Impact of previous abdominal surgery on perioperative outcomes of laparoscopic pancreaticoduodenectomy: a propensity score matching study.

Laparoscopic pancreaticoduodenectomy (LPD) is a complex procedure. The impact of different types of previous abdominal surgeries (PAS) on its perioperative outcomes remains unclear. Clinical data of 794 LPD patients were collected and divided into major-PAS, minor-PAS, and no-PAS groups based on PAS type and extent. Patients with current or previous complicated intra-abdominal infectious conditions were excluded. Propensity score matching (PSM, 1:2:4) was performed using baseline variables. Intraoperative and postoperative outcomes were compared among groups. Multivariable logistic regression

Converting From Laparoscopic Cholecystectomy to Open Cholecystectomy: A Systematic Review of Its Advantages and Reasoning.

Cholecystectomy is the standard treatment for symptomatic cholelithiasis and asymptomatic impending biliary obstruction, which is typically carried out laparoscopically. However, difficult gallbladders, due to distorted anatomy or increased risk of bleeding, can necessitate conversion to open surgery. This systematic review evaluates the advantages, disadvantages, complications, and outcomes of laparoscopic versus converted open cholecystectomy. We screened articles published from 2011 to 2024 by utilizing advanced filters of PubMed, Cochrane, and Scholar databases. Exclusion criteria included

Does conversion of a laparoscopic colectomy adversely affect patient outcome?

PURPOSE: Conversion during laparoscopic colectomy varies in frequency according to the surgeon's experience and case selection. However, there remains concern that conversion is associated with increased morbidity and higher hospital costs. METHODS: From January 1999 to August 2002, 430 laparoscopic colectomies were performed by two surgeons, with 51 (12 percent) cases converted to open surgery. Converted cases were matched for operation and age to 51 open cases performed mostly by other colorectal surgeons from our department. Data collected included gender, American Society of Anesthesiolog

Laparoscopic cholecystectomy for acute cholecystitis. Can preoperative factors predict conversion?

OBJECTIVE: To determine if preoperative clinical, laboratory and radiology data can predict conversion of laparoscopic cholecystectomy for acute cholecystitis to open procedure. METHODS: Retrospective analysis of 44 laparoscopic cholecystectomies were performed for acute cholecystitis between August 2000 and July 2002 at King Khalid University Hospital, Riyadh, Kingdom of Saudi Arabia. Data related to age and sex of patients, maximum body temperature, white blood cell count, gallbladder wall thickness on ultrasonography and timing of surgery from onset of symptoms were collected. RESULTS: Th

Evaluation of preoperative risk factors for converting laparoscopic to open cholecystectomy.

Performing laparoscopic cholecystectomy (LC) always carries the risk of having to convert from laparoscopic to open cholecystectomy (LOC). Being able to identify these patients preoperatively may allow better preoperative planning and lowering operative cost. All LC and LOC were performed by the Eastern Virginia Medical School Department of Surgery retrospectively identified between January 2008 and December 2009. Preoperative risk factors identified in both groups included: age, gender, body mass index greater than 30 kg/m(2), diabetes mellitus, previous upper abdominal surgery, previous abdo

Assessment of Predictors of Difficult Laparoscopic Cholecystectomy by Clinico-Radiological Parameters at a Tertiary Hospital in Eastern India.

BACKGROUND: Laparoscopic cholecystectomy has become the standard treatment for gallbladder (GB) stones, favored for its minimally invasive approach. Despite its benefits, the procedure sometimes requires conversion to open cholecystectomy due to intra-operative challenges, with conversion rates varying between 1% and 13%. There are various preoperative predictors that help in identifying such difficult cases and help to proceed safely. This study aims to identify the preoperative factors that could predict the difficulty of laparoscopic cholecystectomy, thus anticipating the need for conversio

Central tumour location should be considered when comparing N1 upstaging between thoracoscopic and open surgery for clinical stage I non-small-cell lung cancer.

OBJECTIVES: Nodal upstaging is a quality indicator for oncological thoracic surgery and is found in up to 25% of patients with clinical stage I (cStage-I) non-small-cell lung cancer (NSCLC). In large retrospective series, lower N1 upstaging was reported after video-assisted thoracic surgery (VATS) resections. We studied the impact of central primary tumour location on nodal upstaging in cStage-I NSCLC. METHODS: Consecutive patients operated for cStage-I NSCLC were selected from a prospectively managed surgical database. Tumour location was classified as central if the lesion was visible durin

Repeated surgery for recurrence is not a risk factor for conversion in patients undergoing laparoscopic ileo-colonic resection for Crohn disease.

BACKGROUND: Patients who undergo ileo-colonic resection for Crohn disease are at risk of repeated surgery because of recurrence. Minimally invasive approach is debated in recurrent cases. This study aims to identify predictors for conversion to open surgery and explore if a repeated laparoscopic procedure represents a risk factor for conversion. Outcomes of recurrent versus primary surgery were also evaluated. METHODS: This is retrospective single-center cohort study enrolling all patients undergoing laparoscopic surgery for primary and recurrent ileo-colic Crohn disease between January 2017

Conversion during laparoscopic surgery: frequency, indications and risk factors.

PURPOSE: There are limited data on the indications for open conversion during laparoscopic surgery. The frequency of conversion for various procedures is poorly quantified and the degree to which this changes with time is not well understood. Risk factors for conversion are not defined. We addressed these issues in a large series of laparoscopic operations. MATERIALS AND METHODS: We reviewed our database of 2,128 laparoscopic operations performed between 1993 and 2005, including radical nephrectomy in 549 patients, simple nephrectomy in 186, partial nephrectomy in 347, donor nephrectomy in 55

Factors associated with conversion to laparotomy in patients undergoing laparoscopic appendectomy.

BACKGROUND: Laparoscopic appendectomy (LA) has been increasingly adopted for its advantages over the open technique, but there is a possibility of conversion to open appendectomy (OA) if complications occur or the extent of inflammation prohibits successful dissection. This study aimed to identify the preoperative predictors for conversion from laparoscopic to open appendectomy. STUDY DESIGN: Medical records of 705 consecutive patients who underwent surgery for suspected appendicitis were reviewed retrospectively. LA was attempted in 595 patients by 25 different surgeons.
