US Emergency Department Use and Operations Amid Natural Disasters: A Narrative Review.

In the United States from 2014-2024, an average of 18.2 national disasters per year caused over a billion dollars in inflation-adjusted damage, compared with 3.3 national disasters per year during the 1980s. The increased frequency and intensity of severe weather phenomena-attributed by climate science experts to climate change-have raised concerns about national emergency preparedness. One aspect of emergency preparedness is the functioning of emergency departments (ED). In this narrative review, we examine patterns of ED use and operations amid natural disasters in the US, with a special foc

Creating a state medical response system for medical disaster management: the North Carolina experience.

The purpose of this work was to examine the creation and evolution of the North Carolina state medical response system (SMRS). During the past 30 years, states and local communities have developed a somewhat incongruent patchwork of medical disaster response systems. Several local or regional programs participated in the National Disaster Medical System; however, aside from the Disaster Medical Assistance Teams, most of these local resources lacked national standards and national direction. The September 11, 2001 terrorist attacks in Washington, DC and New York, and the anthrax-laced letters m

From Compliance to Competence: Accelerating Readiness to Change in Hospital CBRNE Preparedness.

Hospital preparedness for CBRNE events remains an Achilles' heel-not because plans do not exist, but because the gap between documented protocols and executable competence is vast. This editorial describes a transformative workshop for a hospital disaster team, operationalizing their Emergency Operations Plan through high-fidelity simulation, iterative practice, and structured hot wash after-action reviews. Anchored in the Transtheoretical Model, our assessment captured not merely knowledge retention but the team's readiness to change-their shift from passive compliance to active ownership. Po

Active shooters: The underutilisation of emergency operations centres - the need for a checklist to guide response and recovery.

The emergency operations centre (EOC) is a critical emergency response and recovery component that provides information management and resource allocation. EOCs are often used during all hazards; however, after reviewing over 25 after-action reports for active shooter incidents, they are frequently underutilised. Not activating or delaying activation can slow recovery efforts and lead to chaos for the first responders and the public due to a lack of situational awareness. Historical active shooter incidents, such as the San Bernardino attack, Uvalde school shooting and Aurora theatre shooting,

Trajectories of community fire adaptation: Social diversity, social fragmentation and the temporal evolution of wildfire action.

There is increasing acknowledgement that the unique characteristics (i.e., social contexts) of human communities influence variable means for adapting to the growing risks posed by wildland fire. However, there has been less work documenting how community social contexts evolve over time, and the ways they might influence collective mitigations pursued in partnership with professionals when addressing wildfire planning. We conducted 73 semi-structured interviews with 112 residents, emergency management professionals, government officials and members of community organizations in two Nevada Cou

Conflict by design: Stafford Act incompatibility with Alaska tribal sovereignty.

The Robert T. Stafford Act unintentionally marginalizes Alaska's Tribes and hinders their ability to exercise sovereignty following disasters. Although there has been significant academic analysis of the Alaska Native Claims Settlement Act since its passage in 1971, the monumental agreement that settled Alaska Native aboriginal land claims was not critically considered during the creation of the Stafford Act, nearly two decades later in 1988. The consequences have resulted in increased confusion and controversy as Alaska's Tribes attempt to exercise their sovereign option following emergencies

Earthquake Survival Strategies: Potential Advantages of the Fetal Position in the Triangle of Life to Survive Over the "Drop, Cover, and Hold On".

This article explores 2 key earthquake survival strategies: the widely endorsed "Drop, Cover, and Hold On" (DCH) method and the alternative fetal position within a survival triangle. While DCH provides mechanical protection from falling debris, its effectiveness in scenarios involving structural collapse and prolonged entrapment remains uncertain. Drawing on recent field data and thermodynamic considerations, this paper argues that the fetal position may offer survival advantages by minimizing heat loss and conserving metabolic energy-especially under cold conditions and delayed rescue. Integrating anticipatory action in disease outbreak preparedness and response in the humanitarian sector.

In the humanitarian sector, anticipatory action entails acting ahead of predicted hazardous events to prevent or mitigate potential impacts and needs. It leverages early warnings to bridge preparedness and response, with a core principle being the provision of ex-ante emergency funding for preagreed early actions. Traditionally applied to extreme climatic events, there is growing interest in integrating anticipatory action into disease outbreak preparedness and response. We present an analytical framework for trigger development for climate-sensitive infectious disease outbreaks based on a rev

Get it together, together: Creating whole community continuity through cross-sector collaboration in Texas.

