[Urofacial Ochoa's syndrome: a clinical case].

An 11 year-old girl had an urinary diversion because of a complex uropathy with vesical sphincter dyssinergism. Her inexpressive face with lost eyes and forced gestures, especially when smiling--her smile looking rather like weeping--was the clue to the diagnosis of Ochoa's syndrome. She underwent psoic bladder fixation, right ureteral reimplantation, bladder neck loosening and cystostomy undiversion. At follow-up, renal function deteriorated. She has recently received a kidney transplantation in another institution, and is now symptom-free.

Continent bladder stoma.

The formation of an aesthetically desirable urinary diversion through a continent bladder stoma requires a long-term commitment by both patient and urologist to monitoring patient progress and addressing problems, both urological and otherwise, which arise over time. In this manuscript, issues relating to physical aspects of surgical management are discussed. These include the nature of and site of the stoma and its catheterizing track, the continence mechanism, provision of a low-pressure storage system of adequate capacity and management of the bladder neck/urethra when incompetent. It is im

Continent vesicostomy with an ileal conduit. A case report.

A patient is described who had construction of a vesicocutaneous ileal conduit with a continent intussusception valve allowing for intermittent self-catheterisation. The procedure was performed on a 13-year-old male patient with a normal bladder but with a totally unreconstructable urethra as a result of previous traumatic urethral disruption and unsuccessful urethral alignment procedures.

Suprapubic catheter placement through ileal mesentery causes mechanical small bowel obstruction six years later in a female patient with turners syndrome.

Percutaneous suprapubic cystostomy (SPC) is a procedure performed to manage urinary retention when urethral catheterization is contraindicated or to improve the quality of life in cases such as neurogenic bladder. Although a simple procedure, it is associated with serious complications, increasing the morbidity and mortality. This case study demonstrates a delayed presentation of small bowel obstruction caused by a suprapubic catheter traversing through the ileal mesentery in a patient with no prior bowel surgeries. Few cases report this complication and this is possibly the first case to be r

[Pediatric urinary tract catheters.].

Urinary tract catheters are hollow and flexible tubes used to facilitate urine output or to achieve urine transit through them. We can find different types of catheters depending on the purpose they have been designed for: urethral, cystostomy and ureteral catheters among others. We review the indications for insertion of these catheters, their designs, composition, covering materials, insertion techniques and associated complications. Pediatric catheters are behind what is produced for adults, so we perform a bibliographic review about urinary catheters in general with emphasis on the pediatr

Clinics in diagnostic imaging (54).

A 29-year-old man sustained a pelvic fracture and haemorrhagic shock after a road traffic accident. He underwent an exploratory laparotomy open cystostomy, and iliac artery embolisation. Subsequent antegrade cystography and computed tomography showed a complete traumatic posterior urethral rupture with a "high riding" bladder. Delayed repair of the posterior urethra was performed 6 months later with good functional outcome. The risk of urethral injury in pelvic fractures, the mechanism of injury, and the role of imaging in the diagnosis of possible urethral injury in pelvic fractures are discu

[Congenital urethral diverticulum in a male infant: a case report].

A case of congenital anterior urethral diverticulum of a male infant is reported. The patient was a 3-year-old boy who had complained of a sac-like swelling and urinary fistula of the anterior penis. The diagnosis was made by means of retrograde urethrocystography. Diverticulectomy and cystostomy were done. The post-operative course was uneventful. The microscopic examination showed that the diverticulum was lined with normal urethral epithelium. We made a statistical survey on 47 male children up to 15 years of age with congenital anterior urethral diverticulum reported in the Japanese litera

Prostatomembranous urethral injuries: a review of the literature and a rational approach to their management.

Controversy surrounds the management of prostatomembranous urethral injuries. We herein present 38 patients and review the findings of 538 in 19 reported series. Results indicate a high risk of stricture, impotence and incontinence if conventional early urethral realignment techniques are used. Therefore, it is suggested that this approach be reserved for cases demanding immediate intervention (high riding bladder, associated rectal tear, concomitant bladder neck injury or continued bleeding), and that all others be managed by initial suprapubic cystostomy alone and delayed urethroplasty. [Indirect electrochemical blood oxidation in the prophylaxis of suppurative-inflammatory complications after transurethral resection of the prostate].

To investigate efficiency of indirect electrochemical blood oxidation in prophylaxis of infectious complications, 28 patients were treated with 0.06% hypochlorite solution (2 infusions, 2-3 days before TURP). These patients belonged to a high-risk group of infectious complications (80% of them had chronic prostatitis, 8 patients had bladder stones, 8 had undergone cystostomy). The results demonstrated that the complication rate in the above prepared patients fell four times as compared to the control group and the postoperati hospital stay was reduced by 3 days.

Nonoperative management of iatrogenic prostatorectal fistula.

Iatrogenic prostatorectal fistula is a rarely seen urologic complication of transurethral resection of the prostate and stricture dilatation with sounds. In the past, such cases have been managed with colostomy diversion of the fecal stream and suprapubic cystostomy diversion of the urinary stream and in most cases operative closure of the fistula. This case illustrates that a more conservative nonoperative approach can lead to successful closure of such a fistula in selected cases. The management of the case described here utilizes low residue dietary supplements, Foley catheter drainage, and

[Continent urostomy using the intestine].

The hydraulic ileal valve, which we developed in 1975, ensures continence by adapting to 5 different urinary reservoirs. An isolated ileal segment (14 cm) is folded inward on itself throughout its entire length. We performed 260 continent urostomies. An ileocecal reservoir was used in 179, detubularised ileocecal reservoir in 32, ileum 34, sigmoïd 5, rectum 1 and bladder (continent cystostomy) in 8. Of the patients 82% were continent immediately. Continence was obtained after repair of the valve in 12.5% therefore, 94.5% patients over all were continent.

[Condylomatous carcinoma of the urethra that detected human papillomavirus type 16 genome: a case report].

This is a report on a rare case of male urethral condylomatous carcinoma that detected human papillomavirus type 16 genome (HPV16). He had been administered immunosuppressant, since he had a kidney transplant in 1977. Papillary tumors spread from the external urethral opening to the membranous urethra. Histologically, the tumors presented papillary growth and koilocytotic atypia on the surface part. Moreover, we observed corpus spongiosum penis invasion and vein invasion, and detected HPV16 by immunostaining. We diagnosed condylomatous carcinoma. We treated with anterior urethrectomy and cysto
