Laparoscopic cholecystectomy for acute cholecystitis.

One hundred and eight patients with histopathologically confirmed acute cholecystitis underwent laparoscopic or attempted laparoscopic cholecystectomy in the Security Forces Hospital, from October 1991 to April 1996 were retrospectively reviewed. All the patients had routine laboratory works including abdominal ultrasonography. Females represented 75% and 57% had previous admission. Diabetes mellitus was found in 43.5%. Laparoscopic cholecystectomy was successfully completed in 71.2%. The main reasons for conversion in 31 patients were adhesions and unclear anatomy in 87%. The mean operative t

[Laparoscopic cholecystectomy in acute cholecystitis].

AIM: Results evaluation of laparoscopic cholecystectomy in acute cholecystitis. MATERIAL AND METHOD: Between 1994-1997 we performed 65 laparoscopic cholecystectomies for histopathologically proved acute lithiasic cholecystitis. We studied clinic and echographic diagnosis, operative moment, conversion rate, operative time, postoperative morbidity and hospitalization. The cholecystectomy was performed within 72 hours in 18 patients (trial I), 4 to 7 days in 25 patients (trial II) and over 7 days in 22 patients (trial III). RESULTS: Diagnosis of acute lithiasic cholecystitis was always possible

[Necrotizing fasciitis after laparoscopic cholecystectomy].

Necrotizing fasciitis is a rare and potentially fatal infection of the fascia and subcutaneous tissue. We found only few reports about this disease following laparoscopic surgery. A contamination of the abdominal wall at the trocar position with pathogenic aerobic and anaerobic microorganisms in an "aerosol" is discussed. An increased incidence is associated with risk factors which lead to compromised tissue perfusion and immunosuppression. Early recognition and aggressive surgical management combined with intravenous antibiotics are crucial to a successful outcome. Various aspects of the path

Laparoscopic cholecystectomy: two-port technique.

BACKGROUND AND STUDY AIMS: Conventional laparoscopic cholecystectomy requires four ports. The present study describes a new laparoscopic cholecystectomy technique using two ports only. PATIENTS AND METHODS: Sixty-two consecutive patients with symptomatic gallstones underwent laparoscopic cholecystectomy using this two-port technique. The operation requires a straightforward laparoscope and a percutaneous sling to retract the gallbladder laterally and cephalad. RESULTS: The two-port method was successful in 52 patients; eight required an additional port, and two required conversion to open su

Laparoscopic cholecystectomy: the Finnish experience.

Between January 1992 and December 1994, 5,742 patients were treated by laparoscopic cholecystectomy in 35 Finnish hospitals. The operation was converted to open laparotomy in 360 (6.3%) patients, the most common causes for conversion being technical difficulties in dissection of the gall bladder (2.8%), bleeding (0.9%) and bile duct injury (0.48%). Intraoperative cholangiography was performed selectively in 18%, and common bile duct stones were found in 10.2% of these cases. Postoperative complications occurred in 208 (3.6%) patients, of whom 65 (1.1%) required reoperation. Twenty-eight (0.48%

[Laparoscopic cholecystectomy in acute cholecystitis].

Contrary to earlier opinions, the laparoscopic cholecystectomy (LC) is not a contraindication in the acute cholecystitis. The most important parameter in determining the feasibility of attempting laparoscopic cholecystectomy in the setting of acute inflammation appears to be the experience of the surgeon. 59 cases with LC are analyzed: 50 LC and 9 conversion. The operations were more difficult and lasted longer. The patients appeared to be at a greater intraoperative risk and the conversion rate was also higher. Neither lesions of the common bile duct nor deaths were recorded. The advantages o

Laparoscopic cholecystectomy: 700 consecutive cases.

From March 1993 till June 1995 700 laparoscopic cholecystectomies have been performed on 590 female and 110 male patients who ranged form 10 to 82 years of age at the Departments of Surgery of County Hospital and Alexander's University Hospital in Sofia. Ninety seven of the patients (13.8%) who have had laparoscopic cholecystectomies (LC) were diagnosed as acute cholecystitis and the rest as chromic calculous cholecystitis. In 16 cases (2.28%) a conversion to standard open cholecystectomy was necessary, the most frequent causes being intraoperative bleeding and unsuspected preoperatively commo

Laparoscopic cholecystectomy in acute cholecystitis.

Between January 1991 and December 1992, 220 patients underwent laparoscopic cholecystectomy and were monitored prospectively. Twenty-seven patients (12.3%) had acute cholecystitis, according to strict clinical and pathologic criteria. Their average age was 60.4 years. The average operative time was 121 min. Two of the 27 patients (7.4%) required conversion to open laparotomy. The postoperative morbidity rate was 11%. The average hospital stay was 1.9 days. This series provides further evidence that laparoscopic cholecystectomy can be used safely and effectively to treat acute cholecystitis whe

Laparoscopic cholecystectomy for acute cholecystitis.

