Diagnostic value of computed tomography colonography (CTC) after incomplete optical colonoscopy.

INTRODUCTION: This study evaluated the role of computed tomography colonography (CTC) in patients who previously underwent incomplete optical colonoscopy (OC). We analyzed the impact of colonic lesions in intestinal segments not studied by OC and extracolonic findings in these patients. METHODS: Between January 2014 and May 2015, 61 patients with a history of abdominal pain and incomplete OC examination were studied by CTC. CTCs were performed by 320-row CT scan in both the supine and the prone position, without intravenous administration of contrast medium. In all patients both colonic findi

Colorectal Cancer Screening.

Colorectal cancer (CRC) is a leading cause of cancer morbidity and mortality in the Western world. Advances in surgical and medical management have led to improved outcomes; however, the prognosis of CRC is often poor when detected at a symptomatic stage. Most cases of CRC develop over years from removable well-defined precursor lesions, and asymptomatic, curable disease may be detected by convenient noninvasive tests. These features make CRC a suitable candidate for screening, and several options are available. This article outlines the evidence for established CRC screening tests along with

CT colonography: same-day tagging regimen with iodixanol and reduced cathartic preparation.

The purpose of the study was to test the tagging performance and patient's acceptance of a reduced cathartic preparation, based on iodixanol and PEG, administered to patients 3 h before the exam. One hundred and thirty-two asymptomatic patients were enrolled. As colonic cleansing we used PEG macrogol 3350. For fluid tagging iodixanol was orally administered 3 h before the exam, in a total dose of 50 mL mixed with 34.8 g of PEG in 750 mL of water. Image's review showed 446 segments (56.4%) clean of feces and 346 segments (43.6%) with feces. Untagged fluid was observed in 74/706 (10.5%) segments

The use of computed tomography in the detection and characterisation of large bowel obstruction.

Computed tomography (CT) has become the imaging modality of choice for the investigation of patients with clinico-radiological suspicion of obstruction. Appropriate CT technique is discussed, with recommendations on how to improve diagnostic confidence and to reduce radiation dose. The performance of CT in the investigation of LBO is compared to that of the plain abdominal radiograph and contrast enema. CT is unrivalled in its ability to detect and characterise the cause of large bowel obstruction (LBO), identify complications, and guide appropriate treatment.

Comparison of polyp distance on CT colonography between supine and prone scans using an automated path-distance measurement tool: correlation with colonoscopy.

OBJECTIVE: To investigate the proximity of polyp distance on CT colonography (CTC) between supine and prone scans using an automated path-distance measurement tool and to correlate the path distance of polyps with that of colonoscopy. METHODS: Of 196 polyps in 85 patients, 98 polyps (25 <6 mm, 42 6-9.9 mm, 31 > or =10 mm) detected on supine and prone positions in 72 patients, were included. The location of each polyp, which was expressed as the distance from the tip of the rectum, was measured using an automated path-distance measurement tool and was compared between the two positions. Magnetic resonance colonography versus colonoscopy as a diagnostic investigation for colorectal cancer: a meta-analysis.

AIMS: Magnetic resonance colonography (MRC) is emerging as a potential complementary investigation for the diagnosis of colorectal cancer (CRC) and also for benign pathology such as diverticular disease. A meta-analysis reporting the use of MRC is yet to be performed. The aim of this study was to evaluate the diagnostic accuracy of MRC compared with the gold-standard investigation, conventional colonoscopy (CC). METHODS: A literature search was carried out to identify studies containing comparative data between MRC findings and CC findings. Quantitative meta-analysis for diagnostic tests was

The Quebec Association of Gastroenterology position paper on colorectal cancer screening - 2003.

Colorectal cancer is a leading cause of death and the third most common cancer in Canada. Evidence suggests that screening can reduce mortality rates and the cost effectiveness of a program compares favourably with initiatives for breast and cervical cancer. The objectives of the Association des gastro-entérologues du Québec Task Force were to determine the need for a policy on screening for colorectal cancer in Quebec, to evaluate the testing methods available and to propose one or more of these alternatives as part of a formal screening program, if indicated. Fecal occult blood testing (FOBT

Effect of carbon dioxide insufflation on the delay-attenuation relationship of mesenteric veins in contrast-enhanced CT colonography.

To evaluate whether carbon dioxide (CO₂) insufflation modifies the relationship between scan delay and mesenteric venous attenuation during fixed-delay portal venous phase scans in contrast-enhanced CT colonography (CTC). This retrospective study included 170 patients (mean age, 66.6 ± 13.0 years; 72 men) who underwent contrast-enhanced abdominal CT between April 2017 and October 2021 with portal venous phase scans acquired at fixed delays of 25, 30, or 35 s after the start of arterial-phase scanning. Examinations were classified as the CO₂ insufflation group (n = 86) or the non-insufflation g

Colorectal Cancer and Advanced Adenoma after Single CT Colonography or Biennial FIT Screening in the SAVE Randomized Controlled Trial.

