Antibacterial activity of ibezapolstat against antimicrobial-resistant clinical strains of Clostridioides difficile.

Antimicrobial resistance is emerging in clinical strains of Clostridioides difficile. Ibezapolstat (IBZ) is a DNA polymerase IIIC inhibitor that has completed phase II clinical trials. IBZ has potent in vitro activity against wild-type, susceptible strains but its effect on C. difficile strains with reduced susceptibility to metronidazole (MTZ), vancomycin (VAN), or fidaxomicin (FDX) has not been tested. The primary objective of this study was to test the antibacterial properties of IBZ against multidrug-resistant C. difficile strains. The in vitro activity, bactericidal, and time-kill activit

Spore-Forming Clostridium (Clostridioides) difficile in Wastewater Treatment Plants in Western Australia.

There is growing evidence that shows Clostridium (Clostridioides) difficile is a pathogen of One Health importance with a complex dissemination pathway involving animals, humans, and the environment. Thus, environmental discharge and agricultural recycling of human and animal waste have been suspected as factors behind the dissemination of Clostridium difficile in the community. Here, the presence of C. difficile in 12 wastewater treatment plants (WWTPs) in Western Australia was investigated. Overall, C. difficile was found in 90.5% (114/126) of raw sewage influent, 48.1% (50/104) of treated e

Laboratory markers predictive of fulminant Clostridioides difficile infection refractory to fluid resuscitation.

BACKGROUND: Old age, leucocytosis, hypoalbuminemia, and elevated creatinine have been identified as risk factors for fulminant Clostridioides difficile infection (CDI). High ATLAS scores have also been linked to fatal disease. The affiliated studies, however, involved patients prescribed metronidazole - a regimen no longer standard of care. The variables were thus reassessed in patients prescribed optimal therapy. METHODS: Adults hospitalized with CDI at University of Kentucky Medical Center were retrospectively reviewed. Enrolled subjects were separated according to disease classification i.

In Vivo Targeting of Clostridioides difficile Using Phage-Delivered CRISPR-Cas3 Antimicrobials.

Clostridioides difficile is an important nosocomial pathogen that causes approximately 500,000 cases of C. difficile infection (CDI) and 29,000 deaths annually in the United States. Antibiotic use is a major risk factor for CDI because broad-spectrum antimicrobials disrupt the indigenous gut microbiota, decreasing colonization resistance against C. difficile Vancomycin is the standard of care for the treatment of CDI, likely contributing to the high recurrence rates due to the continued disruption of the gut microbiota. Thus, there is an urgent need for the development of novel therapeutics th

Toxic Clostridioides (formerly Clostridium) difficile colitis: No longer a diarrhea associated infection.

BACKGROUND: Clostridioides difficile infection (CDI) is traditionally taught to be an antibiotic associated diarrheal infection. This diagnosis is based on the presence of clinical symptoms (usually defined as more than 3 watery, loose or unformed stool within 24 h) coupled with a diagnostic test. There is now a new presentation of CDI, including progression to toxic megacolon, in patients without diarrhea. METHODS: We report a case series of 9 surgical patients from a single institution who developed CDI without preceding diarrhea. RESULT: All 9 patients had CDI with positive laboratory tes

Effect of fluoroquinolone resistance mutation Thr-82→Ile on Clostridioides difficile fitness.

OBJECTIVES: Fluoroquinolone resistance is common among epidemic Clostridioides difficile PCR ribotype (RT) 027 and may have contributed to outbreaks of C. difficile infection (CDI). We investigated the impact of fluoroquinolone mutations on the bacterial fitness (BF) of C. difficile RT027 isolates. METHODS: The BF of seven RT027 mutants with reduced susceptibility to moxifloxacin (moxifloxacin MIC 4-32 mg/L) was compared with their susceptible (moxifloxacin MIC 1-2 mg/L) progenitor strains in competitive batch culture (CBC), cell cytotoxicity and maximal growth rate assays. Effects of sexual dimorphism and estrous cycle on murine Clostridioides difficile infection.

