Conjunctival dermoid cyst seen on examination as a chronically red eye.

We describe a patient who was seen with a unilateral red eye and purulent discharge refractory to antibiotic treatment with multiple antibiotic regimens over the previous 4 months. Initially it was believed to be a bacterial conjunctivitis, but when conventional treatments failed it was thought to be a viral conjunctivitis with bacterial superinfection. Cultures yielded only Enterococcus. There was a small mass with a draining fistulous tract at the lateral canthus. Symptoms persisted despite courses of topical and systemic antibiotics followed by a tapering dose of steroids. The patient was t

Differentiating conjunctivitis of diverse origins.

While symptoms can be very distressing, patients with conjunctivitis general maintain good vision and recover completely without permanent sequelae. The great majority of cases of conjunctivitis are infectious or allergic in origin. Seen with increasing frequency are external eye diseases related to contact lens wear or prolonged use of ophthalmic medications. The various forms of conjunctivitis are often not associated with pathognomonic features. A thorough history and ophthalmic examination often permit a presumptive diagnosis and initiation of empiric therapy. For example, a chronic bilate

Relation of the outcome of conjunctivitis and the conjunctivitis-otitis syndrome to identifiable risk factors and oral antimicrobial therapy.

The epidemiology, microbiology and clinical outcome of the conjunctivitis-otitis syndrome (CJ-AOM) was investigated in a rural private practice concurrent to a double blind placebo-controlled study of orally administered amoxicillin for prevention of acute otitis media (AOM) secondary to conjunctivitis. Bacterial pathogens were isolated when greater than 15 polymorphonuclear leukocytes/high power field were observed on Gram-stained smear of conjunctival secretions. Nontypable Haemophilus influenzae biotype 2 predominated in CJ-AOM; however, Streptococcus pneumoniae was isolated nearly as frequ

Gonococcal conjunctivitis: A case report.

We report a case of acute gonococcal conjunctivitis in a 36-year-old woman who presented with eye redness and a history of discharge for one month. Prior to presenting to us, she was treated for bacterial conjunctivitis with three courses of local antibiotics by three different clinics after brief assessments and without improvement. The final diagnosis of gonococcal conjunctivitis was made after a complete history was elicited and supported by the presence of Neisseria Gonorrhoeae in the eye swab culture test. She and her sexual partner were treated successfully with intramuscular Ceftriaxone

Laboratory studies in acute conjunctivitis.

In 84% of 700 consecutive cases of untreated acute conjunctivitis subjected to conjunctival cultures and scrapings, a presumptive etiologic diagnosis could be made based on standard microbiologic and cytologic criteria. Of these 583 cases, 264 were bacterial, 245 were viral, 51 were allergic, and 17 were chlamydial. In 75% of the cases, clinical impressions were consistent with laboratory findings. On the basis of these results, laboratory workup is recommended in (1) follicular conjunctivitis to differentiate viral from chlamydial processes, (2) purulent conjunctivitis to identify pathogens a

[Isolation of Haemophilus aegyptius associated with Brazilian purpuric fever, of Chloropidae (Diptera) of the genera Hippelates and Liohippelates].

The recognition of the Brazilian purpuric fever (BPF) in 1984 led to a number of studies which showed a relation between this disease and conjunctivitis caused by Haemophilus aegyptius. The increase in cases of conjunctivitis in children associated with higher population density of eye gnats (Chloropidae: Hippelates) has been reported since last century. This phenomenon is related to the attraction that those flies show for the eyes, secretions and wounds, from where they feed on. Although there are evidences on the role of these flies in the mechanical transmission of seasonal bacterial conju

Incidence of Moraxella conjunctival infection.

Thirty-three cases of Moraxella conjunctival infection, many incorrectly diagnosed as Chlamydia, epidemic keratoconjunctivitis, and herpes simplex infections, were confirmed by bacterial cultures and Giemsa staining during a one-year period. Although Moraxella conjunctivitis is associated with adolescent girls, 13 of our 27 female patients with Moraxella infection (48%) were 20 years or older. Although corneal involvement, preauricular adenopathy, and ocular nodules are not classically associated with Moraxella conjunctivitis, these symptoms were present in 15 cases (45%).

Conjunctival Intraepithelial Lymphocytes, Lacrimal Cytokines and Ocular Commensal Microbiota: Analysis of the Three Main Players in Allergic Conjunctivitis.