Background A 5-year screening interval for CT colonography (CTC) and a 2-year screening interval for fecal immunochemical tests (FITs) have been recommended on the basis of the natural history of colorectal cancer (CRC). Purpose To assess whether the recommended 5-year interval for screening CTC is appropriate and whether CTC is associated with earlier diagnosis relative to biennial FIT screening. Materials and Methods This secondary analysis of the SAVE randomized controlled trial was conducted between March 2012 and March 2018; 14 981 participants aged 54-65 years were randomly assigned to g

Evolving trends in CT colonography: A 10-year analysis of use and associated factors.

PURPOSE: Computed tomographic colonography (CTC) is a non-invasive screening test for colorectal cancer (CRC) with high sensitivity and low risk of complications. We used a nationally representative sample of screening-eligible adults to examine trends in and factors associated with CTC use. METHODS: We examined CTC use among 58,058 adults in the National Health Interview Survey in 2010, 2015, 2018, 2019, and 2021. For each survey year, we estimated CTC use by sociodemographic and health factors. We used multivariable logistic regression to identify factors associated with CTC use. RESULTS:

Use of Screening CT Colonography by Age and Race: A Study of Potential Access Barriers Related to Medicare Noncoverage Based on Data From the ACR's National CT Colonography Registry.

OBJECTIVE: The primary objectives of this investigation were to evaluate the use of screening CT colonography (CTC) examinations by age comparing individuals of Medicare-eligible age to younger cohorts and to determine if the association between use of CTC and Medicare-eligible age varies by race. Although the Affordable Care Act requires commercial insurance coverage of screening CTC, Medicare does not cover screening CTC. MATERIALS AND METHODS: Using the ACR's CTC registry, the distribution of procedures by age was evaluated using a negative binomial model with patient age (to capture overa

ACR Appropriateness Criteria colorectal cancer screening.

Colorectal cancer is the third leading cause of cancer deaths in the United States. Most colorectal cancers can be prevented by detecting and removing the precursor adenomatous polyp. Individual risk factors for the development of colorectal cancer will influence the particular choice of screening tool. CT colonography (CTC) is the primary imaging test for colorectal cancer screening in average-risk individuals, whereas the double-contrast barium enema (DCBE) is now considered to be a test that may be appropriate, particularly in settings where CTC is unavailable. Single-contrast barium enema

Clinical validation of high-resolution fast spin-echo MR colonography after colon distention with air.

To evaluate the feasibility of MR colonography (MRC) with air using two-dimensional (2D) T1-weighted fast spin-echo (T1wFSE) in patients scheduled for conventional colonoscopy (CC) after classic bowel preparation, and assess the ability of the technique to detect colonic lesions. The distention was sufficient for diagnosis, and the technique provided adequate delineation of the wall in the majority of segments. Residual fluid obscured the wall in different segments, especially in the ascending and descending colon (supine position) and in the cecum, transverse, and sigmoid colon (prone positio

Actual colonic perforation in virtual colonoscopy: report of a case.

Computed tomography colonography, also termed virtual colonoscopy, is a new imaging method to investigate the colon, which may be a potential alternative to the conventional endoscopic colonoscopy in some cases. The high safety profile of this imaging method was considered as an additional advantage of this procedure. A case of colonic perforation in computed tomography colonography is presented, highlighting a potential risk related to this procedure. It is assumed that perforation was the result of overinflation of air into an obstructed colon caused by a lesion at the rectosigmoid junction.

Virtual gastroduodenoscopy: a new look at the bypassed stomach and duodenum after laparoscopic Roux-en-Y gastric bypass for morbid obesity.

BACKGROUND: After open or laparoscopic Roux-en-Y gastric bypass (RYGBP) for morbid obesity, the bypassed stomach and duodenum are not readily available for radiological and endoscopic evaluation. Furthermore, little is known about the long-term physiologic and histologic changes that occur in the bypassed GI segments following these procedures. Many alternative radiological and endoscopic techniques have been described to access the distal gastric pouch and the duodenum after RYGBP. Apart from percutaneous gastrografin studies, all these techniques require the insertion of a gastrostomy tube i

Magnetic resonance colonography for colorectal cancer screening.

Colorectal cancer screening has vast potential. Beyond considerations of cost and diagnostic accuracy, the effectiveness of any colorectal screening strategy will be dependent on the degree of patient acceptance. Magnetic resonance (MR) colonography has been shown to be accurate regarding the detection of clinically relevant colonic polyps exceeding 10 mm in size, with reported sensitivity and specificity values exceeding 95%. To further enhance patient acceptance, strategies for fecal tagging have recently been developed. By modulating the signal of fecal material to be identical to the signa
