PURPOSE: To present an improvised method for repositioning a proximally displaced stent using only a cystoscope and a guidewire.
METHODS: A Glidewire guidewire (Boston Scientific) was passed through a cystoscope and into the distal ureter, and manipulated up the ureteral stent into the renal pelvis, reflected, and passed antegrade down the ureter and into the bladder. The guidewire was then grasped in the bladder, clamped at the penis, and retracted, pulling the stent back into the bladder.

A 29-year-old woman suffered a vesicouterine fistula following cesarean section. The fistula was treated successfully after cystoscopic identification and fulguration of the tract. Although various surgical approaches to this problem have been described, and a number of cases of spontaneous resolution are reported, to date there have been no reports of treatment of this problem via cystoscopic fulguration. We advocate this simple technique as a primary approach to vesicouterine fistulas.

The authors describe a modification of the classical double J stent designed to improve its clinical acceptability. It consists of cutting the lower J so that the catheter, inserted in the ureter, does not protrude into the bladder. A loop of nonresorbable suture attached to the lower extremity of the stent is passed through the ureteric orifice, allowing its easy extraction. This procedure has been used in eight patients with no complications.

OBJECTIVES: The extent of the required preoperative workup of patients who are candidates for radical prostatectomy is still controversial. Traditionally, cystoscopy has been reserved only for those patients who present with hematuria. Ureteral stent exchange is usually performed under both fluoroscopic and cystoscopic guidance. We experienced two cases with retrograde placement of metallic ureteral stent via urethra under fluoroscopic guidance. The original concept of laparoscopy developed as a branch of cystoscopy. Since the use of the Nitze cystoscope at its inception, laparoscopy has kept pace with the innovations in our cystoscopic inventory. The recently emerging interest of urologists in utility of the laparoscope for a variety of urologic surgery is a welcome renascence of this procedure, which has been used mainly by our gynecologic colleagues, in part because of our own lack of pursuit in its development. Laparoscopy should now become an integral part of our urologic teaching and practice.
