Surface-oriented diving with nitrogen-oxygen breathing gases (nitrox) other than air will commonly be planned according to the Equivalent Air Depth (EAD) principle. EAD is the depth at which a diver breathing air will inhale the same pN₂ as the nitrox-breathing diver. Using conventional air decompression tables based on the EAD depth will allow longer bottom times or shorter decompression times compared to air breathing at the same water depth. This scoping review aimed to investigate the validity of the EAD principle. A literature review identified 13 eligible studies that reported DCS incide

INTRODUCTION: To develop the diving capacity in the Swedish armed forces the current air decompression tables are under revision. A new decompression table named SWEN21 has been created to have a projected risk level of 1% for decompression sickness (DCS) at the no stop limits. The aim of this study was to evaluate the safety of SWEN21 through the measurement of venous gas emboli (VGE) in a dive series.
METHODS: A total 154 dives were conducted by 47 divers in a hyperbaric wet chamber. As a proxy for DCS risk serial VGE measurements by echocardiography were conducted and graded according to t

Scuba diving is a popular watersport in the United States, with over 9 million certified divers. This activity uses specialized equipment that enables swimmers to explore deeper into the ocean for a more extended period than free diving. This popularity has led to the development of diving medicine, a multidisciplinary field that includes dermatologic management. An extensive literature review was conducted on PubMed and Google Scholar, using key search terms related to diving and dermatology to compile relevant peer-reviewed articles. This review examines the dermatological impacts of recreat

The human body is well adapted to dealing with small variations in atmospheric pressure. However when our pursuit of sport and recreation takes us to extreme altitudes or ocean depths, the change in surrounding pressure has the potential to cause significant morbidity. Sports with more extreme changes in atmospheric pressure such as skydiving and scuba diving commonly place the athlete at risk for barotrauma injuries, especially in the middle ear and sinuses. Middle ear barotrauma occurs when a pressure differential develops between the middle ear and the pressure outside of the tympanic membr

INTRODUCTION: Prophylactic recompression treatment (PRT) is an empirical treatment conducted in China for divers who conduct repetitive diving or asymptomatic insufficient decompression to eliminate residual nitrogen and microbubbles and prevent decompression sickness (DCS).
CASE REPORT: We report a case of a 43-year-old healthy diving fisherman who experienced cerebral dysfunction after PRT. The fisherman suddenly experienced dizziness accompanied by vomiting, vertigo, nausea, confusion, and aphasia six hours after a 500kPa-PRT. Laboratory tests revealed a significantly elevated serum creati

Novel therapies for the next decade include hyperbaric oxygen, nitric oxide, and extracorporeal membrane oxygenation. Hyperbaric oxygen delivers oxygen at a pressure greater than one atmosphere and has been used in diseases ranging from decompression sickness to carbon monoxide poisoning. Inhaled nitric oxide, a potent vasodilator, has been used in the acute respiratory distress syndrome and for the diagnosis and treatment of pulmonary hypertension. Extracorporeal membrane oxygenation (ECMO) has been used to provide cardiopulmonary bypass support, particularly in the pediatric and neonatal pop

Decompression sickness has not previously been reported at altitudes below 16,000 ft (4,878m), unless the patient had been previously exposed to hyperbaric conditions (SCUBA). There are some underdocumented reports at lower altitudes. We are reporting a suspected case of bends appearing at 14,000 ft (4,268m) in a young student Naval aviator. The patient's only predisposing factors were an old athletic injury repaired with a permanently retained screw (foreign body) and a regular exercise program. A short discussion of bends is included.

During the period April 1965 to April 1968 observations were made on 285 patients and 357 staff exposed to air and oxygen at pressures of 1 to 3 atmospheres absolute in a hyperbaric unit. Decompression sickness was not noted and an annual radiological skeletal survey failed to show aseptic bone necrosis; the incidence of otitic barotrauma was 4.7%. Oxygen toxicity was not observed in any patient. It is suggested that pressures in this range are relatively safe in a specialized medical hyperbaric environment.

