[Newborn with Bilateral Lower Limb Amputation Due to Twin Anemia Polycythemia Sequence].

We present a case report of a newborn with bilateral lower limb ischemic gangrene caused by spontaneous Twin anemia polycythemia sequence (TAPS). Due to the extent of tissue damage, bilateral lower limb amputation was performed on the fourth day after birth-left knee disarticulation and right transfemoral amputation. The postoperative course was uneventful, and the wounds healed by primary intention. This case represents an exceptionally rare complication of TAPS, in which timely prenatal diagnosis could have enabled early intrauterine intervention.

Complications of suppurative arthritis and osteomyelitis in children.

Three children with suppurative arthritis and osteomyelitis are described to emphasise that delayed or incorrect diagnosis may lead to serious cardiopulmonary complications. In two patients, bilateral bronchopneumonia developed with pneumatocoeles, pneumothorax and empyema. The other had cardiac failure from septic pericarditis. In one case, disarticulation of the knee was needed as a life-saving measure, and the other leg developed an infected pseudarthrosis of the tibia. The causative organism in each case was staphylococcus aureus and no evidence of immunodeficiency was demonstrated.

[Gas gangrene: an extreme surgico-medical rather than medico-surgical emergency].

A diabetic woman developed spontaneous gas gangrene of upper limb extending into trunk and due to clostridium septicum. Recovery was obtained after major surgery involving disarticulation of upper limb and excision of soft tissues of left hemithorax, and a difficult intensive care programme (hyperbaric oxygen therapy, assisted ventilation). The presence of this gas gangrene revealed a bipolar colon cancer treated in parallel. The conventional therapeutic hierarchy is discussed: surgery should be carried out as an extreme emergency and should not be delayed to allow transfer to a center equippe

Clostridium septicum gangrene complicating a closed femoral fracture.

One of the basic principles of orthopedic surgery is that gas gangrene does not develop in closed fractures. That there are exceptions to this rule is demonstrated by the following report of an extremely rare case. Clostridium septicum myonecrosis developed after surgical treatment of a closed femoral fracture in a fit 45-year-old man, treated by antibiotics. The patient survived after having a right hip disarticulation and administration of high doses of intravenous penicillin. Prophylactically administered cephalosporins failed to prevent the Clostridial infection.

Ilizarov lengthening of a posttraumatic below elbow amputation stump. A case report.

Lengthening of a nonfunctional posttraumatic below elbow stump via Ilizarov technique is described. Preoperatively, the 5 cm ulna in the residual forearm provided an insufficient lever arm for practical use of a standard below elbow prosthesis. An additional 5 cm of ulnar length was achieved (100% increase) by distraction histogenesis, ultimately limited by impending necrosis of adherent skin covering the tip of the residual limb. Utilizing the Ilizarov technique, ulnar lengthening converted the functional level of the patient's amputation from elbow disarticulation to below elbow status.

[Profuse hemorrhage from the axillary artery in electric burn: prevention and treatment].

A total of 53 cases of electric burns were admitted to our unit during the 1977-1985 period. In 19 cases an upper limb was involved, accounting 35.8 percent of the total. In 15 cases disarticulation of the shoulder was done, and in 3 of them a profuse hemorrhage from the axillary artery occurred as a result of infection of the amputation stumps. The second segment of the axillary artery was ligated during the amputations in the other 4 cases, no hemorrhage occurred in them.

A Surgeon's Guide to Obtaining Hemorrhage Control in Combat-Related Dismounted Lower Extremity Blast Injuries.

The application of lessons learned on the battlefield for timely surgical control of lower extremity hemorrhage secondary to blast injuries to the civilian practice for similar wounding patterns from industrial accidents or terrorist activities is imperative. Although simple cut-down procedures are commonly sufficient for the control of blood vessels for distal extremity traumatic amputations, high-thigh or disarticulation wounding patterns often require more complex surgical methods. The following details both the decision-making process and operative techniques for controlling hemorrhage fro

[An atraumatic approach in stapes reconstruction].

In order to prevent complications which occur in classical stapedoplasty, authors applied a new atraumatic procedure in 138 patients with otosclerosis. Along with preserved ossicular chain, stapedotomy was performed and a piston-wire prosthesis placed. Disarticulation of incudostapedial joint was the next part of the procedure with removal of suprastructure of the stapes. Early and late cochlear disorders were rare while the final functional effects were much better. This paper analyzes numerous advantages of this procedure with directives for promotion of our work in the future.

Striving for Consensus: Neuroma Prevention and Preferred Approaches in Surgical Digit Amputations-A Survey of Hand Surgeons.

Background: Although surgical digit amputations are common procedures in hand surgery, substantial differences in technical approaches have been reported, with the development of symptomatic neuromas posing a major challenge. Given the absence of a universally accepted technique, especially concerning painful neuroma prevention, this survey study's rationale was to explore the preferred surgical approach for traumatic noninfected digital amputation injuries (excluding the thumb) at the interphalangeal (IP) joint levels. Methods: A 10-point online questionnaire was sent to ÖGH, DGH and SGH memb

Residual rotation of forearm amputation: cadaveric study.

BACKGROUND: The purpose of this study was to investigate residual rotation of patients with forearm amputation and the contribution of involved muscle to residual rotation. METHODS: Testing was performed using five fresh-frozen cadaveric specimens prepared by isolating muscles involved in forearm rotation. Amputation was implemented at 25 cm (wrist disarticulation), 18 cm, or 10 cm from the tip of olecranon. Supination and pronation in the amputation stump were simulated with traction of involved muscle (supinator, biceps brachii, pronator teres, pronator quadratus) using an electric actuator

Osteosarcoma in a patient with neurofibromatosis type 1: a case report and review of the literature.

