, separated from their mothers or other relatives, and the length of court dependency. We also determined the mothers' compliance with court orders, the availability and use of rehabilitative services, factors used by the court to determine the final disposition, and the eventual placement of the infants. METHODS: The cohort sample consisted of all infants from San Mateo County (CA) born at Stanford Uni

Primarily Null Associations Between the Nursing Child Assessment Feeding Scale and Maternal Feeding Behaviors, Infant Eating Behaviors, Diet, and Growth: A Prospective Cohort Study.

Background/Objectives: The predictive value of the Nursing Child Assessment Feeding Scale (NCAFS) for eating, diet, and growth outcomes among healthy infants is not well-described. Methods: In this prospective cohort study, healthy infants (n = 227) were assessed with the NCAFS at 0.5, 2, and 4 months and outcomes were measured concurrently and at 12 months. Maternal feeding and infant eating behaviors were observed during a separate feeding and assessed with the Infant Feeding Styles Questionnaire, Baby Eating Behavior Questionnaire, Children's Eating Behavior Questionnaire-Toddler, and a die

Through the nurse's lens: Bereavement photography and creative interventions in nursing care.

Art has long supported healing, and, in nursing, creative practices such as music, writing, and photography enhance holistic care by fostering connection, emotional well-being, and comfort during end-of-life moments. Bereavement photography, particularly in neonatal and pediatric settings, provides families with a tangible way to honor and remember their child, while also helping nurses and caregivers process their grief. Research shows that images are deeply valued as keepsakes of life and love, and when offered with ethical, culturally sensitive, and trauma-informed approaches, photography b

Heritable Genome Editing: Who Speaks for "Future" Children?

Approximately 80% of rare and often incurable and serious conditions affect newborns and children, and roughly half of all rare diseases are considered to have an onset in childhood. Somatic gene therapies are already in clinical trials for spinal muscular atrophy, beta thalassemia, and macular degeneration. If proven to be safe and effective, could heritable genome editing be seen as a form of preventive personalized medicine and as fostering the right to health of the child? The latest calls for global moratoria on clinical applications of heritable genome editing are troubling in that they

Spotlight on Juvenile Justice: Intersecting the Child Welfare System.

The authors disclose that this is a fictional case created by the authors. Julian, a 16-year-old adolescent who entered the juvenile justice system facing armed robbery charges, was 8 years old when he entered the child welfare system. He and his younger sister, Jessica, were placed in protective custody after school staff discovered that they were physically abused and witnessed domestic violence. Jessica further disclosed sexual abuse by the mother's boyfriend who was subsequently incarcerated. The siblings were placed together in several foster homes until a biological aunt assumed custody

Kinship Care and Child Protection in High-Income Countries: A Scoping Review.

Kinship care is a global phenomenon with a long history, which in high-income countries (HICs) at least, is being increasingly formalized through legislation and policy. There are many benefits to kinship care, including improved child mental health and well-being when compared to other types of out-of-home care. Despite this, kinship care is not without its risks with a lack of support and training for kinship carers putting children at an increased risk of abuse and neglect. This scoping review was conducted across 11 databases to explore the breadth and depth of the literature about abuse a

Plain Language Summary: Tympanostomy (Ear) Tubes in Children.

This plain language summary explains tympanostomy tubes, also known as ear tubes, to patients and families. The summary applies to children aged 6 months to 12 years with tympanostomy tubes or children being considered for tympanostomy tubes. It is based on the "Clinical Practice Guideline: Tympanostomy Tubes in Children (Update)," published in 2022 as a major update to the original guideline from 2013. This plain language summary is written explicitly for consumers, patients, and families as a companion publication to the full guideline, which provides greater detail for health care providers

Scaling Up Housing Services Within the Child Welfare System: Policy Insights From Simulation Modeling.

