[Functional evaluation of auditory cortex by functional magnetic resonance before cochlear implantation: a pilot study].

OBJECTIVE: To investigate the functional evaluation of auditory cortex by functional magnetic resonance imaging (fMRI) before cochlear implantation (CI). METHODS: Twenty-one children with severe to profound sensorineural hearing loss (SNHL) (7 cases with normal inner ear structure, 3 cases with large vestibular aqueduct syndrome, 1 case with cochlear nerve hypoplasia) and 7 children with normal hearing were examined by fMRI under sedation. The subjects received acoustic stimuli at four random frequencies (500, 1 000, 2 000, 4 000 Hz) during fMRI examination. Tones were interleaved with silenc

Round window membrane intracochlear drug delivery enhanced by induced advection.

Delivery of therapeutic compounds to the inner ear via absorption through the round window membrane (RWM) has advantages over direct intracochlear infusions; specifically, minimizing impact upon functional hearing measures. However, previous reports show that significant basal-to-apical concentration gradients occur, with the potential to impact treatment efficacy. Here we present a new approach to inner ear drug delivery with induced advection aiding distribution of compounds throughout the inner ear in the murine cochlea. Polyimide microtubing was placed near the RWM niche through a bullaost

Clinical implications of experiments on alteration of the labyrinthine fluid pressures.

Hearing is affected by changes in hydrostatic pressure of the perilymph, but very high pressures are required to produce a significant loss. Among the observed findings in cats are a rapidly reversible decrease in the sensitivity of the cochlear microphonic that is greater for low frequencies, harmonic distortion and reduction of dynamic range as measured by the input-output function for the cochlear microphonic, and a change in the summating potential from negative to positive. These changes are attributed to a temporary reduction in the size of the cochlear duct due to a shift of water molec

Comparison of Endolymphatic Duct Dimensions and Jugular Bulb Abnormalities Between Meniere Disease and a Normal Population.

BACKGROUND: The pathogenesis of Meniere disease (MD) has not been fully understood. According to the widely accepted theory, imbalances due to overproduction and/or impaired absorption of endolymph may cause endolymphatic hydrops, which is the hallmark pathological finding in MD. Some developmental temporal bone abnormalities may impair endolymph circulation and absorption, and these abnormalities could be a part of MD pathophysiology. However, structural features of the temporal bone cannot explain MD pathophysiology definitively. The authors aimed to determine the length and width of the end

Large vestibular aqueduct syndrome: a case study.

A 23-month-old female was referred for hearing aid fitting after failing newborn hearing screening and being diagnosed with significant hearing loss through subsequent diagnostic testing. Auditory brainstem response (ABR) and behavioral testing revealed a moderate-to-severe bilateral mixed hearing loss. Prior to the hearing aid evaluation, tympanostomy tubes had been placed bilaterally with little or no apparent change in hearing sensitivity. Initial testing during the hearing aid fitting confirmed earlier findings, but abnormal middle ear results were observed, requiring referral for addition

Unpredictable hearing loss after intratympanic gentamicin treatment for vertigo. A new theory.

A new hypothesis is advanced suggesting that unpredictable cases of profound hearing loss after intratympanic gentamicin treatment (IGT) may be caused by decreased patency of the communication routes between the inner ear and the cerebrospinal fluid, primarily of the cochlear aqueduct. A tympanic displacement analyzer, which can indirectly analyze inner ear and intracranial pressure changes and can also evaluate the efficiency of communication between these two compartments, was used. Two cases are presented: in the first, a patient who became deaf after IGT showed signs of decreased patency o

The classification and treatment of Menière's disease.

The classification and treatment of Menière's disease are especially difficult because the natural history of Menière's disease is so variable, and not possible to relate to the cochlear and vestibular pathology present, which is constantly changing and cannot be known during life. In the many new papers written each year on Menière's disease and the hundreds of older papers already in the medical literature, there is no good agreement about the definition of Menière's disease, the pathology and natural history, and the real benefit of the medical and surgical treatments given. Only recently,

[Inner ear hearing loss following cerebrospinal fluid puncture: a too little appreciated complication?].

Lumbar puncture for myelography, spinal anaesthesia and aspiration of cerebrospinal fluid is a very common procedure. Although it has been known for a long time that hearing impairment may be a late complication, only a few case reports have been published, and not all have included audiograms. We present nine cases of hearing loss following myelography, lumbar puncture and spinal anaesthesia. It appears likely that this rare complication arises only in subjects with a wholly or partially patent cochlear aqueduct, allowing loss of perilymphatic fluid into the cerebrospinal space. Hearing loss

Mondini-like malformation mimicking otosclerosis and superior semicircular canal dehiscence.