Dysbaric osteonecrosis is a serious complication for those exposed to a hyperbaric environment, with prevalence of 17% amongst compressed air workers and 4.2% amongst divers. Bone lesions are characteristically multiple and bilateral, occurring frequently in the shafts of the femora or tibiae and the heads of the humeri or femora. A proportion of the lesions will lie next to the joint surface, the so called juxta-articular lesion, and these may progress to a structural failure and secondary osteoarthritis. These lesions can be severely disabling, especially in a young adult male. When related

Scuba diving fatalities post-mortem diagnosis presents a higher level of forensic complexity because of their occurrence in a non-natural human life environment. Scuba divers are equipped with diving gas to breathe underwater. It is essential for them to be fully trained in order to be able to manage their dive safely despite the varying increase of ambient pressure and temperature decrease. Throughout the dive, the inhaled diving gas is dissolved in the diver's tissues during the descent and if the decompression steps are not respected during the ascent, the balance between the dissolved gas

Beaked whales are a poorly known but diverse group of whales that have received considerable attention due to strandings that have been temporally and spatially associated with naval sonar deployment. Postmortem studies on stranded carcasses have revealed lesions consistent with decompression sickness, including intravascular gas and fat emboli. These findings have been supported by analyses of intravascular gas emboli showing composition dominated by nitrogen gas. To increase our understanding of the pathophysiology of nitrogen bubble formation and intravascular embolization, we examined the

The first of these reviews described the physiological factors defining the limits of static apnea, while the second examined performance in apneic distance swimming. This paper reviews the factors determining performance in depth disciplines, where hydrostatic pressure is added to the stressors associated with apnea duration and physical work. Apneic duration is essential for performance in all disciplines, and is prolonged by any means that increases gas storage or tolerance to asphyxia or reduces metabolic rate. For underwater distance swimming, the main challenge is to restrict metabolism

Decompression illness (DCI) is a well-known medical emergency among scuba divers, aviators, astronauts, and compressed-air workers. It includes two diseases: decompression sickness (DCS) and arterial gas embolism (AGE). Vision changes are one of the symptoms that can happen in patients with DCI, but acute complete loss of vision is extremely rare. We report a 28-year-old male patient, a novice scuba diver who presented to our hospital with complete loss of vision while he was ascending from 18 m depth. The diagnosis of AGE was made depending on the history, clinical examination, and radiologic

Understanding cell-bubble interactions is crucial for preventing bubble related pathologies and harnessing their potential therapeutic benefits. Bubbles can occur in the body as a result of therapeutic intravenous administration, surgery, infections or decompression. Subsequent interactions with living cells, may result in pathological responses such as decompression sickness (DCS). This work investigates the interactions that occur between bubbles formed during decompression and cells in a 3D engineered tissue phantom. Increasing the tissue phantoms' cellular density resulted in decreased dis

There is a dearth of studies in undersea and hyperbaric medicine (UHM), likely due to the limited number of clinicians currently specializing in UHM. Due to the high-consequence nature of diving and the effects of pressure on the human body, medical clearance is of the utmost importance. Despite all efforts to mitigate the possibilities of dive maladies, divers occasionally succumb to the effects of prolonged submersion. This article provides an in-depth look at three individuals who suffered from separate dive-related medical events. In each example, UHM was applied and successfully mitigated

INTRODUCTION: Extreme deep technical diving presents significant physiological challenges. While procedures often blend elements from both recreational and commercial diving, many remain empirical and unvalidated for this purpose. The rise of closed-circuit rebreathers has reduced gas cost and logistical barriers, enabling more divers to reach unprecedented depths. This study, based on the experience of deep divers, explores the limits of extreme-depth diving and the strategies developed to overcome them.
METHODS: Eight rebreather divers (one female, seven males) with experience beyond 200 me

A documented case of beginning aseptic necrosis of the femoral head associated with pregnancy together with a review of the literature about this rare complication of pregnancy is presented. The known risk factors of osteonecrosis are; steroid use, alcoholism, organ transplantation, especially after kidney transplant or bone marrow transplantation bone, systemic lupus erythematosus, dyslipidemia especially hypertriglyceridemia, dysbaric decompression sickness, drepanocytosis and Gaucher's disease. Among the less established factors, we mention procoagulations abnormalities, HIV infection, chem

INTRODUCTION: There are increasing anecdotal reports of in-water recompression in freedivers who surface with neurological symptoms, likely suffering from decompression illness (DCI). Given the remote locations where many cases occurred, divers often struggled to access medical care, including the gold-standard hyperbaric oxygen treatment (HBOT), thus resorting to in-water recompression (IWR). Currently, IWR guidelines have only been discussed for scuba and surface supplied divers in specific scenarios, with protocols prescribing oxygen breathing at depths ≤ 9 metres maximum for around 1-3 hou