Clostridioides difficile infection (CDI) causes the majority of identifiable antibiotic-associated diarrhea. Epidemiological studies have shown that biological human females are more susceptible to CDI than males. In this study, we show that female mice developed more severe CDI than males under all conditions tested. We found time-delayed effects of the female estrus cycle on CDI. Indeed, animals in proestrus at any time during CDI progression developed severe signs 1-2 days later. In contrast, animals that were in the estrus stage were protected. Consistent with the delayed effect of the est

Understanding the Pathophysiology of Clostridioides difficile Infection, Colonization, and Making an Accurate Diagnosis.

Clostridioides difficile is a gram-positive bacterium and major cause of infectious colitis, in hospital and health care settings, where it remains a significant public health threat. Accurate diagnosis of C difficile infection (CDI) is challenging and requires a clinical assessment supported by laboratory stool testing. Reliance on laboratory tests can lead to both overdiagnosis and underdiagnosis. This article outlines the pathophysiology of C difficile, with emphasis on aspects related to diagnostic testing, and discusses the clinical approach to clinical diagnosis. New therapies for the tr

A programmed reset mechanism primes Clostridioides difficile virulence at the onset of infection.

Clostridioides difficile is a major cause of antibiotic-associated diarrhea. This pathogen produces toxins essential for disease and flagella that promote intestinal colonization. Flagella and toxins are coregulated in an ON/OFF manner leading to a heterogeneous population of toxigenic, flagellated cells (flg-ON) and aflagellate cells with attenuated toxin production (flg-OFF). How selective pressures affect ON/OFF switching dynamics and overall population composition is unknown. Here, we use a mouse model of disease to evaluate the ON/OFF makeup of the C. difficile population in vivo. Two key

Eravacycline in sepsis complicated by Clostridioides difficile infection: a brief case series.

Clostridioides difficile infection complicating severe bacterial infections presents a therapeutic challenge, when antimicrobial discontinuation is unfeasible. Eravacycline, a synthetic fluorocycline with broad-spectrum activity against Gram-positive, Gram-negative and anaerobic bacteria, including C. difficile, carries a relatively low risk of inducing C. difficile. Although approved for complicated intra-abdominal infections, emerging evidence indicates its potential for other severe infections. We report three cases of C. difficile infection during systemic antibacterial therapy for severe

Comparison of Anaerobic Culture Methods for Detecting Clostridioides difficile in Bovine Faeces.

BACKGROUND: The study of the epidemiology of Clostridioides difficile in populations is greatly facilitated by the ability to isolate and further characterize individual organisms, which requires effective culture protocols. In cattle, where little is known about the epidemiology of C. difficile, no studies have assessed or compared the performance of different assays for detecting C. difficile. OBJECTIVES: This study compared two culture protocols for detecting C. difficile in bovine faeces from 121 gestating cows and 70 of their neonatal calves, while situating results obtained with each pr

Challenges and Strategies in Managing Recurrent Clostridioides difficile Infection in Older Adults.

BACKGROUND: Clostridioides difficile infections (CDIs) are caused by a Gram-positive, spore-forming bacillus and are defined by more than three episodes of watery diarrhoea per day. CDI is a major cause of morbidity and mortality in older adults, particularly over 65 years. Recurrent CDI leads to higher mortality and prolonged, debilitating illness. CASE PRESENTATIONS: This article presents two patients, aged over 80 years old, who developed recurrent CDI causing complicated and prolonged treatment courses. Patient 1 required an extended course of antibiotics for treatment of discitis and a c

The efficacy and underlying mechanisms of berberine in the treatment of recurrent Clostridioides difficile infection.

Recurrent Clostridioides difficile infection (rCDI) is a global health threat that has received considerable attention. Berberine (BBR), a natural pentacyclic isoquinoline alkaloid, has been used as a cost-effective treatment for intestinal infections in Asia for many years. However, the effect of BBR on rCDI is not clear. The efficacy and underlying mechanisms of BBR were evaluated in a vancomycin-dependent rCDI mouse model and an intestinal organoids model. The study findings showed that BBR treatment alleviated the severity of infection and increased survival rate in rCDI mice. Mechanistica

Novel Enterococcus faecium P2 as a probiotic for mitigating Clostridioides difficile infection.

