Heterogeneity and atypical presentation in infantile systemic hyalinosis with severe labio-gingival enlargement: first Egyptian report.

Infantile systemic hyalinosis (ISH) (MIM 236490) is a rare, progressive, fatal autosomal recessive condition characterized by widespread deposition of hyaline material in many tissues. Our proband was a 4-year-old male with growth retardation, severe labio-gingival enlargement, generalized stiff skin, joint contractures, and intractable diarrhea. We discovered a history of a brother and sister who suffered a more severe disease course. A final diagnosis of systemic hyalinosis was made; we report this case and discuss the clinical and orodental heterogeneity among these siblings in the first re

Gut Microbiota Profile and Functional Gastrointestinal Disorders in Infants: A Longitudinal Study.

BACKGROUND/OBJECTIVES: The gut microbiota is involved in modulating gastrointestinal function and consequently contributes to the manifestation of functional gastrointestinal disorders (FGIDs). The aim of the study was to analyze the composition of the gut microbiota in infants with functional gastrointestinal disorders (infantile colic, functional constipation, gastroesophageal reflux, functional diarrhea) according to age, environmental factors, and clinical manifestations. METHODS: The study involved the clinical and laboratory examination of 134 infants divided into two groups: group I (n

[Clinical picture of celiac disease in children].

The celiac disease was first described over one hundred years ago. English doctor S. Gee selected a syndrome characterized by growth inhibition, diarrheas and malabsorption disorders among the group of enteropathies in children in 1888. He assumed that the large intestine was affected in such cases. That is the reason why this disease was called celiac affection. C. Herter described a similar disease in 1908 and called it intestinal infantilism. Only as early as in 1950 Dutch pediatrician W. Dicke described the inability to uptake gluten (a component found in some herbs) as an important reason

Pompe disease: dramatic improvement in gastrointestinal function following enzyme replacement therapy. A report of three later-onset patients.

Pompe disease is a lysosomal storage disease due to deficient acid α-glucosidase (GAA) activity. Infants with the classic infantile-onset subtype present with severe hypotonia and cardiomegaly, and most expire in the first year of life, whereas the severity of the muscle-based manifestations in patients with the late infantile/juvenile and adult-onset subtypes depends on the level of GAA residual enzymatic activity. The clinical features of later-onset Pompe disease are still emerging, and even the natural history and progression of muscle weakness and respiratory failure, hallmarks of the lat

Pompe Disease: Cyanosed Hypotonic Infant with Normal Respiratory Rate.

Infantile hypotonia or floppy infant is a diagnostic challenge when it presents with other presenting complaints such as fever, cough or diarrhea. Many times the hypotonia goes unnoticed when other symptom covers the hypotonia and child continues to receive the treatment for other symptoms. We report a rare case from Nepal of infantile Pompe disease who presented with the history of fever and cough in the recent earthquake disaster camp at remote part of Sindhupalchowk, Nepal. He was being treated as a case of pneumonia. Pompe disease can be diagnosed clinically by taking detailed history and

Rotavirus vaccines and the prevention of hospital-acquired diarrhea in children.

Rotavirus, the major cause of severe acute dehydrating gastroenteritis in children less than 5 years of age, is responsible for an estimated 20-50% of all hospitalizations for diarrhea and approximately 440,000 deaths annually, primarily in the developing world. Rotavirus vaccines are considered the most promising means for disease prevention. While the prime rationale for developing rotavirus vaccines has been the enormous burden of rotavirus infection leading to severe and fatal disease, a secondary benefit may be the prevention of nosocomial rotavirus diarrhea. We have reviewed the burden o

Aetiology of chronic diarrhoea in children: experience at King Khalid University Hospital, Riyadh, Saudi Arabia.

Forty-eight cases of chronic diarrhoea in children seen at King Khalid University Hospital over a 5-year period were analysed. The mean age at presentation was 1.8 years (range 0.08-10 years); 34 were boys and 14 girls. Forty-four patients were Saudi and four were non-Saudi Arabs. Most children presented with failure to thrive and pallor. The aetiological factors identified were: the post-gastro-enteritis syndrome with or without lactose intolerance in 16 (33%); coeliac disease in ten (21%); congenital chloride diarrhoea in five (10%); glucose-galactose malabsorption and acrodermatitis enterop

Rotavirus as an important cause of diarrhoea in a hospital for children in Dammam, Saudi Arabia.

