[Preliminary study on the effects of smoking on gingival microcirculation in chronic periodontitis].

OBJECTIVE: To explore the mechanism of smoking that promotes chronic periodontitis from the perspective of gingival microcirculation. METHODS: In experiment one, upper anterior teeth (n=102) from smokers with chronic periodontitis (Group A), nonsmokers with chronic periodontitis (Group B), and nonsmokers with healthy periodontal conditions (Group C) were selected to undergo gingival blood flow (GBF) through laser doppler flowmetry. In experiment two, the tissues obtained from gums during periodontal flap surgery were divided into smoking (Group A') and nonsmoking (Group B') groups, and the gi

Comparison of the number of gingival blood vessels between type 2 diabetes mellitus and chronic periodontitis patients: An immunohistological study.

BACKGROUND: The relationship between diabetes and periodontitis has been studied for more than 50 years and is generally agreed that the periodontal disease is more prevalent in diabetic patients compared to nondiabetics. Vascular changes like increased thickness of basement membrane in small vessels has been reported in diabetic patients, but the quantity of blood vessels in gingiva of diabetic patients has not been discussed much. The aim of this study was to compare the number of blood vessels in gingiva between chronic periodontitis (CP) patients, CP with diabetes (type 2), and normal heal

Local concentration of systemic amoxicillin and metronidazole in healthy and inflamed gingiva: a comparative in vivo study.

BACKGROUND: One of the most recommended methods of systemic antibiotic administration in periodontics is the combination of amoxicillin and metronidazole, which has great inhibitory effect on periodontal pathogens. The aim of this study is to determine the local concentrations of these drugs in gingiva and compare its distribution in healthy and inflamed tissues. METHODS: The study population was selected from patients referred to our department. Fifteen subjects were referred for crown lengthening, and another 15 subjects required flap surgery because of severe periodontitis. All 30 patients

Aesthetic Treatment Related to Clinical Need--An Illustrated Case Report.

Aesthetic treatment may be patient driven and usually by a 'want', rather than a 'need'. This paper describes the management of a patient who presented with aesthetic wants and clinical needs, both of which were caused as a result of unsuccessful aesthetic treatment which the patient had received previously. The diagnostic process, discussion of the treatment plan and clinical procedures, which produced a satisfactory result, are described and illustrated. Clinical Relevance: This case demonstrates that aesthetic treatment may commit the patient to future dental treatment needs, particularly i

Treatment options of untreatable traumatized anterior maxillary teeth for future use of dental implantation.

The anterior maxilla is the most traumatized region during childhood. Posttraumatic complications occasionally lead to tooth loss as well as the need for future implants. Unfortunately, it is contraindicated to place dental implants during childhood. A waiting period of approximately 8 to 10 years before completion of growth is required. For this patient to become a candidate for future dental implants, it is necessary to ensure the continuous growth and to preserve the dimensions of the alveolar process until growth has ceased from time of injury until joint maturation. To achieve these goals

Semidirect Restorations in Multidisciplinary Treatment: Viable Option for Children and Teenagers.

Due to the esthetic necessity required nowadays, the multidisciplinary treatment became a fundamental step in the restoration success. When the patient exhibits dental agenesis of one or more elements, he can show difficulty in social interactions. The age of the patient is a limiting factor to esthetic procedures, however, it should be evaluated as a real indicative with each case. The utilization of semi-direct restorations is a viable option due the cost, esthetic and improvement of physical and mechanical properties. The purpose of this paper is to present a case detailing the confection a

Anterior space management: interdisciplinary concepts.

UNLABELLED: This case report describes the treatment of one patient with maxillary anterior spacing, caused by bilateral lateral peg-shaped incisors, using a planned sequence of multidisciplinary approaches for esthetic treatment. An asymmetrical gingival line was visible when the patient smiled. To evaluate the desired gingival level and the proportion of restoration to be made using the recurring esthetic dental (RED) proportion method, a diagnostic wax-up model was fabricated. Esthetic crown lengthening corrected the gingival line. RED proportion analysis suggested minor tooth movement prio

The concept of the interdental gingival midpoint line in cervical line management.

The first purpose of this study was to investigate the relationship between the lowest cervical point and the interdental gingival midpoint (IGM) line in the maxillary anterior region. After marking the lowest cervical points and the mesial and distal interdental gingival midpoints on study models of 77 patients, the shortest distances from the IGM lines that connect both interdental gingival midpoints to the lowest cervical points were measured on silicone impressions. The findings showed that the average position of the lowest cervical points in the central incisors coincides with the IGM li

Orthodontic Extrusion in Daily Clinical Practice: Management of Fractured or Damaged Anterior Teeth.

Background. Orthodontic extrusion (OE), or forced eruption, is a conservative technique used to recover teeth affected by coronal fractures, traumatic intrusions, or severe caries. It involves applying light, continuous forces to induce vertical tooth movement, promoting tissue remodeling through periodontal ligament stimulation. Materials and Methods. This narrative review included studies investigating OE as a therapeutic approach for the management of deep or subgingival carious lesions, traumatic dental injuries (such as intrusion or fracture), or for alveolar ridge augmentation in implant

Two-Year Outcomes of Regenerative Therapy in an Immature Maxillary Molar With Periapical Abscess: A Case Report.

Trauma or caries in teeth with incomplete root formation can lead to pulp necrosis. Endodontic treatment in such cases requires specific clinical protocols that differ from conventional approaches due to the anatomical and developmental characteristics of immature teeth. This case report describes the clinical management of a maxillary second molar (tooth #17) with incomplete root development and a periapical abscess, treated through pulp revascularization. A 13-year-old male patient presented with spontaneous, throbbing pain in the upper right facial region, accompanied by infraorbital swelli

Restoration of the Anterior Segment in a Cleft Palate in Conjunction with Surgically Facilitated Orthodontic Therapy: An Interdisciplinary Approach.

A 26-year-old woman was referred to a periodontal surgical practice for concerns related to gingival recession. After several consultations among the orthodontist, periodontist, and cosmetic/restorative dentist, she decided to have surgically facilitated orthodontic therapy as part of a collaborative, interdisciplinary treatment planning process to correct her constricted maxillary arch form, augment thin dentoalveolar facial bone, simultaneously with gaining root coverage as well as improving attached gingiva width and mucogingival thickness. As a consequence of changing the arch form, an imp