Clostridioides difficile (C. difficile) is a well-recognized pathogen responsible for severe intestinal disorders, including pseudomembranous colitis and toxic megacolon. Probiotics have shown considerable promise in the prevention and management of gastrointestinal diseases. In this study, we isolated Enterococcus faecium (E. faecium) P2 from the feces of a healthy child and evaluated its probiotic efficacy against C. difficile infection (CDI) using a mouse model. In vitro assessments demonstrated that E. faecium P2 exhibits high safety, tolerates gastrointestinal stressors, forms robust biof

AHM-1: An Inclusion to the Arsenal of β-Lactam Resistance in Clostridioides difficile.

This study delves into a newly discovered MBL (metallo-β-lactamase) in Clostridioides difficile, a formidable pathogen known for causing nosocomial infections and exhibiting resistance to antimicrobial agents. The primary objective was to unravel its structure-function relationship. This research establishes the enzyme AHM-1 as a subclass B3-like MBL. Experimental results reveal that the enzyme's active site consists of two Zn2+ atoms exhibiting tetrahedral and trigonal bipyramidal coordination, similar to B1 and B3 MBLs. Notably, within its active site, it exhibits a lower binding capacity fo

Identification of a hemorrhagic determinant in Clostridioides difficile TcdA and Paeniclostridium sordellii TcsH.

UNLABELLED: Paeniclostridium sordellii hemorrhagic toxin (TcsH) and Clostridioides difficile toxin A (TcdA) are two major members of the large clostridial toxin (LCT) family. These two toxins share ~87% similarity and are known to cause severe hemorrhagic pathology in animals. Yet, the pathogenesis of their hemorrhagic toxicity has been mysterious for decades. Here, we examined the liver injury after systemic exposure to different LCTs and found that only TcsH and TcdA induce overt hepatic hemorrhage. By investigating the chimeric and truncated toxins, we demonstrated that the enzymatic domain

Testing and Diagnosis of Clostridioides difficile Infection in Special Scenarios: A Systematic Review.

INTRODUCTION: Clostridioides difficile infection (CDI) is a clinical and laboratory diagnosis. Populations at higher risk of developing disease require a high clinical index of suspicion for laboratory testing to avoid incorrect assumptions of colonization. Common risk factors include recent antibiotic use, elderly (>65 years old), and immunocompromised patients. C. difficile assays should be ordered in an algorithm approach to diagnose an infection rather than colonization. Screening tests are widely available in hospital systems, but novel molecular testing may aid in diagnosis in patients w

Clostridioides difficile Toxins: Host Cell Interactions and Their Role in Disease Pathogenesis.

Clostridioides difficile, a Gram-positive anaerobic bacterium, is the leading cause of hospital-acquired antibiotic-associated diarrhea worldwide. The severity of C. difficile infection (CDI) varies, ranging from mild diarrhea to life-threatening conditions such as pseudomembranous colitis and toxic megacolon. Central to the pathogenesis of the infection are toxins produced by C. difficile, with toxin A (TcdA) and toxin B (TcdB) as the main virulence factors. Additionally, some strains produce a third toxin known as C. difficile transferase (CDT). Toxins damage the colonic epithelium, initiati

Prevalence of Clostridioides difficile in Canine Feces and Its Association with Intestinal Dysbiosis.

The role of Clostridioides (C.) difficile as an enteropathogen in dogs is controversial. In humans, intestinal bile acid-dysmetabolism is associated with C. difficile prevalence. The relationship between fecal qPCR-based dysbiosis index (DI) and especially the abundance of bile acid-converting Clostridium hiranonis with the presence of C. difficile in dogs was explored across the following 4 cohorts: 358 fecal samples submitted for routine diagnostic work-up, 33 dogs with chronic enteropathy, 14 dogs with acute diarrhea, and 116 healthy dogs. Dogs that tested positive for C. difficile had sign
