Metronidazole prophylaxis in colorectal surgery: the need for additional aminoglycoside?

A randomized double-blind trial was conducted in two phases to compare the efficacy of prophylactic oral and intravenous administration of kanamycin and metronidazole in preventing wound infection and other complications in patients undergoing colorectal surgery, and also to assess the need for inclusion of an aminoglycoside in the prophylactic regimen. Adequate prophylaxis did not depend on the route of antimicrobial administration, but wound infection rates were increased following exclusion of kanamycin.

Effect of prophylactic systemic administration of cephalothin in colorectal surgery.

The effect of standardized prophylactic treatment with systemically administered cephalothin (Keflin) was studied in a series of 120 consecutive patients in whom elective colorectal surgery was planned. The patients were divided by random selection into one treatment group and one control group. The same mechanical cleansing with cathartics and tap water enemas was performed in both groups. The patients in the treatment group received intravenously 2 g cephalothin 2 hours prior to the operation, 2 g during the operation and then 2 g every 6th hour during 4 days. 14 cases were excluded for vari

Estimation of the postoperative fatality window in colorectal cancer surgery.

BACKGROUND: Postoperative death measured 30 days after surgery is a conventional quality metric, whereas intervals up to 90 days are increasingly used, although data-driven time windows have scarcely been investigated. METHODS: The Swedish Colorectal Cancer Registry was used to identify all patients subjected resection for colorectal cancer between 2007 and 2020. All patients were followed up until 180 days after surgery. A join-point statistical hazard model was used to model a declining hazard to a transition point, followed by a stable death rate. This method was subsequently applied to de

Feasibility of on-demand robotics in colorectal surgery: first cases.

INTRODUCTION: The key benefits of robotics are improved precision and control, thanks to fully articulated robotic instruments and enhanced, stable endoscope control. However, colorectal procedures also require large movements such as medialization of the colon where a robotic platform is not always needed. We present the world's first experience in colorectal surgery with a new open platform of on-demand robotics. METHODS AND PROCEDURES: Standard laparoscopic 3-D camera, insufflator, trocars and energy devices, available in all hospitals performing laparoscopic surgery, are used in combinati

Preoperative Identification and Management of Anemia in the Colorectal Surgery Patient.

Preoperative anemia is a common finding in patients undergoing colorectal surgery, particularly those with cancer. While often multifactorial, iron deficiency anemia remains the most common cause of anemia in this patient population. Although seemingly innocuous, preoperative anemia is associated with an increased risk of perioperative complications and need for allogenic blood transfusions, both of which may worsen cancer-specific survival. Preoperative correction of anemia and iron deficiency is thus necessary to diminish these risks. Current literature supports preoperative screening for an

Sugammadex in Colorectal Surgery: A Systematic Review and Meta-analysis.

BACKGROUND: Traditionally, reversal of neuromuscular blocking agents following the completion of surgery was achieved with cholinesterase inhibitors. Recently, sugammadex has been increasingly relied upon. Sugammadex is a γ-cyclodextrin molecule that rapidly reverses steroidal neuromuscular blocking drugs. Its use following colorectal surgery has become more common, and while the rapidity of reversal is undoubtedly improved, whether sugammadex impacts clinical postoperative outcomes is unknown. This systematic review and meta-analysis aims to compare postoperative outcomes in patients receivin

A simple regimen for control of postoperative sepsis in colorectal surgery.

A preoperative schedule based on three principles: rigorous mechanical cleansing of the intestine, nutrition for four days with a formula diet (Biosorbin MCT), and short-time systemic prophylaxis with doxycycline has been tested in a prospective study of 252 patients who underwent elective colorectal surgery. There were 11 wound infections, most of them of minor importance (4.4 per cent). Intra-abdominal abscesses occurred in two patients, both successfully reoperated, giving an overall incidence of septic complications of 5.1 per cent. The schedule was easy for the staff to administer, well t

Goal-directed fluid therapy on laparoscopic colorectal surgery within enhanced recovery after surgery program.

INTRODUCTION: Enhanced recovery after surgery protocols (ERAS) are used in peri-operative care to reduce the stress response to surgical aggression. As fluid overload has been associated with increased morbidity and delayed hospital discharge, a major aspect of this is fluid management. Intra-operative goal-directed fluid protocols have been shown to reduce post-operative complications, particularly in high risk patients.?. OBJECTIVE: To compare 2fluid therapy models (zero-balance versus goal-directed fluid therapy) in patients who were scheduled for laparoscopic colorectal surgery within an

Role of extended venous thromboembolism prophylaxis in patients with colorectal neoplasia who underwent minimally invasive colorectal surgery within an enhanced recovery after surgery protocol.

BACKGROUND: Patients with colorectal cancer are at increased risk of venous thromboembolism (VTE) due to malignancy-associated hypercoagulability and the physiologic stress of surgery. Current guidelines recommend extended-duration thromboprophylaxis for up to 28 days following major abdominal or pelvic cancer surgery. However, its effectiveness in patients managed by Enhanced Recovery After Surgery (ERAS) pathways is uncertain. This study evaluated the effectiveness of extended thromboprophylaxis after minimally invasive colorectal resection managed within a mature ERAS program in reducing cl

Robotic-assisted colorectal surgery in colorectal cancer management: a narrative review of clinical efficacy and multidisciplinary integration.

Colorectal cancer (CRC) remains a formidable global health challenge, ranking among the most prevalent malignancies and a principal contributor to cancer-associated mortality. While traditional open surgery has historically been the cornerstone of CRC treatment, the advent of minimally invasive techniques, particularly robotic-assisted colorectal surgery (RACS), has garnered significant momentum owing to technological advancements in the field. Robotic platforms, exemplified by the da Vinci Surgical System, offer superior three-dimensional visualization, enhanced dexterity, and heightened prec
