Comparison of brain volume increase in patients with intracranial hypertension after decompressive craniectomy and expansion craniotomy.

Intracranial hypertension is a critical condition often resulting from traumatic brain injury or malignant middle cerebral artery ischemia, necessitating surgical intervention when conservative treatments fail. This study compares the efficacy of decompressive craniectomy (DC) and expansion craniotomy (EC) in managing cerebral edema. Sixty-three patients, 52 undergoing DC and 11 undergoing EC, were evaluated for changes in brain volume, midline shift, and other key parameters using pre- and post-operative CT scans. Results indicated that brain volume increased significantly more in the EC grou

Expansive Craniotomy versus Standard Decompressive Craniectomy in Refractory Intracranial Hypertension: A Systematic Review and Meta-Analysis.

BACKGROUND: Raised intracranial pressure can have devastating consequences on mortality and outcome after acute brain injury. Decompressive craniectomy (DC) is an established surgical procedure for controlling refractory intracranial hypertension; however, this requires subsequent cranioplasty. Expansive craniotomy (EC) techniques, where the bone flap is returned but only partially fixed in place, have been developed to avoid the need for cranioplasty. However, comparative safety and efficacy is not well defined. METHODS: A systematic review to identify studies comparing EC with DC was perfor

Long-term prognostic value of Rotterdam computed tomography score in traumatic brain injury after decompressive craniectomy.

OBJECTIVE: Decompressive craniectomy (DC) is a critical intervention for traumatic brain injury (TBI) patients with intracranial hypertension. However, outcomes following the procedure can display considerable variability. While the Rotterdam computed tomography (CT) score was conceived to predict TBI prognosis, its utility in gauging long-term outcomes after DC remains unexplored. This study aimed to elucidate the long-term prognostic value of the Rotterdam CT score in TBI patients undergoing DC. METHODS: This retrospective study enrolled 176 patients who underwent DC for TBI, with a follow-

Post-traumatic hydrocephalus after decompressive craniectomy: a multidimensional analysis of clinical, radiological, and surgical risk factors.

Decompressive craniectomy is a key treatment for refractory intracranial pressure after severe traumatic brain injury (TBI). Post-traumatic hydrocephalus (PTH) occurs in 7.6-36% of cases, and early diagnosis significantly improves rehabilitation outcomes. This retrospective study analyzed risk factors for shunt-dependent PTH in 126 TBI patients (93 men, 33 women, median age 53 years). Patients were divided into those requiring shunts and those who did not. Clinical and radiological characteristics, including volumetric measurements and surgical techniques, were assessed using SPSS® Statistics

Characteristics of Post-traumatic Shunt-dependent Hydrocephalus After Decompressive Craniectomy: Are Computed Tomography Scoring Systems Predictors?

OBJECTIVE: Traumatic brain injury (TBI) significantly contributes to morbidity rates. While computed tomography (CT) scoring systems have been recognized as predictive factors for TBI outcomes, their association with shunt dependency in patients undergoing decompressive craniectomy (DC) has not been investigated. This study aimed to evaluate the predictive utility of CT scoring systems concerning shunt-dependent hydrocephalus in patients post-DC for TBI. METHODS: In this retrospective study, we enrolled 162 patients who underwent DC and survived more than 7 days following TBI. The pre-DC CT s

Outcomes of decompressive craniectomy for malignant middle cerebral artery stroke in an academic hospital in Brazil.

BACKGROUND: Ischemic stroke is an important cause of death in the world. The malignant middle cerebral artery infarction (MMCAI) has mortality as high as 80% when clinically treated. In this setting, decompressive craniectomy is a life-saving measure, in spite of high morbidity among survivors. OBJECTIVE: To evaluate the outcomes of patients with MMCAI treated with decompressive craniectomy in a Brazilian academic tertiary stroke center. METHODS: A prospective stroke database was retrospectively evaluated, and all patients treated with decompressive craniectomy for MMCAI between January 2014

Decompressive craniectomy as a second/third-tier intervention in traumatic brain injury: A multicenter observational study.

OBJECTIVES: RESCUEicp studied decompressive craniectomy (DC) applied as third-tier option in severe traumatic brain injury (TBI) patients in a randomized controlled setting and demonstrated a decrease in mortality with similar rates of favorable outcome in the DC group compared to the medical management group. In many centers, DC is being used in combination with other second/third-tier therapies. The aim of the present study is to investigate outcomes from DC in a prospective non-RCT context. METHODS: This is a prospective observational study of 2 patient cohorts: one from the University Hos

A systematic review and meta-analysis of factors involved in bone flap resorption after decompressive craniectomy.

Decompressive craniectomy (DC) is effective in controlling increasing intracranial pressure determined by a wide range of conditions, mainly traumatic brain injury (TBI) and stroke, and the subsequent cranioplasty (CP) displays potential therapeutic benefit in terms of overall neurological function. While autologous bone flap (ABF) harvested at the time of DC is the ideal material for skull defect reconstruction, it carries several risks. Aseptic bone flap resorption (BFR) is one of the most common complications, often leading to surgical failure. The aim of our study was to systematically rev

Surgery for acute subdural hematoma: the value of pre-emptive decompressive craniectomy by propensity score analysis.

BACKGROUND: Acute subdural hematomas (ASDH) are found frequently following traumatic brain injury (TBI) and they are considered the most lethal type of mass lesions. The decision to perform a procedure to evacuate ASDH and the approach, either via craniotomy or decompressive craniectomy (DC), remains controversial. METHODS: We reviewed a prospectively collected series of 343 moderate to severe TBI patients in whom ASDH was the main lesion (ASDH volumes ≥10 cc). Patients with early comfort measures (early mortality prediction >50% and not ICP monitored), bilateral ASDH or the presence of anoth

Factors associated with the development and outcome of hydrocephalus after decompressive craniectomy for traumatic brain injury.

Posttraumatic hydrocephalus (PTH) is common in patients undergoing decompressive craniectomy (DC) for traumatic brain injury (TBI), but the incidence, mechanisms, and risk factors have not been fully elucidated. This study aimed to determine the incidence of and the factors associated with PTH. We retrospectively reviewed patients who underwent DC for TBI at our institute between January 2014 and December 2018. We identified and compared the demographic, clinical, and radiological data, and 12-month functional outcome (as assessed by the Glasgow Outcome Scale [GOS]) between patients who develo

Decompressive bifrontal craniectomy for malignant intracranial pressure following anterior communicating artery aneurysm rupture: two case reports.

There are a number of causes of raised intracranial pressure (ICP) following aneurysm rupture. These include primary and diffuse hypoxic brain injury, intracranial hematomas, cerebral ischemia or infarction, and obstructive hydrocephalus. More localized brain swelling can also occur: the result of vasogenic and cytotoxic edema resulting from overlying bleeding in the subarachnoid spaces. In the case of rupture of an anterior communicating artery (ACommA) aneurysm and interhemispheric subarachnoid hemorrhage (SAH), this swelling can occur in both frontal lobes and when extensive, and the result

Repeated decompressive craniectomy after head injury in children: two successful cases as result of improved neuromonitoring.

BACKGROUND Decompressive craniectomy in the treatment of posttraumatic brain swelling is not generally accepted. Until now the efficacy of operative decompressive craniectomy in posttraumatic brain swelling of children appeared more promising. However, the criteria for such procedures remain unclearly defined. METHODS We present two children who had repeated decompressive craniectomy following head injury, in order to control intracranial pressure (ICP) sufficiently. Our indications for performing a decompressive craniectomy in the presence of conservatively uncontrollable raised ICP are: (1)

The influence of large decompressive craniectomy on the outcome of surgical treatment in spontaneous intracerebral haematomas.

The outcome of a series of 73 patients with spontaneous intraparenchymatous haematomas treated by surgical evacuation of the clot and decompressive craniectomy has been described. Comparing postoperative mortality of this series with another series of patients (54 cases) treated only with surgical removal of the clot without decompressive craniectomy a statistically significant improvement in the mortality rate after craniectomy could be observed in acute cases demanding surgical intervention for preservation of life in the first 24 hours. If signs of brain-stem suffering appear surgical morta

Triphasic Evolution of Spontaneous Calvarial Neo-Ossification After Decompressive Craniectomy: A 15-Year Serial Imaging Study.

Spontaneous cranial bone regeneration after decompressive craniectomy (DC) is rare in adults and has uniformly been described as a unidirectional, progressive process. The authors report the first case of a triphasic, nonlinear evolution-formation, resorption, and reactivation-of spontaneous calvarial neo-ossification in an adult. A 24-year-old man underwent bilateral DC for severe traumatic brain injury; the native dura was preserved almost intact on the left side, whereas the right side underwent extensive durotomy with expansile duraplasty. Serial computed tomography at 8 time points over 1

Age and Time as Predictors of Outcome in Patients Undergoing Decompressive Craniectomy for Traumatic Brain Injury.

BACKGROUND: Evidence to support the benefit of decompressive craniectomy (DC) in traumatic brain injury (TBI) is diverging. Moreover, in the context of an aging population, there is a notable lack of direct evidence supporting the efficacy of DC in older age groups. In this study, we evaluate the impact of age on outcome after DC for TBI in a single-center cohort. Additionally, we explore the effect of time from injury to DC on outcome. METHODS: In this retrospective cohort study based on prospectively collected data from the Oslo TBI Registry Neurosurgery, we included TBI patients of any age

Novel association of the Rotterdam computed tomography score with decompressive craniectomy in acute traumatic subdural hematoma.

BACKGROUND AND OBJECTIVES: The decision to perform a craniotomy (CO) versus decompressive craniectomy (DC) for acute subdural hematomas (aSDH) after traumatic brain injury (TBI) remains challenging. Rotterdam Computed Tomography (RCT) scoring has been shown to correlate with outcome measures in TBI. This study examines the association between RCT scoring and the decision to pursue CO or DC in TBI patients with aSDH. METHODS: A single-center retrospective review was conducted on adult patients who presented with aSDH and received either a CO or DC between 2018 and 2021. Baseline demographics a
