Acute lumbar disk herniations are the most common cause of sciatica. After excluding emergent causes, such as cauda equina syndrome, epidural abscess, fracture, or malignancy, a six-week trial of conservative management is indicated. Patients should be advised to stay active. If symptoms persist after six weeks, or if there is worsening neurologic function, imaging and invasive procedures may be considered. Most patients with lumbar disk herniations improve over six weeks. Because there is no difference in outcomes between surgical and conservative treatment after two years, patient preference

Bovine biomechanical data have demonstrated adequate stability of a single threaded interbody cage when combined with a unilateral facet screw during posterior lumbar disc excision and interbody fusion (PLIF). Instrumented PLIF surgery using 1 versus 2 interbody cages was studied in 35 military men with disc disease and chronic low back pain. All patients underwent instrumented PLIF with bilateral diskectomy and partial facetectomy, pedicle screws, autogenous iliac crest bone graft, 1 or 2 interbody cages, and posterior lateral fusion. After an average of 15 months' follow-up, the 2-cage group

This retrospective study analyzes the mid-term results of 22 patients who underwent diskectomy following a diagnosis of cauda equina syndrome due to prolapsed intervertebral lumbar disks (mean follow-up: 3 years and 9 months). Postoperatively, 17 of 22 patients had complete urinary function recovery within the follow-up period, 4 patients had a persistent stress incontinence, and 1 patient, incontinent 4 years postoperatively, required catheterization. Thirteen of 17 patients had recovery of motor deficits, 14 of 21 of sensory deficits, and 13 of 15 patients regained perianal sensation. There

A 64-year-old patient with stage IV non-small-cell lung carcinoma and several comorbidities, which include obesity and long-term smoking, was treated with N-allyl noroxymorphone eluting osteoinductive bone graft biomaterial. The patient had multilevel degenerative disk disease (DDD), which has a high rate of failure when osteoinductive bone grafts are not used. Infuse, the most widely administered osteoinductive bone graft, is contraindicated in the spine for patients with active tumor. As such, a novel drug eluting osteoinductive biomaterial was administered to this patient, for whom no other

A rare case of aneurysm of the lateral sacral artery is reported. This 46-year-old female presented with complaints of bowel and bladder incontinence and decreased perianal sensation for the past 15 months. She underwent laminectomy and diskectomy for the diagnosis of a prolapsed disk at peripheral hospital where the surgeon was confronted with a severe and unexpected hemorrhage, and surgery was aborted without effective treatment. Prior medical history includes hypertensive kidney disease with a renal transplant eight years ago. Magnetic resonance imaging and angiographic findings were sugges

Cervical laminoplasty was initially described for the management of cervical myelopathy resulting from multilevel stenosis secondary to ossification of the posterior longitudinal ligament. The general concepts are preservation of the dorsal elements, preservation of segmental motion, and expansion of the spinal canal via laminar manipulation. No clear evidence suggests that laminoplasty is superior to either posterior laminectomy or anterior cervical diskectomy and fusion. However, laminoplasty has its own advantages, indications, and complications. Surgeons have refined the technique to decre

We have carried out temporary silicone implants after diskectomies or arthroplasties in temporomandibular joint surgeries to avoid postoperative adhesion and to maintain articular space. We evaluated 19 joints in 15 patients who had received dacron-reinforced silicone implant after silicone sheet removal through follow-up for at least 6 months. The cases included temporomandibular joint disorder (10 joints in 9 patients), psoriatic arthritis (2 joints in 1 patient), ankylosis (4 joints in 3 patients) and synovial chondromatosis (2 joints in 2 patients). On the basis of the criteria of temporom

Twenty-eight patients with temporomandibular dysfunction problem have been subjected to diskectomy unilaterally. The diagnosis was in 23 patients ADD (anterior disk displacement) without reduction and in another five ADD with reduction. The disk was removed in total according to a routine procedure. Surgical observation of a firm connection between the inferior surface of the disk and the condylar head was noted in eighteen patients. The disk tissue was orientated on a cork sheet according to its position in the fossa and subjected to histological examination. Histopathological analysis showed

Current trends in the surgical treatment of cervical disk disease, when compared with those of the 1960s, have been unimpressive. There is an ongoing refinement of anterior procedures, which seem to be effective for both myelopathy and radiculopathy, ranging from simple diskectomy and/or fusion to variously radical anterior decompressions. This refinement has demonstrated that however simple or radical the anterior procedure is, the results seem to be the same and that bone grafting is not necessarily critical to success. Posterior procedures, both for root exploration and spinal cord decompre

This operative video details treatment of a cervical disk herniation through a minimally invasive posterior cervical foraminotomy and microdiskectomy. A man in his 20s presented with unilateral C6-7 radiculopathy, and imaging findings indicated a paracentral disk herniation at C6-7. There was no evidence of spinal cord compression or cervical myelopathy and no severe spondylosis. Therefore, a posterior cervical foraminotomy was offered rather than an anterior diskectomy and fusion or arthroplasty to preserve segmental motion and limit potential implant-related complications. The patient consen

