Single allograft medial collateral ligament and posterior oblique ligament reconstruction: a technique to improve valgus and rotational stability.

We present a novel and simple method for single hamstring allograft MCL and PMC reconstruction, which can improve both joint valgus and external rotational stability and maximize utilization of allograft. All patients received arthroscopic evaluation through inferomedial and inferolateral knee incisions to ascertain whether there were intra-articular injuries. An 8-cm-length longitudinal incision was made from 1 cm above adductor tubercle to 5-cm proximal medial tibia joint line. The anterior tibia insertion was defined as 15 mm lateral from the medial tibia edge and 45 mm below the medial tib

Corrective osteotomy and ligament repair for longstanding radial collateral ligament tear of the proximal interphalangeal joint: case series.

PURPOSE: To evaluate the clinical outcomes of corrective osteotomy and ligament repair for longstanding radial collateral ligament tears of the proximal interphalangeal (PIP) joint. METHODS: We retrospectively evaluated 4 patients with 5 longstanding tears in the radial collateral ligaments of the PIP joints. The average age at the time of surgery was 51 years (range, 40-62 y). The average time from the initial injury to surgery was 31 years (range, 22-40 y). Plain radiographs revealed an ulnar slope at the PIP joint surface with degenerative changes in all fingers. We corrected the slope usi

Viability of shear wave elastography to predict mechanical/ultimate failure in the anterolateral and medial collateral ligaments of the knee.

The purpose of this study was (1) to determine the utility of shear wave elastography as a predictor for the mechanical failure of superficial knee ligaments and (2) to determine the viability of shear wave elastography to assess injury risk potential. Our hypothesis was that shear wave elastography measurements of the anterolateral ligament and medial collateral ligament would directly correlate with the material properties and the mechanical failure of the ligament, serving as a prognostic measurement for injury risk. 8 cadaveric specimens were acquired, and tissue stiffness for the anterola

A randomized, prospective study of operative and non-operative treatment of injuries of the fibular collateral ligaments of the ankle.

One hundred and forty-six adults who had an isolated injury of the fibular collateral ligaments of the ankle were randomized to be managed operatively or non-operatively. Disruption of the ligaments was diagnosed by means of a physical examination and on the basis of stress radiographs of the ankle made with use of a specially designed device to hold the leg. Operative treatment, performed in seventy-three patients, consisted of suture repair of the disrupted ligaments within seventy-two hours after the injury, followed by immobilization of the ankle in a below-the-knee plaster cast for six we

Arthroscopically Assisted Reconstruction of Lateral Metatarsosesamoid Suspensory and Lateral Metatarsophalangeal Collateral Ligaments in the Management of Iatrogenic Hallux Varus Deformity.

The medial and lateral metatarsosesamoid suspensory ligaments together with the sesamoid apparatus are the main contributors to the rotational stability of the first metatarsophalangeal joint (MTPJ). Rotational instability of first MTPJ is a major factor in the development of hallux valgus and varus deformities. As the great toe starts to rotate, the muscle pulls across the first MTPJ, causing it to become imbalanced and lead to progressive medial or lateral deviation of the great toe. The gold standard and most reliable option for hallux varus deformity is first MTPJ fusion. However, it resul

Anatomic reconstruction of the medial collateral ligament in multi-ligaments knee injury using achilles allograft : a modification of Marx's technique.

Medial Collateral Ligament (MCL) injury may require operative treatment. Marx et al. described the latest technique for reconstruction of MCL. While good results have been reported using the Marx technique, some issues have been observed. To address the mentioned issues, a modification to the Marx technique has been devised. Eleven patients were enrolled and their ligaments were repaired by the fixation of allograft on the proximal and distal attachment footprints of the superficial MCL. For preventing loss of knee ROM, MCL and other ligaments were reconstructed in 2 separate stages. At the la

Single-Stage Anatomic Reconstruction of Anterior Cruciate Ligament, Posterior Cruciate Ligament, Medial Collateral Ligament, and Posterolateral Corner for Multiligament Knee Injury.

Multiligament knee injuries represent rare and challenging injuries that require complex surgical management to restore stability and function. Current evidence supports anatomic single-stage reconstructions when feasible in order to re-establish knee kinematics and enable early rehabilitation. This technique paper describes single-stage anterior cruciate ligament reconstruction with a bone-patellar tendon-bone allograft, double-bundle posterior cruciate ligament reconstruction with Achilles tendon allograft for the anterolateral bundle and tibialis anterior allograft for the posteromedial bun

Change in the collateral and accessory collateral ligament lengths of the proximal interphalangeal joint using cadaveric model three-dimensional laser scanning.

UNLABELLED: The purpose of this study was to assess the lengths of the index and middle finger proximal interphalangeal joint ligaments and determine the relative changes in the collateral and accessory collateral ligament lengths at 0°, 45° and 90° flexion. We generated three-dimensional scans of 16 finger (eight index and eight middle) proximal interphalangeal joints to assess relative changes in ligament length. Significant changes were found between 45°-90° and 0°-90° for the ulnar collateral ligament of the index finger and both collateral ligaments of the middle finger between 45°-90° an

Minimally-Invasive, Retensionable, Onlay Double-Bundle Medial Collateral Ligament Reconstruction.

BACKGROUND: The superficial medial collateral ligament (sMCL) is the most injured knee ligament, and the most commonly coinjured ligament in anterior cruciate ligament tears. The technique's goal is to be less invasive while maintaining the hamstrings' function. INDICATIONS: Surgical indications for this technique include isolated grade 3 injuries that fail nonoperative management and multiligament knee injuries that include the MCL. TECHNIQUE DESCRIPTION: Items needed for the double-bundle reconstruction technique include an anterior tibialis graft > 270 mm, C-Arm for visualization of radio

Short-term outcomes following reconstruction or direct repair of the ulnar collateral ligament.

INTRODUCTION: Medial ulnar collateral ligament (UCL) injuries substantially impair athletic performance due to pain, instability, and diminished throwing capabilities. While UCL reconstruction is the primary surgical treatment due to its reliable restoration of stability and high return-to-play rates, direct UCL repair has re-emerged as an alternative in select cases. For the purposes of this study, both procedures are collectively referred to as UCL reconstruction/repair (ULCR). The long rehabilitation period and complications such as ulnar neuropathy and stiffness remain concerns. Existing l

Lateral Collateral Ligament Calcification: A Rare and Challenging Cause of Chronic Knee Pain.

Lateral collateral ligament (LCL) calcification of the knee represents an uncommon degenerative or post-traumatic change characterized by ectopic mineral deposition within the ligamentous fibers. We report a rare case of LCL calcification in a 49-year-old female presenting with chronic right lateral knee pain with recurrent acute exacerbations. The patient had a prior history of mild knee trauma, and no significant findings were initially identified on radiographs. Despite several courses of physiotherapy and oral nonsteroidal anti-inflammatory drugs (NSAIDs), her symptoms persisted with only

Medial collateral ligament ganglion cyst: a rare cause of medial knee pain.

Ganglion cysts are commonly found in areas of constant mechanical stress such as the joints and tendons of the wrist or hand as well as the anterior aspect of the ankle. In the knee, parameniscal cysts are often encountered secondary to meniscal tears or articular degeneration. Intra-articular ganglion cysts are uncommon and often arise from the cruciate ligaments and are found in the intercondylar notch. However, ganglion cysts arising from collateral ligaments are very rare. We present a rare case of a boy in early childhood with non-specific medial joint line swelling and discomfort and was

Hand size association with ulnar collateral ligament reconstruction in professional baseball players.

BACKGROUND: Ulnar collateral ligament (UCL) injuries in professional baseball remain a significant problem with debate as to which are the greatest risk factors. Currently, it is unknown if hand size should be considered a predictor for UCL injury. A larger or smaller hand size could have implications on fatigue of the forearm musculature and thus varying amounts of strain seen by the UCL. The purpose of this study was to investigate if there is a correlation between hand size and history of UCL tear requiring a reconstruction. We hypothesize that a smaller hand size will be correlated with hi

Ulnar Collateral Ligament Repair With Suture Tape Augmentation: Can You Overtighten?

