Rational use of systematic postoperative CT scans after neurosurgical craniotomy.

BACKGROUND: The aim of this retrospective study was to evaluate the relevance of a systematic postoperative CT scan after neurosurgical craniotomy and to identify predictive factors of complications. METHODS: This retrospective analysis included all the patients at our institution who benefited from a cerebral postoperative CT scan within 24 hours post-craniotomy. Patient characteristics and neuroimaging abnormalities were recorded. Predictive factors were identified using a recursive partitioning analysis. RESULTS: A total of 633 patients were included. Of these, 17.9% of patients suffered

Postarrest targeted temperature management immediately following craniotomy-a case report.

Therapeutic hypothermia or targeted temperature management (TTM) has been shown to improve survival and neurological outcome after cardiac arrest. TTM is not frequently utilized in the postoperative setting because of the concern for exacerbation of bleeding. We present the case of a 65-year-old man who had a cardiac arrest during craniotomy for a brain tumor resection. He was successfully resuscitated from pulseless electrical activity and remained unresponsive. After assessment for postoperative brain hemorrhage, the neurocritical care team initiated TTM. Repeat imaging revealed no additiona

A 3-dimensional computed tomographic procedure for planning retrosigmoid craniotomy.

OBJECTIVE: The planning of retrosigmoid craniotomies often relies on anatomic landmarks on the posterolateral surface of the cranium, such as the asterion. However, the location of the asterion is not fixed with respect to the underlying transverse-sigmoid sinus complex. We introduce a simple procedure that uses 3-dimensional (3D) computed tomographic imaging to project the transverse-sigmoid sinus complex onto the external surface of the cranium. METHODS: We enrolled 8 patients scheduled for retrosigmoid craniotomy (Group 1) and 30 patients without posterior fossa lesions (Group 2). The proc

Image-guided craniotomy for frontal sinus preservation during meningioma surgery.

OBJECTIVE: Preservation of the frontal sinus (FS) during the frontolateral approach to the skull base reduces morbidity, enhances patient comfort, and speeds up the surgical procedure. Due to its irregular outline, mental reconstruction of the borders of FS from two-dimensional images is challenging during surgery. This study was designed to evaluate the impact of neuronavigation on identification and preservation of the FS during frontolateral craniotomies. METHODS: Forty-five patients with pathologies located in the anterior skull base and in the parasellar region were included. A standard

Craniotomy for central nervous system metastases in epithelial ovarian carcinoma.

BACKGROUND: Although central nervous system (CNS) metastases from epithelial ovarian carcinoma are rare, recent studies indicate that the incidence may be increasing. Numerous series have reported various modalities for treatment with median survivals of 3 to 5 months, but the role of craniotomy has not been specifically addressed. METHODS: We conducted a retrospective review of all patients who underwent craniotomy between 1989 and 2001 for pathologically confirmed recurrent epithelial ovarian cancer metastatic to the CNS. RESULTS: We identified 14 patients who had a mean age at diagnosis o

Postoperative headache after the lateral suboccipital approach: craniotomy versus craniectomy.

The lateral suboccipital approach to the cerebellopontine angle is typically performed as a small craniectomy. Incisional pain and headache following cerebellopontine angle surgery have been reported. Adherence of the cervical muscles to the dura, which is richly innervated, with consequent traction has been suggested to be responsible for postoperative headache. Therefore, postoperative headache probably could be reduced by replacing the bone flap between the muscles and the dura. In a prospective non-randomized study this hypothesis was tested by comparing craniectomy and craniotomy. 40 pati

Transciliary subfrontal craniotomy for anterior skull base lesions. Technical note.

Microsurgical approaches are the procedures of choice for high-risk patients with lesions requiring surgical treatment. The use of a microscope reduces the extent of the surgical invasion, thus minimizing the handling of healthy tissues. The authors present a surgical approach described for the first time in 1981, which has been used for the past 17 years in more than 260 patients with different tumors and vascular lesions of the anterior cranial fossa. The modification set forth in this article makes better exposure possible, allows more space for instrument handling, and improves cosmetic re

Mannitol increases cerebral arteriovenous oxygen difference in patients undergoing craniotomy.

BACKGROUND: Monitoring the jugular venous oxygen saturation (SjVO2) and cerebral arteriovenous oxygen difference (AVDO2) is valuable in detecting cerebral ischemia. In the present study, we investigated the effect of mannitol on SjVO2 and AVDO2 in patients undergoing craniotomy. METHODS: We studied 25 ASA class II-III patients undergoing craniotomy. Anesthesia was induced with thiopental, fentanyl and vecuronium. Immediately after induction, a 16-gauge central venous catheter was placed in the internal jugular vein, ipsilateral to the lesion side with cephalic insertion and anchorage in the j

Complications related to pre-craniotomy shunts in posterior fossa tumours.

Among 344 patients with posterior fossa tumours treated from 1979 to 1986 a ventriculoperitoneal, or, less commonly, a ventriculatrial shunt was inserted in 164 cases pre-operatively. Haematoma following the insertion of a shunt was found to be an important cause of deterioration. While the occurrence of cerebrospinal fluid leak (p less than 0.3) and pseudomeningocele (p less than 0.01) was more frequent in the non-shunted cases, seizures (p less than 0.01) were found to occur more frequently in the shunted cases.

A simple and aesthetically pleasing floating craniotomy: How I do it.

BACKGROUND: Decompressive craniectomy and subsequent cranioplasty are associated with significant morbidity (Kurland et al. Neurocrit Care 23:292-304, 2015). Bone flap-preserving techniques, including floating and hinged craniotomies, offer effective intracranial pressure (ICP) control (Mohan et al. Acta Neurochir (Wien) 163(5):1415-1422, 2021), although the optimal technique remains controversial, particularly in the context of traumatic brain injury where recent landmark trials have evaluated decompressive craniectomy outcomes (Patel et al. Trauma Surg Acute Care Open 10: e001784, 2025). ME

Frailty is Not Associated With Awake Craniotomy Outcome: A Single Institution Experience.