OBJECTIVES: Housing insecurity and homelessness contribute to risk of maltreatment among one in five of the nearly 3.5 million children annually investigated for maltreatment in the United States. The Family Unification Program (FUP)-a federal initiative-connects inadequately housed families involved in child welfare with long-term rental subsidies to avoid foster placement. However, FUP remains understudied and underutilized with funding levels that serve only a fraction of eligible households. The present study uses system dynamics modeling to inform decision-making by testing policies for s

[Psychosocial rehabilitation applied to children].

Based on a developmental approach, psychosocial rehabilitation (PSR) for children assumes that a combination of factors points to problems and, as for adults, focuses on recovery. It does not depend solely on the child but on a process involving supportive adults. Recovery in children supports the hope of the parents and builds a new form of parenthood to cope with the child's disability. To optimize psychosocial abilities of a child with a neurodevelopmental disorder (NDD), a personalized assessment and early interventions are needed. Promoting parental skills is the key to preventing additio

Lifetime rates and types of subsequent child protection system contact following a first report of neglect: An age-stratified analysis.

An estimated 1 in 3 U.S. children will be the subject of a child protective services (CPS) investigation during their lifetime, typically for allegations of neglect. Whether and how an initial report of neglect is addressed may place children on divergent trajectories for safety and stability throughout childhood. The purpose of this study is to track subsequent CPS contact among children born in California in 2000 who were first investigated by CPS for neglect allegations alone (no co-occurring abuse) and not permanently separated from their families of origin (i.e., not removed or reunified

Iron status, development, and behavior in young children in the Pennsylvania foster care system.

BACKGROUND: Children in foster care are classified as a highly vulnerable population and struggle with both physical and mental health problems. Medical conditions, like poor nutritional status, remain understudied in children in foster care. To our knowledge, few studies in children in U.S. foster care have quantified the prevalence of anemia, and no studies have examined the association between anemia status and relevant developmental and behavioral outcomes. OBJECTIVE/AIMS: (1) To determine the prevalence of anemia among children in or adopted from Pennsylvania foster care, between the age

When does time matter? maternal employment, children's time with parents, and child development.

This study tests the two assumptions underlying popularly held notions that maternal employment negatively affects children because it reduces time spent with parents: (1) that maternal employment reduces children's time with parents, and (2) that time with parents affects child outcomes. We analyze children's time-diary data from the Child Development Supplement of the Panel Study of Income Dynamics and use child fixed-effects and IV estimations to account for unobserved heterogeneity. We find that working mothers trade quantity of time for better "quality" of time. On average, maternal work

Posttraumatic Stress Symptoms in Preschool Children in Foster Care: The Influence of Placement and Foster Family Environment.

Children in foster care often experience traumatic events which increase their risk for posttraumatic stress symptoms (PTSS). Until now, no research has investigated the developmentally sensitive PTSS criteria for preschoolers among children in foster care. The current study estimated the prevalence of potentially traumatic experiences and clinical PTSS in German foster care children aged 3 to 7 years. The foster parents of 324 children completed questionnaires about children's PTSS, foster parental stress, parenting, and family functioning. Linear regression models tested trauma-related varia

Fetal alcohol syndrome and fetal alcohol effects. The University of Minnesota experience.

Fetal alcohol syndrome (FAS) describes a set of physical and neurologic defects that occur in the newborn as a direct result of maternal alcohol consumption. We report on a review of a series of patients seen at the University of Minnesota Hospital and Clinic (UMHC) who were identified as having been exposed to alcohol in the prenatal period. Of 46 patients, 38 were found to have FAS, and seven were found to have a less severe set of abnormalities termed fetal alcohol effects (FAE). This series at UMHC demonstrated many shocking characteristics. The average IQ of the children with FAS was unde

Structural problems in institutional care for youth.

In examining the relationship of emotional responsiveness and authoritarianism to the success of child welfare institutions as measured by running away behavior structural factors relating to both are discussed. The emotional deficit of the care giving situation seems to be a factor related to running away. Authoritarianism was not a related factor. Though both runaways and nonrunaways rated the institutional experience to be less emotionally responsive than their families, workers and runaways rated runaways experience as the least emotionally responsive. This is thought to relate to both run