Neurofibromatosis type 1 (NF1) or von Recklinghausen's disease is a genetic disease generally characterized by café-au-lait spots and neurofibromas. Malignant tumors of the nervous system, such as malignant schwannomas, gliomas, or astrocytomas, have been well known to coexist with neurofibromatosis. However, occurrence of malignant tumors unrelated to the nervous system is rare. We report an unusual case of a 29-year-old NF1 female suffering from malignant peripheral nerve sheath tumor (MPNST) that eventually developed osteosarcoma in the proximal femur. Osteosarcoma is the most common high-g

Operation Techniques and Outcomes in Patients with Chronic Limb Threatening Ischaemia Undergoing Minor Amputations-A Cohort Study.

INTRODUCTION: For patients with chronic limb threatening ischaemia (CLTI), minor amputations are frequently performed; however, evidence regarding surgical technique and outcomes is limited. This study aimed to investigate the association between toe amputation technique and clinical outcomes. METHOD: A retrospective cohort study was conducted, including all patients with Rutherford 5 or 6 CLTI who underwent a toe amputation at a single university centre between January 2018 and December 2023. Data were extracted from electronic patient records. Surgical techniques included metatarsophalangea

Brachial Plexus Birth Injury Causes Location-Dependent Detriments in Glenohumeral Trabecular Bone Microstructure.

Brachial plexus birth injury (BPBI) is caused during a difficult childbirth when the head and neck are excessively stretched, and the brachial plexus nerve bundle is damaged. Injury causes lifelong arm impairment in 30-40% of those affected, but the extent to which bone microstructure is affected and how that relates to bone morphology alterations is unclear. Additionally, how injury location impacts trabecular bone, and the role limb disuse plays in these differential effects are unknown. Using two rat models of BPBI (postganglionic and preganglionic) and a disuse model (disarticulation), tra

Facial reanimation with end-to-end hypoglossofacial anastomosis: 20 years' experience.

OBJECTIVE: This study aimed to evaluate retrospectively the results of experience with end-to-end anastomosis of cranial nerves VII and XII, performed due to transection of the facial nerve during acoustic neuroma removal. METHODS: We assessed the facial reanimation results of 33 patients whose facial nerves had been transected during acoustic neuroma excision via a retrosigmoid approach, between 1985 and 2006, and who underwent end-to-end hypoglossofacial anastomosis. We compared the facial nerve functions of patients receiving short term (two to three years) and long term (more than three y

Evolution of speech and voice following supracricoid partial laryngectomy.

The evolution of voice and speech parameters following supracricoid partial laryngectomy (SCPL) has been evaluated in a prospective fashion over an 18-month period of time in three patients using the Computerized Speech Lab (from Kay Elemetrics). Preliminary results demonstrate the post-operative instability of voice parameters. Speech parameters remain stable with time. Our data stresses that voice is attained post-operatively even if one arytenoid cartilage is disarticulated at time of resection.

A Functional Larynx Dissection Utilizing Mandibular Rotation: A Technical Report.

In gross anatomy labs, the larynx is usually visualized with either a sagittal hemisection or a posterior disarticulation approach. Both fail to maintain the functional in-situ relationships between the intact larynx and adjacent structures. This makes it challenging for students to understand how suprahyoid and infrahyoid muscles, pharyngeal muscles, the tongue, epiglottis, and intrinsic laryngeal muscles are all involved with airway protection and swallowing. We describe a novel approach to cadaveric larynx exposure using a pharyngotomy and mandibular rotation technique, which keeps the lary

Clostridium septicum infection associated with colonic carcinoma and hematologic abnormality.

Six patients with Clostridium septicum sepsis seen at Duke University Medical Center over a two-year period also had other abnormalities, consisting of hematologic disorders in 3 and colon tumors in 3. Three patients died of sepsis; 2 survived following disarticulation of the arm to control gas gangrene, while the sixth patient survived the sepsis but died of metastatic disease. When anaerobic cultures are positive for C. septicum, antibiotics should be given immediately. The high incidence of underlying colon tumor, especially in the cecum, should prompt consideration of a barium-enema examin

Challenges in managing Stewart-Treves Syndrome: navigating complexities in recurrent lymphangiosarcoma.

Lymphangiosarcoma, or Stewart-Treves syndrome, is a rare and aggressive malignancy. It is associated with poor prognosis due to its high risk of recurrence and metastasis. This case report highlights the surgical challenges faced in managing lymphangiosarcoma. We present a case of a woman in her 60s who underwent modified radical mastectomy and adjuvant therapy for breast carcinoma. She developed lymphangiosarcoma of the upper limb and underwent limb disarticulation. 6 months later, she presented with recurrent angiosarcoma at the amputation site. A wide local excision was performed, followed b

An alternative bent-knee prosthesis.

Prosthetic fitting in patients with below-knee amputations and concurrent knee flexion contractures poses inherent difficulties to the prosthetic rehabilitation team. The standard bent-knee prosthesis is bulky and awkward. It treats the patient functionally as a knee disarticulation and yields no potential for improving the patient's degree of contracture. This paper describes a case report with an alternative to the bent-knee prosthesis that not only offered improved function relative to the standard bent-knee prosthesis, but also acted therapeutically by reducing the patient's knee-flexion c

Clubfoot deformity and tibiofibular diastasis.

Four children with distal tibiofibular diastasis associated to talipes equinovarus deformity are described. Tibial hypoplasia resulting in tibiofibular diastasis is believed to be the underlying cause of the deformity. This complex deformity is usually associated with other congenital malformations including congenital cardiac defects and other limb anomalies. Surgical therapy has centered on establishment of an ankle mortise; however, several authors believe that the best treatment is early ankle disarticulation rather than reconstructive surgery. Radiographs should be made of all apparent cl