In 2003, it was reported that superior semicircular canal dehiscence can mimic otosclerosis because of low-frequency bone conduction hearing gain and dissipation of air-conducted acoustic energy through the dehiscence. We report the case of a 17-year-old girl with left-sided combined hearing loss thought to be due to otosclerosis. Bone conduction thresholds were -10 dB at 250 and 500 Hz and she had a 40 dB air-bone gap at 250 Hz. When a tuning fork was placed at her ankle she heard it in her left ear. Acoustic reflexes and vestibular evoked myogenic potentials could be elicited bilaterally. Im

Impedance measurement as a noninvasive technique for the monitoring of intracranial pressure variations.

In previous reports it has been demonstrated that jugular compression causes a modification of middle ear acoustic impedance; this phenomenon has been named the jugulotympanic reflex (JTR). The aim of the present study was to contribute to the understanding of the origin of this reflex. We have analyzed the middle-ear impedance modifications induced by unilateral and bilateral jugular compression in 12 normal subjects, in 12 patients afflicted with otosclerosis, in 3 patients with Menière's disease and in 6 patients demonstrating intracranial hypertension. In the latter group, the cerebrospina

Perilymphatic fistula in teens and young adults: emphasis on preexisting sensorineural hearing loss.

Sudden hearing loss and new-onset vestibular disorders can be caused by perilymphatic fistulas (PLFs). In children, the existence of a severe or profound sensorineural hearing loss (SNHL) has been associated with a perilymphatic fistula when a sudden hearing loss or vestibular disturbance occurs. Likewise, at the other end of the age spectrum, several large series of PLFs in adulthood have been presented. This report focuses on a neglected age group, those in their teens and young adulthood. Twenty-three patients between the ages of 17 and 26 were studied who had a preexisting SNHL and had dev

Unilateral permanent sudden sensorineural hearing loss after spina-epidural anaesthesia for caesarean section: a case report.

Sudden sensorineural hearing loss (SSNHL) after spinal-epidural anaesthesia is a very rare complication. The patient is a 25-year-old female who developed right-sided hearing loss, unbalance, a sensation of aural fullness and tinnitus 2 days post uncomplicated spinal-epidural anaesthesia for an emergency caesarean section. Initial management by her primary care physician for suspected eustachian tube dysfunction did not relieve symptoms, and 2 weeks subsequently, audiological assessment demonstrated right Sensorineural hearing loss (SNHL). Specialist consultation was sought, and the patient wa

Unilateral Meniere's disease: is the contralateral ear normal?

Certain investigations in patients with unilateral Meniere's disease may on occasion show abnormalities in the completely symptomless contralateral ear. These tests include transtympanic electrocochleography, the acetazolamide cochlear hydration test, vestibular aqueduct tomography, and caloric testing. Eventually these ears may well become symptomatic. Previous studies have shown that otoadmittance changes are a sensitive indicator of glycerol-induced intracochlear pressure alterations in hydropic ears, but do not occur in patients without Meniere's disease. Otoadmittance parameters were eval

Rupture of the round window membrane.

The mechanism of development of rupture of the round window membrane appears to be an increase in pressure in the cerebrospinal fluid transmitted to the inner ear via the cochlear aqueduct. Two cases of rupture of the round window membrane in divers are described in detail. One diver had ruptures of both round window membranes following difficulty in autoinflating when diving, and the other had a rupture of one round window membrane, having presented with acute prostating vertigo and normal hearing. Surgical repair restored the hearing in the former case and preserved normal hearing in the lat

Sensorineural and conductive hearing loss associated with lateral semicircular canal malformation.

OBJECTIVE: Lateral semicircular canal (LSCC) malformation is one of the most common radiological inner ear malformations. Traditionally, inner ear malformations are thought to be associated with sensorineural hearing loss (SNHL). Recent experience with patients with LSCC malformation suggested that LSCC malformation may be associated with both SNHL and conductive hearing loss (CHL). The auditory phenotype associated with LSCC malformation is not well delineated. The objective of this study is to define the nature of the hearing loss associated with LSCC malformation. STUDY DESIGN: Retrospecti