Conjunctival intraepithelial lymphocytes, tear soluble molecules and commensal microbiota have important roles in the ocular mucosal immune response in healthy and diseased subjects. For the purpose of this study, the cellular and microbial populations of the conjunctiva and the lacrimal soluble molecules were analyzed to find the main biomarkers in allergic conjunctivitis. A total of 35 healthy subjects, 28 subjects with seasonal allergic conjunctivitis and 32 subjects with perennial allergic conjunctivitis were recruited to obtain peripheral blood, conjunctival brush cytology, tear fluid and

[Primary meningococcal conjunctivitis: implications beyond the conjunctiva].

BACKGROUND: Neisseria meningitidis is a uncommon cause of acute bacterial conjunctivitis. However, its diagnosis has important therapeutic implications. METHODS: From December 1993 to January 1984, a prospective study on acute bacterial conjunctivitis was performed at the Hospital Universitario Materno-Infantil Vall d'Hebron. Primary meningococcal conjunctivitis (PMC) was diagnosed in 34 patients. The diagnosis of PMC was made on the basis of consistent clinical manifestations together with isolation of Neisseria meningitidis from conjunctival exudate culture. RESULTS: There were 16 men and

Study of an outbreak of epidemic conjunctivitis in Delhi in 1986.

An epidemic of acute conjunctivitis occurred in Delhi during July-September 1986. The clinical presentation in total of 350 patients were characteristic of acute haemorrhagic conjunctivitis (AHC). Bacterial cultures from conjunctival swabs were sterile in 85 out of 105 (80%) patients, virus cultures in Vero and HeLa cells were negative in 30 patients. However, conjunctival scrapes from 20/30 (66.7%) patients showed specific cytoplasmic fluorescence with enterovirus 70 (EV 70) antiserum, suggesting EV 70 to be the etiologic agent of the epidemic.

Conjunctival scrapings and impression cytology in chronic conjunctivitis. Correlation with microbiology.

To assess the utility of exfoliative cytology in the diagnosis of chronic conjunctivitis, we compared its findings with microbiological examination and established the correlation between cytology and microbiology in 32 patients with conjunctivitis lasting four weeks or more, with a nonspecific clinical picture. The material for cytology was collected by scraping and by imprint of the ocular surface on Millipore filter paper. The specimens were stained with Giemsa. Material for bacterial culture and fluorescent antibody test for viruses and Chlamydia were obtained at the same time. In 12 patie

Ophthalmologists with conjunctivitis: are they fit to work?

PURPOSE: To establish current opinion as to whether ophthalmologists with conjunctivitis are fit to work. METHODS: One hundred and sixty ophthalmology units in the United Kingdom were sent a postal survey enquiring about work practices when an ophthalmologist contracts conjunctivitis. RESULTS: One hundred and five replies were received. Twenty-nine per cent of respondents said the ophthalmologist should stay off work while 7% said he or she should continue as usual. There was no concordance as to whether viral or bacterial conjunctivitis posed a greater problem. There is evidence that ophtha

Treatment and prevention of ophthalmia neonatorum.

QUESTION: In my office I occasionally see neonates with conjunctivitis. What are the current recommendations for ocular prophylaxis at birth? Do topical antibiotics alone provide adequate treatment of neonatal conjunctivitis? When is systemic therapy indicated? ANSWER: All infants should receive ocular prophylaxis at birth to prevent gonococcal ophthalmia. Neonates presenting with signs of conjunctivitis should have a conjunctival swab sent for Gram stain and culture. If Gram-negative diplococci are present on the Gram stain results, the infants and their parents should be treated immediately

Enhanced anti-inflammatory and antibacterial efficacy of a novel gum ghatti-infused polymeric film for ocular drug delivery.

BACKGROUND AND PURPOSE: Ocular inflammation is a key challenge after ocular surgery, and resistance has developed in the bacterial strain. Primarily, the short residence time of the ocular formulation makes it more effective in treating and alleviating inflammation. EXPERIMENTAL APPROACH: In this study, a moxifloxacin-incorporated gum ghatti-infused poly(vinyl alcohol-chitosan) polymeric film was developed by solvent casting. Various properties of the ocular composite film, including mucoadhesiveness, strength profile, and crystal size, were evaluated. KEY RESULTS: The prepared composite fil