BACKGROUND: There has been a renewed interest and, recently, wider implementation of ulnar collateral ligament (UCL) repair in throwing athletes because of improvement in ligament repair technology and corresponding outcome data. PURPOSE: To compare the biomechanical parameters and failure mode between 2 brace-tightening techniques for UCL repair. STUDY DESIGN: Controlled laboratory study. METHODS: Eleven matched pairs of cadaveric arms were procured. One limb from each pair underwent UCL repair with suture tape augmentation with either (1) attempted restoration of physiologic ligament tens

Radial Collateral Ligament Laxity of Thumb Metacarpophalangeal Joint Following Trapeziometacarpal Arthrodesis.

PURPOSE: Trapeziometacarpal (TM) arthrodesis may increase adduction motion of the thumb metacarpophalangeal (MCP) joint, causing radial collateral ligament laxity. Stability of the MCP joint is important to the long-term functional outcome after TM arthrodesis. This study assessed preoperative and postoperative radial collateral ligament laxity using dynamic radiographs to confirm whether laxity was exacerbated after surgery and examined whether there is a relationship between the fixation angle of arthrodesis and the degree of laxity. METHODS: Forty-four thumbs in 33 patients who underwent T

Posterolateral Elbow Dislocation: An All-Arthroscopic Reconstruction of the Lateral Ulnar Collateral Ligament.

The elbow is one of the most commonly dislocated joints. Although simple dislocations of the elbow usually resolve with conservative management, certain patients can experience residual chronic instability. Posterolateral rotational instability accounts for approximately 80% of elbow chronic instability cases. We propose an all-arthroscopic reconstruction of the posterior, or ulnar, fascicle of the lateral ligament complex using an autograft or allograft, performed with a 5-mm-thick and 8-cm-long graft. The graft is first inserted distally into the supinator crest with an Arthrex 4.75-mm Swive

Thumb Metacarpophalangeal Joint Ulnar Collateral Ligament Injuries: Management and Biomechanical Evaluation.

First described in 1955 as "gamekeeper's thumb," injuries to the ulnar collateral ligament (UCL) of the thumb metacarpophalangeal joint are common and can cause pain and instability, especially during key pinch and grasp. Although primarily diagnosed on physical examination, stress radiographs, ultrasonography, and magnetic resonance imaging can be used to diagnose UCL injuries and distinguish partial from complete tears. If complete rupture occurs, the adductor aponeurosis can become interposed between the retracted UCL stump and its insertion on the proximal phalanx, known as a "Stener lesio

Arthroscopic lateral collateral ligament imbrication of the elbow: short-term clinical results.

INTRODUCTION: Chronic posterolateral rotatory instability (PLRI) of the elbow results from an insufficient lateral collateral ligament (LCL) complex. Arthroscopic LCL imbrication may prove a minimally invasive alternative to open lateral ulnar collateral ligament (LUCL) reconstruction with a quicker rehabilitation. The purpose of this study is to analyze the validity of a modified arthroscopic imbrication technique. We hypothesized that arthroscopic LUCL imbrication would yield stable elbows in patients with grade 1 or 2 chronic PLRI at a minimum of 2 year of follow-up. METHODS: We retrospect

Arthroscopic lateral collateral ligament imbrication for treatment of atraumatic posterolateral rotatory instability.

BACKGROUND: Among the causative lesions of lateral elbow pain, atraumatic posterolateral rotatory instability (PLRI) is a rare condition, but it produces pain and disability to the elbow. It still lacks published specific treatment literature. According to the development of arthroscopy, the arthroscopic lateral collateral ligament (LCL) imbrication can possibly be an alternative minimal invasive procedure that can eradicate the atraumatic PLRI. Hypothesis/Purpose: To report clinical results of arthroscopic LCL imbrication for the treatment on atraumatic PLRI elbow. Methods: Thirty-three patie