Posterior lumbar apophyseal ring fractures are rare in adolescents. We report 4 such cases in Chinese adolescents. Two of the patients had a slipped capital femoral epiphysis; 3 of them were overweight/obese. All presented with low back pain and radicular pain. Apophyseal fractures of the upper lumbar spine usually involve the lower end plate, whereas those of the lumbar sacral spine usually involve the upper end plate. The radiological features and pathophysiology are discussed. Two of the patients were treated with laminotomy and diskectomy after conservative treatment failed. All patients h

BACKGROUND: Lumbar disc herniation is one of the commonest conditions affecting the lumbar spine, with disc fragments migrating in 35-72% of patients. It can be treated with either microdiscectomy or percutaneous endoscopic lumbar discectomy (PELD) surgeries.
MATERIALS AND METHODS: We searched these databases; Web of Science, PubMed, Cochrane Library, and SCOPUS. Overall, 26 studies, both randomized controlled trials and observational studies, were included. We analyzed different efficacy, safety, and functional outcomes including operation time, estimated blood loss (EBL), length of hospital

OBJECTIVE: To determine the safety and efficacy of percutaneous endoscopic lumbar discectomy (PELD) combined with platelet-rich plasma (PRP) hydrogel injection in patients with lumbar disc herniation (LDH).
METHODS: A total of 98 consecutive patients with LDH who underwent either PELD combined with PRP hydrogel injection or PELD alone were reviewed. This retrospective study was performed between January 2019 and January 2021. Clinical outcomes were compared in the visual analog scale (VAS) for low back pain and leg pain, Oswestry disability index (ODI), Japanese Orthopaedic Association (JOA)

BACKGROUND AND OBJECTIVES: En bloc spondylectomies are essential for controlling malignant spinal tumors, but existing classification systems are often complex and impractical. This study introduces the Column, Line-of-Sight, Margins, Ring (CLiMR) framework, a novel system simplifying and standardizing en bloc spondylectomy planning and classification.
METHODS: A retrospective review of patients who underwent single- or multistage en bloc spinal tumor resections was conducted at a single academic center (October 2019-December 2024). The CLiMR framework assigns scores to 4 components: Column (

BACKGROUND: Management of anterior cervical corpectomy and fusion (ACCF)-derived adjacent segment disease (ASD) represented a challenge facing the surgeons.
METHODS: A 41-year man diagnosed as C3-4 level ASD derived from C5-level ACCF surgery 13 years ago was admitted to the hospital for numbness and pain in the right shoulder and upper limb. Percutaneous full-endoscopic anterior transcorporeal cervical discectomy (PEATCD) was performed, and pre- and postoperative clinical and imaging data were collected.
RESULTS: The operation was completed within 70 min, and no clinical or radiological com

OBJECTIVE: To first report the application of percutaneous full-endoscopic anterior transcorporeal cervical discectomy (PEATCD) with channel repair for a patient with adjacent segment disease (ASD) after anterior cervical discectomy and fusion.
METHODS: PEATCD with channel repair was performed for a patient with ASD at the cranial level adjacent to previous fusion at the C5-C6 level. The pre- and postoperative clinical symptoms were evaluated with Japanese Orthopedic Association and visual analog scale (VAS). The radiological examinations included magnetic resonance imaging, computed tomograp

BACKGROUND: Cervical disk arthroplasty is now a widely accepted alternative to anterior cervical interbody fusion, which is known to reduce normal cervical motion and increase the incidence of adjacent segment disease. Although multiple studies report the use of cervical disk arthroplasty to treat multilevel cervical disease, this is the first report in the literature detailing the placement of multiple, noncontiguous artificial disks.
CASE DESCRIPTION: We describe a 41-year-old male who presented with myelopathy and left upper extremity radiculopathy resulting from 2 cervical disk herniation

BACKGROUND: Iatrogenic vertebral artery injury is an uncommon but well recognized complication during cervical spine surgery. Intraoperative surgical repair is extremely challenging, and options for endovascular repair are limited because of the lack of proper equipment in the operating room setting.
CASE DESCRIPTION: A 53-year-old woman who presented with myelopathy underwent anterior cervical diskectomy and fusion of C3-7. A significant laceration injury of the left vertebral artery was encountered during surgery, which was salvaged by intraoperative endovascular repair with a covered stent

BACKGROUND: Damage to the intervertebral disk is usually corrected by means of a prosthesis.
OBJECTIVES: To report the outcome of the artificial lumbar disk replacement with the Charité SB III disk prosthesis in 20 patients after a 48 month follow-up.
METHODS: The 20 patients were evaluated clinically and radiographically during this period. Preoperative diagnosis included degenerative diskopathy in 17 patients and failed posterior conventional diskectomy in 3. The prosthesis was implanted at one level in 17 patients and bi-level implantation was performed in the other 3 patients.
RESULTS:
