Public health aspects of breast cancer gene testing in Canada. Part 2: selection for and effects of testing.

Criteria set by clinical services for referral for counselling and genetic testing are variable and often arbitrary. Empirical data and computer models are available to estimate the probability of being a mutation carrier, based on family and personal history. Surveys show that high proportions of women at risk of cancer and of women in the general population are interested in being tested, but this may be based on inflated perceptions of personal risk and limitations in understanding of the tests used and their implications. A high proportion of women with a positive family history have a gre

[Current status of pediatric home mechanical ventilation in Japan].

The first nationwide survey of pediatric home mechanical ventilation (HMV) was conducted. From 35 out of 149 medical centers investigated, 49 cases of HVM were reported. The patients ranged in age from 1 to 20 years, and the causes of respiratory failure were neuromuscular diseases (51%), central hypoventilation (33%), respiratory disorders (10%) and cervical cord injury (6%). Since 1983 when 2 patients were discharged home on mechanical ventilation, more and more ventilator-dependent children have been sent home every year. However, home mechanical ventilation is not yet well recognized in Ja

The diffusion of new genetic tests for predicting disease.

This paper examines the pathways by which new genetic tests will become available to the public. In view of the scarcity of genetic specialists, the pathway is likely to involve primary care physicians. Other pathways entail state-mandated testing, community-based programs, or testing by laboratories without much involvement of primary care physicians. When testing does become available the "destination" will be either family-centered testing or population-oriented screening. The deterrent to screening will not be the inability to detect disease-causing mutations but the costs and attitudes of

Brazilian Immigrant Parents' Perspectives on Oral Health in Early Childhood and Suggested Strategies for Education, Access, and Care: Qualitative Study.

BACKGROUND: Oral health in early childhood is vital for long-term well-being, yet dental caries is highly prevalent among young children in the United States, especially in low-income and immigrant families. Brazilian immigrants, a rapidly growing Latin American population in the United States, face distinct barriers to oral health care, such as language differences, limited access to care, and a lack of culturally tailored resources. Despite this, Brazilian immigrants are understudied in public health research. Understanding Brazilian immigrant parents' perspectives is essential to advancing

Effectiveness of three pneumococcal conjugate vaccines to prevent invasive pneumococcal disease in Quebec, Canada.

BACKGROUND: In Quebec, a pneumococcal conjugate vaccine (PCV) program was implemented in December 2004. The recommended schedule is 2+1 doses for low-risk infants. PCV-7 was first used (including catch-up for children <5 years of age), replaced by PCV10 in June 2009, and by PCV13 in January 2011 (no catch-up in both instances). From the beginning, >90% of children received the recommended number of doses. OBJECTIVE: To assess the effectiveness of the three PCVs sequentially used to prevent invasive infectious disease (IPD). METHODS: IPD cases in children 2-59 months during the years 2005-201

A Qualitative Analysis of State Medicaid Coverage Benefits for Allogeneic Hematopoietic Cell Transplantation (alloHCT) for Patients with Sickle Cell Disease (SCD).

Sickle cell disease (SCD) is the most common inherited hemoglobin disorder, affecting approximately 100,000 people in the United States. Allogeneic hematopoietic cell transplantation (alloHCT), also known as bone marrow transplant (BMT), is currently the only established curative option for SCD. However, alloHCT is an optional benefit under Medicaid. This study of alloHCT coverage for patients with SCD aims to understand the scope of state Medicaid coverage benefits and BMT financial coordinators' experience working with their state Medicaid programs. States estimated to have more than 50 newb

Surveillance of Vaccination Coverage Among Adult Populations - United States, 2014.

PROBLEM/CONDITION: Overall, the prevalence of illness attributable to vaccine-preventable diseases is greater among adults than among children. Adults are recommended to receive vaccinations based on their age, underlying medical conditions, lifestyle, prior vaccinations, and other considerations. Updated vaccination recommendations from CDC are published annually in the U.S. Adult Immunization Schedule. Despite longstanding recommendations for use of many vaccines, vaccination coverage among U.S. adults is low. REPORTING PERIOD: August 2013-June 2014 (for influenza vaccination) and January-D

Association of clinical and social factors with risk of fracture in children with atopic dermatitis.

BACKGROUND: Children with atopic dermatitis (AD) have multiple risk factors for accidental fractures, injuries that can lead to significant morbidity and mortality. However, little is known about the factors that mediate the relationship between AD and fracture in children. OBJECTIVE: Our purpose was to examine the association of AD with fracture and to identify potential mediating factors. METHODS: This retrospective cohort study examined children with and without AD from a longitudinal matched cohort database of 353,040 children registered in the national health insurance service and parti

The costs of public services for teenage mothers post-welfare reform: a ten-state study.

BACKGROUND: More information is needed on the use and costs of public services by teens after the passage of major national polices in the 1990s. Both the 1996 welfare reform and later changes to the Medicaid program have affected the access of low-income adolescents to public assistance programs. In turn, these changes have affected teenaged mothers and their infants and the costs that taxpayers incur in the 50 states. STUDY QUESTION: What public services do teenage mothers use and what are their costs in the decade after the major policy changes to public assistance programs? How do pattern

Why Aren't We Achieving High Vaccination Rates for Rotavirus Vaccine in the United States?

BACKGROUND: Rotavirus vaccine (RV) coverage levels for US infants are <80%. METHODS: We surveyed nationally representative networks of pediatricians by internet/mail from April to June, 2019. Multivariable regression assessed factors associated with difficulty administering the first RV dose (RV#1) by the maximum age. RESULTS: Response rate was 68% (303/448). Ninety-nine percent of providers reported strongly recommending RV. The most common barriers to RV delivery overall (definite/somewhat of a barrier) were: parental concerns about vaccine safety overall (27%), parents wanting to defer (2

The Effect of Socioeconomic Status and Parental Demographics on Activation of Department of Child and Family Services in Pediatric Burn Injury.

Burns resulting from child maltreatment are tragic causes of significant morbidity and mortality, most commonly affecting children under 3 years of age. More than one third of nonaccidental burns occur in single-parent homes or have parents with history of mental illness, substance abuse, incarceration, or Department of Children and Family Services (DCFS) involvement. The authors sought to profile pediatric burn injuries associated with DCFS investigations. They performed a retrospective chart review of pediatric burn patients, admitted between January 1, 2011 and December 31, 2014. They analy

Federal Policy Changes Endanger Medicaid's Critical Role in Perinatal Care and Health Equity.

Medicaid is the nation's largest public health insurer and the primary payer of perinatal care; it covers more than 70 million low-income residents and 41% of births. The One Big Beautiful Bill Act (OBBBA), which was signed into law on July 4, 2025, introduced the largest Medicaid funding reductions in the program's history-an estimated $1.1 trillion over 10 years. In this article, we describe the major OBBBA provisions related to eligibility and Medicaid financing and the anticipated effects of these provisions on the delivery of perinatal care services and related outcomes. Although pregnant