BACKGROUND AND OBJECTIVES: Patient frailty has been shown to be a powerful predictor of poor surgical outcome across specialties and may guide patient selection. In awake craniotomy, patient selection is particularly important for completion of intraoperative mapping and to reduce conversion to general anesthesia. We evaluated whether frailty is associated with unsuccessful awake craniotomy or poor outcome. METHODS: We performed a single-center retrospective study of adult patients with tumor, epilepsy, and vascular pathologies that underwent first-time awake craniotomy between 2018 and 2024.

From Craniotomy to Caesarean: A Multidisciplinary Approach to Meningioma During Pregnancy.

This case report addresses a brain tumour - specifically, a meningioma - occurring during pregnancy. It describes the initial diagnosis, management, and outcome of a woman in her early 40s. She is para 2, and at 24 weeks + 3 days' gestation, she presented to the Emergency Department with confusion and neurological deficits. A computed tomography (CT) scan of the brain revealed a 6.4 cm meningioma in the left frontal lobe. Following a multidisciplinary discussion, the obstetric and neurosurgical teams opted for surgical intervention: a bi-frontal craniotomy with Simpson Grade I resection, perfo

Post-craniotomy leptomeningeal cyst: a new presentation of an old problem.

Leptomeningeal cyst (LMC) is a known complication of pediatric head injury but has not been described following a craniotomy other than for craniosynostosis. We present the case of a 20-month-old boy who underwent craniotomy for a traumatic epidural hematoma. There was an inadvertent tear of the dura which was repaired with a pericranial patch and dural sealant. The patient presented with a progressive surgical site swelling 5 months post-surgery and a CT scan revealed an LMC with elevation of the bone flap. He underwent re-exploration with watertight repair of the dural defect and rigid fixat

Feasibility of awake craniotomy for brain arteriovenous malformations: A scoping review.

BACKGROUND: Brain Arteriovenous Malformations (AVMs) located in proximity to eloquent brain regions are associated with poor surgical outcomes, which may be due to higher rates of postoperative neurological deterioration. Current treatment protocols include stereotactic radiosurgery, transarterial embolization, and surgical resection under general anesthesia. Awake Craniotomy (AC) allows intraoperative mapping of eloquent areas to improve post-operative neurologic outcomes. OBJECTIVES: We reviewed the current literature reporting surgical outcomes and assessed the feasibility of AC for AVM re

Anaesthetic management in a cat undergoing emergency craniotomy for meningioma excision.

CASE SUMMARY: A 15-year-old female spayed domestic shorthair cat underwent an emergency craniotomy to remove an intracranial meningioma causing marked midline shift, caudal transtentorial and foramen magnum herniation. Because intracranial structures are enclosed in the cranium, any volume-occupying lesions might raise intracranial pressure (ICP), compromising cerebral perfusion. RELEVANCE AND NOVEL INFORMATION: This case report discusses the anaesthetic management of a cat that presented with marked bradycardia and concomitant hypotension. Cushing's reflex (CR) is a well-recognised cardiovas

The Myth of Prometheus in metastatic meningioma to the liver: from craniotomy to hepatectomy.

Metastases from intracranial meningiomas are rare, and among them, meningiomas with hepatic dissemination are extremely rare. Therefore, there are currently no guidelines for staging and treatment of metastatic disease in meningioma, a disease that is a challenge for both the clinician and the pathologist. Our literature review revealed 24 cases of liver metastases originating from intracranial meningiomas. We used them to analyze the pathological patterns of dissemination and to assess the different management strategies available, the most efficient and beneficial being surgery and chemother

Anesthetic Management of Asleep and Awake Craniotomy for Supratentorial Tumor Resection.

Understanding how anesthetics impact cerebral physiology, cerebral blood flow, brain metabolism, brain relaxation, and neurologic recovery is crucial for optimizing anesthesia during supratentorial craniotomies. Intraoperative goals for supratentorial tumor resection include maintaining cerebral perfusion pressure and cerebral autoregulation, optimizing surgical access and neuromonitoring, and facilitating rapid, cooperative emergence. Evidence-based studies increasingly expand the impact of anesthetic care beyond immediate perioperative care into both preoperative optimization and minimizing

Emergency craniotomy in patient with intracranial metastatic choriocarcinoma: a case report.

Choriocarcinoma is a highly malignant gynaecological tumour. This disease becomes life-threatening once brain haemorrhage or brain herniation occurs. Timely and accurate brain surgery can gain treatment time for patients that have a large number of cerebral haemorrhages and/or brain herniation. This current report describes a case of choriocarcinoma secondary to a hydatidiform mole in a 55-year-old woman that presented with neurological symptoms. Following admission to hospital, computed tomography examination found that lung and brain metastases were accompanied by cerebral haemorrhage. Cereb

Awake Craniotomy in a Child: Assessment of Eligibility with a Simulated Theatre Experience.

BACKGROUND: Awake craniotomy is a useful surgical approach to identify and preserve eloquent areas during tumour resection, during surgery for arteriovenous malformation resections and for resective epilepsy surgery. With decreasing age, a child's ability to cooperate and mange an awake craniotomy becomes increasingly relevant. Preoperative screening is essential to identify the child who can undergo the procedure safely. Case Description. A 11-year-old female patient presented with a tumour in her right motor cortex, presumed to be a dysembryoplastic neuroepithelial tumour (DNET). We had conc