The safety and efficacy of laparoscopic cholecystectomy for acute cholecystitis were evaluated in a 2-year retrospective review. Results of laparoscopic cholecystectomy in 66 patients with acute inflammation of the gallbladder were compared with those of the standard open procedure for this condition (43 patients) and routine laparoscopic cholecystectomy (227 patients). The laparoscopic procedure for acute cholecystitis was successful in 46 of 66 patients. There was no difference in mean operating time when the inflamed gallbladder was removed laparoscopically or at open surgery (82 versus 84

Incidental cholecystectomy during colorectal surgery.

OBJECTIVE: To assess the risks and benefits of incidental cholecystectomy in patients having colorectal surgery. SUMMARY BACKGROUND DATA: Cholelithiasis is found commonly during abdominal surgery. Previous studies used disparate methods to assess the risks and benefits of incidental cholecystectomy and have reached contradictory conclusions. METHODS: All patients in whom asymptomatic cholelithiasis was noted during colorectal surgery between January 1982 and December 1986 were studied. Operative morbidity and long-term outcome were assessed by chart review and questionnaire. RESULTS: Three

[Cholecystectomy through a surgical tube].

Operations were conducted on 800 patients (83.2% females and 16.8% males) for acute and chronic cholecystitis by means of an apparatus designed by the authors with the use of an operative tube with a light guide and a special set of instruments. The average duration of the operation was 54 min. Fifteen patients underwent choledochotomy with drainage of the common bile duct. The operative interventions had to be repeated on 4 patients due to hemorrhage. Intraoperative damage to the bile ducts occurred in 4 (0.5%) patients. Complications were encountered in 3.2% of cases. The suggested method ma

Venous stasis during laparoscopic cholecystectomy.

The objectives of this research were (a) to determine the effect of insufflation at laparoscopic cholecystectomy to 12 mm Hg on femoral venous blood flow; and (b) to assess the function of intermittent pneumatic compressors (IPC) and intermittent electric calf stimulators (IECS) in the presence of a pneumoperitoneum. Measures of baseline venous blood flow velocity, femoral vein diameter, and maximum blood flow velocity achieved by IPC or IECS were made in the presence or absence of a pneumoperitoneum of 12 mm Hg. The ICP and IECS were randomly allocated to either leg. All measures were made by

Conventional cholecystectomy in elderly patients.

One hundred and seventy two consecutive patients of 70 years or older undergoing cholecystectomy at laparotomy for gallbladder stone disease during a five-year period in Tampere University Hospital were studied to evaluate the present safety of "conventional" cholecystectomy in the elderly. One hundred and six (62%) underwent emergency or urgent cholecystectomy for acute cholecystitis and 66 (33%) elective cholecystectomy. The mortality was zero per cent in elective operations as compared to 5% in emergency/urgent operations (P < 0.001). The complication rate was 8% in elective operations comp

[Percutaneous cholecystectomy in acute cholecystitis].

Seventy-two patients with acute cholecystitis were treated with initial ultrasound-guided percutaneous transperitoneal cholecystostomy (PTCS). PTCS could be performed in all 72 patients almost without procedure related complications, and subsequent elective treatment could be done in all, except three patients where acute cholecystectomy was necessary because of accidental catheter dislocation. Seven patients died of their severe underlying illness without relationship to PTCS or cholecystitis. Eight patients with acalculous cholecystitis had no further treatment after removal of the drainage

Intraoperative ultrasonography during laparoscopic cholecystectomy.

Intraoperative ultrasonography during laparoscopic cholecystectomy is a new procedure for examining the bile duct. The ultrasonic probe used in this study utilized a 7.5-MHz convex array designed for easy insertion through a 10-mm cannula. This probe was 90 cm long, had a 10-mm diameter, and was equipped with an angulation system controlled by levels. Additionally, this ultrasound system could detect the direction of the blood flow using a color Doppler display. Both laparoscopic ultrasound and laparoscopic cholangiography were attempted in 45 patients. In 41 of the 45 patients, laparoscopic u

Laparoscopic cholecystectomy. Postoperative sonographic findings.

Seventeen consecutive patients undergoing elective laparoscopic cholecystectomy (LC) were serially evaluated with transabdominal ultrasound before, one day after, and six days after LC to document what, if any, changes occur in the surgical bed and surrounding parenchyma. The most common postoperative finding was focal sonolucency in the hepatic parenchyma adjacent to the gallbladder fossa in six (35%) of 17 patients. Five patients (29%) had postoperative fluid collections in the gallbladder fossa; in four of these five, it was technically difficult to dissect the gallbladder from the liver at