Continuity of operations for government is an evolving philosophy, much like exercises and after-action reports. Continuity continues to identify areas for growth and improvement as more people become involved in the conversation. This paper briefly describes the evolution of continuity in the USA and its application in the State of Texas. Moving forward, it discusses the application of the concept of 'whole community continuity' as the driving force of the Continuity Council in Texas, which focuses on preparedness at all levels, from individuals to private industry, to all levels of governmen

Building capability and community through cyber-incident response exercises.

While a natural disaster or related threat may impact an organisation at some point, it is more likely (even inevitable) that it will be the victim of a cyber attack. The solution to being better prepared for these imminent attacks is to undertake more lightweight and frequent incident response (IR) exercises to help build capabilities and community through a tighter, recurring cycle of planning, conducting and assessing. To boost the facilitation of IR exercises, organisations must leverage the established relationships between business continuity management (BCM) or resilience staff (both of

Using enterprise risk management to strengthen organisational resiliency : One institution's story.

This paper discusses how Carnegie Mellon University launched a cyclical enterprise risk management framework that incorporates both emergency preparedness and response and business continuity into its purview, to deliver greater organisational resiliency. The paper goes on to describe the governance structure that defines roles and responsibilities throughout the organisation, before discussing how faculty, staff and students are engaged and educated to sense risks, and to collaborate with leadership at all levels in prioritising risks for deep-dive assessments and employing feedback loops to

Communicating for business resilience.

Defining resilience has remained a challenge despite decades of research across multiple disciplines. This paper proposes that resilience can be better understood when examined through the framework of communication. Resilience is proposed to have three interconnected domains (community, organisation and individual) and two dimensions (physical and social-emotional). This work focuses on the social-emotional components of enacted organisational resilience. The impact of organisational culture on sensemaking, organisational narrative and resilience strategies is explored. Communication-based in

Organisational resilience: Shifting from planning-driven business continuity management to anticipated improvisation.

As unpredictable major-impact events are on the rise, many organisations have adopted an organisational resilience (OR) approach for dealing with these so-called 'black swan events. What OR comprises is subject to ambiguity and multiple interpretations. This article presents a perspective that makes a distinction between predictable risks and unpredictable major-impact events. The article argues that predictable risks would benefit from an adaptive and efficient business continuity management (BCM) capability. Using several cases, the article demonstrates how the adaptability and efficiency of

Utilising social media within incident response programmes.

Humans are social creatures that learn from observing others. Until this century, the reach of social learning was confined to small groups or locales. However, the pervasive adoption of social media platforms has provided the means to augment social learning and empower virtual groups by transcending geographic and time boundaries. The broad adoption of social media across all demographic groups gives augmented social learning potentially broad applications, including within the realm of incident response. This article will discuss how augmented social learning can be a powerful tool to manag

Using unmanned aircraft systems for live event monitoring: The benefits of aerial observation.

What started as a project on how to detect unwanted drones during University of Colorado Boulder football games has turned into a programme providing live aerial coverage of campus events. The use of drones (or unmanned aircraft systems) has improved situational awareness for event command and control, enabling adjustments to tactics and strategies during events. This paper will detail the humble start-up of a drone programme, describing the need for partnerships, the basics of flying, the essentials of a drone programme, and examples of drone use during incidents and/or events. The reader wil

Hurricane preparedness, response and recovery: Is everybody ready?

Nothing strikes a heightened level of awareness among coastal and island communities, emergency managers and critical infrastructure owners like the start of another hurricane season. Even though the season starts at the same time every year - 15th May for the Eastern Pacific region, and 1st June for the Atlantic and Central Pacific regions - too many organisations, agencies and communities are ill-prepared for a hurricane to strike. For emergency management professionals and critical infrastructure owners, however, this is not an option: the communities served by these organisations are count

The use of metrics to manage enterprise security risks: Understanding, evaluation and persuasion.

Metrics drive business decisions and behaviour. Properly developed using psychometric principles, metrics provide a solid rationale for senior management to make prudent decisions about organisational growth as well as facilitating the assessment of internal policies and programmes. Until recently, there has been little research on the development of effective performance metrics, especially in the security field. The American Society for Industrial Security International (ASIS International) provided funding for an extensive study into the development of metrics for the security profession. T
