Differential effectiveness of dialectical behavioural therapy and schema therapy in patients with borderline personality disorder: a secondary analysis of a randomised clinical trial.

BACKGROUND: Borderline personality disorder (BPD) can be treated successfully with specific psychological treatments, but there is no clear evidence of superiority of one specific treatment at the group level. Due to high heterogeneity in BPD, individual patients might benefit differently from specific treatments. METHODS: Based on a randomised trial comparing 18 months of dialectical behaviour therapy (DBT) and schema therapy (ST) for BPD, differential effectiveness was examined using causal forest analyses. Baseline variables on BPD criteria, general psychopathology, traumatic childhood exp

A modified DBT skills training program for oppositional defiant adolescents: promising preliminary findings.

A modified skills training component of dialectical behavior therapy (DBT) was implemented in a group therapy format for non-suicidal outpatient young adolescents who met criteria for oppositional defiant disorder (ODD). Thirty-two youths completed the 16-week program, as well as pre- and post-treatment measures. The treatment was effective not only in decreasing negative behaviors, but also in increasing positive behaviors, per caregiver report. The youths reported a significant reduction in externalizing and internalizing symptoms and in depression. Reliable change indices indicated that far

[The role of traumatization in eating disorders. Possible therapeutic modalities with special regard to cognitive behavioural methods].

In the complex pathomechanism of eating disorders (especially that of bulimia nervosa and binge eating disorder) the traumatic experiences (sexual, physical, emotional abuse, neglect) often play an important role. The consequence of traumas can be the borderline personality disorder, or different disorders of the emotional regulation. These occur mainly in the multiimpulsive subtype of eating disorders with impulse control disorders. In relation to the trauma dissociative phenomena are frequent. In the diagnostics of eating disorders the history of traumatization is essential. Among the therap

Multi-Family Therapy for Bulimia Nervosa in Adolescence.

Existing randomized controlled trials of family therapy for treatment of bulimia nervosa in adolescence highlight the need for further development of treatments. This article describes the development of multi-family treatment for bulimia nervosa in adolescents aged 13-18. It outlines the theory guiding this development, the areas of need identified by previous studies, and the treatment that has been designed to meet these needs. Particular attention is given to the need to increase communication between family members, strategies to reduce high levels of criticism or hostility, and skills to

Emotion regulation and impulsivity evolution during transdiagnostic DBT implementation in addictology - a 3-year naturalistic study.

INTRODUCTION: Addictive and eating disorders are the two deadliest psychiatric disorders. Emotional dysregulation and impulsivity are central processes in both disorders. Maladaptive emotion regulation strategies play a key role in the development and maintenance of these disorders. The standard treatment for emotion regulation is Dialectical Behavioral Therapy (DBT); its implementation is often a challenge, but transdiagnostic groups can facilitate it. Due to the lack of data available on these groups in addictology, our study aimed to determine whether emotional dysregulation and impulsivity

Experiences of dialectical behaviour therapy for adolescents: A qualitative analysis.

OBJECTIVES: To explore how former patients in dialectical behaviour therapy for adolescents (DBT-A) experience their treatment, and specifically if there were aspects of the treatment that they retrospectively identify as particularly meaningful, helpful or unhelpful. DESIGN: From a larger sample of 75 former DBT-A patients 19 were selected for a qualitative semi-structured interview study. METHODS: Young adults (N = 19; 18 females, one male), who as adolescents had been enrolled in DBT-A due to self-harming behaviours and features of borderline personality disorder, were interviewed up to 8

Identifying early dropouts from a rehabilitation program for psychiatric outpatients.

The attendance of 112 patients referred to an outpatient psychiatric rehabilitation program was retrospectively examined, and characteristics of dropouts were compared with those of patients who remained in treatment during the first eight weeks. Discriminant function analysis revealed that a combination of three variables correctly predicted 81 percent of the dropout group. The variables were a hostile, labile, or bizarre affect; a diagnosis of personality disorder; and impaired orientation. Patients with these characteristics require a thorough assessment of specific needs and deficits and o

Clinicians' perspectives of interfering behaviors in the treatment of anxiety and obsessive-compulsive disorders in adults and children.

Patient behaviors that may interfere with the process and outcome of therapy have been examined in the context of dialectical behavior therapy, but no measures exist to systematically characterize patient (or caregiver) treatment interfering behaviors (TIBs) in cognitive behavioral therapy (CBT) for anxiety disorders and obsessive-compulsive disorder (OCD). Accordingly, the primary aims of this study were to develop preliminary measures of TIBs, asking clinicians who provide CBT for anxiety disorders and/or OCD to adults and/or children to retrospectively reflect on the presence of TIBs in a r

Phone consultation and burnout among providers of dialectical behaviour therapy.

Adherent dialectical behaviour therapy (DBT) includes between-session phone consultation to help clients generalize skills, solve problems during crises, and repair relationships. Despite benefits of phone consultation, it is frequently not implemented in outpatient settings. The perceived burden phone consultation places on providers is one of the most frequently cited reasons for its omission. The current study examined phone consultation in relation to providers' burnout using a cross-sectional design. We hypothesized that (1) DBT experience and support from peer consultation team members,

Target-user preferences, motivations, and acceptance for a dialectical behaviour therapy smartphone application for eating disorders.