A major cause of morbidity and mortality among children in developing countries is acute diarrhoea and rotavirus, a reovirus-like agent, is found to be a leading causative agent. We report here the incidence of rotavirus infection among infants and young children with gastro-enteritis treated at the Children's Hospital in Dammam, Kingdom of Saudi Arabia. One hundred and fifty children, aged between 1 and 60 months, suffering from diarrhoeal illness, were selected as the study cohort; another 150 hospitalized children of the same age group, but not suffering from diarrhoea, served as controls.

ACUTE DIARRHOEAL DISEASE IN LESS DEVELOPED COUNTRIES. I. AN EIDEMIOLOGICAL BASIS FOR CONTROL.

Some of the acute diarrhoeas of man are specific infectious diseases. Another and major proportion have no demonstrated infectious agent. No characteristic clinical pattern regularly distinguishes one specific disease entity from another, nor specific from non-specific infections, nor epidemic from endemic disease. Since clinical and laboratory differentiation is at present possible for only a small fraction of the diarrhoeas, an alternative in community control, especially where facilities are limited, is to regard all diarrhoeal disease as constituting a clinical syndrome, "acute undifferent

Secretory IgA-mediated protection against V. cholerae and heat-labile enterotoxin-producing enterotoxigenic Escherichia coli by rice-based vaccine.

Cholera and enterotoxigenic Escherichia coli (ETEC) are among the most common causes of acute infantile gastroenteritis globally. We previously developed a rice-based vaccine that expressed cholera toxin B subunit (MucoRice-CTB) and had the advantages of being cold chain-free and providing protection against cholera toxin (CT)-induced diarrhea. To advance the development of MucoRice-CTB for human clinical application, we investigated whether the CTB-specific secretory IgA (SIgA) induced by MucoRice-CTB gives longstanding protection against diarrhea induced by Vibrio cholerae and heat-labile en

Bronchial and gastrointestinal cryptosporidiosis in AIDS.

Cryptosporidiosis is a coccidial protozoan initially reported in domestic animals. It is primarily a gastrointestinal organism that does not invade mucosa. It was first described in 1976 in developing countries as an etiology of infantile diarrhea with inanition and malnutrition. Gastrointestinal involvement in patients with AIDS has been increasingly reported. We report a case of combined gastrointestinal and bronchial cryptosporidiosis. Cryptosporidium is an acid-fast organism which was successfully treated with erythromycin.

Role of colostrum in gastrointestinal infections.

Colostrum is breast milk produced after the birth of the newborn and lasts for 2-4 days. Colostrum is very important part of breast milk and lays down the immune system and confers growth factors and other protective factors for the young ones in mammals. This is the source of passive immunity transferred to the baby from the mother. The biological value of bovine colostrum in present day medical practice is documented in clinical trials and large databases containing case reports and anecdotal findings. The main actions include an antibacterial effect and modulation of immune response with th

Mediastinal ganglioneuroblastoma-secreting vasoactive intestinal peptide causing secretory diarrhoea.

In this case report we describe a case of mediastinal ganglioneuroblastoma-secreting vasoactive intestinal peptide (VIP), causing secretory diarrhoea in an 18-month-old child. An 18-month-old girl presented with a 2-month history of diarrhoea, abdominal distension and weight loss. Investigations revealed secretory diarrhoea with hypokalaemia, hyponatraemia and hypochloraemia and metabolic acidosis. Her stool output was 2.5-3.lday(-1) with increased stool sodium. VIP levels were strikingly high with normal glucagon and gastrin levels. X-ray of the chest revealed a well-defined mass in the right

[Rectal prolapse in a neonate with Shigella diarrhea].

UNLABELLED: A case of neonatal rectal prolapse during Shigella diarrhea is described. A 10 days neonate exclusively breast fed, whose 2 years old brother has had the same process 2 weeks ago. Medical treatment with intravenous ceftriaxone resolved rectal prolapse in 48 hours. Stool cultures showed the presence of Shigella flexneri II resistant to amoxicillin; blood and cerebrospinal fluid, cultures were negative. CONCLUSION: Rectal prolapse is an uncommon complication seen in infants with Shigella diarrhea. It has been described in older children, but never before in neonates.
