Egg donation beyond the standard model: fertility practitioners' clinical reasoning in single, combined and shared treatment options in Sweden.

The use of donor eggs, sperm and embryos in medically assisted reproduction (MAR) provide new possibilities for reproductive assistance and family-making. In clinical practice, it also brings to light questions of responsibility and ethical conduct. Despite this, fertility practitioners' reasoning in clinical decision-making remains surprisingly understudied. Drawing on Annemarie Mol's work on ontologies in medical practice (2002), we examine how fertility practitioners' clinical reasoning varies across different types of reproductive assistance involving donor eggs. Following Sweden's legal c

Factors in the infertile couple influencing the success of artificial insemination with donor semen.

Factors influencing the success of artificial insemination with donor semen (AID) were investigated in a series of 928 infertile couples. A simple approach with one insemination/cycle, timed only by cycle length and basal body temperature, was used. The overall life-table cumulative conception rate was 82% and the average fecundability was 10%. The following factors were associated with a higher success rate: women less than 36 years, azoospermic compared with oligozoospermic husband and women with no history of abdominal surgery. The success rate decreased with increasing duration of infertil

Obstetric and perinatal outcomes in pregnancies conceived with donor versus partner sperm: a systematic review and meta-analysis.

BACKGROUND: Male-related factors contribute to 30-40% of infertility cases, with donor sperm serving as a critical solution for severe male infertility or paternally inherited genetic disorders. While cryopreservation ensures virological safety, concerns persist regarding sperm DNA damage, oxidative stress, and epigenetic impacts on embryogenesis. Previous studies have shown inconsistent evidence regarding obstetric and perinatal outcomes using donor versus partner sperm. This meta-analysis aimed to compare these outcomes to guide evidence-based clinical decisions. METHODS: To identify studie

Relationships of survival time, productivity and cause of death with telomere lengths of cows produced by somatic cell nuclear transfer.

The reproductive ability, milk-producing capacity, survival time and relationships of these parameters with telomere length were investigated in 4 groups of cows produced by somatic cell nuclear transfer (SCNT). Each group was produced using the same donor cells (6 Holstein (1H), 3 Holstein (2H), 4 Jersey (1J) and 5 Japanese Black (1B) cows). As controls, 47 Holstein cows produced by artificial insemination were used. The SCNT cows were artificially inseminated, and multiple deliveries were performed after successive rounds of breeding and conception. No correlation was observed between the te

Lesbian Motherhood and the Artificial Insemination by Donor Scandal of 1978.

In January 1978, the London Evening News informed its readers of its shocking discovery that British lesbians were conceiving babies by artificial insemination by donor (AID). Assisted by a respected London gynaecologist, Dr David Sopher, the women were planning and raising children in the context of lesbian relationships, challenging conventional family models and the widespread presumption that lesbianism and motherhood were mutually exclusive identities. The debate which was sparked by the Evening News expose and taken up in Parliament, the national and local media and on the streets in the

Sharing information with adults conceived as a result of donor insemination.

A growing number of jurisdictions have introduced legislation to give children the right to information about, and identity of, 'their' gamete donor. This has been accompanied by a reduction in the social stigma associated with infertility and building families with the assistance of donated gametes. This changing culture has meant that an increasing number of parents are seeking assistance regarding how to share their family history, including the donor insemination conception story. The literature suggests that the sharing of information is best undertaken when the children are young. There

New challenges for gamete donation programmes: changes in guidelines are needed.

In the UK, the Human Fertilisation and Embryology Act 1990 prevents children born as a result of donor-assisted conception from gaining access to identifying information about their genetic origins. There is growing concern that current screening protocols regarding gamete donation are ill-suited, especially in relation to genetic disease. There are no guidelines addressing the issues of confidentiality that might arise if a disease emerges after insemination and establishment of pregnancy. Donors may become aware that they are at risk of a familial condition after they have donated gametes or

Single versus double insemination: a retrospective audit of pregnancy rates with two treatment protocols in donor insemination.

Our objective was to evaluate the effect of a change in treatment protocols, suggested following an inspection visit by the regulatory authority, from single to double inseminations during donor insemination treatment cycles. We therefore conducted a retrospective audit of pregnancy rates in the reproductive medicine clinic of a major teaching hospital. All patients were treated for male factor infertility by donor insemination, without ovulation induction with gonadotrophins between October 1992 and December 1995. The main outcome measures were cumulative conception and live birth rates. Duri

Report From a Multidisciplinary Symposium on the Future of Living Kidney Donor Transplantation.

Virtually all clinicians agree that living donor renal transplantation is the optimal treatment for permanent loss of kidney function. Yet, living donor kidney transplantation has not grown in the United States for more than 2 decades. A virtual symposium gathered experts to examine this shortcoming and to stimulate and clarify issues salient to improving living donation. The ethical principles of rewarding kidney donors and the limits of altruism as the exclusive compelling stimulus for donation were emphasized. Concepts that donor incentives could save up to 40 000 lives annually and conside

Meteorological elements associated with vasovagal reactions in blood donors: Influence of collection setting and donation type.

