[RECONSTRUCTION OF DEGLOVED THUMBS WITH FREE SECOND TOE DORSAL FLAP COMBINED WITH MIDDLE OR RING FINGER ISLAND FLAP].

OBJECTIVE: To investigate the effectiveness of free second toe dorsal flap combined with middle or ring finger island flap for repairing degloved thumbs. METHODS: Between August 2009 and June 2013, 6 patients with degloving injury of the thumb were treated using free second toe dorsal flap combined with middle or ring finger island flap. There were 4 males and 2 females, aged 19-44 years (mean, 32 years). The left thumb was involved in 2 cases and the right thumb in 4 cases, including 5 cases of type II and 1 case of type III degloving injury. The size of wound was 5.5 cm×2.5 cm to 6.5 cm×5.0

A Rare Presentation of a Morel-Lavallee Lesion of the Lower Leg Successfully Treated With Ultrasound-Guided Aspiration.

A Morel-Lavallee lesion (MLL) is a relatively rare condition that is caused by a traumatic shearing force. This force leads to a closed degloving injury of the subcutaneous tissue and fascia that creates a potential space that can fill with lymph, blood, and necrotic fat. The MLLs are traditionally seen after high impact trauma and typically located at the greater trochanter and pelvis, although recent reports have found them to be located at the knee, thigh, and lower leg. The MLLs typically present as swelling at the site of injury, which can be difficult to differentiate from several other

Salvage of degloved digits with heterodigital flaps and full thickness skin grafts.

The purpose of this paper is to report the salvage of degloved digits (class III ring avulsion injuries [Urbaniak et al classification] [J Hand Surg Am. 1981;6:25-30]) using the heterodigital neurovascular island flap (originally developed by Moberg and Littler) in addition to full thickness grafting.Six fingers with class III ring avulsion injuries (Urbaniak et al classification) were included. The volar surface is covered with a heterodigital island flap from the adjacent finger, while a full thickness graft is applied to the dorsum. All flaps survived. Mean graft take was 85%. Mean static tw

Expanded screening criteria for blunt cerebrovascular injury: a bigger impact than anticipated.

BACKGROUND: We implemented expanded screening criteria for blunt cerebrovascular injuries (BCVIs) in an attempt to capture the remaining 20% of patients not historically identified with earlier protocols. We hypothesized that these expanded criteria would capture the additional 20% of BCVI patients not previously identified. METHODS: Screening criteria for BCVI were expanded in 2011 after identifying new injury patterns. The study population included 4 years prior (2007 to 2010; classic) and following (2011 to 2014; expanded) implementation of expanded criteria. RESULTS: BCVIs were identifie

Heel coverage using a distally based sural artery fasciocutaneous cross-leg flap: report of a small series.

UNLABELLED: One of the goals in the management of severe open injuries of the foot is to obtain adequate soft tissue coverage. In extreme conditions of pedal soft tissue loss, in patients who are not satisfactory candidates for local or free-tissue transfer, the cross-leg flap remains an option for surgical reconstruction. We present the results of 7 patients with multiple lower limb open fractures associated with ipsilateral degloving injuries, and/or secondary pressure ulcers of the hindfoot with exposure of the calcaneus, in which a distally based sural artery island fasciocutaneous flap, e

Resect or Retain Bone Fragments in Children and Teenagers with Severe Open Fractures?: A Case Report.

CASE: A 14-year-old pedestrian was hit by a car and encountered similar bilateral Gustilo IIIB open tibial fractures. The right tibial fracture involved a large borderline vital butterfly fragment without periosteal contact, which was retained and proceeded to sufficient healing within 12 weeks. The left tibial fracture was treated according to the principles for the treatment of severe open fractures in adults, involving resection of devitalized fragments and bone transport, and healed within 15 months. CONCLUSIONS: Teenagers do possess larger bone healing potential than adults. Therefore, a

Vascular consideration in repair of total scalp avulsion.

Successful total scalp replantation was performed in our case. Based on the angiosome concept and anatomical study, the avulsed scalp survived with unilateral anastomosis of the superficial temporal artery and superficial temporal vein, largely due to the presence of rich arterial and venous arcades in the scalp. The patient currently has no problems with activities of daily living, although total hypoaesthesia and dysfunction of the left frontal muscle of the forehead are present. In addition, the combined findings of hair growth pattern indicated the vascular territories of the scalp skin.

Traumatic amputation. Mechanisms of injury, treatment, and rehabilitation.

Traumatic amputations are one of many injuries that can occur to the body's musculoskeletal systems. Degloving, partial and total amputations are common types of traumatic injuries. Since traumatic amputations are very life threatening, emergency treatment must be initiated quickly and directed toward profuse blood loss and potential hypovolemia. Not only do traumatic amputees undergo extreme physiological changes, but they must also encounter the psychological trauma of an amputation. Due to a loss of body part(s) and alteration in body image, the amputee often experiences the stages of griev

Morel-Lavallée-Like Periosteal Degloving Lesion of the Tibia Following Trauma: A Novel Observation.

Morel-Lavallée lesion (MLL) is a closed degloving injury resulting from the separation of the subcutaneous tissue from the underlying deep fascia due to shearing forces. It most commonly occurs around the pelvis, hip, and thigh. We describe an unusual post-traumatic lesion resembling an MLL but occurring at the periosteal interface of the tibia. A 49-year-old man presented with a localized swelling over the upper anterior aspect of the right leg that developed two months after a blunt injury sustained while kick-starting a motorcycle. Physical examination revealed a firm, mildly tender swellin

Reconstruction of a Circumferential Upper Extremity Soft Tissue Defect With a Dermal Regeneration Template and Skin Grafting.

Extensive degloving injuries of the upper extremity are rare and pose unique reconstructive challenges. Circumferential loss of soft tissue coverage over the elbow treated by skin grafting is often complicated by elbow contracture and decreased range of motion, requiring secondary contracture release and free-flap reconstruction to restore function. As an alternative approach, we report a good outcome after the use of a dermal regenerative template and subsequent split-thickness skin grafting. A 38-year-old right hand dominant man presented with circumferential degloving injury of the entire r

Quadriceps muscle injuries in athletes: a narrative review.

The quadriceps muscles are a large group of 4 muscles in the anterior compartment of the thigh, comprising the rectus femoris, vastus medialis, vastus intermedius, and vastus lateralis, which in combination act as the primary extensors of the knee joint. The rectus femoris is also responsible for hip joint flexion. Quadriceps muscle injuries are frequently encountered in sports and athletic activities and present a significant challenge in the realm of sports medicine, impacting athletes across various disciplines and levels of competition. A spectrum of sporting injuries and imaging findings

Endoscopic-Assisted Palatal Surgery Via Transoral and Intranasal Approach.

Palate carcinoma often challenges to the treatment options. It depends on the histologic type, local invasion, and nodal or distant metastasis. Hard palate tumors that invade the nasal cavity can be operated by midfacial degloving approach, lateral rhinotomy approach, and lip splitting incision with infrastructure maxillectomy. These approaches inevitably coincide with facial scars, nerve injuries, facial swelling, and long hospital stay. Transoral robotic assisted surgery can be applied; however, this needs high price and has a weakness of accurate handling about intranasal lesion. The author

Secondary reconstruction by temporoparietal free fascial flap for ring avulsion injury.

We report the secondary reconstruction of an amputated finger following a ring avulsion injury using a temporoparietal fascial flap (TPFF). There have been reports of preserving degloved fingers using a cross-finger flap and an abdominal flap, but it is difficult to obtain good results with these procedures. The TPFF is an ideal, thin flap that gives excellent results, preserving movement of the phalangeal joint.

High-energy roller injuries to the upper extremity.

Eleven cases of high-energy industrial roller injuries treated between 1980 and 1984 were retrospectively reviewed. The dominant extremity was affected in nine. Six patients sustained fractures and/or dislocations, and three of these patients required fasciotomies for clinical signs of impending compartment syndromes. All fracture/dislocations, with the exception of a scapula fracture, anterior dislocation of a thumb interphalangeal joint, and a fractured coronoid process of the ulna, required open reduction with internal fixation. Three patients required split-thickness skin grafting for exte

Primary toe-to-thumb transfer in the acutely avulsed thumb.

The case is presented of a primary toe-to-thumb transfer by microvascular anastomosis for reconstruction of an acute degloving amputation of the thumb. This approach is not recommended as the standard practice for this type injury, however, there were certain advantages in this patient: (1) one stage procedure, (2) restoration of length, sensibility, and cover, (3) cosmetic improvement, and (4) minimal length of hospitalization.

Decapitation due to car accident.

The occurrence of complete decapitation as a consequence of car accident is an extremely rare event. This fatality is generally seen in pedestrians run over by trains and also in motorcyclists who impact against the tailboard of trucks. Moreover, complete transection of pedestrians and occupants of cars has been described in road accidents especially in case of vehicles traveling at a high speed. We present a case of decapitation with complete degloving injury of the neck in a patient involved in a traffic accident, and we briefly discuss the possible mechanisms producing this injury.

A Traumatic Loss of the Calcaneus and Heel Pad From a Lawnmower Injury in a Pediatric Patient: A Case Report.

Traumatic calcaneal injuries with combined soft tissue loss are extremely rare in pediatric patients, posing significant reconstructive challenges due to contamination and the need to preserve growth potential. We report a case of a 10-year-old female who sustained a lawnmower-induced degloving injury with partial calcaneal loss, plantar heel pad avulsion, and posterior calf soft tissue damage. Management included emergent debridement followed by staged reconstruction aimed at preserving the distal calcaneal growth plate. At 11 months post-injury, the patient achieved complete wound closure, i

Repair of penile rupture through a high-scrotal midline raphe incision.

We report a case of penile rupture after a self-inflicted injury, which was repaired through a 4-cm midline raphe incision in the high-scrotal position. A single injury to the right lateral corpora cavernosum was identified and repaired. This incision allows for exposure of all three corporal bodies and results in excellent cosmesis. Creating artificial tumescence with intracorporal saline injection can be useful in identifying small or multiple sites of corporal injury. This may obviate the need for performing a routine degloving procedure, which results in unnecessary dissection, increased e
