Psychiatric symptomatology in borderline and other personality disorders: dissociation and fragmentation as mediators.

This study considered the role of dissociation and personality fragmentation as psychological factors that might distinguish borderline and personality disorder (BPD) patients, and that might explain why BPD patients have higher levels of other psychiatric symptomatology than those with other personality disorders. Two groups of personality-disordered patients (personality disorders including BPD; personality disorders other than BPD) completed measures of dissociation, personality fragmentation, and psychiatric disturbance. The BPD group had higher levels of a number of aspects of psychiatric

Impact of Cluster C personality disorders on outcomes of contrasting brief psychotherapies for depression.

Twenty-seven of 114 depressed clients, stratified for severity of depression, obtained a Diagnostic and Statistical Manual of Mental Disorders (3rd ed.; DSM-III; American Psychiatric Association, 1980) diagnosis of Cluster C personality disorder--that is, avoidant, obsessive-compulsive or dependent personality disorder (PD clients)--whereas the remaining 87 did not (non-personality-disorder [NPD] clients). All clients completed either 8 or 16 sessions of cognitive-behavioral (CB) or psychodynamic-interpersonal (PI) psychotherapy. On most measures, PD clients began with more severe symptomatolo

Body image concerns in bipolar I and II disorders: their relationships with personality styles and affective states.

BACKGROUND: Body image concerns are associated with the poor prognosis of bipolar disorder, but it is unknown whether bipolar I (BD I) and II (BD II) types differ in these concerns and their associations with personality styles or affective states. SUBJECTS AND METHODS: We therefore invited 89 BD I, 91 BD II patients, and 159 healthy volunteers to undergo the tests of the Body Image Concern Scale (BICS), the Mood Disorder Questionnaire, the Hypomania Checklist - 32, the Plutchik - van Praag Depression Inventory, and the Parker Personality Measure. RESULTS: Both BD I and BD II displayed highe

[Personality disorders in a psychiatric unit: retrospective study].

From those patients who were admitted to the psychiatric in-patient unit of the Hospital Ramón y Cajal of Madrid across a period of eight years, those who received a diagnosis of personality disorder according to the axis II of DSM-III or DSM-III-R were selected. We analyzed the patterns of comorbidity between axis I and II, length of the admission, the percentage of readmissions and general variables as age and sex. 17.35% of the patients admitted for the first time and 27.03% of those readmitted received a clinical diagnosis of personality disorder. The most frequent disorders were: unspecif

Relationship between hypomania and personality disorders before and after successful treatment.

OBJECTIVE: To examine the effect of hypomanic states on maladaptive personality traits and personality disorders, the authors evaluated personality traits and disorders of patients during an episode of hypomania and after successful somatic treatment. METHOD: The authors used the Structured Interview for DSM-III Personality Disorders to study 66 outpatients who had a lifetime diagnosis of bipolar disorder and who met the minimum Research Diagnostic Criteria for hypomania. All patients had a knowledgeable informant separately undergo the Structured Interview for DSM-III Personality Disorders d

Taxometric evidence for the dimensional structure of cluster-C, paranoid, and borderline personality disorders.

Despite a lively debate about the dimensional vs. categorical nature of Personality Disorders (PDs), direct empirical tests of the underlying structure are missing for most PDs. Taxometrics can be used to investigate whether latent structures are categorical or dimensional. We investigated the latent structure underlying Avoidant, Dependent, Obsessive-Compulsive, Depressive, Paranoid, and Borderline PD by means of three types of taxometric analyses. SCID-II based DSM-IV PD criterion scores from 1,816 patients from Mental Health and Forensic Institutes, and 63 nonpatients, were analyzed with th

Metacognitive mastery moderates the relationship of alexithymia with cluster C personality disorder traits in adults with substance use disorders.

Cluster C personality disorder traits have been observed in substance use disorders and linked with poorer outcome. One potential factor which may cause these disturbances in personality function is alexithymia, or the inability to name and express emotion. There may be other proximate factors which moderate the impact of alexithymia on the expression of cluster C traits, such as metacognitive mastery, which is the ability to use knowledge about mental states of self and others to cope with distress and solve social problems. To examine the possibility that mastery mediated the effects of alex

Comorbid disorders in patients with bipolar disorder and concomitant substance dependence.

OBJECTIVE: Substance dependence is common in bipolar disorder and is associated with an increase in Axis I and II comorbidity. Little research has compared the relative rates of comorbidity among bipolar patients with dependence on different substances. METHODS: The Mini International Neuropsychiatric Interview (MINI) was used to assess 166 outpatients involved in one of three clinical trials of medications for bipolar disorder and substance dependence. Patients had concurrent alcohol dependence, cocaine dependence, or both conditions. RESULTS: Generalized anxiety disorder and current depres

Separation anxiety disorder in OCD.

BACKGROUND: A history of separation anxiety disorder (SAD) is frequently reported by patients with obsessive-compulsive disorder (OCD). The purpose of this study was to determine if there are clinical differences between OCD-affected individuals with, versus without, a history of SAD. METHODS: Using data collected during the OCD Collaborative Genetic Study, we studied 470 adult OCD participants; 80 had a history of SAD, whereas 390 did not. These two groups were compared as to onset and severity of OCD, lifetime prevalence of Axis I disorders, and number of personality disorder traits. RESUL

Diagnostic specificity of the dependent and self-critical personality dimensions in major depression.

