Paraplegia following intraaortic balloon assistance.

In a patient receiving intraaortic balloon counterpulsation following myocardial revascularization, paraplegia developed two days post-operatively. Postmortem examination demonstrated a dissecting hematoma of the thoracic aorta and spinal cord infarction. The neurological deficit is thought to be due to embarrassment of spinal cord blood supply, and mechanisms of injury are discussed.

[Physical training integration with physiotherapeutic methods of myocardial metabolism recovery in the rehabilitation of patients after acute coronary syndrome and cardiosurgical myocardial revascularization].

UNLABELLED: Successes achieved in the treatment of patients with coronary heart disease (CHD) after acute coronary syndrome and cardiosurgical interventions require the development of new, more advanced methods for their rehabilitation. This is because of metabolic disturbances persistence in the deeply ischemic myocardium in patients with coronary heart disease even after restoration of blood flow through the coronary arteries by surgical or endovascular methods due to the development of the phenomenon of «stunning» and hibernation. The discovery of the development of their own adaptation to

Feasibility of Pump Speed Modulation for Restoring Vascular Pulsatility with Rotary Blood Pumps.

Continuous flow (CF) left ventricular assist devices (LVAD) diminish vascular pressure pulsatility, which may be associated with clinically reported adverse events including gastrointestinal bleeding, aortic valve insufficiency, and hemorrhagic stroke. Three candidate CF LVAD pump speed modulation algorithms designed to augment aortic pulsatility were evaluated in mock flow loop and ischemic heart failure (IHF) bovine models by quantifying hemodynamic performance as a function of mean pump speed, modulation amplitude, and timing. Asynchronous and synchronous copulsation (high revolutions per m

The Influence of Different Operating Conditions on the Blood Damage of a Pulsatile Ventricular Assist Device.

Because of pulsatile blood flow's benefit for myocardial recovery, perfusion of coronary arteries and end organs, pulsatile ventricular assist devices (VADs) are still widely used as paracorporeal mechanical circulatory support devices in clinical applications, especially in pediatric heart failure patients. However, severe blood damage limits the VAD's service period. Besides optimizing the VAD geometry to reduce blood damage, the blood damage may also be decreased by changing the operating conditions. In this article, a pulsatile VAD was used to investigate the influence of operating conditi

Iatrogenic vascular trauma associated with intra-aortic balloon pumping: identification of risk factors.

The most important noncardiac consequence of use of the intra-aortic balloon pump (IABP) is associated iatrogenic vascular trauma. A retrospective analysis was undertaken of all patients at our institution (n = 90, mean age 60 years) who had insertion of an IABP over a 10-year period to assess the possibility of preoperative identification of patients at high risk of IABP associated vascular injury. Catheters were introduced via the common femoral artery (n = 84; 30 percutaneous, 4 open Seldinger technique, 50 via a dacron sleeve), external iliac artery (n = 2), and ascending aorta (n = 4). Th

The Effect of EECP on Ischemic Heart Failure: a Systematic Review.

PURPOSE OF REVIEW: Heart failure is a serious global health problem, and coronary artery disease is one of the main causes. At present, the treatment options for ischemic heart failure (IHF) are limited. This article mainly aims to explore the evidence of enhanced external counterpulsation (EECP) as a non-invasive cardiac rehabilitation method in patients with IHF and to make a preliminary exploration of its mechanisms. RECENT FINDINGS: According to the existing evidence, the standard course of EECP is safe in patients with IHF and can significantly improve the quality of life of these patien

Vasopressin effective in reversing catecholamine-resistant vasodilatory shock.

A patient with perforated appendicitis developed progressive vasodilatory shock which was complicated by perioperative acute myocardial infarction. Cardiovascular support included dopamine infusion, and later, intra-aortic balloon counterpulsation balloon pump and noradrenaline and dobutamine infusion. Vasopressin was introduced as a final attempt to reverse the refractory shock and was associated with recovery. The experience with this case suggests that vasopressin may be a valuable adjunct to the treatment of catecholamine-resistant vasodilatory shock.

Percutaneous left ventricular support in cardiogenic shock and severe aortic regurgitation.

Severe aortic regurgitation (AR) is a well-established contraindication to intra-aortic balloon counterpulsation therapy. We report two cases of medically refractory cardiogenic shock in patients with severe AR who were successfully bridged to surgical left ventricular assist device implantation by percutaneous left atrial to femoral artery mechanical bypass. Further investigation into the clinical utility of percutaneous mechanical support in the setting of AR and shock is required.

Tricuspid valve replacement for cardiogenic shock after acute right ventricular infarction.

A 66-year-old man had an acute inferior wall myocardial infarction complicated by hypotension, high-grade atrioventricular block, and distended neck veins, suggesting associated right ventricular infarction. He failed to respond to volume loading, intra-aortic balloon counterpulsation, and pharmacologic afterload reduction. Cardiac catheterization five days after the infarction disclosed severe tricuspid regurgitation and a hemodynamic pattern resembling pericardial constriction. Emergency tricuspid valve replacement was dramatically successful.

[Intra-aortic balloon pump in patients with myocardial infarction and cardiogenic shock of stages A and B].

This article presents two clinical cases of patients with myocardial infarction and stage A (at risk) and B (beginning) cardiogenic shock who underwent intra-aortic balloon counterpulsation (IABP). In patients with a high risk of classic cardiogenic shock and/or the no-reflow phenomenon, stenting of the infarct-related coronary artery during this type of mechanical circulatory support was performed without complications. Theoretical and practical aspects of using IABP at different stages of cardiogenic shock are discussed.

Use of the intra-aortic balloon pump to stop gastrointestinal bleeding.

Temporary aortic occlusion can be lifesaving in selected conditions. We describe the unorthodox use of an intra-aortic balloon pump without counterpulsation to achieve temporary vascular control in a patient with shock caused by rapid upper gastrointestinal bleeding. The technique of aortic balloon occlusion has been reported in several clinical circumstances, primarily trauma. However, its use to increase blood pressure and gain time for resuscitation before laparotomy for catastrophic gastrointestinal bleeding has not previously been described.

Simplified method of heterotopic cardiac transplantation for left ventricular assist.

A method of heterotopic cardiac transplantation has been developed which is performed through a left thoracotomy and requires only two vascular anastomoses. Systemic heparinization and cardiopulmonary bypass are not necessary. Preliminary experiments demonstrated that this heterotopic transplant technique was effective for left ventricular assist; in some experiments, a multimode atrioventricular programmable pacemaker was used to provide sequential pacing for "counterpulsation." This technique or similar techniques may have potential clinical benefit for patients with intractable left ventric

Evaluation of an intraventricular balloon pump for short-term support of patients with heart failure.

The high cost of ventricular assist devices results in poor cost-effectiveness when used as a short-term bridging solution, thus a low-cost alternative is desirable. The present study aimed to develop an intraventricular balloon pump (IVBP) for short-term circulatory support, and to evaluate the effect of balloon actuation timing on the degree of cardiac support provided to a simulated in vitro severe heart failure (SHF) patient. A silicone IVBP was designed to avoid contact with internal left ventricular (LV) features (ie, papillary muscles, chordae, aortic, and mitral valves) based on LV com

[S3-Guideline: Recommendations for intra-aortic balloon pumping in cardiac surgery].

Although intra-aortic balloon pumping (IABP) is the most frequently used mechanical cardiac assist device in cardiothoracic surgery, there are only guidelines for substantive sections of aortic counterpulsation including prophylactic and postoperative use. In contrast, evidence-based recommendations are still lacking concerning intraoperative use, management, contraindication and other relevant issues. According to international surveys, important aspects of IABP usage show a wide variation in clinical practice. The results of a national questionnaire performed before initiation of this guidel

Echographic assessment of visceral perfusion under intra-aortic balloon pump support.

BACKGROUND: Intra-aortic balloon pump (IABP) is a classical mechanical hemodynamic support device. However, the influence of IABP counterpulsation on visceral flow is still undetermined. METHODS: This prospective study included 33 patients who were treated with the IABP in a single cardiovascular center. Transabdominal echography was used to evaluate blood flow in the visceral branches under IABP support. The aim of this study is to evaluate the influence of the IABP counterpulsation on visceral perfusion and identify the risks of visceral arterial compromise. RESULTS: Visceral perfusion was

Intra-Aortic Balloon Pumping in Acute Decompensated Heart Failure With Hypoperfusion: From Pathophysiology to Clinical Practice.

Trials on intra-aortic balloon pump (IABP) use in cardiogenic shock related to acute myocardial infarction have shown disappointing results. The role of IABP in cardiogenic shock treatment remains unclear, and new (potentially more potent) mechanical circulatory supports with arguably larger device profile are emerging. A reappraisal of the physiological premises of intra-aortic counterpulsation may underpin the rationale to maintain IABP as a valuable therapeutic option for patients with acute decompensated heart failure and tissue hypoperfusion. Several pathophysiological features differ bet

Role of nitrates in acute myocardial infarction.

In patients with acute myocardial infarction, intravenous nitroglycerin lowers left ventricular filling pressure and systemic vascular resistance. At lower infusion rates (less than 50 micrograms/min) nitroglycerin is principally a venodilator, whereas at higher infusion rates more balanced venous and arterial dilating effects are seen. Patients with left ventricular failure demonstrate increased or maintained stroke volumes, whereas patients without failure show a decrease in stroke volume. All hemodynamic subgroups show a decrease in left ventricular filling pressures and a reduction in elec

[Treatment of cardiogenic shock with exclusion of inotropic drugs].

Cardiogenic shock comprises peripheral hypoperfusion and pulmonary vascular overload. The goals of therapy are to reduce pulmonary congestion by lowering pulmonary capillary wedge pressure and to increase cardiac index. Volume loading is the first step of treatment. It helps to place the patient on the Franck-Starling relationship. This challenge studies the effects of an increased preload on stroke volume. It has to be done even in case of major heart failure. The main effect of venous vasodilators is to decrease myocardial oxygen consumption. Arteriolar vasodilators also decrease left ventri

Does tirofiban prevent platelet loss in patients after cardiogenic shock during continuous renal replacement therapy?

Link and colleagues present a pilot study investigating platelet function and platelet numbers in patients with cardiogenic shock and acute kidney failure undergoing continuous venovenous haemodialysis. Their data indicate a significantly reduced platelet loss with combined therapy of unfractionated heparin plus tirofiban, the glycoprotein IIb/IIIa antagonist, compared with unfractionated heparin therapy alone. Owing to the small sample size, however, the potential impact of additional treatment variables (antiplatelet agents, intraaortic counterpulsation) could not be clarified. A substantial