BACKGROUND AND OBJECTIVES: Vasovagal reactions (VVRs) are common adverse events associated with blood donation. While previous research has focused on intrinsic donor-related risk factors, we previously identified an association between specific meteorological elements and VVR incidence. This study aims to build on our earlier findings by investigating the potential modifying effects of collection setting and donation type. STUDY DESIGN AND METHODS: This study analysed VVR incidence data from blood donations collected in northern Nara Prefecture, Japan, over a 3-year period (April 2020-March

Why uterine transplantation requires us to rethink the role of the pre-conception welfare principle.

Uterine transplantation (UTx) is a programme of treatment aimed at providing a unique solution to absolute uterine factor infertility, enabling patients to have children as a result of their own pregnancies. As a transplant procedure performed for fertility purposes it may be thought obvious that the welfare of any children created should be assessed prior to treatment provision. However, major concerns about the breadth and scope of such requirements, and the potential threat they pose to patients' reproductive autonomy, have been raised. In this paper, I analyse novel questions regarding the

Spontaneous Pregnancy in the Setting of Primary Ovarian Insufficiency and Breastfeeding: Does Immunosuppression Play a Role?

BACKGROUND Primary ovarian insufficiency is defined as primary hypogonadism in a woman under the age of 40 years. It commonly presents clinically with amenorrhea and infertility. It is often thought to be an autoimmune process. Breastfeeding also reduces the rate of conception by reducing pulsatile gonadotropin secretion, resulting in lactational amenorrhea. CASE REPORT Here, we present a case of a patient with primary ovarian insufficiency. FSH and LH levels at diagnosis were in the menopausal range. After undergoing fertility treatments and failing to become pregnant, she achieved a first pr

Liver repopulation: a new concept of hepatocyte transplantation.

Hepatocyte transplantation has been recognized as an alternative strategy for organ transplantation because the supply of donor livers is limited. However, in conventional hepatocyte transplantation, only 1%-10% of the liver replaced with transplanted hepatocytes. Recently a novel concept termed "liver repopulation" has been established, where the whole recipient liver can be replaced by a small number of donor hepatocytes. To induce liver repopulation, growth advantage of the donor hepatocytes against the host liver seems to be required according to the data of previous studies. Additionally,

A review of family donor constructs: current research and future directions.

This review addresses research on gamete donors, recipients, and offspring and demonstrates that the foci on all three within the triad are largely directed at disclosure or anonymity; and each in turn centers on the perceived importance of the genetic link. The importance attached to genetics has led some countries to review the ethics of anonymous gamete donation (e.g. New Zealand's 'open system' of information sharing) and has led other countries (Sweden, Austria, Victoria, Australia; the Netherlands, the UK) to change their laws allowing donor gamete offspring the right to obtain identifyi

Fragile X Syndrome Secondary to in Vitro Fertilization With a Family Egg Donor: A Case Report and Review of the Literature.

Objective: To evidence the need for screening fragile X syndrome (FXS) in egg donors in assisted reproduction protocols. Case report : This is the report of a boy with FXS who inherited the mutated allele from an ovule donated by the mother´s sister through an assisted reproduction protocol. Identifying premutation (PM) carriers of FXS amongst gamete donors isn't part of the obligatory genetic analysis for donors and is only considered by most of the in vitro fertility societies and guidelines as part of the extension screening tests. Conclusion: It is cost-effective to do pre-conceptional scr

The ethical case for non-directed postmortem sperm donation.

In this article we outline and defend the concept of voluntary non-directed postmortem sperm donation. This approach offers a potential means of increasing the quantity and heterogeneity of donor sperm. This is pertinent given the present context of a donor sperm shortage in the UK. Beyond making the case that it is technically feasible for dead men to donate their sperm for use in reproduction, we argue that this is ethically permissible. The inability to access donor sperm and the suffering this causes, we argue, justifies allowing access to sperm donated after death. Moreover, it is known t

Renal transplantation from extended criteria cadaveric donors: problems and perspectives overview.

The critical shortage of organs available for renal transplantation has led to the consideration of alternative strategies for increasing the donor pool. Recently, the cadaveric kidney donor pool extended to donors who might have been deemed unsuitable in early times, leading to the concept of marginal donors and more recently to the notion of expanded criteria donors. Such organs are eligible for organ donation but, because of extreme age and other clinical characteristics, are expected to produce allograft at risk for diminished post-transplant function. Thus, the challenge is now to reduce

Female-partnered and single women's contact motivations and experiences with donor-linked families.

STUDY QUESTION: What are female-partnered and single mothers' motivations and experiences at one donor insemination (DI) program with regard to contacting other families who share the same sperm donor? SUMMARY ANSWER: By and large, women reported seeking contact to obtain (i) support for their children and/or themselves, and (ii) information about shared traits and medical problems, ultimately describing a range of contact experiences, both positive (e.g. special bond created) and negative (e.g. uncomfortable encounters). WHAT IS KNOWN ALREADY: There is a growing phenomenon of donor insemina

Robot-assisted donor hysterectomy in uterus transplantation - a modality to increase reproducibility.

Uterus transplantation is a non-lifesaving vascularized composite allotransplantation procedure requiring immunosuppression until removal of the graft. The focus of uterus transplantation is changing regarding refining individual treatment procedures included in this complex treatment of absolute uterine factor infertility, such as robot-assisted donor hysterectomy. The inferior hypogastric nerve plexus should be preserved during robotic dissection of the ureter and uterine vessels to prevent postoperative complications such as urine and fecal evacuation disturbances and sexual disorders.
