BACKGROUND: Percutaneous cervical nucleoplasty (PCN) is a minimally invasive technique used for the treatment of cervical disc herniation. However, if the lesion is located at the neural foramen, complete access and effective neural decompression are often challenging because of the special anatomical condition of the uncovertebral joint, osteophyte, and facet joint of the cervical foraminal space.
OBJECTIVES: To investigate the effect of PCN on radiculopathy caused by foraminal cervical disc herniation (FCDH).
STUDY DESIGN: Retrospective case series.

BACKGROUND: Percutaneous endoscopic lumbar discectomy (PELD) via the transforaminal approach is difficult at L5-S1 in patients presenting with high iliac crests (HIC). The conventional wisdom is that measurement using lumbar radiography, computed tomography (CT), or magnetic resonance imaging (MRI) is necessary.
OBJECTIVES: The objective of this study was to introduce a lumbo-iliac triangular (LI-Tri) technique based on biplane oblique fluoroscopy and verify whether it facilitated transforaminal PELD for patients with L5-S1 lumbar disc herniation (LDH) combined with HIC.

OBJECTIVE: To present a unique case of guide wire breakage as a rare complication during posterolateral endoscopic lumbar discectomy.
BACKGROUND: Posterolateral endoscopic lumbar discectomy (PELD) has become a routinely performed minimally invasive spinal procedure. However, several complications of PELD have attracted our concerns, including the intraoperative injury to neural and vascular structures and failure of the surgery. We have countered a rare intraoperative complication of guidewire breakage during a PELD procedure, as far as we know, this has not been reported previously, and we w

Lumbar disc infection, either after surgical discectomy or caused by haematogenous spread from other infection sources, is a severe complication. Specific antibiotic treatment has to be started as soon as possible to obtain satisfactory results in conservative treatment or operative fusion. The aim of this study was to analyse 16 cases of lumbar disc infection, treated with percutaneous lumbar discectomy (PLD) to obtain adequate amounts of tissue for histological examination and microbial culture. Between 1990 and 1994, 26 patients with vertebral osteomyelitis were treated. Sixteen patients, w

BACKGROUND: Currently, unilateral biportal endoscopy (UBE) and percutaneous endoscopic lumbar discectomy (PELD) are increasingly being used for the treatment of lumbar disc herniation (LDH). However, there are few studies comparing the efficacy of UBE and PELD in the treatment of LDH. The aim of this study is to compare the clinical efficacy and radiological outcomes between UBE and PELD in the treatment of LDH. METHODS: A retrospective analysis was conducted on 108 patients with LDH treated with spinal endoscopy in our hospital from January 2021 to July 2023. Of these, 47 patients were includ

OBJECTIVE: Creating a 3D lumbar model and planning a personalized puncture trajectory has an advantage in establishing the working channel for percutaneous endoscopic lumbar discectomy (PELD). However, existing 3D lumbar models, which seldom include lumbar nerves and dural sac reconstructions, primarily depend on CT images for preoperative trajectory planning. Therefore, our study aims to further investigate the relationship between different virtual working channels and the 3D lumbar model, which includes automated MR image segmentation of lumbar bone, nerves, and dural sac at the L4/L5 level

OBJECTIVE: Currently, there is no established guideline on whether to opt for percutaneous endoscopic lumbar discectomy (PELD) or traditional transforaminal lumbar interbody fusion (TLIF) surgery based on specific types of lumbar disc herniation (LDH). Based on the Michigan State University (MSU) classification system, this study conducted a medium- to long-term follow-up analysis of two surgical methods over 5 years for the first time, aiming to provide empirical evidence to assist in making more informed decisions before surgery for LDH treatment.
METHODS: This was a retrospective study tha

BACKGROUND: Percutaneous endoscopic discectomy (PED) includes 2 main procedures: percutaneous endoscopic lumbar discectomy (PELD) and percutaneous endoscopic interlaminar discectomy (PEID), both of which are minimally invasive surgical procedures that effectively deal with lumbar degenerative disorders. Because of the challenging learning curve for the surgeon and the individual characteristics of each patient, preventing and avoiding complications is difficult. The most common complications, such as nucleus pulposus omission, nerve root injury, dural tear, visceral injury, nerve root induced

BACKGROUND: Percutaneous cervical nucleoplasty (PCN) is an effective treatment for cervical herniated intervertebral disc (C-HIVD). In this retrospective study, we evaluated clinical predictors that affect the successful outcome of PCN.
METHODS: Fluoroscopically guided PCN was conducted for C-HIVD by one pain physician. Successful outcome was defined as a combination of greater than 50% pain relief on the numerical rating scale pain score, no increase in analgesics, and no cervical epidural steroid injection during the 3-month follow-up period. The relationship between outcomes and independen

PURPOSE: Percutaneous endoscopic cervical discectomy (PECD) is an emerging surgical treatment for cervical disc herniation in recent years, but the complications of PECD are rarely reported. In this case report, we aimed to report two cases of total spinal anesthesia, an unusual complication of PECD.
METHODS AND RESULTS: One patient became unconscious with unstable vital signs and another patient appeared with numbness of both arms and legs during PECD operation.
CONCLUSIONS: From these two cases, we concluded that the operation of anchoring during PECD should be cautious and standardized, i

BACKGROUND: Percutaneous endoscopic lumbar discectomy is a common surgical treatment for lumbar disc herniation, and percutaneous endoscopic interlaminar lumbar discectomy (PEID) is commonly used for direct decompression of L5-S1. Like microdiscectomy, recurrence of herniation after endoscopic discectomy is an important problem. In this study, we aimed to decrease the recurrence after PEID using a new surgical technique.
OBJECTIVES: We propose a new surgical technique for reducing the recurrence after PEID for lumbar disc herniation. The new technique uses annular sealing after fragmentectomy

BACKGROUND: Since the descriptions by Mixter and Barr of open surgical treatment for rupture of the intervertebral disc in 1934, open surgical procedures have become a common practice. Disc herniations have been reported as being contained and non-contained. The results of open surgical discectomy for contained disc herniation have been poor. Consequently, several alternative techniques have been developed which are minimally invasive including percutaneous laser disc decompression.

BACKGROUND: In recent years, a number of minimally invasive nuclear decompression techniques for lumbar disc prolapse, protrusion, and/or herniation have been introduced, including the Dekompressor a device utilizing an Archimedes screw. The primary goal of the surgical treatment of nerve root compression from a disc protrusion continues to be the relief of compression by removing the herniated nuclear material with open discectomy. However, poor results have been reported for contained disc herniations with open surgical interventions. The results with several alternative techniques including

BACKGROUND: The L4-5 and L5-S1 intervertebral disc spaces are the most frequent sites of discal spinal pathology, hence, diagnostic and therapeutic interventions are commonly performed at these levels. While performing fluoroscopically guided spinal procedures such as discography or intradiscal electrothermal anuloplasty (IDEA), antero-posterior (AP), lateral, and oblique views are utilized. However axial projection is not typically possible without three-dimensional imaging such as computerized tomography (CT). Intraprocedural CT is not commonly available. Instead, post-discography CT axial v
