The following report describes an unusual case of smothering by a medical nebulizer of a psychiatric in-patient and discusses the manner of death. A 43-year-old man, with previous medical history of paranoia was hospitalized at a psychiatric department because of a suicide attempt. He was found dead by the medical staff in the seclusion room. The patient is known suffering from asthma attacks treated by Ventoline(®) spray. Autopsy findings showed a ventoline's nebulizer impacting within the pharynx and in the upper part of oesophagus. These findings were compatible with a death by smothering.

The aim of this study was to determine the prevalence of mental disorders among offenders admitted at the health establishments in Bizzah Makhathe Correctional Centre, South Africa. BACKGROUND: Borderline personality disorder (BPD) is characterized by a pervasive pattern of instability and impulsivity. There is a high prevalence of BPD patients among those admitted to the emergency department for suicide attempts. However, little empirical research exists to assist clinicians in deciding whether to hospitalize a suicidal patient. Some authors have argued that hospitalization does not prevent suicide and could actually harm these patients, thereby leading to psychosocial regression. Parasuicidal behaviors could be reinforced by the attention given during hospitalization.

OBJECTIVE: Specific ethnic groups appear to be disproportionally represented in emergency compulsory admissions. This may be due to a parallel higher risk of psychopathology, but different pathways to care in patients from ethnic minorities may also be an explanatory factor. Forensic consultations with psychotic inpatients frequently include issues of risk management, such as dangerousness and civil commitment.