PURPOSE: Evidence shows that dialectical behavioural therapy (DBT) is efficacious for eating disorders (ED), yet few people have access to specialized treatments like DBT. Translating key DBT skills for delivery via a smartphone application may broaden the dissemination of evidence-based interventions. However, prior to developing a DBT-based app, it is crucial to gather information on target-user needs and preferences. Assessing overall acceptance and identifying predictors of acceptance, informed by the UTAUT framework, is also important. This process ensures not only a demand for such an ap

Parent and Carer Skills Groups in Dialectical Behaviour Therapy for High-Risk Adolescents with Severe Emotion Dysregulation: A Mixed-Methods Evaluation of Participants' Outcomes and Experiences.

BACKGROUND: There is an established evidence-base for dialectical behaviour therapy for adolescents (DBT-A) in the treatment of young people with severe emotion dysregulation and related problems, including repeated self-harm and suicidal behaviours. However, few studies have reported on parental involvement in such treatments. This study aims to explore the outcomes and experiences of participants of a dedicated skills group for parents and carers embedded within an adapted DBT-A programme in the United Kingdom. Mental Health and Personality Disorders: Personality Disorders.

Personality disorders describe enduring, pervasive, pathologic patterns of behavior and inner experiences that deviate from a patient's culture. Personality disorders are divided into three clusters depending on core features. Diagnosis of a personality disorder is generally made based on criteria from the Diagnostic and Statistical Manual of Mental Disorders, although other diagnostic modalities exist. The most common disorders in primary care settings include obsessive-compulsive personality, narcissistic personality, and borderline personality disorders. Obsessive-compulsive personality dis

Emotion dysregulation is heightened in autistic females: A comparison with autistic males and borderline personality disorder.

BACKGROUND: Emotion dysregulation is increasingly recognized as highly prevalent and impairing in autistic individuals. Yet, a large majority of studies have considered emotion dysregulation in youth only, and most of them did not consider sex differences in emotion dysregulation manifestation. OBJECTIVES: In the present study, we aim to investigate sex differences relative to emotion dysregulation in autistic adults without intellectual disability as well as its relationship with different factors potentially involved in emotion dysregulation (e.g. camouflaging, alexithymia, suicidality, qua

Telehealth and Collaboratively Delivered Dialectical Behaviour Therapy: An Opportunity for Increasing Access to Effective Treatment for People With Borderline Personality Disorder Living in Rural Areas.

AIMS: The aim of this commentary is to consider how telehealth and a collaborative model of service delivery may offer a way of making Dialectical Behaviour Therapy available in rural areas. CONTEXT: Dialectical Behaviour Therapy (DBT) is an effective treatment for Borderline Personality Disorder (BPD). However, there are many barriers to making this sort of therapy available within routine care, particularly in rural areas. APPROACH: This commentary provides a summary of the literature relevant to the role that telehealth could play in increasing access to DBT. A new model of care could uti

Clinical outcomes for sexual and gender minority adolescents in a dialectical behaviour therapy programme.

BACKGROUND: Sexuality and gender minoritised (SGM) adolescents are at increased risk of self-injury and suicide, and experience barriers to accessing mental health support. Dialectical behaviour therapy (DBT) is an effective treatment for self-injury and emotion dysregulation in adolescent populations, but few studies have published outcomes of DBT for SGM young people. AIMS: This study aimed to investigate treatment outcomes and completion for SGM adolescents and their cisgender and heterosexual peers, in the National & Specialist CAMHS, DBT service (UK). Does impulsivity predict treatment outcomes in PTSD with borderline personality disorder features? Results from a randomized clinical trial.

BACKGROUND: Trauma-focused psychotherapies are first-line treatments for posttraumatic stress disorder (PTSD). However, a substantial proportion of clients do not respond adequately or drop out of therapy prematurely. This has sparked interest in identifying individual-level predictors of treatment outcomes, including improvement in PTSD severity and dropout. Impulsivity may be a predictor because it may interfere with key therapeutic processes, such as cognitive restructuring and emotional processing. Consequently, we present a hypothesis-driven secondary analysis of a 15-month randomized cli

Othello syndrome. Preventing a tragedy when treating patients with delusional disorders.

Othello syndrome, sometimes referred to as delusional, pathological, morbid, or erotic jealousy, is a rare delusional disorder with high-risk implications. This article provides an overview of Othello syndrome and includes an individual example as well as a review of the literature. Pharmacological interventions and dialectical behavior therapy are discussed as options for pathological jealousy. Nursing considerations and assessment strategies of delusional jealousy are described for identification and evaluation of risk factors. Patient safety is addressed, and collaborative interventions are

Psychiatric comorbidity and depression in older adults.

Late-life depression rarely occurs in isolation. Thus, screening for and assessment of late-life depression should always involve screening for other psychiatric disorders, including anxiety, personality disorders, and alcohol misuse. Comorbidity often makes treatment to relieve depressive symptoms more complicated, more comprehensive, and longer. Current late-life depression research indicates the strongest support for antidepressant medication and/or cognitive-behavioral therapy for comorbid anxiety, antidepressants and dialectical behavior therapy for comorbid personality disorders, and ant

Personal agency and borderline personality disorder: a longitudinal study of outcomes.

BACKGROUND: Low personal agency is the concept of attributing successes and failures to external factors rather than personal characteristics. Previous research supported links between low personal agency and symptoms of borderline personality disorder (BPD). The present research followed patients in an outpatient dialectical behavioural therapy (DBT) group from intake to 12 months follow up to examine the impact of personal agency on outcome. METHODS: Patients (N = 57, age 18-72, 91.5% female) were assessed at intake, after three months of DBT treatment, and 12 months follow up on measures o
