OBJECTIVES: To assess the period prevalence and severity of dizziness and vertigo in adolescents.
METHODS: In 1661 students in 8th-10th grade in twelve grammar schools in Munich, Germany information on vertigo/dizziness was assessed by a questionnaire in the class room setting. Three month prevalence of dizziness/vertigo was estimated; symptoms were categorized as orthostatic dizziness, spinning vertigo, swaying vertigo or unspecified dizziness. Duration of symptoms and impact on daily life activities were assessed.
RESULTS: 72.0% (95%-CI = [69.8-74.2]; N = 1196) of the students (mean age 14

PURPOSE OF REVIEW: Recent advances in next generation sequencing techniques (NGS) are increasing the number of novel genes associated with cerebellar and vestibular disorders. We have summarized clinical and molecular genetics findings in neuro-otolology during the last 2 years.
RECENT FINDINGS: Whole-exome and targeted sequencing have defined the genetic basis of dizziness including new genes causing ataxia: GBA2, TGM6, ANO10 and SYT14. Novel mutations in KCNA1 and CACNA1A genes are associated with episodic ataxia type 1 and type 2, respectively. Moreover, new variants in genes such as COCH,

Although vertigo is a common complaint with patients presenting to an ED, some uncommon causes also must be considered. This report focuses on a woman who developed sudden-onset vertigo with associated mental status changes after a long-distance flight. Her symptoms were consistent with mal de debarquement syndrome, an uncommon cause of vestibular dysfunction.

INTRODUCTION: Vertigo, dizziness, and nausea encompass a spectrum of balance-related symptoms caused by a variety of etiologies. Balance is affected by many systems: Proprioceptive pathways and visual, cerebellar, vestibulocochlear, and vascular / vasovagal systems. Vertigo is a subtype of dizziness, in which a subject, as a result to a dysfunction of the vestibular system, improperly experiments the perception of motion. The most useful clinical subdivision is to categorize vertigo into true vertigo and pseudovertigo, whereas from a pathophysiological point of view, vertigo can be classified

Only few patients with dizziness are referred to a medical specialist, who is sometimes unable to come to a conclusive cause of the dizziness. We have no information about the course of unexplained dizziness, from the literature or from our own population. We therefore undertook this follow-up study to assess the outcome of patients with unexplained dizziness, after assessment in a multidisciplinary outpatient clinic. In 292 patients (out of our total cohort of 2,244 patients), a diagnosis could not be established. The median dizziness handicap inventory (DHI) score at the diagnostic visit was

BACKGROUND: Dizziness is among the most frequent neurological chief complaints in emergency room (ER) patients. Although the majority of underlying disorders are benign, serious causes that require immediate in-hospital treatment may occur that are difficult to identify clinically.
METHODS: Retrospective study of 475 consecutive ER neurological consultations with dizziness as the chief complaint.
RESULTS: Of all ER dizziness patients, 73% were initially assigned to benign and 27% to serious diagnoses. The two most frequent disorders were benign paroxysmal positional vertigo (22%) and stroke

Dizziness is not a unique clinical picture, but rather is the unspecific symptom of numerous diseases. Dizziness always develops when the real incoming vestibular, visual and somatosensory information contradict each other and the expected input signals. Disorders of the vestibular apparatus are the most frequent cause of dizziness; in contrast, cervicogenic causes play a secondary role. Dizziness syndromes usually have a favourable prognosis and can be successfully treated.

The middle ear pressure was analyzed in 112 patients with dizziness. In 37 patients with Meniere's disease, the middle ear pressure on the low-pressure side was significantly lower, the middle ear pressure difference (between the high and low sides) was significantly larger, and the maximum compliance on the low-compliance side was significantly lower than in normal volunteers. The middle ear pressure difference was significantly larger during periods of dizziness or recurrent dizziness than at the time of remission. In patients with Meniere's disease, a middle ear pressure difference of more

The objective of the study was to validate the effectiveness of a questionnaire and computer-assisted algorithm in diagnosing children with dizziness or vertigo. Dizziness and vertigo are common complaints in children, causing an extensive, often unnecessary evaluation. A pediatric "dizziness questionnaire" was designed and a computer-assisted algorithm was developed to facilitate the diagnostic task. A retrospective medical record review was conducted on all children presenting to the clinic for dizziness or vertigo throughout a 2-year period. The information was used by one investigator to c

The diagnosis of cervicogenic dizziness is characterized by dizziness and dysequilibrium that is associated with neck pain in patients with cervical pathology. The diagnosis and treatment of an individual presenting with cervical spine dysfunction and associated dizziness complaints can be a challenging experience to orthopaedic and vestibular rehabilitation specialists. The purpose of this article is to review the incidence and prevalence, historical background, and proposed pathophysiology underlying cervicogenic dizziness. In addition, we have outlined the diagnostic criteria, evaluation, a

Complaints of vertigo and dizziness are common problems referred to otolaryngologists for evaluation. Awareness of uncommon causes of dizziness increases the physician's ability to diagnose and treat these patients. We present the case of a middle-aged woman who presented with episodes of vertigo and symptoms suggestive of vertebrobasilar insufficiency. These symptoms were the result of a persistent trigeminal artery (PTA) and occlusive carotid artery disease. A PTA is a carotid-basilar anastomosis that has been reported to be demonstrated on 0.1% to 0.6% of all cerebral angiograms. Persistenc

Dizziness is a common symptom in patients presenting to an otorhinolaryngologist. Hyperventilation accounts for up to 5% of cases with dizziness and is a contributory factor in a further 20% of cases. Six cases of dizziness due to hyperventilation are presented to illustrate the author's simple management policy. A high index of suspicion in the absence of an organic cause of dizziness and a simple provocation test will identify these cases. Management is aimed at demonstrating resting hypocapnia, investigations to exclude organic causes of hyperventilation and rehabilitation in collaboration

Because of the benefit-risk ratio in their favor, calcium channel antagonists are considered to be a safe form of treatment of hypertension in the elderly. However, the outcome of dizziness in some patients over 65 yr old during the first few days of treatment with these drugs has to be considered. Five cases of "malaise" have been mentioned, which occurred during the first few days of treatment. The imputability of the drug in question has been evaluated. Additional factors have been found, ie orthostatic hypotension whatever its mechanism, and association with diuretics, with or without hypo

Vertigo is a distressing symptom, but in the majority of cases it can be treated effectively. Whenever possible, treatment should be directed at the underlying disorder. Usually, however, symptomatic treatment alone or in combination with specific therapy is effective. Symptomatic treatments include antivertiginous medications, vestibular exercises, and surgical therapy. The choice of medications is determined by the known side effects of the drugs and the time course of symptoms. An early vestibular exercise program ensures a more rapid return to normal physical activity. Surgical therapy is

Much diagnostic confusion arises from the fact that dizziness is a vague symptom with multiple individual interpretations. A systematic approach to eliciting a history from a dizzy patient is presented, differential diagnoses of vertigo are delineated, and appropriate treatment for peripheral labyrinthine disorders is suggested.