Validational studies of self-critical and dependent personality dimensions as vulnerability factors for depression have been tested primarily with depressed samples, employing research designs devised to address state vs. trait and trait-situational congruity issues. In this study we examined the diagnostic specificity to depression of these two personality dimensions, comparing Self-Criticism and Dependency scores as measured by the Depressive Experiences Questionnaire (DEQ) in two samples of outpatients: (1) panic disorder with agoraphobia; and (2) non-psychotic, unipolar major depression. Neuroticism associated with cocaine-induced psychosis in cocaine-dependent patients: a cross-sectional observational study.

BACKGROUND: Cocaine consumption can induce transient psychotic symptoms, which has been correlated with more severe addiction and aggressive behavior. However, little is known about the nature of the relationship between personality traits and psychotic symptoms in cocaine-dependent patients. This study examined the relationship between neuroticism and cocaine-induced psychosis. METHODS: A total of 231 cocaine-dependent patients seeking treatment were recruited to the study. Personality was evaluated by the Zuckerman-Kuhlman Personality Questionnaire. Cocaine-induced psychosis questionnaire,

Predictors of personality disorders in prisoners.

Personality disorders can lead to difficult social or occupational functional processes rooted in chronic maladaptive thoughts, feelings, and behaviors. This study aimed to investigate factors of personality disorder in prisoners from the central prison of Sanandaj, Iran. We conducted a cross-sectional study, which included all prisoners in the central prison of Sanandaj, Iran. The study sample includes 343 prisoners, of which 329 were male, and 14 were female, selected by randomized multistage sampling method. Participants filled in Millon's multi-axis clinical questionnaire. The data were an

The prediction of violent and nonviolent criminal behavior in a methadone maintenance population.

The utility of traits associated with Antisocial Personality Disorder in making risk assessments of violent and nonviolent crimes was examined in 254 subjects sampled from a methadone maintenance population. A factor analysis of a number of baseline measures resulted in five factors measuring hostility, insecure attachment, impaired reality testing, antisocial personality, and empathy. These factors were used in logistic regression analysis to predict charges for violent and nonviolent crimes over a 2-year period. Individuals with high scores on the antisocial personality factor had an increas

Comorbidity and its effects on panic disorder.

Comorbid conditions require attention in the planning of treatment strategies for panic disorder, as they affect the course and prognosis of the disorder as well as its response to treatment. The literature documents high rates of comorbidity for depression, bipolar disorders, generalized anxiety disorder, social phobia, other anxiety disorders, and personality disorders (especially dependent and avoidant personality disorders). The alcohol abuse research shows comorbidity rates that are lower than expected from clinical reports. Although agoraphobia is no longer considered a comorbid disorder

[The capacity for mentalization in depressive patients: a pilot study].

OBJECTIVES: Mentalization has been primarily investigated in Borderline Personality Disorders. This pilot study takes the first step toward investigating mentalization in depressive patients. METHODS: First, we present the theoretical background of the concept of mentalization and previous studies. Secondly, we report the results of this pilot study. 20 female inpatients suffering from Major Depressive Disorder were investigated with SKIDI+II, depression-scales (HAM-D, BDI), cognitive tests (TMT, MCVCT), and the Adult Attachment Interview (AAI), rated according to the Reflective Functioning S

Alexithymia and personality disorder functioning styles in paranoid schizophrenia.

OBJECTIVES: Personality disorder functioning styles might contribute to the inconclusive findings about alexithymic features in schizophrenia. We therefore studied the relationship between alexithymia and personality styles in paranoid schizophrenia. METHODS: We administered the Chinese versions of the Toronto Alexithymia Scale (TAS-20), the Parker Personality Measure (PERM), the Positive and Negative Syndrome Scale as well as the Hamilton Anxiety and Depression Scales to 60 paranoid schizophrenia patients and 60 healthy control subjects. RESULTS: Patients scored significantly higher on the

The general neurotic syndrome: a coaxial diagnosis of anxiety, depression and personality disorder.

The validity of the general neurotic syndrome, a combination of anxiety, depression and dependent personality disorder, was examined in a 2-year study of outpatients with dysthymic, panic and generalized anxiety disorder diagnosed using a structured interview schedule. The general neurotic syndrome, found in a third of the patients, was associated with greater mental disorder and a significantly worse outcome than patients without the syndrome. It did not, however, predict response to treatment. Further analysis revealed that the general neurotic syndrome was a better predictor of short- and l

The comorbidity of multiple personality disorder and DSM-III-R axis II disorders.

Our ability to differentiate MPD from DSM-III-R Axis I disorders has become increasingly refined. Differentiation of MPD from the Axis II personality disorders is an area of more recent clinical investigation. MPD can be found comorbidity with many other psychiatric conditions. It is found in association with each of the DSM-III-R personality disorders. At the present time, however, we lack research data that define the prevalence of the comorbidity of MPD with the personality disorders. Objective study of this area is complicated by the paucity of instruments available to assess personality d
